However, its pathophysiology has not been completely elucidated, therefore various approaches have been applied to its therapeutics. The management including pharmacological treatment is still evolving, and guidelines of atrial fibrillation have been updated frequently. This review article will introduce the principles and practical information on the pharmacological management in patients with atrial fibrillation based on the recently updated guidelines. (Korean J Med 2011;81: ) Keywords: Atrial fibrillation; Pharmacological treatment 서 론 심방세동의약물치료원칙 심방세동 (AF) 은가장흔한지속성부정맥이며또한가장복잡한부정맥이다. 하지만아직까지정확한병태생리를알지못해다양한접근법이치료에동원되고있으며, 이에바탕을둔새로운임상시험결과가계속보고되고있다. 본리뷰에서는최근업데이트된유럽심장학회 (ESC) 2010 가이드라인 [1], 미국심장학회 (ACCF/AHA/HRS) 2006 가이드라인에대한 2011 업데이트 [2], 캐나다심장학회 (CCS) 2010 가이드라인에 [3] 바탕을둔심방세동의치료중항부정맥제에대한내용을다루고자한다. 심방세동은임상적으로발작성 (paroxysmal), 지속성 (persistent) 및영구형 (permanent) 으로구분할수있다. 저절로동율동으로전환되는경우를발작성, 동율동전환을위해항부정맥제또는직류전기심율동전환이필요한경우를지속성, 동율동으로전환되지않는경우를영구형심방세동이라한다. 각유형별치료의알고리듬은그림 1, 2, 3에서제시된바와같다. 약제를이용한심박수조절방법지속성및영구형심방세동환자에서심박수조절의표적이과거와달라졌다. 이는 RACE II 연구의결과에의한것 Correspondence to Seil Oh, M.D., Ph.D., FHRS Department of Internal Medicine, Seoul National University College of Medicine, 101 Daehak-ro, Jongno-gu, Seoul , Korea Tel: , Fax: , seil@snu.ac.kr

Pharmacological management of patients with newly discovered AF. 이다 [4]. 즉안정시심박수를 < 80 bpm으로한군과 < 110 bpm으로한군사이에임상적결과가차이가없다는것이다. 유럽가이드라인은안정시심박수 < 110 bpm으로치료를시작하는것을그대로인정하고있다 ( 권고강도 IIa, 증거수준 B). 물론이정도의심박수를유지해도증상이지속되거나빈맥성심근병증이있으면 < 80 bpm을권장한다. 미국가이드라인은 LVEF > 0.40 이고증상이없는환자에서는안정시심박수 < 110 bpm보다더조절하는것이이득이없으므로이런행위는권고강도 III ( 증거수준 B) 로분류하고있다. 하지만캐나다가이드라인에서는 < 100 bpm으로할것을권고하고있다. 이전연구들에서심박수가 bpm 인환자수는매우적었기때문에 100 bpm 이상인경우의안 Figure 2. Verapamil 은혈중 digoxin 레벨을상승시킬수있으므로함께투여할경우 digoxin의용량을줄여야한다. 표 1, 2 에흔히사용되는약제를요약하였다. 약제를이용한동율동전환방법동율동으로전환시키는방법에는약제를이용하는방법과전기충격이있다. 동율동전환율은전기적인방법이우수하다. 동율동전환시에는혈전색전증의위험이있으며, 두방법간에혈전색전증발생위험은유의한차이가없다. 전기충격을이용하는경우진정 (sedation) 또는마취가필요하지만약제투여에의한경우는필요하지않다. 전기적동율동전환법과직접비교한연구는없지만약제에의한동율동전환은적용이간편한대신전환율이전기충격에비해낮다. 약제에의한전환법의가장중요한부작용은항부정맥제의독성이다. 약제에의한동율동전환은 AF 발생후 7일이내에시작할때가장효과적이며, 7일이상경과한지속성 AF는동율동전환율이급격히떨어진다. 또한동율동전환효율성은약제의용량과투여경로및투여속도에의해좌우된다. 발생 7일이내의 AF에효과적인약제들로는 dofetilide, flecainide, ibutilide, propafenone이 ACC/AHA 가이드라인권장등급 I 약제이며, amiodarone이 IIa, procainamide, quinidine이 IIb이다. 발생한지 7일이상된 AF의경우 dofetilide가등급 I, amiodarone 과 ibutilide가 IIa, flecainide, propafenone, Quinidine, procainamide 가 IIb이다. Digoxin과 sotalol 은효과가없으므로동율동전환목적으로투여하면안되는약제들이다. 약제를이용한동율동유지요법 AF는만성질환이기때문에동율동으로전환된환자라도대부분재발하게된다. 따라서동율동을유지하려면예방적인약물치료가필요하다. 약제의선택은환자의증상정도, 심질환의종류, 부정맥의부담정도, 합병증의위험정도, 환자의선호도등에따라결정된다. Digoxin은최근발생한 AF 의동율동전환이나재발의예방에효과가없다. 베타차단제는동율동유지에적은효과가있다. Propafenone, flecainide, sotalol은동율동유지에효과적이다. 완전히 AF의재발을막지못하는경우에도재발간의간격을연장시키는효과가있다. 하지만아직까지발작성 AF에서재발의횟수가감소할경우혈전색전증의빈도도감소하는지에대해서는데이터가없는실정이다. 또한무증상성으로 AF가나타나는경우도있으므로, 동율동유지약제를복용중이라하더라도여전히혈전색전증의위험은있다. 또한이들항부정맥제는심박수조절능력이있으므로 ( 특히 sotalol), 증상이있었던발작을무증상성으로전환시킬수있게된다. Amiodarone은약제의부작용으로인해일차약제로사용할경우주의가필요하다. 그러나심부전이있는환자에서는약제로얻을수있는이득이뚜렷하다. 유럽과미국의가이드라인은심비대환자에게 class IC 항부정제와 sotalol을권장하지않는다. 하지만캐나다가이드라인에서는증거수준이약하다고판단하여이를허용하고

Dronedarone의동율동유지효과가별로크지않고, 심박수조절효과가 ATHENA 연구에서의입원율감소에기여했을수있다는판단에서이다. 영구형심방세동환자를대상으로진행중이던 PALLAS 연구에서 dronedarone군의예후가문제되어 2011년중반에조기종료되었다 [6]. 이로인해조만간 dronedarone의사용에대한권고안도변경이불가피할것으로본다. 유럽과캐나다가이드라인은불안정한심부전환자가아닌경우 dronedarone을심박수조절의 2차약제로도권장하고있다. 각약제별동율동전환및유지에사용되는권장용량및가능한부작용은표 3과같다. 업스트림치료유럽가이드라인에서는선택적인환자군 ( 심부전, 고혈압,

5 - 오세일. 심방세동의약물치료 - 심혈관계수술후환자등 ) 에서안지오텐신전환효소억제제, 안지오텐신수용체차단제, 스타틴등을심방세동의 1차또는 2차예방을위한업스트림치료로권장하고있다. 하지만미국과캐나다가이드라인에서는이에대한특별한권고를하고있지않다. 이론적으로는설득력이있으나아직증거수준이권고안을만들기에부족하다는판단을갖고있기때문이다. 결론심방세동에서항부정맥제치료의방향항부정맥제를이용한부정맥치료는그효과가만족스럽지않고부작용발생의위험으로인해비록현재심방세동치료의주류를이루지만사용에주의를요한다. 또한최근눈부시게발전하는전극도자절제술의높은치료성적으로항부정맥제의입지는점차위축되는경향이다. 항부정맥제의표적은궁극적으로부정맥의발생기전에가장효과적으로작용할수있는이온통로, 감수체, 운반체 / 펌프등이다. 하지만심방세동은복잡한발생기전으로말미암아이러한논리적접근의적용이쉽지않다. 따라서항부정맥제의역할은제한적일수밖에없고, 단독으로이용되기보다는비약물적치료와병행하여이용될전망이다.
