College of Medicine As mammography has become more popular for the evaluation of breast symptoms and for the screening of asymptomatic women. the smaller lesions are being detected before they become palpable. therefore preoperative localization became necessary. This has led to the development of several methods for preoperative localization of nonp외p able lesions. Authors analyzed 50 cases with preoperative locallzations In 45 patients whlch had abnormal findings on film mammogram or ultrasonomammogram since October 1985 through March 1990 at Yongdong Severance Hospltal. College of Medlcine, Yonsei University. The results were as follows ; 1. Techniques of localization were spot method in 8 cases. conventional needle locallzation method in 38 cases and Kopan s needle locallzation method in 4 cases 2. The most common mammographic indlcation for locallzation was focal calcifications in 29 cases(58 %). which was followed by a newly developed mass in 25 cases(50 %) 3. Outcome of pathologically confirmed dlagnosis put into benign lesions in 37 cases(74 %) 없ld malignant leslons in 13 cases(26 %). 4. Index Words : Breast. biopsy Breast neoplasm. 0.3 Breast radiography. technology needle 4 5 ) 을이용하여병소에유구강선 ( h ooked wire) 1. 서 르 a 을꽂아두는방법을 4 예에서실시하였다. 전예에서의위치결정은 Aloka SSD 280 와 7.5MHz 1. 대상 1985 년 1 0월부터 년 3월까지연세대학교부속영통세브란스병원에서필름유방촬영술과초음파유방촬영술상이상소견을보이면서엄상적으로종괴가촉지되지않는여자환자 4 5 명 ( 50 회위치결정 ) 을대상으로하였으며연령은 26세에서 63세까지다양하였다. 저자들이시행한위치결정의적응증은아래와같이하였다. 필름유방촬영술상 2. 3 ) (1 ) 악성을시사하는석회화 (2) 경계가불분명하거나결절형의유방종괴 ( 3 ) 별모양의유방종괴 (4) 어느다른종괴보다의미있게큰경계가좋은종괴 (5 ) 비정상적형태나증가된음영분포를모인경우, 그리고초음파유방촬영술 상 (1 ) 촉지되지않는병소로악성종양이의심되는미세병소 ( 2) 유방에작은결절과이차적인유방암소견 ( 3) 양성인종괴소견을보여도유방암의발생가능성 이많은사람에게추적검사상없던종괴가새로발생시 (4) 국소병소는뚜렷하지않으나한군데모이는 탐촉자, 그리고 Acuson 1 28 과 5. 0MHz 탐촉자에물 주머니나 Sonopad 을이용하여초음파유도 (G uid ance) 로시행한후위치결정의성공여부는상하, 필름유방촬영술로최종적위치를확인하였다. 적출된조직은즉시 내외측 방사선유방표본촬영 (specimen m am m og r aphy ) 을하여처음의심했던병소가정확히 적출되었는지확인하고필요한경우정확한국소의병 리조직검사를위하여조직표본에다시위치결정을하 였다. Ill. 결 대상은 45 명의여자환자였으며 3 명에서양쪽유방 과다수위치결정의예를포함하여 50 회의위치결정을 대상으로하였다. 연령분포는 26 세에서 63 세로평균 47 세였다. 50 회위치결정중에서 8 예에서색소주입법 (Fig. 1). 38 예에서단순주사침법 (Fig.2 ), 그리고 4 관의확장이나상피세포의비후와더불어미세한석회 화모임으로후방에차폐음영이보이는경우로하였 다. 2. 방법필름유방촬영술은블란서제 CGR 회사의 Sonograph 500 T 를이용하여유방의상하, 내외측촬영을행하였으며초음파유방촬영은 Aloka SSD 280 과 7. 5MHz 탐촉자, 그리고 Acuson 12 8과 5. 0MHz 탐촉자에물주머니나 So n opad 을이용하여촬영하였다. 위치결정은 3 가지방법으로행하였는데첫째는의심되는병소에 Gentian Violet 혹은 Gentian Vio l e t에조영제를흔합하여주입하는색소주입법 (Spot Met h od) 을 8 예에서시행하였고, 둘째는 34 예에서의 심되는병소에 19G 주사칩을꽂은후에 Gentian Violet 을주입하는단순주사침법그리고셋째는 Kopan s Fig. 1. 4 - 大韓放射線醫學會誌 : 第 26 卷第 6 號 예에서 Kopan s needle 사용볍 ( Fig. 3 )( Fig. 4 ) 을실로분류해보았을때석회화를보인경우가 29 예 (58 시하였다 ( Tab l e 1 ). %) 그리고자라나는종괴가 25 예 ( 50 % ) 로가장많았 수술전진단은다양하였으며양성으로진단을내린 q (Table 2 ). 경우가 2 1 예, 악성이나악성암의증으로진단을내린수술후병리조직학적소견으로 37 예 ( 74 % ) 가양성 경우가 29 예였다. 유방촬영술상적웅증은크게 4 가지병변이었고 1 3 예 ( 26 %) 가악성병변으로관찰되었다. 악성병변증에서유관내선암 ( I n trad uctal carcinóma) 이 5 예였으며, 침윤성관선암 Onf il tra tin g ductal carcino m a ) 가 7 예이고전이 ( Me t astas i s ) 암이 1 예였다 (Table 3 ). 수술전후진단을비교해보았을때, 수술전진단이 양성인경우는모두양성으로나왔으며수술전진단이악성이나악성의증 29 예중에서악성은 13 예 ( 44 %) (Fig 2 ), 양성병소가 1 6 예였으나, 리소견중에유방암의전구단계로알려진 6 ) 양성으로나온병 비전형적 상피비후 (Atypical epithelial hyperplasia ) 가 3 예, 유 선증 ( Ad e nosi s) 이 4 예로수술전악성으로진단된예 에서악성혹은그전구단계로확진된경우는 20 예 ( 69 % ) 를보이고있다 (T ab l e 4 ). However multiple metastases should be nαled out (c) By way of Kopan's and conventional needle localizati on, multiple atypical epi야1eli외 hyperplasia shows in the right breast 되기전에촉지되지않는병소로발견되었다. 따라서 수술전위치결정의필요성이증가되며여러방법의발 전이있었다. 이제까지알려진방법 J. 7, 8 ) 에는크게유방외부에서 의위치결정법과유방내부에서의위치결정법으로분 류되며유방외부에서의위치결정법은피부에지워지 지않는잉크혹은방사선불투과성표적의사용이 으나촬영시압박상태의유방위치와생겸시누워있는 상태의유방위치의변화그리고처지거나아주큰유 방에서는이방법으로는정확한위치결정이어렵다. 유방내부의위치결정법은촉지되지않는병소가까이 혹은그안에표적을세우는방법으로단순하게주사 침을꽂고위치결정하는방볍과생체염료로위치결정 하는색소주입법 9, 10 ) 유구강선법 4, 5.!l ) 그리고최근 에는가역성말단굴곡상선법 ( C urve d end retractable wire ) 1 2- J4 ) 이대두되고있다. 유구강선을이용하는것이 있 병소위치결정후침의 이동을줄이는최근에소개된방법으로알려져였으나 주사침의가격이비싸고구하기어려우며외국여성의 지방유방과는달리우리나라여성의유방은크기가적

(c) Specimen mammographic localization for pathologic diagnosis and pathology confirmed fibrocystic disease. 기때문에저자들은쉽게구할수있는단순주사침을많이이용하였디. 이미알려진방법들에의하면, 위치결정을하는과정중에서정확힌위치를확인하기위하여여러번의상하, 내외측필름유방촬영술을시행하여야했다 7-14 ) 그러나이와같이여러번반복되는필름유방촬영술은과대한방사선에노출되며유방압박에의해주사 침의이동을초래하는단점이있다. 이에저자들은위치결정시초음파유도로이러한단점을피하고자하였다. 적출된조직은방사선유방표본촬영에의하여의심되는병소부위가정확하게적출되었는지확인하고 ( Fig 2) 펼요한경우에는병리조직검사를위한조직표본위치결정을더실행하였다 ( Fig. 4 )

Preoperative Diagnosis Versus Postoperative Diagnosis p 때 9M D 맹m p- 딴의떤뽑: 냉펀ι M & n 맹i de rηιbenign (21) Malignancy (29) 16* 13 7 cases; Atypical epithelial hyperplasia (3cases) Adenosis (4cases) Gallagher 와 Martin 1 5 ) 은조기병기 (early stage) 와 같은크기의병변에근거하여좋은예후를갖는유방 암을구별하여미세유방암 ( minim al breast cancer) 이 라는개념을소개했으며, 여기에는 0.5 c m 를넘지않 는침윤성혹은비침윤성암을포함하였다. 미세유방암혹은미세잠복유방암 ( minimal occult breast ca n cer) 은흔히촉지되지않는작은병소이고 주위에섬유화변화가적어서, 펼름유방촬영술이나 초음파유방촬영술상악성과양성의소견을함께갚기 때문에, 수술전병소의정확한위치결정을한후외과 뼈- 적으로접근해서적은부위를절제하여진단을얻는 것이필요하다. Rosenberg1 6 ) 퉁은 927 예의위치결정에서 270 예 (29 % ) 가악성으로나왔으며 Homer 7 ) 등에의해서는 100 예의위치결정에서 24 예 ( 24 %), 그리고 Schwartz l 7) 등은 1 89 예의위치결정중에서 52 예 ( 27.5 % ) 가악성 으로나왔다. 저자들은 50 회위치결정으로 13 예 ( 26 % ) 가악성으로확진되어서다른보고자들과거의일 치하는악성율을보였고더구나수술전진단이악성이 었던 29 예중에서 44 % 가병리조직학적으로악성암, 그리고 25 % 가악성전구단계로확인되었던점이의미 가있었다. 엄상적으로촉지되는종괴에서실시한위치결정은 과거의맹목적인유방사분절제술같은많은양의유방 조직절제를피하고, 신체적결손 1 을적게주면서정확 한병리조직학적진단을얻어조기에유방암을발견하 여예후를좋게하는데도움이된다하겠다. v. 결론 저자들은비촉지유방병소로수술전위치결정을실 시하고수술후병리조직학적진단을얻은 45 명 (50 회 위치결정 ) 에서다음과같은결과를얻을수있었다. 1. 위치결정방법으로는초음파유도하에 8 예에서 색소주입법, 38 예에서단순주사침법, 그리고 4예에서 Kopan s needl e 을이용하였으나경제성이나한국인 여성유방의특정때문에단순주사침법이효과적이었 다. 2 유방촬영술상새로생기거나자라나는종괴 25 예 ( 50 % ) 와악성의증을나타내는석회화 29 예 (58 %) 의경우에위치결정을시행하였다 예중에서 37 예 (7 4 % ) 는양성그리고 13 예 ( 26 % ) 는악성으로확진되었으며미세유방암의조기발견 에기여하였다. 4. 수술전진단이양성인경우는 100 % 에서양성으 로확진되었다. 그러나수술전진단이악성이나악성의증이었던경우에서병리조직학적으로악성암 44 % 그리고악성암의전구단계 25 % 로확진되어실제로는악성암이나악성암의발전가능한병리조직병변은 69 % 로서진단에도움을주었다고보겠다. 결론적으로비촉지성유방병소의수술전위치결정 은신체적결손을적게주면서정확한병리조직진단에
