1 대한방사선의학호 l 지 1997; 36: 우측후복막강구조의 CT 소견 : 우측신주위강혈종의파급에관한분석 l 서광원 검경락 이 혁 김영화 조원수 검일영 목적. 우측외상성신손상환자에서우측후복막강혈종의파급을분석하여그후복막강의구조와특정을알아보고자하였다. 대상및방법 : 저자들은우측신손상에의해우측신상부후복막강혈종이있는 13 명의복부 CT 를후향적으로조사하였다. 신장위치에서는요근에접한혈종의다른후복막강혈종과의관계를분석하였다. 신상부에서는신주위강혈종과횡격막에접하는혈종에대하여간내하대정맥및그하대정맥주위혈종과의관계를조사하였다. 결과 : 신문이하위치에서요근과접한혈종은 8예가관찰되었고그혈종은모두전신방신후연장혈종과연결되었다. 신상부에서간내하대정맥주위혈종은횡격막에접하는혈종과는연결되지않는반면, 신주위강혈종과 13 예모두에서연결되었다. 신상부의위부분에서내측의신주위강혈종을포함하는간내하대정맥주위혈종은평균열번째흉추체의상부삼분의일부위까지파급되었다. 결론 : 전신막은내측을제외하고신주위강을감싼다. 신상부의위부분에서는전신막의전후면이합쳐지고횡격막근막과결합하지만, 내측에서는각각간내하대정맥주위의결체조직과후내측횡격막에결합한다. 후복막강은질환발생시에병소의종류와위치및경중에따 라다 OJ= 한구조적변화와체액축적이동반되기때문에그구조 를정확히파악하는것이중요하다. 후복막강구조에있어서요근및횡격막에접핸혈종이신 주위캉아니면후신방 (posterior pararenal space) 혈종인지 에대한논란이있다 (1). 또한우측신상부후복막구조 (right suprarenal retroperitoneal str ucture) 는간의무장막공간 (bare space of liver) 과간내하대정맥 (intrahepatic in ferior vena cava) 이있어좌측보다복잡한데, 신상부에서전 후신막이서로합쳐져횡격막근막 (diaphragmatic fascia) 에 결합한다는설명이있다 (2). 그에반하여간의무장막공간과 신주위강이잠재적인교통을할수있으므로전신막은복막에 결합하여굴절 (reflection) 되고후복막은횡격막에부착한다 는보고도있다 (3). 이에저자들은외상성우측신손상시동반 된요근및횡격막에접하는혈종과신상부후복막강혈종의 CT 소견을분석하여우측후복막강구조를파악하고자하였다. 대상및방법 최근 6 년동안 (90 년 1 월 -95 년 12 월 ) 본원에서임상적흑 1 순천향대학교천안병원진단방사선과학교실이논문은 년 12 월 27 일접수하여 년 2 월 6 일에채택되었음. 은수술등으로우측신손상이확진된환자로서 CT상우측신손상에의한우측후복막강혈종이있는 3 1 예중, 우측신상부의후복막강에혈종이관찰된 13 예를대상으로하였다. 그중신적출 (nephrectomy) 이시행된 7 예모두가혈복강 (hemoperitoneum) 의소견이보였다. 연령범위는 1 세에서 55 세 ( 평균 : 23.8세 ) 이었고남녀비는 9:4로남자가많았다. 혈종의원인으로는교통사고가 7 예, 추락사고나념어침 (slip down) 이 5 예, 그리고물체에맞은경우가 l 예였다. 신손상의정도는 Fanney 등의분류에의하여 (4) 범주 (category) II 에해당하는신피수질열상이 5 예, 신장골절 (fracture) 2 예, 뇨관신우접합부 (ureteropelvic junction) 파열이 1 예였고, 범주 m 에해당하는신피막 (capsule) 이파열된분쇄 ( sha t tered) 신장이 5 예였다. CT 검사는 CT W -700 (Hitachi Co, Tokyo, Japan) 을사용하였으며, 1 3 명의환자에서조영증강전후를 10 mm 간격으로횡격막으로부터골반강까지 10mm 절편두께로촬영하였다. 분석방볍은신장위치에서는요근혹은횡격막에접하는혈종의특성과다른혈종과의연결이있는지를조사하였으며, 신상부에서는신주위강혈종과횡격막에접하는혈종의위치에따른변화와간내하대정맥주위의혈종과의연관성을파악했다. 또한신상부후복막강혈종의끝을척추체의위치로파악하고, 그혈종에의한간내하대정맥의전방이동은부선위치에서하대정맥후변의중심점에서수직으로우측횡격막

2 서광원오 1: 무측후복막강구조의 CT 각 (diaphragmatic c rur a) 의전면까지의거리로측정하였다. 결과총 13 예중 8예에서요근에접하는혈종이신문이하위치에서관찰되었으며, 그외측으로전신방의신후연장 (retrorenal extension of anterior pararenal space) 혈종과연결되었으며, 신주위강혈종보다저농도였으며, 후신방으로의파급은없거나경마한침상화 (spiculation) 형태였다 (Fig.1). 또한그중상 부요관파열에의한 l 예는요근과접하는요종 ( urinoma) 이 외측으로파급되어전신방의신후연장과연결되었다 (Fig. 2). 1 3 예전예에서횡격막에접히는혈종이신 -^J- 극 (upper pole) 위치에서관찰되었는데, 간내하대정맥주위혈종과구분이되 고신주위강혈종보다고농도흑은균질한혈종이었으며, 신 상녁의바로아래부분에서신주위강혈종과연결되였다 (Fig. 3C, Fig. 3D, Fig. 4D). 1 3 예전예에서신상부의신주위장혈종은비균질하고간과 접하며간내하대정맥주위의혈종과경계를구분할수없었으 A B Fig. 1. 3 대한빙사선의학회 AI 1997: 36: 나, 횡격막에접하는혈종은균질하고간내하대정맥주위의혈종과구분되였다 (Fig. 3B, Fig. 4B, Fig. 4C). 상부로진행하면서신주위강혈종의대부분이횡격막에접하는혈종보다먼저급격히사라졌는데, 그중후복막강혈종이심한 7예에서횡격막에접한혈종의전변에저농도의혈종이보였으나 (Fig. 3A) 후복막강혈종이경미한 6예에서는관찰되지않았다 (Fig. 4A). 신상부의간내하대정맥주위의혈종은다른혈종보다상부로지속되어평균열번째흉추체의상부상분의일부위까지파급되였으며, 주로간내하대정맥의후변에위치하여그대정맥을전방으로평균 O.98cm 이동시켰다 (Table 1). 이중 9 예는간내하대정맥의전변에도혈종이관찰되었다 (Fig. 3B, Fig. 4A). 전신방혈종은신싱극과신상부에서 13 예모두에서 Fig. 3. 5 대한밤사선의학호 IAI1997;36: 관찰할수없었다. 고찰신막은보통 1-3 mm 의두께를가지는데후신막이전신막보다더두껍다. 신막은교원성결체조직 (collagenous connec tive tissue) 으로구성되며, 이것에의해형성되는신주위강은신장, 상부요관, 부신, 신혈관, 그리고지방을포함한다 (1). 신막은또한교격막 (bridging septae) 으로형성된다발성격방 (mul ti -<:ompartmen ts) 으로구성된다 (5). 이러한신주위강에혈종이발생할경우그파급은신막의한계와형태에의해결정될것이다. 이연구의결과에서관찰된바와같이, 후복막강혈종중신장의위치에서후내측의요근혹은횡격막각과접하여그경계를불분명하게하는혈종이기존의몇몇연구에서후신방혈종으로분류되었다 (2, 6, 7). 또한 Mindel 등은후신막이신장과요근사이를통과하여하대정맥과복부대동맥의전면을넘는다고하여신주위강의교통을제시하였다 (8). 그러나다른연구들은이혈종이요근과접한다고후신방혈종으로만볼수없고신주위강혈종일수도있다고보고하였다 (9, 10, 11). Raptopoulos 등은신장의상하부및개언에따라변이가있으나요추방형근 (quadratus lumborum muscle) 혹은요근의전외측에후신막이부착한다고주장하였다 (1 2). 임등은, 신의 상하부에서는후신막의한계가요근의외측에있고 (1 3), 후신방은신상부에서는후내측으로존재하지않는다고하였다 (3). McClennan 등은요근과요추방형근에근막 (aponeuroses) 이없다고하였으므로 (1 4), 이요근에접한혈종은교격막과 요근의사이에있는신주위강혈종일것이다. 그러나이연구결과, 요근등에접하는혈종이나요종이신문이하위치에서전신방의신후연장혈종과연결되고, 후신방으로의파급은없거나경미하였으며, 신상극및신상부에서는횡격막에접하는혈종이신주위강혈종과명확히분리되어신주위강혈종을전방으로멀고있었다. 그러므로이요근과횡격막에접하는혈종은전신막의신후연장과후복근사이의혈종, 즉전신방의신후연장혈종으로생각된다. 결국후신막의두엽 (Iaminae) 중에서전 (anterior) 엽이후엽에비하여상당히내측으로진행하여요근과횡격막각을덮고하대정맥주위의결체조직에결합할것이다. 물론전엽의신후연장부분에대한명확한한계는알려져있지않고, 다만전신방의내측한계를규명하는데있어서후엽과상대되는전엽을부수적으로요근및요추방형근의외측에결합하는것으로추론했을뿐이다 (2, 12). 이러한이유는후신막을구성하는전엽이후엽에비해앓으므로 (1) 혈종의압력에쉽게파괴되어, 이연구의결과에서처럼신문이하의위치에서신주위강및후신방혈종과구분이힘들기때문일것이다. 그러나이연구결과처럼신상극위치에서전신방의신후연장혈종이잘보이는이유를설명하는데는, Condon 등이제시허듯신장의상부에서섬유성엽이많지않고신막이신하부에 비하여덜단단하므로 (1 5), 신주위강혈종이신후연장전신막 을뚫고전신방의신후연장에혈종을형성시켰을것이다. 또한신주위캉의후방에풍부한지방조직이있고누워있는상태에서혈종이후방으로더많이진행하기때문에 (1, 2), 전신방의신후연장에혈종이더많이파급될수도있을것이다. 그러나저자들이추가하고싶은원인은, 전신방보다전신방의신후연장에혈종의침입이용이한것은신장은직립이나호홉시하부이동을한다. 이때신을지지하고감싸는교격막은신막과결합하고 (5), 복강측의신막은같이이동할수있는신축력있는복막과결합할수있으므로 (3, 15), 신의이동에의해전측의전선방은변형이경미할것이다. 그러나신후연장전신막은함께이동할수없는비탄력적인요근혹은요추방형근과, 신문의위부분에서는후복벽에결합한횡격막각과접하므로, 신의이동에따라그사이에있는전신방의신후연장이심하게변형되어잠재적인공간이형성되므로혈종의침투가용이할것이다. 이연구의결과에서신상부에서전신방의신후연장혈종과신주위강혈종은상부로올라갈수록좁아지고사라지므로저자들은 Meyers 등이제시하는전신막의전면과후면은신상부의상부에서결함하여횡격막근막이된다 (2) 는것에동의한다. 그러나신주위강혈종의내측은간내하대정맥주위의혈종과경계를구분할수없고전신방의신후연장혈종은횡격막각에서끝나므로, 저자들은전신막의전면은내측으로는간내하대정맥주위의결체조직과결합하고후면은후내측의횡격막에결합할것으로생각한다. 이구조는호홉시나기립시의간과간내하대정맥의종축이동에탄력적으로작용할것이며, 신주위강혈종은간내하대정맥주위특히후방으로잘교통할것이다. Kneeland 등은하대정맥은신주위강에포함되므로신주위강혈종이간내하대정맥주변특히후방을통하여종격동부위까지도달할수있다고하였는데 (9), 이연구의 13예에서선주위강혈종이흉곽내종격동까지파급된것은복부 CT상으로는확인할수없였다. 이연구결과에서간의무장막공간의혈종은명확히보이지않았다. 다만 9예의간내하대정맥전변에있는혈종과신상부에서비균질한신주위강혈종이끝난후에형성된 7예의균질한신상부후복막강혈종은간의무장막공간혈종으로추정했다. 그러나 CT상간의무장막공간혈종과다른후복막강신상부혈종의구분이힘든것은, 이연구의결과처럼내측에서간내하대정맥이전방으로평균 0.98cm 이동하듯이원발성신주위강혈종의압력을더받는신주위강과신후측전신방혈종이전면의간내하대정맥과전신막및횡격막근을전면으로압박하여간의무장막공간혈종이두께를형성못하기때문일것이다. 무장막공간으로혈종이파급되는이유로혈종의압력과외상에의하여신막의손상이있어신주위강혈종이간의무장막공간으로누출 (diapedesis) 이된다는가능성이제시되었다 (16). Meyers 등도신막의전후면이신상부에서서로붙어횡격막근막이된후횡격막에부착하므로이신상부의신막은혈종을차단을할수없을만큼불완전한결합을하거나손상을받았을가능성을제시했다 (2). 그러나임등은전신막은간의하부관상인대주위에서후복막과부착하고후신막은횡격막에 부착하여간의무장막공간과전신방은교통할수없고대신신 %

6 서광원오 1: 무측후복막강구조의 CT 주위강혈종이교통할수있다고하였다 (3). 이연구결과에서 심한신상부후복막강혈종시에는무장막혈종이출현하고그 위치에서신주위강과그일부인간내하대정맥후방의혈종이 있으며, 후면으로는횡격막에접하는전신방의신후연장혈종 이있으므로 1 저자들은신상부에서신주위강혈종은불완전한 면을형성하는전신막과간내하대정맥주위의결체조직을, 전 신방의신후연장혈종은그위의불완전한변인횡격막근막을 통하여각각간의무장막공간으로파급할것으로생각된다. 더 구나전신방의염증혹은악성질환이간의무장막공간으로파 급할수있고 (1 7), 신상부에서신막의연약성과섬유성엽이 척다는 (15) 보고를참고하면전신막은신상부에서후복막과 부착할지라도굴절되지않고혈종의누출을허용하는불완전한 전신막이간의무장막공간의후변에존재할것으로추론한다. 이연구의한계는수술당시의임상기록에후복막강의구조 가세밀히기술되지않아서후향적인 CT 소견을확인할수없 다는것이다. 또한이미세한후복막강구조를수술시에육안적 으로확인하기에는한계가있을것이다. 그러므로시신의신주 위강에조영제를투여하여방사선학과해부학적으로연구하든 지, 후복막강혈종이있는환자를 MRI 로시상 (sagittal) 스 캔을하면더많은후복막강의구조를이해하는데도움이될것 이다. 결론적으로전신막은대혈관을포함하는내측을제외하고신 주위강을감싸며신상부의상부에서는전신막의전, 후면은횡 격막과결합하고횡격막근막이되지만내측에서는각각간내 하대정맥주위의결체조직과후내측의횡격막에결합한다 (Fig.5). 참고문헌 1. Materials and Methods: We retrospectively reviewed CT scans of 13 patients with right retroperitoneal hematomas caused by right renal injury. At the renallevel, we analyzed the relation of a hematoma contacting psoas muscle with other retroperitoneal compartmental hematomas. At the suprarenal level, a perirenal hematoma and a hematoma contacting the diaphragm were analyzed according to their relation with intrahepatic IVC and pericaval hematoma. Results: Below renal hilar level, all hematomas contacting psoas muscle, observed in eight cases, were connected with retrorenal extension of anterior pararenal hematoma. At the suprarenal level, intrahepatic pericaval hematomas were not, in all 13 cases, connected with a hematoma contacting the diaphragm, but with a perirenal hematoma. At the upper suprarenal level, the only pericaval hematomas containing a medial component of perirenal hematoma extended superiorly to the upper one third of the tenth thoracic vertebral body. Conclusion : The anterior renal fascia envelops perirenal space except in its medial aspect. In the upper suprarenal region, the anterior and posterior planes of the anterior renal fascia unite to fuse with diaphragmatic fascia, but along the medial aspect they fuse with intrahepatic pericaval connective tissue and posteromedial diaphragm, respectively.
