Methods: Fasting plasma D-dimer was measured in 290 consecutive patients (61.7% men, mean age 67.0±12.3 years) within 3 days after the onset of acute ischemic stroke. The outcomes were measured at 3-months after stroke onset, by the modified Rankin Scale (mrs). Results: Atrial fibrillation, hypertension, diabetes and involvement of the insular cortex, the levels of serum high sensitive C-reactive protein and D-dimer, as well as incidence of women and age, were all significantly higher in the poor outcome group (P<0.05). After the adjustment of National Institutes of Health Stroke Scale (NIHSS) scores on admission, the plasma D-dimer levels positively correlated with mrs (Spearman partial rho=0.247, P<0.001). The cut-off value of D-dimer level for prediction of poor outcomes was 0.35 mg/l (sensitivity 0.70, specificity 0.63, area under the curve 0.71). Furthermore, two multivariated logistic regression analysis models were performed. One model excluded the NIHSS score as an independent variable, and demonstrated that D-dimer (odds ratio, 1.24 for every 1.0 mg/l, P<0.05) was independently associated with poor functional outcome. Conversely, the other model which included the NIHSS score, did not show any such association. Conclusions: Plasma D-dimer level is a useful marker for short-term outcomes in acute ischemic stroke, and may have a role in risk stratification for predicting a poor outcome. 2 혹은영상학적예후를알기위한생체표지자에대한연구 들이증가하고있다. 염증성표지자나전염증성사이토카인 (proinflammatory cytokine) 이뇌졸중의초기신경학적증상 의악화와불량예후에관련이있는것으로보고되고있다. 3,4 혈장 D- 이합체는섬유소분해산물 (fibrin degradation product) 이며, 섬유소침착과안정및응고항진을나타내는 신뢰할수있고민감한표지자이다. 5 D- 이합체는생체외에서 도비교적안정된상태를유지하고반감기가길어서다른혈 전생성과관련된표지자들보다임상에서사용하기유용하 다. 6 심장성색전증에의한뇌경색에서다른기전에의한뇌경 색보다 D- 이합체가높다. 7 비판막성심방세동과관련된뇌경 색에서 D- 이합체가병변의크기와도관련이있었다. 8 혈장 D- 이합체와뇌졸중예후와의연구에서 D- 이합체가 불량예후의독립적인인자라는보고들과 9-12 그렇지않다는 보고들이 6,13-16 있다. 혈장 D- 이합체의급성허혈성뇌졸중예 후에대한연구는전반적으로많지않으며, 9,17 국내에서도허 혈성뇌졸중에서 D- 이합체에관한연구는부족한상태이 다. 12,18 본연구는급성허혈성뇌졸중환자에서혈장 D- 이합체가 단기예후에어떤영향을미치는지를살펴보고, 급성허혈성 뇌졸중의단기예후에대한예측인자로서의가치를알아보 고자하였다. 대상및방법 1. 대상본연구는 2011 년 3 월부터 2014 년 6 월까지한대학병원신 경과에입원하여신체검사와뇌영상을통해급성허혈성뇌 졸중으로진단된환자를대상으로하였다. 허혈성뇌졸중은 Trial of Org in Acute Stroke Treatment (TOAST) 에의해 분류하였다. 19 연구대상군의수집기간동안에내원한뇌졸 중환자는총 879 명이었다. 발병후 3 일이지난허혈성뇌졸 중, 혈장 D- 이합체가측정되지않은경우, 뇌출혈, 폐렴, 패혈 증등의열성질환, 암, 2 등급이상의판막성심질환, 만성신부 전으로투석중, 3 개월후추적관찰이되지않은경우는연구 대상에서제외하였다. 대상군은총 290 명이었다. 2. 방법연구대상에포함된환자들의임상적인자료는의무기록과 면담을통해얻었다. 대상환자의병력, 과거력, 뇌혈관질환에 대한위험인자들이수집되었고, 혈액검사, D- 이합체, 심전도, 심초음파, 컴퓨터단층촬영, 자기공명영상, 컴퓨터단층촬영 혈관조영술이나자기공명영상혈관조영술이시행되었다. 고 혈압은뇌졸중급성기를지나고안정된상태에서수축기혈압 140 mmhg 이상이거나, 이완기혈압 90 mmhg 이상이두번 이상있는경우로하였고, 현재고혈압약을복용중인경우 는혈압의여부에상관없이고혈압으로정의하였다. 당뇨병 치료를받았거나입원후공복혈당이 126 mg/dl 이상이거 나경구포도당내성검사에서 2 시간째혈당이 200 mg/dl 이 상인경우, 당화혈색소가 6.5 이상인경우를당뇨병으로하였 다. 고지혈증은공복시총콜레스테롤 240 mg/dl 이상혹은 저밀도지질단백 160 mg/dl 이상이거나고지혈증약을복용 중인경우로하였다. 혈장 D- 이합체검사는입원후다음날아침공복에시 행되었으며, 대부분의대상군에서내원 24 시간이내에검 사가시행되었다. D- 이합체는 STA-R Evolution 응고검사법 (Diagnostica Stago, Asnieres, France) 을이용한면역혼탁법 (immunoturbidometry) 으로측정하였다. 모든대상군에는뇌자기공명영상이시행되었고, 섬피질침 범은확산강조영상 (diffusion-weighted image) 을통하여침범 여부를확인하였다. 예후는발병 3 개월후 mrs (modified Rankin Scale) 로평가 하였다. 20 평가의척도인 mrs 는환자의전반적인기능적예후 를평가하는데유용하다. 평가점수는 0-5 단계로구성되며 6 은사망을의미한다. mrs 는일상생활의독립적수행능력과 타인도움의필요정도를기준으로전반적기능을평가한다. 좋은예후는 mrs 가 0-2 점인경우로하였고, 불량예후는 3 점 이상으로하였다. 3. 통계및분석연속형변수는평균 ± 표준편차로, 범주형변수는빈도와퍼 센트로표현했다. 좋은예후군과불량예후군간의여러변수 들의특성을알기위해 Chi-square, t-test, Fisher s exact test, Mann-Whitney U test 를이용하여분석하였다. 허혈성뇌졸중 의 TOAST 분류에따른혈장 D- 이합체의차이를보기위해일 원배치분산분석을하였다. 연속형변수는혈장 D- 이합체와 관련을보기위해상관분석과편상관분석을하였다. 좋은예 후군과불량예후군간의유의미한차이를보인변수들로예 후에미치는영향을확인하기위해로지스틱회귀분석을시행 하였고, 다른변수들을조정하기위해다중로지스틱회귀분 석을시행하여교차비 (odd ratio) 와 95% 신뢰구간 (confidence 20

D-dimer Predicts Short-term Outcome in Acute Ischemic Stroke interval) 을확인하였다. 혈장 D-이합체의결정치 (cut-off value) 는 ROC 곡선 (receiver operating characteristic curve) 을이용하여분석하였다. 통계는 SPSS Version 22.0 (SPSS Inc., Chicago, IL, USA) 을이용하였고, 유의수준은 0.05로하였다. 결과 1. 대상군의임상적특성급성허혈성뇌졸중환자는총 290 명이고, 평균연령은 67.0±12.3 세이다. 남성은 179 명 (61.7%) 이며, 여성은 111 명 (38.3%) 이다. 고혈압 77.6%, 당뇨병 38.6%, 고지혈증 29.0%, Table 1. TOAST, tial of org in acute stroke treatment; LAA, large-artery atherosclerosis; SVO, small-vessel occlusion; CE, cardioembolism; OD, other determined; UD, undetermined. 심방세동 19.0%, 관상동맥질환 9.0% 의순으로동반질환이있었고, 흡연은 47.1%, 음주는 39.4% 를차지했다. 자기공명확산강조영상으로확인된섬피질의침범은 49명 (16.9%) 이었다. 입원시의미국국립보건원뇌졸중척도 (National Institutes of Health Stroke Scale, NIHSS) 점수는 4.1±4.7점이었고, 3개월후 mrs는 1.8±1.7을보였다 (Table 1). mrs는 0점 17.9%, 1점 42.8%, 2점 16.6%, 3점 5,2%, 4점 6.6%, 5점 4.8%, 6점 6.2% 를차지했다. mrs 1점이가장높은빈도를보였고, 사망률은 6.2% 였다. 허혈성뇌졸중의 TOAST 분류에따르면, 큰동맥죽경화증 (large artery atherosclerosis) 64명 (22.1%), 작은동맥폐색 (small-vessel occlusion) 79명 (27.2%), 심장성색전증 (cardioembolism) 50명 (17.2%), 기타원인 (other determined) 3명 (1.0%), 원인불명 (undetermined) 94명 (32.4%) 이었다 (Table 2). 분류에따른혈장 D- 이합체의유의한차이는없 었다 (P =0.128, Fig. 1). D- 이합체는나이, 혈청 high sensitive C-reactive protein (hs-crp), 크레아티닌, 추정사구체여과율 (estimated glomerular filtration rate, egfr), 알부민, 저밀도지 질단백질과유의한상관관계가있었다 (P<0.05). 혈장 D- 이 합체와입원시 NIHSS 점수 (Spearman rho =0.165, P =0.006), 3 개월째 mrs (Spearman rho =0.239, P<0.001) 와약한양의 상관관계가있었다 (Fig. 2). D- 이합체와 3 개월째 mrs 는입 원시 NIHSS 점수를조정한후에도양의상관관계가있었다 (Spearman partial rho =0.247, P<0.001). 2. 좋은예후군과불량예후군의비교좋은예후군과불량예후군은각각 224 명과 66 명이었다. 좋은예후군은남성 (63.6%) 이많았고, 불량예후군은여성 22

(51.5%) 이많았다. 나이는불량예후군에서좋은예후군보다많았다. 고혈압, 당뇨병, 관상동맥질환, 울혈성심부전, 심방세동의병력에서는불량예후군에서유의하게빈도가높았다. 흡연과음주는오히려좋은예후군에서높게나타났다 (P<0.05). 내원시의혈압, 신체비만지수, 총콜레스테롤, 고밀도지질단백질, 저밀도지질단백질, 호모시스테인은두군에서차이는없었다. 심박수, 공복혈당, hs-crp, CK-MB (creatine kinase-mb), D-이합체, 섬피질침범은불량예후군에서높았고, 혈청알부민, 혈청중성지방, 추정사구체여과율은좋은예후군에서높았다 (P<0.01, Table 1). TOAST 분류에따라좋은예후군에서원인불명, 작은동맥폐색, 큰동맥죽경화증, 심장성색전증순의빈도를보였고, 불량예후군에서는심장성색전증, 큰동맥죽경화증, 원인불명, 작은동맥폐색순을보였다 (Table 2). D-이합체는좋은예후군에서 0.5±1.0 µg/ml이었고, 불량예후군에서 1.9±3.6 µg/ml로불량예후군에서유의하게높았다 (P<0.01). 입원시의평균 NIHSS 점수는좋은예후군은 2.3±2.3점이었고, 불량예후군은 10.0±5.9점이었다. 3개월후의평균 mrs는좋은예후군은 1.0±0.7점이었고, 불량예후군은 4.5±1.1점이었다 (Table 1). TOAST 분류에따라서좋은예후군과불량예후군에서 D-이합체의차이를비교해보았다. 큰동맥죽경화증과원인불명에서불량예후군은좋은예후군에비해각각유의하게높은 D-이합체수치를보였다 ( 각각 0.5±0.6 vs. 1.9±4.3, P<0.05, 0.7±1.6 vs. 2.3±2.5, P<0.01). 작은 Figure 3. ROC curve. Area under the curve (AUC) 는 0.71 이었다 (Fig. 3). 4. 불량예후에영향을미치는독립적변수불량예후에관련된인자들은성별, 나이, 고혈압, 당뇨병, 심방세동, 섬피질침범, 심박수, 추정사구체여과율, 혈청알부 민, hs-crp, CK-MB, D- 이합체, NIHSS 점수였다. 불량예후에 관련된인자들을독립변수로, 불량한예후를종속변수로하 였으며, NIHSS 점수를독립변수에포함하지않은모형과포함 한모형으로다중로지스틱회귀분석을하였다. NIHSS 점수를독립변수에포함하지않은모형에서불량예 후와독립적으로관련이있는혈청생체표지자는 D- 이합체 와 hs-crp 이었고, D- 이합체의교차비는 1.24 였다 (P<0.05). 예 후와관련된다른독립적임상변수는나이와당뇨병이고, 영 상학적변수는섬피질침범이었다. NIHSS 점수를포함한모형 에서불량예후와독립적으로관련이있는변수는나이, 낮은 혈청알부민과 NIHSS 점수이었고, D- 이합체와 hs-crp, 당뇨병 은포함되지않았다 (Table 3). 고찰 본연구는급성허혈성뇌졸중환자의단기예후를예측하 는데도움이되는표지자로서혈장 D- 이합체의유용성을알 아보고자하였다. D-이합체가급성허혈성뇌졸중환자의단기예후를예측할수있는인자인지확인하기위해두모형으로다중로지스틱회귀분석을하였다. NIHSS 점수를포함하지않은모형에서 D-이합체가독립적으로관련된변수로선정되었지만, 포함한모형에서는그렇지않았다. D-이합체가단기예후에직접적으로영향을주기보다는발병당시뇌졸중중증도와관련이있고, 이차적으로단기예후에영향을주었을가능성이있다. 하지만, 본연구에서 D-이합체는입원시 NIHSS 점수를조정 (adjust) 한후에도 3개월째 mrs와양의상관관계를보였다. 상기가능성에대해서는추가적인연구가필요할것으로생각된다. 다른연구들보다본연구의대상군은비교적임상상태가심하지않은환자들이많이분포되어있었다. NIHSS 점수가평균 4점정도로낮았고, mrs가 2점이하도 77% 를차지했다. 연구기간동안의뇌졸중환자중에추적관찰이이루어지지않은환자들이제외되었는데, 이중에장애정도가심한환자가제외되었을가능성이높다. 퇴원후요양병원이나재활병원으로전원되어외래추적관찰이되지않았거나, 장애가심하여외래방문을하지않았을가능성이높다. 따라서, 불량예후군의환자수가적었을것이다. 본연구는단일기관연구이고, 대상군에예후가좋았던환자들이상대적으로많이포함되어있어전체허혈성뇌졸중을모두대변한다고보기는어렵다. 혈장 D-이합체는일반적으로정맥의혈전색전증과폐동맥 24

D-이합체의증가는응고항진을의미하며, 결국불량한예후를보이게된다. 허혈성뇌졸중에서 D-이합체의증가는뇌혈관의진행하는혈전형성이나전신응고상태의항진을의미할수있으며, 22 증가된 D- 이합체가염증반응을촉발시킬수도있을것이다. 결국증가되고활성화된염증과응고가뇌졸중예후의악화에관여할것이다. 혈장 D-이합체측정은발병 3일이내에입원한사람을대상으로입원 24시간이내에대부분이루어졌고, 주로발병 24시간이내에측정되었다. 혈장 D-이합체의수치의변동은뇌졸중의아형에따라차이가있다라는연구결과도있으며, 뇌경색발병 7일까지는거의일정한수준을유지하는것으로알려져있다. 23 여러연구들에서는발병 1일, 3일, 7일이내의급성허혈성뇌졸중환자를대상으로연구들이주로진행이되었는데, D-이합체가 7일정도는일정하게유지되므로측정시간이연구결과에미치는영향은크지않을것이다. 이런특성은혈장 D-이합체의표지자로서의장점에해당될것이다. 하지만허혈성뇌졸중의초기악화를보이는경우도많기때문에악화를보이는경우에표지자값의변화가일어나지않을가능성이있는것은표지자로서단점에해당할수도있을것이다. 진행성뇌졸중이나재발성뇌졸중에서 D-이합체의연속적인변화에대한연구도향후관심있는연구주제가될수있을것이다. 허혈성뇌졸중의예후결정에관여한다고알려진인자는나이, 성별, 심부전, 뇌졸중병력, 뇌졸중중증도, 당뇨병이있다. 2,24 혈액검사로는 C반응성단백, 10,15 D-이합체, 9,25 섬유소원 (fibrinogen), 15 plasminogen activator inhibitor-1 16 등이알려져있다. 본연구도여성에서예후가좋지않았다. 심방세동의빈도는남성에서 16.2%, 여성에서 23.4% 를보였다. 심장기원의색전이여성에서더많이생겨, 큰뇌병변을초래하고예후에영향을미쳤을것이다. 그외에도여성에서보다많은위험인자, 고령, 근력저하가예후에영향을줄것이다. 고령, 심부전, 당뇨병이예후에나쁜영향을미쳤고, 이런결과는다른연구들과유사하였다. D-이합체는고령, 신경학적장애가심한경우, 허혈성뇌병변이큰경우, 심장색전과큰동맥죽경화증인경우, 불량예후인경우에증가한다고한다. 4,8,9,12,26 허혈성뇌졸중에서심장색전으로인한경우가가장높은 D-이합체치를보인다. 7 본연구에서 D-이합체는나이, NIHSS 점수, 혈청 hs-crp와크레아 티닌과상관관계가있었고, 불량예후군에서 D-이합체가높은결과를보였다. TOAST 분류에따른 D-이합체의차이는없어서다른연구결과와는달랐다. 아마도각아형의수가많지않고, 분류가불명확한군의수가많아서유의한차이가나지않은것으로여겨진다. D-이합체가예후와관련에대해서관련이있다는보고 4,8,9-12 와관련이분명하지않다는보고 6,13-16 가있다. 이전의연구에서 D-이합체와예후사이의관계가다른결과를보였던것은연구에따라서예후를판단하는기준으로본연구와같이 mrs를사용한경우, 8,13,27 재발, 12,13 심혈관계질환의발생, 13,14 사망, 15,16 NIHSS가호전되었는지여부 22 등으로다양하였다는점과관련있는것으로생각된다. mrs를예후의기준으로한연구에서는예후와 D-이합체가관련이있었다. 8,11,13,27 급성허혈성뇌졸중에서의연구는 D-이합체가급성뇌졸중의정도와단기예후와관련이있다고한다. 27 중국인을대상으로한연구에서도급성기의 D-이합체의증가는불량예후와관련이있었다. 22 아시아인종에서는두개내의혈관질환이많고, 백인에서는두개외의혈관인목동맥질환의빈도가높다. 본연구는한국인을대상으로한연구이며다른아시안계인중국연구들과비슷한결과를보였다. 한국인에서도 D-이합체가단기예후의예후인자임을알수있었다. 뇌졸중에서 D-이합체의임상적인가치는분명히있다. 뇌졸중유형간의차이가있고, 뇌졸중에서암과의관련이나정맥혈전이나폐색전과의관련도알려져있다. 본연구에서는큰동맥죽경화증과원인불명에서불량예후군은좋은예후군에비해각각유의하게높은 D-이합체를보였지만, 다른유형에서는좋은예후와불량예후군에서 D-이합체의유의한차이는없었다. 각군에서의대상자수가적어서한계는있지만, TOAST 분류각유형에서불량예후군에서 D-이합체평균치는좋은예후군보다높았다. D-이합체가과응고성을보이지않는것으로알려진군에서도불량예후군에서 D-이합체평균치가좋은예후군보다높게나온것은높은 D-이합체가뇌혈관내에서혈전형성을반영하였거나, 전신적인과응고성, 9 D-이합체의염증반응과관련된상승 28 으로인해불량한예후가발생하였을가능성등을고려할수있다. 본연구의제한점은다음과같다. 첫째, 대상을급성허혈성뇌졸중환자로한정하여정상대조군과의비교가없다는점이다. 둘째, 불량예후의환자수가좋은예후군보다많이적다는점이다. 셋째, 본원에방문한환자만을대상으로한단일기관연구여서전체허혈성뇌졸중환자를대변한다고보기에어려움이있다. 넷째, 급성허혈성뇌졸중에서심부정맥 25

8 혈전증을동반한경우를배제하기위한말초혈관초음파가이루어지지않아심부정맥혈전증에의한 D-이합체상승을배제할수없다. 추후에더많은대상자로전향적연구를하는것이필요할것이다. 본연구는급성기허혈성뇌졸중환자를대상으로예후를평가하는혈액표지자의역할에대한연구이다. 기존의여러질환에서혈장 D-이합체가가지는진단및예후인자로서역할을허혈성뇌졸중에적용하여 D-이합체가단기예후의예측에주요인자가될수있음을알수있었다. 결론 급성허혈성뇌졸중환자에서높은혈장 D-이합체는불량예후와관련이있었고, TOAST 분류에따른큰동맥경화증과원인불명에서높은 D-이합체는불량예후와관련이있었다. 혈장 D-이합체는급성허혈성뇌졸중의단기예후위험도예측에역할을할수있을것으로여겨지며, 전향적인연구를통해단기및장기예후에미치는연구를하는것이 D-이합체의예후인자로서의역할을이해하는데좀더도움이될것이다.
