Twenty-one(9 1.3%) of 23 patients with tuberculous spondylitis showed mixed osteolytic and osteosclerotic patterns of bony destruction. Six(66.7 %) of 9 patients with pyogenic infection and 1(7l.4%) of 14 malignant lesions showed osteolytic pattern of bony destruction. Thirty(9.9%) of 33 infectious lesions including pyogenic infection and tuberculous spondylitis involved intervertebral disc, while the involvement of intervertebral disc was not found in malignant lesions. The Swisscheese appearance of bony destruction was commonly seen in tuberculous spondylitis, but pyogenic infections and malignant lesions more commonly revealed geographic or moth- eaten appearance. The sequestral pattern and sclerotic rims in and around bony destruction were mainly seen in tuberculous spondylitis, and they were thought to be specific findings in tuberculous spondylitis. CT of the spine appears to offer the detailed findings of vertebral body destruction and may be a useful adjunct in differentiation between tuberculous spondylitis, pyogenic infections and malignant lesions of the spme. 뼈e x w o 띠g- s s s p m p m p m U 빼때m m 잃m m 료르서 대부분의척추체파괴를일으킨는질환은그증세가비 특이적이며이들질환의감별은치료의방향및질병의 예후를결정하는데중요한역할을하게된다 (1). 특히척 추체파괴를일으키는질환들중그진행이빠르고조기진단이치료의방향결정에큰역할을하는화농성척추염, 척추결핵및악성종양의감별진단은단순척추촬영으로도가능하나때로는그감별이어려울때가있다 ( 2, 3). 전산화단층촬영 ( CT ) 은이렇게척추체를파괴하는질환에있어척추의골파괴와인접구조물및연조직의변 * 본논문은 년인제연구장학재단의연구비보조에의한것임. 이논문은 1993 년 4 월 27 일접수하여 년 6월 3 일에채택되었음

2 대한방사선의학회지 1993; 29(5) : I 39~ I44 화를더자세히관찰할수있어척추질환의감별진단에 유용한방볍으로알려져왔으며 ( 4-8), 최근에는그장비 가더욱발달하고비교적널리보급되어사용되어지고있 다. 이에저자들은척추체의골파괴가의심되어 CT 를시 행한환자들의 CT 소견을분석하고이들질환사이의감 별점을알아보고자한다 년 1 월부터 1991 년 6 월까지 6 년간척추체병변이 의심되어 CT 를시행한환자들중척추체의골파괴가있 었던예들에서수술및병리검사로확진된 47 예를대상으 로하였으며이들질환중 4 예는수술로, 임상소견으로진단되었다. 대상및방법 7 예는병리및 퇴행성변화와외상에의한척 추골절및양성종양은대상에서제외하였다. 사용된기기는 TCT - 8 A 와 TCT 3S(Toshiba, ] apan) 이었으며전예에서조영증강이실시되었고이중 14 예에서척추강조영술이동시에실시되었다, CT 소견 은전예에서후향적분석을하였고단순척추촬영, 척수강 조영술및때로는자기공명영상들을참고하여분석하였다. 주요감별기준은척추체골밀도에따른파괴양상을골용해형 (osteolytic pattern), 골경화형 ( osteösclerotic pattern), 복합형으로, 골파괴의모양을 swiss-cheese 상, 지도상 ( geographic appearance), 좀먹은상 ( moth -eaten appearance) 으로나누고, 침범된척추수, 주위 골조직및연조직의변화에대하여분석하였다. 원인별질환은총 47 예중척추결핵 23 예, 염 9 예, 화농성척추 매독성척추염 1 예의염증성병변과 5 예의원발 성종양및 9 예의전이암이었다. 원발성종양은 Ewing 육종 3 예, 발병소가간암 3 예, 었던예가 5 예였다. 7" <= 골육종 1 예및럼프종 1 예이었으며, 전이암은원 폐암 l 예이었으며원발장소를알수없 임상증상은비특이적인요통과다양 한신경증상들이가장많이나타났으며화농성척추염의 경우발열을동반한예를볼수있었다. 병변의위치는 염증성인경우대부분흉추및요추부를침범하였으며, 원발성종양은경추를포함한전척추에서발생하였고, 전이성암은흉추를침범한경우가가장많았다 ( Table 과 Table 1. ti J1(1) u?ι 최석진외 : 척추체의골파괴를일으키는병소 1). 침범된척추의수는염증성병변에서화농성 1예를제외하고는모두 2개이상의척추를침범하였고, 특히결핵성인경우 3개이상이인접한척추체를침범한경우가 56% 에서나타났으나, 악성종양에서는하나의척추만을침범한경우가 86% 였다 ( Tab!e 2).

3 ti J1(1) u?ι 최석진외 : 척추체의골파괴를일으키는병소 1). 침범된척추의수는염증성병변에서화농성 1예를제외하고는모두 2개이상의척추를침범하였고, 특히결핵성인경우 3개이상이인접한척추체를침범한경우가 56% 에서나타났으나, 악성종양에서는하나의척추만을침범한경우가 86% 였다 ( Tab!e 2). 척추체골밀도에따른파괴양상은골용해형, 골경화형, 혼합형으로구분할때결핵성의 91% 가혼합형이었고, 화농성및악성종양에서는주로골용해형을보였다 ( Tab! e 3). 매독성척추염은결핵성과유사한골파괴를보였다. 대한방사선의학회지 1993 ; 29 (5) : 1 39~ 1 44 골파괴의모양은다발성골용해가있는동시에골편들이 흔합된부스러진연탄재모양을나타내는경우를 swiss - cheese 상 ( Fi g. 1), 하나의큰골용해형병변이주로있 을경우를지도상 ( Fig.2 ), 그리고골편없이다발성골용 해형병변을나타낼경우좀먹은상으로분류하였다 ( Fig. 3).

4 대한방사선의학회지 1993 ; 29 (5) : 1 39~ 1 44 골파괴의모양은다발성골용해가있는동시에골편들이 흔합된부스러진연탄재모양을나타내는경우를 swiss - cheese 상 ( Fi g. 1), 하나의큰골용해형병변이주로있 을경우를지도상 ( Fig.2 ), 그리고골편없이다발성골용 해형병변을나타낼경우좀먹은상으로분류하였다 ( Fig. 3). 이를기준으로살펴보면결핵성에서는 swiss- cheese 상이가장많이나타났으며화농성및악성종양에서는지 도상및좀먹은상이더많이보였다 ( Table 4). 그외골파괴와동반된소견으로부골형 (seq uestra l pattern) (Fig. 4a ) 과골용해성병변주위의경화성변연 (sclerotic rim) (Fig. 4b) 은결핵성에서주로나타났으며 화농성및악성종양에서는보이지않거나매우드문소견 이었 q (Table 5). 신경궁 ( neural arch ) 이나척추교 ( pedicle ) 의침범은염증성병변에서 33%, 악성종양에서 5% 로나타났다. 추간판의침범은대부분의염증성병변에서나타났으나악성종양에서는찾을수없었다 ( T ab le 6). 기타주위조직의농양및석회화는결핵성에서주로나타났으나농양처럼보였던 Ewing 육종의중앙괴사와골육종의석회화를제외하고는기타화농성병변이나악성종양에서는보이지않는소견이었다 (F ig.5 ) 병변이있는척추체를기준으로한주위연조직의종창정도는각질환에서다양하게나타났으며유의한차이는보이지않았다. 척수강으로의파급은염증및종양에서모두보였 q(table 7). Other CT Findings Abscess or cystic calcification Spinal canal lesion(%) or ossification(%) extension(%) Tuberculous (23) Syphilitic ( 1) Malignant tumors Primaη ( 5) 12(52.1) 15(65.2) 17(73.9) 9( 1) 1(1) 1 (2.) 1 (2.) 5( 1) 6(66.7) 고찰척추체의파괴를일으키는원인은외상이원인이되지않는경우퇴행성질환, 감염, 양성종양, 원발성및전이성악성종양으로구별할수있다. 이러한척추체의파괴를일으키는원인중감염에의한경우와악성종양에의한경우는질병의진행이빠르고, 척추감염의조기발견및적절한치료는완치까지가능하여이들질환의조기발견과감별진단은그치료및예후에결정적인역할을하는것으로알려져있다. 척추결핵은골결핵중가장높은빈도를차지하는것으로알려져있고, 전이성암의경우척추는 39% 까지보고된가장빈도가높은부분의하나로알려져있어 ( 2, 11 ) 척추체의파괴를일으키는이들질환의감별은영상진단에있어하나의목표라하겠다. 이들각질환의방사선학적소견은각영상진단방법에따라비교적잘알려져있으며, 특히척추결핵의경우는국내에서비교적많은예가보고되어왔다 ( 1,3, 4, 6, 9). 이들질환의감별은단순척추촬영만으로어느정도가능하나때로는감별이곤란한경우가있으며, 이들질환의진행정도, 골파괴의정도및감염과종양을구분하는데필요한이차적소견인주위조직의변화를정확하게파악하기 는어렵다 ( 2, 3). CT를이용할경우주위연조직의변화및척수강의침범여부등을비교적정확히파악할수있고 (4, 6-9, 12), 추간판의변화까지알수있다고알려져있으며 (5), 특히척추체의작은골파괴양상은비교적정확히파악할수있다 ( 6, 12). 이들각질환의방사선학적소견을살펴보면전형적인척추감염의 CT소견은척추주위연조직의종창, 농양형성과척추체파괴를보인다고알려져있으며 ( 6, 13), 척추결핵의경우 2개이상의척추체에연결형으로나타나는경우가 9% 내외로알려져있다 (4, 9 ). 저자들의경우척추결핵 23예전예에서다발성연결형으로나타났으며이 는 CT 가비교적조기척추결핵에서도인접한척추체의 미세한골조직변화를알수있기때문에나타난결과로 생각된다. 한편화농성감염의경우 89.9% 의연결형골 파괴를보여지금까지알려진보고와유사한소견을보였 다. 골밀도에따른파괴양상으로척추결핵의경우이등 ( 9) 은흔합형이 44.6% 로보고하였고, 일반적으로용해형 이많은것으로알려져있으나, 저자들의경우 91. 3% 에 서혼합형으로나타났고그중에서도연탄재가부스러진 모양의 sw i ss-cheese" 모양이 6.9% 로척추결핵에서 비교적특징적인방사선학적소견으로나타났으며, 반변 화농성감염의경우골경화성변화는비교적낮은비율로 나타났고, 용해성변화가 66.7% 로많은비율을차지하였 다. 이는척추결핵이만성적경과를취하는데반해화농 성척추엽은비교적급속히진행하는데따른반응성골변 화에서기인하는것으로생각되며, 이러한소견은척추체 주위에서나타나는연조직의종창이나농양의석회화와 더불어척추감염에있어화농성과결핵성의차이점으로 생각된다. 특히부골형성과경화성변연은대부분경우 에서결핵성에서나타나는특정을보여다른염증성질환 과악성종양과의감별에중요한소견으로생각된다. 또다 른중요한소견중의하나인추간판의파괴는과거알려진 바에의하변화농성감엽의경우단백질분해효소의생산 으로결핵성인경우보다그파괴가빨리일어난다고하였 으나 (4, 13) 저자들의경우그차이를찾아볼수없었으며 이는 CT 가가진횡단면영상이미세한추간판의병변을 찾는데불리한점으로생각되며자기공명영상과의합작이 정확한평가에필요할것으로생각된다. 그외소견으로 알려진척수강내로의연조직팽창과드물게보이는신경 궁의침범 ( 7 ) 은양질환의감별에큰의의를갖지못했다. 악성종양의방사선학적소견은큰연조직종괴와골피 질의파괴그리고전이암의경우다발성으로발생하는것 으로알려져있으며 ( 1 4 ), 척추교와신경궁의침범빈도가 높고 ( 2 ), 추간판의침범은거의없는것으로알려져있다 - 143

6 대한방사선의학회지 1993; 29(5) : I39~I44 (2, 3). 저자들의예에서악성종양은골파괴양상이주로 용해형으로나타났으며모양은지도상혹은좀먹은상이 대부분을차지하였고, 골육종 l 예 ( Fig.4) 에서부분적으 로 sw is s - cheese 형태를보였으나일반적으로알려진연 조직종괴는다양한형태를보였다. 추간판의침범은전 예에서볼수없었으며, 척추교와신경궁의침범이 5% 에 서보여지금까지알려진방사선학적소견과일치하였고 또한감염에의한척추골파괴와는차이를보였다. 저자 들의결과에서전이암이일반적으로알려진다발성으로 나타나지않는것은다발성척추전이가있을경우특별히 척추의 CT 를시행한예가적기때문인것으로생각되며 단발성으로생긴전이암은원발성악성종양과감별점을 찾을수없었다. 결론적으로척추전산화단층촬영에서척추체골파괴를 일으키는대표적질환인척추감염과악성종양의감별점을 정리하면, 척추감염은대부분추간판을침범하지만악성 종양에서는추간판이잘유지되었고, 척추감염은침범한 척추의수가대부분둘이상의다발성이며인접해있으나 악성종양은주로단발성으로나타났으며, 골밀도에따른 파괴양상은척추감염에서는흔합형이많으며그형태는 sw i ss-c heese 상이많았으나악성종양은주로용해형으로 나타났고, 부골상과경화성변연은척추감염에서만볼 수있었다. 점은, 척추감염중척추결핵과화농성척추염의감별 골밀도에따른파괴형태는결핵성의경우혼합형 이많으며주로 swiss-cheese 모양이특징적으로나타나 며화농성의경우용해형의좀먹은상을많이보였고, 골형성과경화성변연은화농성에서거의보이지않는결 핵성의특징적인소견이었으나추간판의파괴, 부 척수강및 척추주위연조직으로의파급은양질환에서큰차이를찾 을수없었다. 참고문헌 1989; 172: Sartoris D], Clopton P, Nemcek A, Dowd C, Resnick D.
