Case summary: We report a case of an isolated abscess in an extraocular muscle in a patient who was treated with systemic chemotherapy for precursor B lymphoblastic leukemia. A 54-year-old female who had undergone systemic chemotherapy for precursor B lymphoblastic leukemia presented with right ocular pain and limited eye movements. On ophthalmic examination, she had elevated intraocular pressure (IOP) and limited upward and downward gaze. MRI (magnetic resonance imaging) examination revealed an isolated abscess in right inferior rectus muscle. Although the patient was treated with empirical intravenous antibiotics and IOP-lowering agents, the size of the abscess increased, as confirmed by MRI findings. Therefore, we performed a pus drainage procedure by the transconjunctival approach. We were not able to find any residual abscess lesions on CT scans 3 months postoperatively. The patient s ocular pain disappeared and the limited eye movements improved significantly 6 months postoperatively. Conclusions: There have been no case reports of an isolated abscess in an extraocular muscle in Korea. For immunocompromised patients unresponsive to systemic empirical antibiotic treatment, an early pus drainage procedure by the transconjunctival approach may be a useful and effective therapeutic method in the management of an idiopathic isolated abscess in an extraocular muscle. (B) Brain M RI scan showed a large (6.5 mm 5.0 mm) cystic lesion (white arrow) with surrounding rim enhancement and irregular wall structure in the right inferior rectus muscle, suggesting an intramuscular abscess. 단 후 고용량 항생제 정주를 통해 효과적으로 농양을 제거 고 혈액내과에서 항암 화학 요법 치료 중이었다. 협진 의뢰 한 고립형 근농양 1예를 보고하였다. 아직까지 국내에서는 당시 나안시력은 우안 0.5, 좌안 1.0 이었고 교정시력은 우 고립형 근농양에 대해 보고된 바가 없다. 이에 저자는 면역 안 0.7, 좌안 1.0 이었다. 안압은 우안 28 mmhg, 좌안 20 억제 치료를 받고 있는 환자에서 안와동통 및 안구운동장 mmhg였다. 제일안위 원거리에서 4프리즘디옵터의 외사시 애를 동반한 고립형 근농양 1예로 경결막 농양 흡인 제거술 가 관찰되었다. 외안근 기능검사에서 상방 주시시 경도의 로 호전을 보인 증례를 경험하였기에 문헌고찰과 함께 보 운동제한과 하방 주시시 뚜렷한 운동제한이 관찰되었으며 (Fig. 1A), 상방 및 하방 주시시 복시를 호소하였다. 외안부 고하는 바이다. 검사에서 우안 안검에 경도의 부종을 보였으나 안구돌출계 증례보고 검사상 안구돌출은 없었다. 세극등현미경 검사에서 우안 하 결막 부종과 군날개 소견을 보였고, 시신경유두 촬영을 포 54세 여자 환자가 약 4일간의 우안 안구동통 및 상전, 하 함한 안저검사에서 특이소견은 발견되지 않았다. 색각검사 전 제한의 안구운동장애를 주소로 안과에 협진 의뢰되었다. 와 시야검사에서도 특이소견은 나타나지 않았다. 뇌자기공 과거력상 1개월 전 전구 B림프모세포 백혈병으로 진단 받 명영상에서 우안의 안와연부조직의 미만성 부종을 동반한 617

(B) Brain M RI scans showed a larger (9.0 mm 8.0 mm) cystic lesion (white arrow) in the right inferior rectus muscle, suggesting that the size of abscess pocket did not decrease but rather became larger despite systemic empirical antibiotics treatment. 우안 하직근의 농양이 mm 크기로 관찰되었다(Fig. 관찰되지 않았으며(Fig. 2A) 복시는 호소하지 않았다. 반면 1B). 우안 하직근 근농양 진단 하에 농양 흡인술이 필요한 상 뇌자기공명영상에서 우안 하직근 내에 보이던 농양의 크기 황이나 백혈구, 혈소판 수치가 낮아 전신적 항생제 치료 가 mm로 증가되었으며 주위 조직과의 경계가 뚜 (meropenem 1.0 g을 하루에 2회 정맥주사)를 유지하기로 하 렷해지는 소견이 확인되었다(Fig. 2B). 치료 1개월째 전신 였다. 또한 안압 조절을 위해 timolol (Timoptic-XE, MSD, USA), dorzolamide hydrochloride (Trusopt, MSD, USA), la tanoprost (Xalatan, Pfizer, Belgium), brimonidine tartrate (Alphagan-P, Allergan, USA)를 점안하였다. 치료 2주째 나 항생제 치료에도 불구하고 농양의 크기가 감소되지 않고 증상 호전 가능성이 없다고 판단되어 전신마취 하에 결막 경유절개술을 통하여 우안 하직근 농양의 흡인제거술을 시 행하였다. 제거술 시행 후 전신적 항생제 치료(meropenem 안시력은 우안 0.8, 좌안 1.0 이었으며 안압은 우안 15 mmhg, 1.0 g을 하루에 2회 정맥주사)와 스테로이드 정주를 하며 경 좌안 14 mmhg였다. 외안근 기능검사에서 상방 주시시에만 과 관찰하였다. 농양 흡인액에 대한 배양검사에서는 균이 경도의 안구운동제한이 관찰되었고 하방 주시 운동제한은 동정되지 않았다. 술 후 1일째 안구돌출계검사에서 우안이 618

6 - 박종혁 전루민 : 외안근에발생한고립형근농양 - 연조직염과골막하농양을감별하는데는전산화단층촬영보다우수하다는보고도있다. 8,9 안와주위염증의치료는 Chandler의분류로농양전단계인 1, 2군에서는전신적인항생제치료를 3, 4군에서는수술적치료를우선으로하고있다. 보통결막부종이나안구운동장애등의증상을보이면심한안와사이막앞연조직염을의미하고, 시력저하는절대적으로수술적치료를해야하는가장중요한지표로알려졌고, 이밖에도만져지는유동성종괴가있거나전산화단층촬영에서골막하농양이나안와농양이보이는경우, 항생제치료에도열이떨어지지않거나, 안구돌출의지속적인증가, 이마굴및두개내합병증이의심되면수술적배농을해야한다. 10 하지만골막하농양과안와농양에서항생제만으로합병증없이치료했다는보고가있어시력이나동공반사, 색각이모두정상이라면수술적치료는필요없다는주장도있다. 11,12 본증례에서와같이체내에다른감염의증거나일차병소가확인되지않고감염의전신증상또는징후가나타나지않는상황에서외안근에발생한고립형근농양의경우는감별진단이반드시필요하다. 외안근에발생한낭성병변은극히드물고, 그원인으로는낭충증 13 또는포충낭포 14 의안와침입, 망막박리수술이후에발생한봉입체낭포, 15 그리고유피포낭 16 등이보고되어있으며이들은모두수술적제거로치료된경우이다. 그러나본증례에서와같이외안근의고립형농양은다른일차성병변없이도자연발생적으로형성될수있다. 그리고약간이라도근농양이의심되는상황이라면낭성공동에대해흡인술을시행하여정확한진단을내리고합병증이발생하는것을예방할수있다. 낭성병변이기생충에의한병변의가능성이있어염증반응을줄이기위해스테로이드가사용될수도있음에도흡인술은낭종성분에대한정확한정보를제공해줄수있다는점에서필요하다. 본증례는전구 B림프모세포백혈병진단하에항암화학치료를받고있는면역억제상태의환자에서단안안구동통과안구운동장애를호소하여전산화단층촬영및자기공명영상을시행한결과외안근내부에발생한고립형근농양으로진단할수있었고, 일차적으로혈액학적지수저하및면역억제상태임을고려하여수술적치료보다는전신항생제치료를시행하였으나, 농양의크기가증가하고증상이호전되지않아농양흡인제거술을시행하였다. 이를통해안구운동장애도크게호전되었으며술후 3개월째시행한전산화단층촬영에서도농양의소견은거의보이지않았다. 본증례와같이면역억제상태에서특별한원인없이발병한외안근의고립형근농양의경우, 전신항생제치료에반응이약하거나악화되는경우농양흡인제거술을시행한다면증상의더욱빠른호전과병변의효과적인제거를기대할수있겠다. 7 - 대한안과학회지 2014 년제 55 권제 4 호 - = 국문초록 = 외안근에발생한고립형근농양 1 예 목적 : 전구 B 림프모세포백혈병환자에서발생한외안근의고립형근농양에대하여항생제치료및흡인제거술시행후추적관찰한 1 예를보고하고자한다. 증례요약 : 전구 B 림프모세포백혈병으로진단받고전신항암치료를시행받은 54 세여자환자가우안안구동통및안구운동장애를호소하여안과로협진의뢰되었다. 진찰소견상안구운동의상전, 하전제한및안압상승이관찰되었고, 뇌자기공명영상결과우안하직근의근농양이발견되었다. 경험적항생제정주및안압조절후추적관찰한뇌자기공명영상결과농양의크기가증가되어경결막흡인제거술을시행하였다. 흡인제거술후 3 개월째전산화단층촬영에서농양의소견이거의관찰되지않았으며안구운동제한도현격하게감소하여추후경과관찰하기로하였다. 결론 : 성인에서발생한외안근의고립형근농양은현재까지국내에보고된바가없었으며, 면역억제환자에서경험적전신항생제치료에악화되거나반응이약한경우경결막농양흡인제거술로적절히치료할수있다고생각된다. < 대한안과학회지 2014;55(4): > 622
