EDS can result from multiple causes such as sleep deprivation, sleep fragmentation, neurological, psychiatric and circadian rhythm disorders. Treating the underlying causes of EDS remains the mainstay of therapy but in those who continue to be excessively sleepy, further treatment may be warranted. Traditionally, the amphetamine derivatives, methylphenidate and pemoline(collectively sympathomimetic) psychostimulants were the commonest form of therapy for EDS, particularly in conditions such as narcolepsy. More recently, the advent of modafinil has broadened the range of therapeutic options. Modafinil has a safer side effect profile and as a result, interest in this drug for the management of EDS in other disorders, as well as narcolepsy, has increased considerably. There is a growing school of thought that modafinil may have a role to play in other indications such as obstructive sleep apnea/hypopnea syndrome already treated by nasal continuous positive airway pressure but persisting EDS, shift work sleep disorders, neurological causes of sleepiness, and healthy adults performing sustained operations, particularly those in the military. However, until adequately powered randomized-controlled trials confirm long-term efficacy and safety, the recommendation of wakefulness promoters in healthy adults cannot be justified. Key Words : Excessive daytime sleepiness, Psychostimulants, Antidepressant 최근비공식적인국내연구결과에서성인 10명중한명은심한주간졸림을호소하는것으로확인되었다. 그러나현실적으로많은이가주간졸림을병으로생각하지않고있어많은병적주간졸림환자들이무방비상태로지내고있다. 낮에참을수없는졸음이쏟아져서사고를낸적이있거나야간수면을늘려자도주간졸림의호전이없고짧은낮잠으로도주간졸림이크게호전된다면기면증을의심해봐야한다. 기면증은주간졸리움, 탈력발작, 가위눌림, 입면시환 * Address of correspondence Jinkyu Han, M.D. Department of Neurology, College of Medicine, Korea University Anamdong 5 ga, Sungbuk gu, Seoul, Korea Tel: , Fax: dystonia@hanmail.net 각, 수면곤란등의증상을특징으로하는질환으로전체인구의 0.09% 정도의유병률을가지고있다. 1 통계에의하면국내에도 40만명정도의기면증환자가있을것으로추정되나현재병원을찾아와서치료받고있는환자수는만명이내로대부분의기면증환자가치료를받고있지않음을알수있다. 기면증이외에다양한야간수면장애들이주간졸림의원인이되고심한주간졸림은원인을분석한후치료를받으면일상생활에큰문제가없을정도로호전이될수있는질환이므로신경과의사들이관심이필요하다. 많은향정신성약물이수면장애의치료나혹은다양하게수면을변화시킬목적으로사용되고있다. 수면약물학은임상실제적인면에서보면약물의수면에대한작용양상을이해함으로써합리적인처방을할수있게하며, 이론과연구의측면에서보면수면의생리적기전을이해하는데있어필수적인분야이다. 본장은주간졸림에관여되는약물치료를중심으로 Vol.1, No. 2 / December,

2 한진규 살펴보기로한다. 1. 기면증(narcolepsy) 치료 1) 행동치료 규칙적인수면-각성주기를유지하고수면위생을철저히지키게환자에게교육하며, 가족의이해와협조를유도해야한다. 미리가장졸린시간대를정하고 10~20분정도낮잠을취하는방법이다. 이렇게낮잠을자고나면 90~120분정도는개운한상태가유지되므로주간활동을유지하는데도움이된다. 2 또한규칙적인취침시간과기상시간을지키고숙면을위해수면에방해가될수있는알코올복용이나야간운동등은피하는것이좋다. 고탄수화물식사는졸림을악화시키므로운전을앞두고있거나하는경우에는주의가요구되고만성질환에준하는치료가필요하므로환자와가족, 그리고고용자들의병에대한이해와환자에대한배려가필요하다. 또한환자로하여금지지그룹과유대를형성하고, 기면병환자모임에참여하도록돕는것이매우효과적인방법이다. 2) 약물치료 크게세가지측면에서치료가이루어지는데첫째, 중추신경흥분제를이용한주간과다수면의치료, 둘째는탈력발작, 수면마비, 입면환각등의REM 수면관련증상에대한치료이며, 마지막으로불량한야간수면에대한조절이다. 주간과다수면의치료 교감신경계를흥분시키는효과를보이고 amphetamine, methylphenidate, pemoline등이대표적인약물이다. 약물기전은신경접합부에서 dopamine, noradrenaline, 그리고 seratonie 등의분비증가와재흡수(reuptake) 을억제시킴으로써 mono-aminergic 전달을증강시킨다. 3 Sympathomimetic drug 치료이후기면증환자의 65~85% 에서주간졸림의호전을보였고약물에대한내성은사용양과기간그리고병합치료제의종류에따라다양하게관찰되었다. 임신시태아에끼치는영향은확실히규명되지는않았지만가임기간동안에는되도록적은용량의약물치료를권장하고있다. 4 1 Amphetamine 1927 년처음만들어진후1935 년부터 Prinzmetal 과Bloom- berg가처음으로기면증치료제로서사용하였다. 3 D와 L isomer 중D isomer 인dexamphetamine이좀더효과적이고현재미국에서두번째로많이기면증환자를위해처방되고있다. 하루에 60mg까지사용이가능하며짧은반감기때문에 (3~5 시간) 하루에 3~4 차례처방이필요하다. 사용자의10% 에서교감신경증가에따른흥분된행동, 심계항진, 불면증, 신경과민등이발생될수있다. 4 하루 50~60mg 이상용량은야간수면에방해를줄수있고정상혈압을보인대상군에서약물복용이후혈압상승의부작용은관찰되지않았다. 4 2 Pemoline 과거가장많이사용되던약물로, 반감기가 24시간정도로길며, 발현시간이다른약물보다늦고, 치료의순응도가높은장점이있으나, 간독성이드물게알려져현재는다른약물에반응이없는경우선택적으로사용할수있는약물로분류된다. 3 3 Methylphenidate 1950년 Daly와 Yoss가기면증치료를위해처음으로사용한이후현재미국에서기면증환자의주간졸림치료제로서가장널리처방되고있는약물이다. 5 반감기가 3~4시간으로발현시간이빠르며, 부작용이나내성의발생이다른 amphetamine 계열약물보다적다고알려졌다. 5, 10, 20mg 로하루60mg 까지사용할수있고하루최대 60~80mg까지사용시기면증환자의84% 에서만족할만한결과를나타냈다. 5 야간수면에끼치는영향은 REM 잠복기를증가시키고, 부작용으로는신경과민, 두통, 불면증, 식욕부진, 심계항진등의교감신경항진증상이나타날수있다. 5 비 비sympathomimetic 약물로는 caffeine과 modafinil 등이대표적인약물이다. 1 Caffeine 자연알카로아드계로서각성효과를위해가장널리이용되고있다. 6 수면을유도시키는아데노신수용기를억제시킴으로써각성을증진시킨다. 인스턴트커피한컵에 40~105mg의 caffeine이포함되어있고디카페인커피에는 1~4mg의 caffeine 이포함되어있다. 반감기가 3~12시간이므로오후늦게복용은불면증을유발시킬수있다 수면

3 기면증의약물치료 2 Modafinil 대표적인비sympathomimetic 약물로서반감기가 15시간이상으로하루에한번복용하면되므로기존의약물보다복용하기간편하다.8 90% 이상이간으로대사되고복용 2~4 시간이후에혈장농도가최대가된다. 간이나쁜환자들은통상사용량의 1/2 정도사용을권장한다. 작용기전은정확하지않지만많은동물실험에서대뇌를전반적으로흥분시키는다른각성제들과는달리시상하부근처에서국한적으로작용이된다고보고하고있다. 9 그외글루타민 (excitatory glutamine) 을증가시키고국소적으로 GABA의전달을억제시켜각성효과를증진시킨다는보고도있다. 10 이런시상하부를통한각성은기존의교감신경항진을동반한각성제보다심계항진과같은부작용이적게나타나는것으로알려져있다. 약을끊은뒤에반동적으로발생되는졸림도적은것으로되어있고야간수면구조에영향을주지않는점도다른각성제와다른점이라할수있다. 8 발생되는비렘수면약물상호관계에있어서는간의 cytochrom P450에관여하여 CYP 3A4, CYP1A2, CYP2B6 등의효소를유도시켜이러한효소들에의해대사되는약물( 경구피임약, cyclosporine A, theophylline) 들의혈중농도가감소될수있고 CYP2C19, CYP 2C9등의효소들은저해시켜이러한효소들에의해대사되는약물 (diazepam, propranol, phenytoin, wafarin) 들의혈중농도는증가될수있다. 11 그리고 sympathomimetic 각성제인 methylphenidate, dexamphetamine 등과같이복용시에는별다른부작용은없으나탈력발작치료제인 tricyclic antidepressants(tca) 제재인 clomipramine 과같이사용시에는 72% 에서부작용이보고있었다 주장기간사용하였을때도 9주간사용할시와비슷하게두통이가장흔한부작용이었고그외위장관장애, 어지럼증등이보고되었다. 12 고혈압환자및심근경색과거력이있거나신장및정신, 간장질환환자에게는부작용이아직까지규명되지않아사용시주의를요한다. 12 기면증환자에서 200mg~400mg 오전한번복용으로주간졸림증상이위약과비교했을시3주뒤부터의미있게개선됨을보여주었고 13 그외충분한기간동안지속적상기도양압술을사용해도주간졸림을호소하는수면무호흡환자 14 와야간교대근무환자들의졸림을개선시키는데효과가있다고보고되고있다. 15 탈력발작치료탈력발작이란감정변화에따른양측성근력저하를가리키며이는하행신경주행의부적절한자극에의해서발생된다. 16 이증상은기면증환자에서특징적으로관찰되기때문에탈력발작과참을수없는과도한주간졸림증상만으로도기 면증진단이가능하다. 16 렘수면중에발생되는근력저하가감정의자극에의해시간장소의구분없이발생되는현상을탈력발작의기본병태생리로설명하고렘수면을유발시키는뇌교의 LDT(laterodosal tegmentum) 세포와 PPT(Pedunculopontine nuclei) 세포의활성과함께렘수면을억제시키는LC (locus celluleous), Raphe 세포의활성감소가척수운동신경의과분극(hyperpolarization) 을일으켜운동저하를일으킨다고설명한다 삼환계항우울제(Tricyclic antidepressants, 이하 TCA) 작용기전을보면이들모두 norepineprine( 이하 NE) 과 serotonin( 이하 5-HT) 의합성과대사를감소시켜 noradrenergic, serotonergic 활동을증가시키고항콜린, 알파억제, 항히스타민양상을보인다. 18 중추β-adrenoreceptor의수를감소시키는것외에여러전두엽부위에서 serotonin 수용체의수와기능을변화시키고 18 전기생리학적연구에서 TCA가 noradrenergic 그리고 serotonergic 신경전달시스템사이에기능적인연결을이룬다는증거도있다. 19 생화학적효과의측면에서 TCA가 selective serotonin reuptake inhibitor( 이하 SSRI) 와구별되는점은전반적으로 NE 재흡수를억제하는능력과다양한정도로 5-HT 재흡수를억제하는성질이며이는TCA가 SSRI보다신경전달시스템에미치는영향에서훨씬 넓은 spectrum 을가졌다고말할수있다. 20 부작용으로는두통, 위장장애, 성기능저하, 입마름, 변비, 어지럼 21 증, 불면증, 손떨림, 졸음, 저혈압이보일수있고 TCA의수면구조에대한영향은렘수면을억제시키고제 2단계수면을증가시키며서파수면에대한영향은아직불분명하다. 1960년에탈력발작의치료제로서 TCA 중 imipramine이처음사용된이후 22 이중맹검연구를통한자료는없지만탈력발작, 수면마비, 입면시환각등에효용성이입증되었다 Clomipra mine은보다강력한 serotonin 재흡수억제재이며상대적으로약한 norepinephrine 재흡수억제재로서강력한렘수면억제를보인다. Clomipramine은유럽에서탈력발작치료제로가장많이처방되고있고 26 modafinil과동시사용시에는 clomipramine 혈중농도가증가되므로유의해서사용해야한다 ~74mg 사이의용량이가장많 28 이처방되고사용 2~4 주부터는내성이생길수있다. Imipramine, clomipramine 같이졸음이유발될수있는약물은야간에처방을권장하고 Protryptiline 같이각성을유발시키는약물은오전에처방을권장한다. Vol.1, No. 2 / December,

4 한진규 2 선택적세로토닌재흡수억제제 (Selective Serotonin Reuptake Inhibitor) 대부분의 SSRI들은 histaminergic, adrenergic에대해억제활성이아주작은범위이나 sertraline은예외적으로대략 imipramine 의 1/4가량으로 histaminergic, adrenergic 에대해억제활성이있다. 29 Muscarinec 수용체에대한SSRI의활성은 fluoxetine, fluvoxamine, citalopram, 그리고 sertraline 에서는무시할정도이나, paroxetine는항콜린성작용을나타날수있다. 29 SSRI 약물은기존의 TCA약물에서보이는두통, 위장장애, 성기능저하, 입마름, 변비, 어지럼증, 불면증, 손떨림등의부작용들이훨씬적은것으로보고되고있다. 30 SSRI의수면에대한작용을보면 fluoxetine 은동물모델에서 5~40mg/kg의용량에서 REM 수면의감소, 33 NREM 수면의증가를보였고 citalopram과 fluvoxamine 또한실험실동물모델에서 REM 수면을감소시킨다고보고되었다. 33 외상후기면증환자에서fluoxetine 20mg을매일사용한후시행한수면다원검사에서도렘수면의저하, 서파수면의증가를포함한야간수면의증가가관찰되었다. 33 SSRI 의탈력발작의효과는 clomipramine 과비교한실험에서 clomipramine 을 25~150mg사용한환자 11명을 fluoxetine 60mg으로바꾼결과 4명에서증상완화를보였고 5명에서별다른변화가없었으며 2 명에서악화되는소견을보였다. 34 4명의기면증환자에게 fluoxetine 20mg을사용한결과 35 92% 에서탈력발작의감소를보였으며 TCA와 SSRI에반응이없던 3명의기면증환자에게 citalopram투여 8~10개월후탈력발작의저하가관찰되었다는보고도있다. 36 사용시주의점으로는당뇨, 간장질환신장, 질환간질, 환자에게는주의를기울여야하고특히 selegiline 같은 MAO 억제재나 TCA 복용환자에서같이사용시serotonin 신드롬발생에주의해야한다 선택적세로토닌-노르에피네프린재흡수억제재 (Selective serotonin-ne reuptake inhibitor) 선택적세로토닌-노르에피네프린재흡수억제재는 SSRI 와는다르게 5-HT를막는효과는 NE의재흡수영향에의해보충이된다. 이는한번투여로 β-adrenergic와결합된 camp 의 down-regulation을촉진하는유일한항우울제이다. 탈력발작에관여하는 seratonin과 norepineprine을동시에증진시켜주므로탈력발작에큰효과가있다고보고되고있고 venlafaxine 은최근미국에서탈력발작치료제로가장많이처방되는약물중에하나이다. 38 어떠한약물로도탈력발작조절이어려운 4명의기면증환자에게 venlafaxine을처방한결과 3 명에서효과를보았다는보고가있다. 38 그외새 로운약물인 viloxazine, reboxetine, atomoxetine에의한효과도연구중에있다 야간수면의치료 1 Gammahydroxybutyrate ( 이하 GHB) GHB는수면유도에관여하는내생 (endogenous) 적인물질로서렘수면과서파수면모두를증가시켜견고한 (consolidate) 수면에관여한다. 42 GHB를복용한기면증환자에서주간졸리움, 탈력발작, 가위눌림, 입면시환각, 수면곤란등의증상이호전됨이보고되었고 42 탈력발작의효과도탁월하여현재유일하게 FDA( 미국) 에서허가된탈력발작치료제이다. GHB 의약물효과는빠르나반감기가짧아입면직전에복용하고수면중이차복용이필요하다. 부작용으로는위장장애, 체중감소, 졸림등이있고적정용량이상으로사용될시경기혹은사망까지보고되어환자에게충분한교육이후허가받은기관에서처방되고있다. REFERENCES 1. Roth T, Roehrs TA.
