1 J Lung Cancer 2010;9(1):1-8 The Clinical Approach to Nodular Ground Glass Opacity in the Lung The introduction of low dose chest computed tomography for health screening in Korea has resulted in increased detection of solitary pulmonary nodules, including nodular ground glass opacity. In contrast to the classic solitary pulmonary nodule, nodular ground glass opacity (GGO) has special characteristics especially in Koreans. More than half of nodular GGOs are transient and they are caused by a pulmonary infiltrate of eosinophils. However, persistent nodular GGO (nggo) showed a high malignant potential such as atypical adenomatous hyperplasia and bronchioloalveolar cell carcinoma. The increasing use of video assisted thoracoscopic surgery (VATS) for diagnosis and treatment is the current trend for managing nodular GGO. Even though lobectomy is still the standard management for malignant nggo, limited resection (wide wedge resection or segmentectomy) is widely used for the small malignant GGO (Noguchi types A and B). Multifocal nodular GGOs are mostly of a synchronous origin rather than intrapulmonary metastasis. Therefore, aggressive surgical resection is warranted. This review contains the current concepts for managing nodular GGO and it especially focuses on the Korean data. SPN이란단일의구형의경계가뚜렷한방사선학적음영으로직경이 3 cm 이내이며공기가통하는폐실질로완전히싸여있는경우로정의되어있다 (1). 결절성간유리음영 (nggo) 은국소적결절성희미한폐침윤으로기관지, 혈관의경계가잘그려지는결절로정의 된다 (2). 이는감염이나간질성폐렴에서보이는광범위한다발성으로간유리성폐침윤과대조적인의미이다. 특히 1996년고해상폐 CT에서보이는국소적인 GGO 소견이기관지폐포세포암 (bronchioloalveolar cell carcinoma, BAC) 의초기소견이라는국내논문 (3) 이후주로일본과한국을중심으로많은연구가발표되고있다. SPN은결절내의고형성분 (solid component) 의유무에따라 1) 고형성분이전혀없는순수결절성간유리음영 (pure nggo, pggo), 2) 간유리음영과고형성분이혼합되어있는혼합결절성간유리음영 (mixed nggo, mggo, subsolid nodule) (Fig. 1) 및 3) 간유리음영성분이전혀없는순수고형폐결절 ( 과거의폐결절 ) 로구분된다. nggo 결절의진단및치료에중요임상적인논점을정리하여보면 1) Transient nggo or Persistent nggo, 2) 지속성 nggo의자연적경과, 3) 지속성 nggo의특성, 4) nggo 를어떻게추적검사를할것인가?, 5) nggo의조직학적진단법, 6) nggo의치료, 7) 다발성 GGO 병변을어떻게추적및치료할것인가?, 8) 아직풀리지않고있는문제로정리 1

그러므로 ielcap의 protocol에서도염증이의심될경우에는항생제치료후 3개월후다시 chest CT 촬영을권장하고있다 (4). 지속성 nggo는 3개월이상크기의변화가없거나커지는 GGO 결절을의미한다. 본원에서 GGO로의뢰된환자에서 3개월후 chest CT를촬영하여 pggo의 37.6%, mggo의 48.

특히 solid portion을동반한 mggo가지속될경우는악성일가능성이 90% 이상으로보고되고있다. 일시적으로 GGO로보일수있는질환은국소적폐렴 ( 결핵포함 ), 염증, PIE ( 특히기생충질환 : toxocariasis, paragonimiasis 등 ), 폐출혈 (e.g., thoracic endometriosis), BOOP (COP), aspergillosis 등이있으며국소적폐섬유화반응 (focal interstitial fibrosis), aspergillosis는장기간 GGO로발현할수있다.

Transient nggo or Persistent nggo nggo로발현하는흉부의병변은많은경우염증, 감염, 알레르기병변 (pulmonary infiltrate with eosinophilia 등 ), 출혈등의일시적인경우가많이있다. 그러므로 ielcap의 protocol에서도염증이의심될경우에는항생제치료후 3개월후다시 chest CT 촬영을권장하고있다 (4). 지속성 nggo는 3개월이상크기의변화가없거나커지는 GGO 결절을의미한다. 본원에서 GGO로의뢰된환자에서 3개월후 chest CT를촬영하여 pggo의 37.6%, mggo의 48.7% 에서 GGO가소실되거나현저히줄어드는현상을관찰하였다 (5). 그러나 3개월이상지속되는 pggo, mggo의병변은 atypical adenomatous hyperplasia (AAH), BAC (adenocarcinoma in situ), adenocarcinoma with BAC feature 등의전암성병변또는악성일가능성이높다. 특히 solid portion을동반한 mggo가지속될경우는악성일가능성이 90% 이상으로보고되고있다. 일시적으로 GGO로보일수있는질환은국소적폐렴 ( 결핵포함 ), 염증, PIE ( 특히기생충질환 : toxocariasis, paragonimiasis 등 ), 폐출혈 (e.g., thoracic endometriosis), BOOP (COP), aspergillosis 등이있으며국소적폐섬유화반응 (focal interstitial fibrosis), aspergillosis는장기간 GGO로발현할수있다. 처음 nggo가발견되었을경우이결절이일시적인결절인가또는지속성결절인가를추정하는것은임상적으로매우중요하다. 이에관한연구로 2010년 Lee 등 (6) 의보고에의하면 93명에서관찰된 126개의 partly solid nodule (mixed nggo) 를관찰한결과 88예 (69.8%) 에서 3개월후사라지거나크기가줄어드는일시적결절이었다. 또한일시적인경과를보이는 nggo와지속성을보이는 nggo를비교한결과, 일시적경과를보이는 nggo는 1) 혈액내호산구가높고, 2) 고형부분이크고, 3) 다발성이많으며, 4) 특히경계가불분명한 (ill-defined) 결절인경우가많았다 (Fig. 2). 지속성을의심하게하는 nggo는 1) 경계가뾰족하거나 (spiculated border), 2) 결절내기관지-공기음영 (air-bronchogram), 3) 공기방울음영 (bubble lucency), 4) 흉막또는 fissure의당김 (retraction) 등이있는경우이다. 지속성을의심하게하는요인은악성 nggo를의심하게하는요인도된다 (Fig. 3). 위의결과에서많은 nggo가 3개월이내에소실되는일시적결절임을알수있었다. 그러면이런일시적 nggo를보이는원인은무엇일까? 일시적인폐감염, 출혈등의여러원인이있을수있으나혈중호산구가증가하고다발성인경우가많은예를차지한다. 본원에서의연구에의하면검진을목적으로저선량흉부 CT에서발견된원인이불확실한결절성폐침윤환자 102명을대상으로 toxocara ELISA를시행하여 68명 (66.7%) 에서양성률을보였고혈중내호산구의상승과소간을생식한병력이유의하게높았다 (7). 이를근거로추론하면특히우리나라에서발견되는일시적인 nggo의많은예가소간생식에의한 toxocariasis로추

단, 많은 GGO가 전술한 대로 초기 은 경우는 소간을 생식한 적이 있는지를 꼭 확인하고 있는 에 소실된다.

GGO의 경과를 관찰한 연구를 보면 1) GGO의 크기가 증 가, 2) solid portion의 발생 (mggo), 3) mggo에서 solid 지속성 nggo의 자연적 경과 portion이 증가, 4) pure solid nodule로 변함, 5) 다발성으로 관찰, 5) 변화 없음 등의 경과를 보일 수 있다(8). nggo가 임상에서 주목을 받기 시작한지는 약 10년 정도 nggo의 많은 부분을 차지하는 AAH, BAC는 다른 폐암 밖에 되지 않고 진행이 매우 느려 아직 확실한 자연경과는 과 달리 volume doubling time이 각각 988일 및 567일이라는

Papillary Papillary adenocarcinoma Part-solid nodule with greater increase in solid component or solid nodule CT: computed tomography, GGO: ground glass opacity. 보고도있어장기간의관찰이필요하다 (8). 또한 2008년일본의연구에의하면처음 3개월간변화가없었던 GGO 병변을 1,048일관찰한결과 125예중 26예 (21%) 에서크기가증가하였으며 5년동안추정성장가능성 (estimated growth population) 은 30% 이었다. 처음크기가큰경우 (10 mm 이상 ) 와폐암의병력이있는경우유의하게성장의위험성이높았다 (9). 지속성 nggo의특성 3개월이상지속되는 nggo는 AAH, BAC 및 adenocarcinoma with BAC feature가대부분을차지한다. AAH는 BAC의전암병변으로확인되고있다. Noguchi 등 (10) 은 2 cm 이하의작은 adenocarcinoma의병리소견및 CT 소견을비교하여 AAH, Noguchi type A, B, C, D, E 및 F의소견으로구분하였고 type A는 pure GGO 결절이고이후 solid portion이생겨나가는 type으로구분하였으며이는임상에많이사용되고있다. Table 1에는 Noguchi type과 2004년 WHO 병리분류와 CT 소견을비교정리하였다 (10-12). 최근비소세포폐암중선암 (adenocarcinoma) 에는여러형태의암이포함된다는선암의이형성 (heterogeneity) 이주장되고있다. 폐의선암에는 pggo mggo solid nodule로진행하여병리학적으로는 AAH BAC ( 새분류에서는 adenocarcinoma in situ) adenocarcinoma (ADCC) 로진행하는경우도있으나흔한폐암처럼종괴로발현되는경우가있다. 이에따라 ADCC는한종류가아니라발생적으로다른 것이아닌가하는연구들이많이이루어져있다. 폐암조직에서 genome wide analysis를이용하여폐암의분류하여보면 ADCC는크게 2 group으로분류가된다. 여러논문의결과를종합하여보면폐선암은 clustering pattern이 left branch의 ADCC와 right branch의 ADCC로구별이되며 GGO의 pattern을거쳐발생하는폐선암은 left branch에속한다 (13). 또한 GGO의변화과정에대한유전자변화를연구하여정상폐조직에서 AAH ( 주로 pggo) 의변화에주요한유전자의변이는 EGFR의 mutation이고, BAC (ADC in situ) 에서 invasive ADCC로진행하는데중요유전자변화는 EGFR gene amplification, ADCC에서 metastasis 하는데중요변이는 TTF-1 gene amplification으로알려져있다 (13,14). nggo를어떻게추적검사를할것인가? nggo를포함한 SPN의추적검사지침으로 ielcap의지침과 ACCP, radiology의지침등이있다. 전반적으로비슷하나약간의차이가있어소개한다. 1) Recommendation for SPN including GGO: ielcap (2009.2) (4) Solid or partly solid nodule (mggo) (<5 mm) or nonsolid nodule (pure GGO) (<8 mm): repeat LDCT after 1 year. 5 Nodular GGO 5 (2) PET if solid component >10 mm: if positive, biopsy (3) multiple or infection suspected: antibiotics (including anaerobe coverage) and CT f/u at 3 month Any nodule 15 mm or larger (1) immediate biopsy if malignancy suggested (2) infection suspected: antibiotics and CT f/u at 3 month 2) 2007년 ACCP의 SPN management guideline은 Fig. 4에정리하였다 (15). 3) Radiology guideline (2009) (12) (1) Pure GGO <5 mm: no routine follow up ( 특히노인 ) 5 10 mm - initial follow up at 3 6 month (± antibiotics) - if no change, annual follow up ( 최소 3년이상 ) 10 mm - persistence or growth at 3 6 month: recommend resection (2) Mixed GGO: 크기와상관없이악성의가능성이있다. 적절한시기에 PET/CT 등의검사가필요하다. 4) nggo 자체의특성중악성을의미하는소견 (12,15) (1) 크기 : 10 mm 이상. 악성의가능성이매우높다. (2) 경계 : Spiculated margin. 악성가능성이 solid nodule에서는높으나 GGO에서는뚜렷하지않다. Round nggo가악성이많다는보고도있다. (3) 고형부분 (solid portion) Mixed nggo: 가장악성일가능성이높다. Solid portion이발생 : 악성이높아진다. Ratio of solid/total GGO: 클수록악성의가능성이높다. CT number (Hounsfield Unit) histogram (4) 지속성 : 양성질환의대부분은 3개월이내에감소하거나소실된다. (5) 크기의변화일반적인 SPN: 2년이상크기의변화가없으면양성, 비활동성병변 Fig. 4. 6 6 J Lung Cancer 2010;9(1):1-8 pggo는 5년이상경과후에도진행가능성 (6) 공기기관지음영 (air bronchogram) 의존재 (7) 공기방울음영 (bubble lucency) (8) Pleural or fissure retraction (9) Aerogenous spread의가능성 : GGO 환자의경과관찰중가장우려되는점이 multiple GGO의출현이다. 특히 HRCT의정밀성에따라 GGO가여러곳에서발견되는예가흔하다. 5) pggo, mggo의추적검사법 ielcap 또는 AACP의 guideline에서 GGO와 pure solid nodule 사이에큰차이를두지않고있다. 단, ielcap의 protocol에의하면 GGO가 15 mm 이상이고악성의의심이되면조직검사, 15 mm 이상이고염증등의양성질환이의심되면항생제투여후 3개월 CT에서지속되면조직검사, mm에서는 PET 양성이면조직검사, 그외에도경과관찰중크기가커지거나 solid portion이발생하면조직검사를추천하고있다 ACCP guideline에서는 8 mm 이상의 SPN (GGO 포함 ) 에서악성의가능성이 5% 이상에서는검진을권장하고있다 (Fig. 4). Pure GGO는주로 AAH임. 그러나일부에서는 BAC (Noguchi type A, B) 및일부의 adenocarcinoma도발견된다. 일본의보고에의하면 BAC의 doubling time을 880일정도로예상되고 (8), AAH가 BAC로진행되고 BAC가 ADCC with BAC feature로진행되므로 (13,14,16), pggo는이를기준으로 2.0 cm 이하에서는 1년간격으로 HRCT, 기간은무제한또는최소 5년을경과관찰을권장한다. mggo는 BAC의가능성이높아지므로 10 mm가넘으면조직검사, PET 또는수술적절제가권장된다. nggo의조직학적진단법조직검사방법으로는경피성폐침생검술 (percutaneous needle aspiration biopsy, PCNB) 또는경기관지폐생검법이주로이용되며일부기관에서는 navigation을이용한기관지내시경을시행한다. 1) PCNB의진단적가치 : GGO 병변은일반적으로작고 consistency가낮아 CT 유도 PCNB를하여도정확한병리소견을얻기가쉽지않다. 그러므로많은경험이필요하다. 본원의결과를보면 pggo, mggo의전반적인악성진단율은 33 true positive, 3 false negative로 92% 의정확성을보였고 10명의양성질환에서 1예의 false negative를보였다 (17). 일반적으로 PET의진단율은매우낮아 pggo에서는추천되지않으며 mggo에서도 false negative가많을수있음을감안하여야한다. 단, solid nodule에서는 PET의진단적가치가높다. 2) 사전조직검사없이수술을진행하는경우도많다. 그이유로 (1) nggo는크기가작아검사의어려움이있고시술자에방사선노출이많고, (2) PCNB의합병증가능성 ( 기흉, 혈흉, 암세포의확산등 ) 과, (3) HRCT 소견과병리소견과의일치성이높아사전병리검사의필요성이낮아지며 (10,11,16,18), (4) video assisted thoracoscopic surgery (VATS) 의보편화로 VATS를이용한 wide wedge resection 또는 lobectomy로진단과치료를동시에시행하는예가늘고있다. nggo의치료법 : AAH, BAC (ADCC in situ), 또는 ADCC의치료법현재의 GGO 또는 solid nodule에서악성으로진단되는경우는대부분 BAC 또는 adenocarcinoma with BAC feature이다. 이경우 TNM stage로 T1N0M0 (stage 1A) 로 lobectomy가원칙이다. 그러나병변이매우작고림프절전이가없고병리학적으로 BAC Noguchi type A, B의경우는 limited resection (wedge resection, segmental resection) 을하기도한다 (19-21). 그러나 limited resection은대규모임상연구가진행된후특정조건하에서만시행되어야한다는주장도있다 (22). 그외에수술이어렵거나다발성 GGO인경우에는 radiofrequency ablation, 광역학치료 (photodynamic therapy) 등이이용되기도한다. 수술전 AAH로진단된경우는원칙적으로경과관찰이추천되나일부에서는 limited resection을시행하기도한다. Limited resection의 rationale은 1) limited resection후에도예후가좋다 (Noguchi type A, B) (5년생존율 95% 이상 ), 2) 다발성으로발현할가능성이높으므로나중의수술을위해최소한의수술을시행한다. 3) 수술전 coil, dye 등으로 GGO의위치를정확히알수있기때문이다. 그러나 Noguchi type C이상에서는 lobectomy가금기사항이없는한원칙이다. 다발성 GGO 병변을어떻게추적및치료할것인가? GGO 환자중에호흡기내과의사로가장판단하기어려

7 Nodular GGO 7 운점중의하나가다발성 (multifocal) GGO 환자의대처법이다. Persistent GGO의대부분이 BAC이고 BAC는 aerogenous spread를잘하기때문에 multifocal GGO환자를만나면이병변이 metastasis인가또는 multifocal origin인가를판단하여야한다. 또한 GGO 환자를경과관찰도중에원 GGO는변화없으나새로운 GGO가발견되는경우고민을많이하게된다. 다발성 GGO가전이라고한다면같은 lobe일경우는 T3 로수술적제거가아직가능하나같은측다른 lobe일경우는 T4, 반대쪽폐인경우는 M1a가되어수술같은국소적치료는의미가없게된다. 전이또는다발성 origin을감별하는방법으로병변에서얻은특정유전자를비교분석하는 clonality assay와전체유전자특성을비교하는 genomic profiling이많이이용되고있으며치료의선택및예후판단에이용되고있다 (23,24). 본원에서다발성 GGO 병변으로수술받은 24명의환자를대상으로각 GGO 조직 (2-5 조직 / 환자 ) 에서얻은 DNA에서 EGFR exon 18 21번과 K-ras (codon 12,13,61) 를대한유전자분석을시행하였다. 24명의환자중 18명 (75%) 에서서로다른 EGFR 변이가관찰되어서로상이한 clone임을확인하였다. 이상의결과로대부분의 multiple AAH, BAC가폐내전이가아닌 multiple origin의종양임을확인할수있었다 (25). 삼성의료원의발표에의하면 multiple pggo로발현된 23 명의 BAC 환자에서 multiple lobectomy 또는 wedge resection 을시행하여 5명에서는모든 GGO 병변을제거하고 (group I) 18명에서는일부의 GGO 병변만제거후 (group II) 경과를관찰하였다. Group I의 4명의환자는재발이없었고 1명이새 GGO가생겨재수술을사행하였고 Group II에서는 15명에서남은 GGO의변화가없었고 3명에서는소실되었다 (26). 위의결과를보아 multifocal GGO로발현하는 BAC의경우가능하면적극적인수술적치료를권장할수있다. 예로 1개의큰 mggo로발현하는 BAC가있으며같은 lobe에 GGO병변이있으면 lobectomy를시행하고다른 lobe 또는다른쪽폐에작은 GGO가있는 mggo-bac부위는 lobectomy 또는 limited resection을하고다른부위의 GGO은일반적인 guideline에준하여경과를관찰하면된다. 아직풀리지않고있는문제 nggo의많은부분이밝혀지고대책이수립되고있으나 nggo가생기는원인은알려져있지않다. 여자에게많고 비흡연자에게많이발생되는점으로보아흡연은원인이아닌것으로판단된다. 간접흡연의가능성도제시되고있으며양에서증명된 Jaagsiekte sheep retrovirus와같은바이러스감염가능성또는여성에많은특성에비추어가정에서발생하는독성증기등이제시되고있으나확인되지는않고있어향후연구의초점이되리라생각된다. 결 본논문을마무리하면서 nggo를다루는임상의로서다음의사항을염두에두어야하겠다. 첫째, nggo 중 3개월이내사라지는일시적인예가많다. 둘째, 일시적인 nggo에는 toxocara 등의호산구성폐침윤의예가특히국내에서는많다. 셋째, 암을의심하게하는 nggo 의특성을이해하자. 넷째, VATS를이용하여진단과치료를동시에하는경우가늘고있다. 다섯째, Noguchi type A, B에해당되는결절의치료로제한적절제가많이사용된다. 여섯째, 다발성 nggo는전이보다는 multifocal origin인경우가많아적극적인수술적치료가필요하다. 론 REFERENCES 1.
