Various anatomical and functional abnormality can cause this phenomenon. Rarely, left ventricular outflow obstruction can result after mitral valve surgery. We experienced a case of left ventricular outflow obstruction 13 years after mitral valve replacement. The diagnosis was made using two-dimensinal Doppler echocardiography and confirmed by cardiac catheterization. The pressure gradient across the left ventricular outflow obstruction was 96mmHg. A second mitral valve replacement was performed. Because severe fibrosis, pannus around the prosthetic mitral valve and a subaortic web were detected during the operation, the subaortic web was removed. 었다. 대동맥아래로는 심한 섬유화를 동반한 10mm의 는 2.39m/sec, 좌심실 유출로와 대동막 판막의 압력 대동맥하 막형성이 있었는데, 대동맥을 통하여 이를 차가 23mmHg로 현저히 감소되었으며(Fig. 5) 인공 모두 제거하였고 St. Jude 29mm로 승모판 대치수술 판막의 기능은 정상이었다. 현재 환자는 항응고제 및 을 시행하였다(Fig. 3, 4). Inderal 투약하면서 외래에서 추적관찰 중이며, 흉통 경 과 2차 수술 후 정상 동조율을 유지하고 있었 및 호흡곤란은 소실되었다. 으며 좌심실 기능은 정상이었다. 1997년 3월의 심초 고 음파에서는 심구혈율 61%, 좌심실유출로의 최대속도 찰 승모판 대치수술후 인공판막이 수축기동안에 좌심 실유출로 주위의 폐색을 초래할 수 있기 때문에 low output syndrome의 원인이 된다는 보고들이 있다7,8). 승모판 역류로 인공판막대치술후의 좌심실 폐색에 관한 연구에서는 수술한 환자 32명중 3명(9%)이 승 모판막 전소엽의 이상 전방운동으로 인한 좌심실 유 출로 폐색을 보고하면서 좌심실 폐색이 인공판막 대 치술이후 합병증의 하나로써 관찰되어야 한다고 하였 다4). Subaortic web with severe fibrosis was detected after incision of aorta at the operataion field (arrow). 후 좌심실유출로 폐색에 관한 연구에서의 폐색의 빈 도는 10%였으며, 수술직후나 후기의 추적관찰에서 좌심실 유출로 폐색이 생기는 빈도의 차이가 없다고 보고하였다2). 또한 Mihaileanu등9)은 307명을 대상으 로 하여 승모판막 대치수술을 받은 승모판 부전의 원 인별로 좌심실 유출로 폐색의 빈도를 관찰하였는데, 퇴행성으로 인한 판막 부전으로 인공판막수술울 받았 을 경우가 16%의 유출로 폐색을 보여 기타 류마치스 성(0%), 세균성(5%), 허혈성(0%), 선천성(0%)에 비 Fig. 4. Gross finding of removed subaortic web shows whitish and yellowish soft tissue fragment. 하여 폐색의 빈도가 높았다. 유출로의 폐색은 인공판막 시술에 견고한 판막륜을 사용한 경우에 있어서7,8), 정상 수축기시에 승모판막 륜이 수축하면서 후방으로 이동하여 승모판막의 전소 엽을 끌어당겨 좌심실 유출을 촉진시키는 작용이 소 실되기 때문에 불가피하게 발생되는 합병증이다2,10). Mihaileanu등9)은 인공승모판막 대치후 좌심실 유출 로 폐색의 기전을 인공판막으로 야기되는 해부학적, 기능적변화로 설명하였다. 즉 인공승모판막으로 인해, 확장된 승모판막륜의 크기가 감소될때 승모판막륜과 대동맥 판막륜사이의 각이 좁아지고, 좌심실 후면이 전방 편위되는 해부학적 변형이 좌심실 유출로 폐색 Fig. 5.
