A 23-year-old woman with Treacher Collins syndrome was scheduled for distraction osteogenesis. She had hypoplasia of mandible and malar bone, bilateral deformities of auricles with partial deafness and antimongoloid slant of the palpebral fissures. A 56-year-old woman with mandibular hypoplasia due to childhood trauma was scheduled for distraction osteogenesis. She had a history of difficult intubation. We anticipated a difficult intubation and ventilation. Fiberoptic bronchoscopic guided awake intubation was selected for anesthesia induction. After intravenous injection of midazolam and remifentanil, 10% lidocaine pump spray on the pharyngolarynx with a direct laryngoscope and on the nasal canal. However fiberoptic bronchoscopic guided awake intubation was failed due to severe gag reflex. After intravenous injection of propofol and remifentanil using the target controlled infusion (TCI), mask ventilation was easily performed and, after intravenous injection of vecuronium, fiberoptic bronchoscopic guided intubation was easily performed using a wire reinforced endotracheal tube. The operation was completed successfully without any adverse events. Kite가처음기술하였고그이후로삽관의방법이나기구등은꾸준히발전되어왔다 (Brandt, 1987). 또한전신마취의유도시기관내삽관이용이할지를판단할수있는기도평가방법들이다양하게소개되고있다. Mallampati 등이경구로혀와인두의구조물을관찰하여상기도를 3단계로분류한것을바탕으로 (Mallampati, 1983), Samsoon과 Young이다시 4단계로분류한 modified Mallampati test (mmt) (Samsoon, 1987) 가가장널리사용되는기도평가법이다 (Fig. 1). 또한어려운기관내삽관이예상되는다섯가지위험인자인체중, 두경부의운동성, 하악의관절가동성, 하악의후퇴정도, 돌출된앞니의여부를점수로환산한 Wilson 평가점수법 (Wilson et al, 1988), 턱끝과흉골의함요부사이의직선거리 (sternomental distance), 턱끝과갑상연골의융기부사이의직선거리 (thyromental distance) 측정 (Janssens, 2001) 등의다양한방법들이있다. 이외에 2003년 Khan 등에의해소개된 upper lip bite test (ULBT) 는턱을앞으로내밀어아랫니로윗입술을물수 있는정도에따라 3등급으로나누어어려운기관내삽관을예측하는방법으로써기관내삽관을증진시키는중심축이되는턱의움직임과자유도, 상악의발달, 치아구조를동시에평가하는단일검사법으로, mmt에비해관찰자간의오류가적게발생하고양성예측도와특이도가높은검사법으로알려져있다 (Khan, 2003)(Fig. 3). 굴곡성기관지경을이용한기관내삽관은 1967년 Murphy에의해소개된이후어려운기관내삽관시직접후두경의대안으로제시된방안중가장안전하고효과적인방법으로인정되고있고 (Murphy, 1967), 기관내삽관뿐만아니라중환자, 경추부손상환자의기도관리등다양한기도관리에이용되고있다 (Fan et al, 1988; Fuchs et al, 1999; Ezri et al, 2003). 원광대학교대전치과병원에서는기관내삽관이어려울것으로예상되는환자들에서굴곡성기관지경유도하에기관내삽관을성공적으로유도하여이를보고하는바이다.

Schematic frontal and lateral view of upper lip bite test. 증례 1. 증례 1 체중 49 kg, 키 153 cm인 23세의여자환자가 Treacher Collins syndrome (TCS) 에동반된하악골의형성부전을교정 ( 신연골형성술, Distraction osteogenesis) 하기위해내원하였다. 환자는 13세때타병원에서 1차례기관내삽관을실패한경험이있었으며, 또다른병원에서는굴곡성기관지경을이용한각성하기관내삽관을통해협골의형성부전과안검열의반몽고증사면을교정하는수술을받은경험이있었다. 환자는협골과하악골의형성부전, 부분적인청각장애를동반한양측성의이개기형, 안검열의반몽고증사면을보였으며, 호흡기폐쇄나수면중무호흡증의과거력은없었다 (Fig. 4). El-Ganzouri 등에의해서고안된 El-Ganzouri multivariate risk index (EGRI) 를이용한기관내삽관의난이도평가에서상하절치간간격은 2.8 cm, thyromental distance는 5.7 cm, modified mallampati grade 는 Ⅳ, 목의운동범위는정상, 전돌능력 (ability of prognath) 이없는하악후퇴증을보여전체 ERGI 점 수는 8점으로어려운기관내삽관이예상되었다 (Table 1). 수술전에시행한혈액검사, 심전도, 흉부방사선, 소변검사는정상이었다. 마취전투약으로수술전 30분에 glycopyrrolate 0.2 mg을근주하였고, ondansetron 8 mg을복용하였다. 수술실에도착한후혈압과맥박수는 100/60 mmhg, 64 beat/min 체온은 36.8 o C였으며동맥산소포화도는 100% 였다. 5 L/min 의산소를마스크를통하여흡입시키며 midazolam 2 mg을정주하였고목표농도조절주입기 (Orchestra, Fresenius vial, France) 를이용하여 remifentanil을 Minto 약동학적모델을설정하여목표농도 1 ng/ml 로투여하였다. 후두경을이용하여 10% lidocaine을후인두벽과구강내에분무하였고, 오른쪽비강내에도분무하여도포마취하였다. 내경 6.0 mm의 cuffed anode tube가거치된굴곡성기관지경 (LF-GP Olympus, Japan) 을오른쪽비강내로삽입하고환자의자발호흡을격려하면서삽관을시도하였으나, 계속된구역반사로실패하였다. 굴곡성기관지경을빼내고마스크로산소를흡입시키며 Propofol을 Marsh 약동학적모델을설정하여 2.5 μg /ml, remi-

predicted. fentanil을 2.5 ng/ml로투여하여의식과자발호흡이소실된것을확인하고, 내경 6.5 mm의 nasal airway 를오른쪽코에삽입한후안면마스크환기를시도하였다.

Composition of the El-Ganzouri and Colleagues Multivariate Risk Index (EGRI) Variable/Score Interincisor gap Thyromental distance Modified Mallampati class Neck movement Ability to prognath Body weight History of D/I 4 cm > 6.5 cm I > 90 Yes < 90 kg None < 4 cm cm II o No kg Questionable < 6.0 cm III, IV < 80 > 110 kg Definite D/I: difficult intubation, total ERGI score 4: difficult intubation predicted. fentanil을 2.5 ng/ml로투여하여의식과자발호흡이소실된것을확인하고, 내경 6.5 mm의 nasal airway 를오른쪽코에삽입한후안면마스크환기를시도하였다. 다행히적절한안면마스크환기가이루어져 vecuronium을 2 mg 정주하였다. 근이완이확실히이루어진후에내경 6.0 mm의 cuffed anode tube 가거치된굴곡성기관지경을오른쪽비강으로삽입하여기관내삽관에성공하였다. 삽관직후 vecuronium을 3 mg 추가투여하였고, 마취는 remifentanil 3.0 ng/ml, propofol 3.0 μg /ml, 산소 50% 로유지하였으며, 수술이종료된후 pyridostigmine 10 mg, glycopyrrolate 0.4 mg을정주하고 100% 산소로용수 환기를시행한후자발호흡이회복되고완전히각성된상태에서발관을시행하였다. 환자는특별한합병증없이수술후 6일에퇴원하였다. 4개월후에금속판제거 (plate removal) 를위해내원하였고같은방법으로무리없이기관내삽관을하였고, 합병증없이 4일후에퇴원하였다. 2. 증례 2 체중 44 kg, 키 141 cm인 58세의여자환자가어린시절의외상에의한왼쪽하악과두돌기의골절을치료받지않고있다가, 부정유합에의한턱관절주위의통증과개구장애, 하악골의비대칭과형성

5 194 대한치과마취과학회지 : 제 10 권제 2 호 2010 부전을증상으로신연골형성술을받기위해내원하였다 (Fig. 4). 환자는한달전타병원에서 1차례기관내삽관을실패한경험이있었다. 그당시의마취기록지에의하면 pentotal sodium으로마취유도하고 succinylcholine으로근이완하여후두경으로시야확보를시도하였으나 Cormack과 Lehane의후두시야등급이 Ⅳ였고 (Fig. 2), 1시간에걸쳐기관내삽관을시도하였으나실패하였다고기록되어있었다. 다행히환자의안면마스크환기는적절하였다. EGRI를이용한기관내삽관의난이도평가에서상하절치간간격은 2.5 cm, thyromental distance는 5.7 cm, modified mallampati grade는 Ⅳ, 목의운동범위는정상, 전돌능력이없는하악후퇴증을보여전체 ERGI 점수는 8점으로어려운기관내삽관이예상되었다 (Table 1). 수술전에시행한혈액검사, 심전도, 흉부방사선, 소변검사는정상이었다. 마취전투약으로수술전 30분에 glycopyrrolate 0.2 mg을근주하였고, ondansetron 8 mg을복용하였다. 수술실에도착한후혈압과맥박수는 110/70 mmhg, 74 beat/min 체온은 36.8 o C였으며동맥산소포화도는 100% 였다. 5 L/min의산소를마스크를통하여흡입시키며 midazolam 2 mg을정주하였고목표농도조절주입기를이용하여 remifentanil을 Minto 약동학적모델로설정하여목표농도 1 ng/ml로투여하였다. 후두경을이용하여 10% lidocaine을후인두벽과구강내에분무하였고, 오른쪽비강내에도분무하여도포마취하였다. 내경 6.5 mm의 nasal airway를오른쪽비강내로삽입하고굴곡성기관지경 (LF-GP Olympus, Japan) 을진입시켜환자의 vocal cord 위치를확인하였다. 굴곡성기관지경을빼고마스크를통하여산소를흡입시키면서 Marsh 약동학적모델로 propofol을 2.5 μg /ml, Minto 약동학적모델로 remifentanil을 2.5 ng/ml로투여하여자발호흡이소실된것을확인하고, 적절한안면마스크환기를확인하고 vecuronium을 2 mg 정주하였다. 근이완이확실히이루어진후에내경 6.0 mm의 cuffed anode tube가거치된굴곡성기관지경을오른쪽비강으로삽입하여기관내삽관에성공하였다. 삽관직후 vecuronium을 2 mg 추가투여하였고, 마취는 remifentanil 3.0 ng/ml, propofol 3.0 μg /ml, 산소 50% 로유지하였으며, 수술이종료된후 pyridostigmine 10 mg, glycopyrrolate 0.4 mg을정주하고 100% 산소로용수환기를시행한후자발호흡이회복되고완전히각성된상태에서발관을시행하였다. 환자는특별한합병증없이수술후 6일에퇴원하였다. 5 개월후에금속판제거 (plate removal) 를위해내원하였고같은방법으로무리없이기관내삽관을하였고, 합병증없이 4일후에퇴원하였다. 고찰전세계적으로지난 20년동안마취와관련된사망은 10,000명당한명꼴로일어났으며, 기도관리의실패는마취와관련된사망의중요한원인이되고있다 (Braz et al, 2009). 미국마취과학회에서는어려운기도관리 (difficult airway) 란안면마스크환기가안되거나기관내삽관이어려운경우로정의하였고, 이중어려운기관내삽관 (difficult intubation) 은기관의병변유무에관계없이기관내삽관을여러번시도하는것으로정의되며그빈도는 1.5% 에서부터 15.8% 까지다양하게보고되고있다 (Pearce, 2005; Shiga T, 2005). 기관내삽관이어려운경우는기관내삽관시도전충분한이학적검사로예측이가능하고전통적인기관내삽관방법이외의방법을사용하면보다안전하게기도유지를시행할수있다. El-Ganzouri 등에의해서고안된 El-Ganzouri multivariate risk index (EGRI) 는간단한신체검사를통해확인할수있는예측인자로턱관절의움직임을판단할수있는위아래앞니사이의거리, 후두의노출정도를예측할수있는변형 Mallampati 검사와턱끝-갑상연골사이의거리, 턱의전방이동정도, 목의신전- 굴곡시의각등의 7가지항목을측정하는다변수위험지수로기관내삽관의난이도평가에자주이용되며비교적예측의정확도가높다 (El-Ganzouri, 1996)(Table 1). 또한어려운기관내삽관의경험, 목의움직임이나고리뒤통수관절의움직임에영향을미칠수있는전신적인질병, 병변이나손상등이어려운기관내삽관을쉽고정확하게예측할수있는요인이된다 (Jacobsen, 1996; Paix, 2005). 기관내삽관방법은다양하며, 직접후두경을이용하여구강을통한기관내삽관방법이일반적으로사용되고있지만, 목, 구강, 인후두부위의선

6 오세리 이진한 : 어려운기관내삽관이예상되는환자의기도관리 195 천적또는후천적인해부학적이상으로성문부를육안으로확인할수없는경우에는후두경을이용한통상적인방법으로기관내삽관을용이하게시행할수없다. 이처럼어려운기관내삽관이예상되는경우굴곡성내시경은기도확보가어려운환자에있어서매우성공률이높은방법으로알려져있다. 후두경으로관찰한후두시야가불량한경우에도기관-기관지분지부까지삽입된굴곡성내시경은기관내튜브가기관내에적절히위치하는데있어서길잡이가되어주며, 내시경을후퇴시키면서그위치의적절함을확인할수있다는점에서큰의미를갖는다. 통상적방법으로는기관삽관이불가능했던환자들의 90% 이상이굴곡성내시경을사용한경우에는쉽게삽관에성공했다는실험결과가있다 (Ovassapian, 1995). 최근굴곡성기관지경을이용한각성하기관내삽관법이많이이용되고있지만시술시동통, 기침, 구역질등의문제점이있어시행에어려움이따른다. 이러한문제점을해결하기위해서수술전환자에게충분한설명과이해, 국소마취제의인후두도포마취, 상후두신경차단, 경기관차단, 진정제, 마약등의사용을통한약물학적처치등이사용되고있다 (Reed, 1992). 그러나실제로 10% lidocaine 분사나 4% lidocaine gargling은구강내와인두부를표피마취하는것은가능하지만후두부와기관내를표피마취하기가어렵다. 또한양측상후두신경차단을위해갑상연골외측상부의설골갑상막에 2% lidocaine을 2 ml씩주입하고, 기관내마취를위해윤상갑상막에 4% lidocaine 2 ml를기도로직접분사하는방법은환자에게많은불편을주고있는실정이다. 일반적으로굴곡성기관지경술시근위부 suction channel에주사기를직접연결하고 2% lidocaine을분사해서후두및기관내표피마취를시도하고있으나이러한방법은원위부 suction channel에방울이맺혀시술자의시야를방해할뿐아니라마취하고자하는부위에정확히분사하기가어렵고여러번 lidocaine을분사해도후두및기관내표피마취가가끔충분치못하여튜브를삽관할때기침, 구역질등이유발된다. 본증례에서는두경우모두에서 10% lidocaine을후두경을통하여인후두부에분사하였으나후두의표피마취가제대로되지않아서심한구역반사로 인해굴곡성기관지경이성문을통과하지못하였다. Propofol과 remifentanil로의식을소실시킨후안면마스크환기가적절하게이루어짐을확인하고기관내삽관의실패를염려하여곧바로근이완역전을시도하기위해 vecuronium을소량만투여하였다. 다행히안면마스크환기는적절하게계속유지되었고, 굴곡성기관지경을통한기관내삽관은 30초이내로원활하게이루어졌다. 그러나돌이켜보면어려운기도확보가예상되는환자들에서응급상황을대비한후두기도마스크 (Laryngeal Mask Airway, LMA) 나경기관제트환기, 기관절개술등의준비없이정맥마취제와근이완제를투약하여기관내삽관을시도한것은무모했던것으로여겨진다. 전신마취를위한기관내삽관은술전철저한과거력, 현병력조사와이학적검사를통해기도평가후에시행하는것이바람직하며, 기도유지가어려울것으로예상되는환자들을사전에정확히예측하여기관내삽관과환기가모두되지않는응급상황이발생하지않도록미리예방하는것이가장중요하다. 어려운기도관리는환자뿐아니라임상적환경과시술자의숙련도와도관련이있다. 특히후두경으로관찰한후두시야가불량한경우에가장먼저고려되는굴곡성기관지경을통한각성하기관내삽관은평소에도꾸준한연습을통하여굴곡성기관지경을숙련되게다룰수있어야성공할수있으며, 후두부와기도내의표피마취방법을좀더환자가편안할수있는방법으로개선하도록더많은연구가필요하겠다. 참고문헌 Benumof JL: Management of the difficult adult airway.
