Although several studies have identified IgAN prognostic factors in Korea, the follow-up period was insufficient to evaluate the natural history of IgAN. Methods: A total of 471 patients were diagnosed with IgAN after percutaneous renal biopsy between April 1985 and March Patients with secondary IgAN and patients with a follow-up < 10 years since their diagnosis were excluded. Thus, 184 patients were enrolled. Results: Among the 184 patients, 97 were males (52.7%) and 87 were females (47.3%). The mean age was 33.7 ± 11.5 years, and the mean follow-up period was ± 46.3 months. During the follow up, 73 patients (36.9%) had progressed to end-stage renal disease (ESRD). The mean duration to ESRD was 98.1 ± 55.9 months. The overall renal survival rate was 60.3%, the 10-years renal survival rate was 74.3%, and the 20-years renal survival rate was 49.3%. Univariate analyses indicated that hypertension, serum creatinine > 1.3 mg/dl, estimated glomerular filtration rate < 60 ml/min/1.73 m 2, serum albumin < 3.5 g/dl, proteinuria 1 g/day, and severe renal pathology by the Haas sub-classification were significantly associated with ESRD. When these factors were included in multivariate Cox regression analyses, only severe renal pathology by the Haas sub-classification was an independent prognostic factor for IgAN. Conclusions: Careful follow-up and treatment is recommended, particularly in patients with IgAN and severe renal pathology by the Haas sub-classification. Prognostic factors of IgA nephropathy - 서론 Immunoglobulin A (IgA) 신장병은 1968년 Berger 와 Hinglais 에의해처음보고된이후, 전세계적으로가장흔한일차성사구체신염으로알려져있다 [1]. 임상양상은다양하여무증상적요이상, 육안적혈뇨, 심한부종을동반한신증후군으로나타나기도하고, 초승달사구체신염의형태를보이는급속진행성사구체신염을일으키기도한다. 이질환의가장흔한임상경과는장기적으로서서히진행되는것으로발병후 10년이내에 6-43% [2], 20년이내에는 25-50% 에서말기신부전으로진행하는것으로알려져있다 [3]. IgA 신장병의임상경과및예후인자를찾으려는다양한노력들이있었고말기신부전으로의진행을예측할수있는예후인자로는진단당시나이, 성별, 고혈압, 신기능, 단백뇨, 혈뇨, 조직학적변화, IgA/C3 비, 혈청알부민등이알려져있다 [4-7]. 또한 IgA 신장병은아시아인과백인에흔하며아프리카인, 특히흑인에는드문것으로알려져있다 [8]. 우리나라에서는최소변화콩팥병증이나막증식사구체신염은감소하는반면 IgA 신장병은상대적으로증가하는양상을보인다 [9]. 우리나라말기신부전의원인질환중세번째로흔한질환이만성사구체신염이지만, 10대에서 40대까지의젊은연령층에서는만성사구체신염이가장흔하며그중 IgA 신장병이가장많다 [10]. 이러한임상적특징으로국내에서도 IgA 신장병의예후와임상양상을예측하기위한많은연구가진행되어왔다. 국내에서시행한연구들은 IgA 신장병의자연경과에비해추적기간이짧으며그대상환자들또한적었다 [11-13]. 이에저자는신생검을통해 IgA 신장병을진단받고 10년이상추적관찰한환자들을대상으로임상경과및예후인자에대하여연구하였다. 대상및방법대상 1985년 4월부터 2003년 3월까지계명대학교동산의료원신장내과에입원하여경피적신생검을시행하고조직소견으로 IgA 신장병으로진단된환자는 471명이었다. 이들중진단당시에진찰소견및검사소견상 Henoch-Schönlein purpura, 전신홍반성낭창, 간경화등에의한이차성인경우를제외하였고, 일차성 IgA 신장병으로진단된환자는 453 명이었다. 이들중 10년이상추적관찰이가능했던 184명을대상으로하였다. 방법환자들의신생검당시나이, 성별, 고혈압, 육안적혈뇨, 현미경적혈뇨, 혈청크레아티닌, 추정사구체여과율, 혈청알부민, 24시간단백뇨량, 혈청 IgA/C3 비, 병리조직학적소견을후향적으로조사하였다. 고혈압은진단당시측정된수축기혈압이 140 mmhg 이상이거나이완기혈압이 90 mmhg 이상인경우, 혹은첫방문시이미고혈압제제를복용중인경우로정의하였다. 현미경적혈뇨는고배율시야당 5개이상의적혈구가관찰되는경우로정의하였다. 단백뇨는 24시간요검사를통해측정하였다. 추정사구체여과율은환자의성별, 혈청크레아티닌, 나이의정보를 Modification of Diet in Renal Disease (MDRD) 공식에대입하여계산하였으며 MDRD 공식은아래와같다. MDRD egfr (ml/min per 1.73 m 2 of body surface) = 186 (serum creatinine [mg/dl]) (age [in years]) (For women, the value was multiplied by 0.742) 위험인자분석을위해혈청크레아티닌은 1.3 mg/dl 미만인군과 1.3 mg/dl 이상인군으로나누었고혈청알부민은 3.5 g/dl 미만인군과 3.5 g/dl 이상인군으로나누었다. 단백뇨는 1.0 g/day 미만인군과 1.0 g/day 이상인군으로나누었다. 혈청 IgA/C3 비는 3.32 미만인군과 3.32 이상인군으로나누었다 [6]. 추정사구체여과율은 60 ml/min/1.73 m 2 미만인군과 60 ml/min/1.73 m 2 이상인군으로나누었다. 신생검시병리조직학적소견은 Haas 분류법에의하여 I 군 ( 사구체경화나반월체가없는분절성미세한세포증식 ), II 군 ( 활동성세포증식이없는초점성분절성사구체경화 ), III 군 ( 초점성증식성사구체신염 ), IV군 ( 미만성증식성사구체신염 ), V군 (40% 이상의사구체전경화를보이거나 40% 이상의피질세뇨관의위축 ) 으로분류하였다 [14]. 통계분석 SPSS ver (SPSS Inc., Chicago, IL, USA) 을이용하여통계처리를하였고연속변수는평균 ± 표준편차로표시하

3 - 대한내과학회지 : 제 88 권제 1 호통권제 653 호 였으며, 명목변수는숫자와백분율로표시하였다. 각군간의임상적특성을비교하기위하여 Student t-test 와 Chi-square 검정법을사용하였으며, 신생존율은 Kaplan-Meier법을이용하였다. Log-rank test 를이용하여단일변량분석을하였으며, Cox 비례위험회귀모형을이용하여다변량분석을시행하였다. 통계적유의수준은 p값이 0.05 미만인경우로하였다. Table 1. Clinical manifestations of patients at the time of renal biopsy Characteristic Value Number of patients 184 Age, yr 33.7 ± 11.5 Sex ratio, male:female 1.1:1 Hypertension, % 35 (19.0) Gross hematuria, % 52 (28.3) Microscopic hematuria, % 114 (61.9) Serum creatinine, mg/dl 1.1 ± 0.5 EGFR, ml/min/1.73 m ± 31.8 Serum albumin, g/dl 4.0 ± 2.8 Proteinuria, g/day 2.5 ± 2.4 Serum IgA/C3 ratio 5.9 ± 5.2 EGFR, estimated glomerular filtration rate; IgA, immunoglobulin A. 결과환자의특성대상환자는총 184명이었고남자 97명, 여자 87명으로남녀비는 1.1:1이었다. 평균연령은 33.7 ± 11.5세 (12-35세) 이었고평균추적관찰기간은 ± 46.3개월 ( 개월) 이었다. 또한신생검당시고혈압 35예 (19.0%), 육안적혈뇨 52예 (28.3%), 현미경적혈뇨 114예 (61.9%) 였고평균혈청크레아티닌은 1.1 ± 0.5 mg/dl, 추정사구체여과율은 78.7 ± 31.8 ml/min/1.73 m 2, 혈청알부민은 4.0 ± 2.8 g/dl, 24시간단백뇨량은 2.5 ± 2.4 g/day, 혈청 IgA/C3 비는 5.9 ± 5.2였다 (Table 1). 신생존신기능을추적관찰한결과, 말기신부전으로진행한환자가 73명 (39.6%) 이었으며진단당시로부터말기신부전으로의진행기간은평균 98.1 ± 55.9개월 ( 개월) 이었다. 전체신생존율은 60.3% 이었고 Kaplan-Meier법을이용한 5년, 10년, 15년, 20년신생존율은각각 87.5%, 74.3%, 59.2%, 49.3% 이었다 (Fig. 1). 신생존군과말기신부전군간의비교에서신생검당시의고혈압, 단백뇨량, 혈청크레아티닌, 추정사구체여과율등이의미있는차이를보였고성별, 나이, 혈청알부민, 혈청 IgA/C3 비, 육안적혈뇨, 현미경적혈뇨는차이를보이지않았다 (Table 2). IgA 신장병의예후인자 - Log-rank test 를이용한단일변량분석에서고혈압 (p = 0.003), 1.3 mg/dl 이상의혈청크레아티닌 (p < 0.001), 60 ml/min/1.73 m 2 미만의추정사구체여과율 (p < 0.001), 3.5 g/dl 미만의혈청알부민 (p < 0.001), 1 g/day 이상의단백뇨량 (p = 0.002), 진행된조직학적소견 (p < 0.001) 등이유의한위험인자로나타났다 (Table 3). 단일변량분석에서통계학적으로유의한것으로나타난인자들을바탕으로 Cox 비례위험회귀모형을이용하여다변량분석을시행한결과진행된조직학적소견만이독립적인위험인자로나타났다. I군은 8명 (4.3%), II군은 67명 (35.8%), III군은 81명 (44.5%), IV군은 19명 (10.3%), V군은 9명 (4.9%) 으로 III군에서가장많았으며, I군에서가장적었다. 각각의조직학적분류에해당하는환자들사이에나이및성별차이는없었다. 신생검당시혈청크레아티닌은조직학적병변이심할수록증가하는경향을보였으며추정사구체여과율은낮아지는경향을보였다. 단백뇨또한조직학적소견이심할수록전반적으로증가하는경향을보였으나 I군에서 4.5 ± 3.6 g/day로 II, III, IV군보다더많은단백뇨의배설이있 Table 4. Renal survival rate according to the Haas sub-classification. 었다. 고혈압, 육안적혈뇨, 현미경적혈뇨, 혈청 IgA/C3 비는조직학적분류간에차이를보이지않았다. 말기신부전으로의진행은조직학적병변이심할수록증가하는양상을보여 I 군에서는 10년신생존율이 87.5% 였고, V군에서는 11.1% 였다 (Fig. 2). 고 IgA 신장병은 1968년처음으로보고된이후, 전세계적으로가장흔한원발성사구체신염으로알려져있다 [1]. 이병 찰 의가장흔한임상경과는천천히진행하여말기신부전으로진행하는것이다 [2]. 이러한질환의특성으로 IgA 신장병의조기발견의중요성이대두되었을뿐아니라 IgA 신장병진행의위험인자를찾고, 말기신부전으로의진행을지연시킬수있는방법에대한연구가많이이루어졌다. Song 등 [15] 은평균동맥압과단백뇨의정도를이용하여 IgA 신장병의진행예측공식을산출한바있고, Berthoux 등 [16] 은고혈압, 단백뇨량및조직학적병변의심한정도를이용한 absolute renal risk를통해투석혹은사망위험예측이가능함에대해보고한바있다. 하지만 IgA 신장병의임상양상의다양성으로인해장기예후를예측하는것은어렵다. 본연구에서는일차성 IgA 신장병을진단받고 10년이상추적관찰한환자들을대상으로 IgA 신장병의임상경과및예후인자에대하여후향적으로조사하였다. IgA 신장병은발병후 10년이내에 6-43% [2], 20년이내에는 25-50% 에서말기신부전으로진행하는것으로알려져있으며 [3], 아시아지역이나유럽에서북미지역에비하여신생존율이높은것으로알려져있다. 우리나라에서는 10년신생존율을 82%, 20년신생존율을 70.8% 로보고하였다 [17]. 일본에서는 10년신생존율을 85%, 20년신생존율은 61% 로보고하였고 [5,18], 중국에서는 10년신생존율을 83%, 20년신생존율은 64% 로보고하였다 [7]. 유럽에서는 10년신생존율을 77-94% 로보고하였고북미에서는 10년신생존율을 57-78% 로보고하였다 [2]. 북미지역에서다른지역에비하여신생존율이낮은것은인종간의유전학적차이때문일수도있으나

Prognostic factors of IgA nephropathy - 호발연령대인소아나청소년기의소변검사시행여부에따른차이, 또는신생검적응증에대한지역차에의한것으로시간단축비뚤림때문일가능성이가장높을것으로추정하고있다 [19]. 본연구에서 10년신생존율은 74.3%, 20년신생존율은 49.3% 로다른아시아지역보다낮으며최근보고된우리나라의다른연구와비교해서도낮은편이다 [17]. 진단당시의단백뇨와혈청크레아티닌등이이전연구에비하여평균값이높으며, 이는본연구가이전의연구들과비교하여평균추적관찰기간이가장긴연구로임상증상이경한환자들이추적관찰기간도중추적되지않은경우가임상증상이심한환자보다많아신생존율이낮다고생각된다. IgA 신장병은전연령층에서발생할수있으나젊은연령층에서주로발생하며, 본연구에서평균연령은 33.7 ± 11.5 세 (12-35세) 였다. 또한성별에따른발생빈도는지역적인차이가있어호주, 북미, 유럽등지에서는 2:1 이상남자에서발생빈도가높았으나, 일본에서는성별에따른차이가없었다 [8]. 국내의다른보고에서는남녀간의발생빈도는차이를보이지않는다고하였고 [17] 본연구에서도남녀비는 1.1:1 로차이가없었다. 여러연구들에서고령과남성은불량한예후인자로보고되었으나 [2,20] 다른연구에서는남성보다는여성에서예후가나쁘다는보고가있었고 [21], 최근일본의대규모연구에서는 30세이하의연령이예후가더나쁘다고보고되었다 [5]. 본연구에서는나이와성별에따른신생존율의차이는없었다. 여러연구들에서고혈압, 많은단백뇨량, 신기능저하는 IgA 신장병의나쁜예후인자로알려져있다 [2,5-7,11,17]. 고혈압은 IgA 신장병환자의 6-49% 에서관찰되며본연구에서는 19% 에서고혈압이동반되었으나조직학적병변에따른차이는보이지않았다 [2]. 단백뇨량은보고에따라 3.5 g/day 이상인경우에예후가나쁘다는보고가있었고 1.0 g/day 이상인경우에예후가나쁘다는연구도있었다 [7,12,22]. Pozzi 등 [23] 은단백뇨량을줄이기위해대부분의 IgA 신증환자에서안지오텐신수용체길항제를투약하고있고, 중등도이상의단백뇨를가진환자에서스테로이드의투약이신기능보전에유의한효과를가져올수있다고보고하였다. Choi 등 [24] 은안지오텐신수용체길항제와스테로이드를함께투약한환자에서단백뇨량이유일한주요예후인자로보고하였다. 본연구에서는단백뇨량이 1.0 g/day 이상인환자에서불량한예후를보였지만다변량분석에서는통계학적으로 의미가없었다. 신기능저하또한단일변량분석에서는불량한예후를보였으나다변량분석에서는의미가없었고, 단백뇨량과신기능저하는조직학적변화가심할수록전반적으로증가하는경향을보였다. 본연구에서약제투약에따른신기능및단백뇨량변화에대한분석은없었던상태로추후이에대한추가적연구가필요할것으로보인다. IgA 신장병은아직까지발병과정이완전하게밝혀지지않았지만 IgA1 의글리코실화패턴이중요할것으로여겨진다. IgA 신장병환자에서중쇄의경첩부위에부착하는 O-linked glycan의갈락토오스결핍을보이는 IgA1 이증가되어있음이발견되었고, 이는기본적인병인으로인정받고있다 [25,26]. 또한동물실험을통해줄기세포단계에서혈청 IgA의변화를결정하며골수이식으로 IgA 신장병이호전될수있음을관찰함으로써 IgA 신장병이골수줄기세포결함임이제시되었다 [27]. IgA 신장병은골수에서 IgA1을과다생산하게되고이것이메산지움에침착되는것이주기전으로알려져있으며, IgA1 의메산지움침착과함께 C3, IgG 등도함께침착하는것으로알려져있다 [28,29]. 또한 IgA 신장병이진행함에따라보체의활성화를통해혈청 C3가감소하게된다 [30]. 이전연구들에서심한조직학적변화를가진 IgA 신장병환자들이경한조직학적변화를가진 IgA 신장병환자들에비해혈청 IgA/C3 비가높다는사실을바탕으로높은혈청 IgA/ C3비가나쁜예후인자로서의의미를가진다는보고들이있었다 [6,30]. 본연구는혈청 IgA/C3 비를연구한이전논문들에비해장기간추적관찰하였으며더많은환자를대상으로한연구로, 혈청 IgA/C3 비는예후인자로서의의미를가지고있지않았다. 또한 Haas 분류법에따라 IgA 신장병을조직학적으로분류하여혈청 IgA/C3 비를비교하였을때에도의미있는차이를보이지않았다. IgA 신장병은미세병변부터미만성증식성초승달사구체신염까지병리조직학적소견이다양하다. IgA 신장병이처음으로보고된이후, 여러가지조직학적분류법들이제시되었으나 IgA 신장병에서관찰되는형태학적소견을모두반영하지는못하고있다. 그러나신조직의병리학적소견은 IgA 신장병의진단에필수적일뿐만아니라많은연구결과에서그자체로중요한예후인자로알려져있다 [4,5]. IgA 신장병의조직학적분류는크게두분류로나뉜다. 하나는주로사구체병변을기준으로하며세뇨관, 간질, 혈관의변화에기반을둔것이고, 다른하나는반정량적분류로서메

7 - 대한내과학회지 : 제 88 권제 1 호통권제 653 호 산지움의과증식, 분절성사구체경화, 모세혈관내과증식, 세뇨관위축 / 간질섬유화로나누어각각의점수를매겨분류하는것이다. 전자에속하는것으로는 Haas 분류법이있으며, 후자에는 Oxford 분류법이포함된다 [14,31,32]. 본연구에서는 Haas 분류법에의하여 IgA 신장병을조직학적으로나누었다. Haas 분류법 III군이 82명 (44.5%) 으로가장많았으며 V군을기준으로 I군, II군, III군은통계학적으로유의하게신생존율이높았고 IV군은 V군에비하여신생존율은높으나통계학적으로유의한정도는아니었다. 이는 Haas의연구에서 III군이가장많으며 I군과 II군이양호한예후를보이고 IV군과 V군이불량한예후를보였던것과일치하는결과이다 [14]. 본연구는이전연구들과비교하여평균추적관찰기간이가장긴연구이나단일기관연구라는제한점이있으며, 후향적연구이기때문에선택비뚤림이있을수있다. 많은연구들에서예후인자로알려진단백뇨와혈청크레아티닌은본연구의단일변량분석에서는통계학적으로의미가있었으나, 다변량분석에서는의미를가지지못했다. 이는단백뇨량이적거나혈청크레아티닌이낮은경우임상적으로증상이없는경우가많아장기간의추적관찰기간동안추적되지않은경우가증상이있는경우에비하여많기때문으로생각된다. 본연구는진단당시임상양상과조직검사를바탕으로조사한연구이지만추적관찰기간동안예후인자들의변화또한결과를예측하는데있어중요할것이라생각된다. 또한치료인자에대한조사를하지못해치료군과비치료군이모두포함되어치료군의치료에의한영향이반영되지않았다. 결론적으로본연구에서는진단당시진행된조직학적소견이 IgA 신장병환자의나쁜예후를예측할수있는의미있는예후인자였다. 향후보다많은환자들을대상으로한전향적인연구를통하여새로운예후인자를찾는노력이필요할것으로생각된다. 요약목적 : IgA 신장병은전세계적으로가장흔한일차성사구체신염으로임상경과가다양하며 25-50% 에서말기신부전으로진행하는것으로알려져있다. 국내에서시행한연구들은 IgA 신장병의자연경과에비해추적관찰기간이짧으며그대상환자들또한적었다. 이에 IgA 신장병을진단받 고 10년이상추적관찰했던환자들을대상으로임상경과및예후인자에대해연구를시행하였다. 방법 : 1985년 4월부터 2003년 3월까지계명대학교동산의료원신장내과에입원하여경피적신생검을시행하여 IgA 신장병으로진단된 471명의환자중일차성 IgA 신장병으로진단받고 10년이상추적관찰이가능했던 184명을대상으로조사하였다. 결과 : 총 184명의환자중남자 97명, 여자 87명으로남녀비는 1.1:1이었다. 평균연령은 33.7 ± 11.5세 (12-35세) 였고평균추적관찰기간은 ± 46.3개월 ( 개월) 이었다. 추적관찰기간동안말기신부전으로진행한환자가 73 명 (39.6%) 이었으며진단당시로부터말기신부전으로의진행기간은평균 98.1 ± 55.9개월 ( 개월) 이었다. 추적관찰기간동안 5년, 10년, 15년, 20년신생존율은각각 87.5%, 74.3%, 59.2%, 49.3% 였다. 신생존군과말기신부전군간의단일변량분석에서고혈압, 1.3 mg/dl 이상의혈청크레아티닌, 60 ml/min/1.73 m 2 미만의추정사구체여과율, 3.5 g/dl 미만의혈청알부민, 1 g/day 이상의단백뇨량, 진행된조직학적소견등이유의한위험인자로나타났다. 나이, 성별, 혈청 IgA/C3 비, 육안적혈뇨, 현미경적혈뇨등은유의한차이를보이지않았다. 다변량분석에서는진행된조직학적소견만이독립적인위험인자로나타났다. 결론 : 일차성 IgA 신장병을평균 15.1년간장기추적관찰한결과, 진단당시진행된조직학적소견을보였던환자에서말기신부전으로의진행률이유의하게높았다. 중심단어 : 사구체신염 ; IgA 신장병 ; 예후 REFERENCES 1.
