However, further study is needed to determine its validity. We studied the relationships between the Oxford classification and established prognostic factors and renal survival. We also examined associations between electron microscopy findings and these parameters. Methods: We reviewed and reclassified 213 patients who were diagnosed with IgAN from 1997 to 2007 using the Oxford and World Health Organization (WHO) classification systems. The patients were also categorized by a pathologist using electron microscopy findings, including foot process fusion, glomerular basement membrane thickness, and electron-dense deposits. We examined the correlations between light and electron microscopy data and known prognostic factors (e.g., age, sex, proteinuria, serum creatinine, estimated glomerular filtration rate [egfr], and blood pressure). The same procedure was applied to renal survival. Results: Patient age increased with the grades of segmental sclerosis (S) and tubular atrophy/interstitial fibrosis (T) (p < 0.05). egfr decreased significantly with increasing mesangial hypercellularity (M) (p = ), S (p = ), endocapillary hypercellularity (E) (p = ), and T (p < ). MSET differed significantly by sex (p < ). The 24-h urine protein/creatinine ratio increased significantly with the degrees of S (p = 0.036), E (p = ), and T (p = 0.015). The serum creatinine level was significantly higher in patients with T2 than T1 or T0 (p < ). At the time of biopsy, the degree of tubular atrophy/interstitial fibrosis affected the doubling of serum creatinine or end-stage renal disease. However, the electron microscopy findings did not predict the renal outcome. Conclusions: Our study suggests that tubular atrophy/interstitial fibrosis is significantly associated with proteinuria and renal progression in IgAN. 2 - The Korean Journal of Medicine: Vol. 88, No. 4, 서론 IgA 신증은가장흔한일차성사구체질환으로질환의임상경과가무증상의혈뇨부터만성신부전에이르기까지다양하며평생동안신기능저하소견을보이지않는환자가있는반면, 6-43% 는진단후 10년이내에말기신부전으로진행한다 [1,2]. 따라서 IgA 신증환자는질환의중증도를파악하고예후를예측하여적극적인관리와치료를받는것이중요하다 [3]. 이에따라 IgA 신증의예후에영향을미치는인자를확인하기위한많은연구가진행되었고고령, 남자, 고혈압, 하루 1 g 이상의단백뇨, 진단당시의신기능감소등이불량한예후인자로알려졌다 [4-6]. 신조직검사의병리학적분류도예후를예측하는인자로알려졌으며 IgA 신증의질환활성도를파악하는데중요한근거가된다 [7-9]. IgA 신증의병리학적분류는전통적으로 Lee, Haas 및 World Health Organization (WHO) 의분류법이사용되어왔다 [10-12]. 하지만 2009년기존의병리학적분류의기능적제한을넘어서예후를예측하기위한적극적이고새로운분류법이제안되었는데바로 Oxford 분류법이다 [13,14]. Oxford 분류법은 IgA 신증의활성도와예후를예측할수있는병리학적인소견으로 4가지를제시했는데메산지움의세포과다 (mesangial hypercellularity), 분절성경화 (segmental sclerosis), 모세혈관내세포과다 (endocapillary hypercellularity), 세뇨관의위축과섬유화 (tubular atrophy/fibrosis) 이다. Oxford 분류법은현재다양한연구를통해검증되고있으며그유용성에대해서는향후더많은연구가필요한상황이다 [15-24]. 본연구에서는후향적연구를통해기존에는 WHO 분류법으로적용된 IgA 신증환자에대해 Oxford 분류법을적용해보았으며이미알려진예후인자와의상관성, 신장생존에대한예측능력을평가해보았다. 또한기존의분류법에서근거가된광학현미경소견이아닌전자현미경소견을근거로환자를분류한후예후인자와의상관관계를확인하고신생존의예측여부를평가하여 IgA 신증의새로운분류법으로서의가치를확인해보고자하였다. 대상및방법대상 1997년 1월부터 2007년 12월까지연세대학교원주세브란 스기독병원에내원하여신조직검사를통해 IgA 신증으로진단된환자 213명을대상으로하였다. 환자대상군에서임상자료가부족한경우, 병리조직이충분하지않은경우, 슬라이드와검체가보관되지않아재평가가불가능한경우, 헨노흐-쇤라인자색반콩팥염 (Henoch-Schonlein purpura nephritis) 으로진단된경우, 만 15세미만, 환자가조직검사이후추적이되지않은경우인 46명은연구대상에서제외하였다. 방법환자의의무기록과혈액및소변검사결과를후향적으로조사하였다. 또한신장생검조직슬라이드를재검토하여기존에보고된 WHO 분류법에추가하여 Oxford 분류법을적용하였다. Oxford 분류법은병리소견에따라다음과같이나누었다. Mesangial score; 0.5 (M0) 또는 0.5 (M1) Segmental glomerulosclerosis; absent (S0) 또는 present (S1) Endocapillary hypercellularity; absent (E0) 또는 present (E1) Tubular atrophy/interstitial fibrosis; 25% (T0), 26-50% (T1) 또는 50% (T2) 광학현미경검사이외에전자현미경검사도재검토하였는데전자현미경검사는발돌기융합 (foot process fusion) 정도, 사구체기저막두께 (glomerular basement membrane thickness), 전자고밀도침착위치 (electron dense deposit site) 를평가하였다. 발돌기융합정도는범위에따라정상 (normal; < 10%), 국소 (focal; 10-50%), 광범위 (diffuse; > 50%) 로나누어평가했고, 사구체기저막두께는정상 (normal), 불규칙 (irregular), 두꺼워진 (thickened), 얇아진 (thinned) 두께로나누어평가하였다. 전자고밀도침착부위는침착부위에따라메산지움 (mesangium), 내피하 (subendothelium), 상피하 (subepithelium) 로나누어평가했고침착부위가메산지움에만침착한경우도있지만메산지움이외에내피하및상피하에도동시에침착된경우가많아메산지움에국한된경우와메산지움이외의곳에도침착이동시에존재하는경우로나누어분석하였다. 모든현미경적소견의평가는병리의사가분석하였고환자의임상적인정보는제공되지않았다. 그동안추적관찰이되지않은환자들은유선으로환자의상태를파악했고가능한경우본원에내원하여혈액및소

Renal outcome of IgA nephropathy - 변검사를통해현재의신기능과단백뇨여부등을확인하였다. 수집된데이터를통해 WHO와 Oxford 분류법을각각적용해보았으며 Oxford 분류법의경우는각각의항목에대해기존에알려진예후인자 ( 고령, 남자, 단백뇨, 신기능저하, 고혈압 ) 와의상관관계를분석해보았다. 또한연구의종료점 (end point) 은혈청크레아티닌이두배로증가하거나말기신부전으로진행하여이식또는투석을시행한경우로정의하여신생존을조사하였고 Oxford 분류법항목각각에대해신생존을유의하게예측할수있는지확인하였다. 또한전자현미경소견에대해서도각각의조사항목과기존의예후인자와의상관관계를확인하였으며신생존의예측여부도조사하였다. 연구윤리및통계본연구는연세대학교원주의과대학연구윤리심의위원회의승인을받았다. 통계는 GraphPad Prism for Windows, version 5.0 (GraphPad Software, San Diego, CA, USA) 을사용하여분석하였다. WHO, Oxford 분류단계와예후인자의관련성은 two sample t-test, Fisher s exact test, chi-square test, ANOVA test 등을이용하였고사후검증은 Bonferroni 방식으로하였으며, 신생존에대한분석은로그순위검정 (Log rank test) 을이용하였다. 통계학적유의성은 p값이 0.05 미만으로정의하였다. 결과전체대상자의임상적특징, 검사실소견, 및병리학적소견총대상환자의진단당시평균연령은 ± (15-85) 세였으며, 총 213명중남성은 115명, 여성은 98명이었다. 남성 의평균연령은 ± 14.26세, 여성의평균연령은 ± 12.76세로두군간에유의한차이를보였다 (p = ). 수축기혈압은남자 ± 16.1 mmhg, 여자 ± 17.5 mmhg 로두군간에유의한차이가있었다 (p = ). 진단당시 24시간소변의단백은평균 1,455 ± 1,822 mg/day 였으며평균혈청크레아티닌은 1.05 ± 0.55 mg/dl, estimated glomerular filtration rate (egfr) 는 ± ml/min/1.73 m 2 였다 (Table 1). 임상경과에서 25명 (11.7%) 이신부전에이르러투석을하거나이식을하였으며투석중에사망을한 2명을포함하여사망한환자는 7명이었다. 조직검사후에 reninangiotensin-aldosterone system (RAS) 을억제하는계열의약제를투여한환자는 123명 (57.8%) 이었고투여한환자군과투여하지않은환자군의 24시간단백뇨는각각 1,727 ± 1,896 mg/day 와 1,076 ± 1,653 mg/day 로유의한차이를보였다 (p = ). 면역억제제를사용한환자는모두 29명 (13.6%) 이었으며면역억제제를사용한환자군과사용하지않은환자군 Table 1. 4 - 대한내과학회지 : 제 88 권제 4 호통권제 656 호 의 24시간단백뇨는각각 2,258 ± 1,763 mg/day 와 1,337 ± 1,819 mg/day로유의한차이를보였다 (p = ). WHO 분류법에서는 class II가제일많았고 class V가제일적었으며, Oxford 분류법에서는 S0, E0, T0가 S1, E1, T1 및 T2에비해서상대적으로많은수를보였으며 M0보다 M1이더많았다. 또한 Kang 등 [15] 의연구에서보인환자의분포와마찬가지로 WHO score 가증가할수록 M1, S1, E1, T1 및 T2가증가하는분포를보였다 (Table 2). Chi-square test를이용한 WHO 분류와 Oxford 분류의항목과의연관성을분석하였으며 M 항목 (p < 0.001), S 항목 (p < 0.001), E 항목 (p < 0.001), T 항목 (p < 0.001) 간에연관성이있음을보여주었으며 WHO 분류와전자현미경소견과의관련성은모두없었다. 전자현미경소견에따른분포는표 3과같다. 발돌기융합소견은국소범위의병소를보이는경우가가장많았고사구체기저막두께에있어서는불규칙한소견이가장많았다. 전자고밀도침착부위는메산지움에침착된경우가 173명으로제일많았으며, 내피하또는상피하와중복적으로침착되는곳도있었으며순수하게메산지움에만침착되는경우는 115명이었다. 내피하나상피하에만국한적으로침착된경우는극소수였으며각각 5명과 1명이있었다. IgA 신증의예후인자와 Oxford 평가항목에따른비교 IgA 신증의불량한예후인자로잘알려져있는다섯가지요소와 Oxford 평가항목간에연관성이있는지분석하였다. Table 3. S 항목에서는 S1이 S0보다많이검출되었다 (p = 0.036). E 항목도 E1이 E0보다더의미있게검출되었고 (p = ), T 항목의경우에는 T0에비해 T2에서월등히많은단백뇨가검출되는것으로나타났다 (p = 0.015) (Fig. 1). 혈청크레아티닌의경우는 E 항목에서 E1이 E0보다더의미있게검출되었고 (p = ) T 항목에서 T2는 T0와 T1보다높았으며 T1 은 T0보다의미있게높았다 (p < ). MDRD 공식을이용한 egfr의경우에는 M 항목 (p = ), S 항목 (p = ), E 항목 (p = ) 및 T 항목 (p < ) 모두에서유의한차이를보였다 (Fig 2). 혈압의경우는 T 항목에서만수축기, 이완기혈압및평균동맥압이유의한차이를보였고다른항목과는유의한상관관계를보이지않았다. IgA 신증의예후인자와전자현미경소견의비교전자현미경소견과예후인자와의상관성을분석하였다. 그결과발돌기융합정도와사구체기저막두께의비교에서는어떤예후인자와도의미있는상관성이발견되지않았다. 다만전자밀도침착부위에따른분석에서는메산지움이외의부위까지침착을보인경우 (extra-mesangium) 와메산지움에국한하여침착되어있는군 (mesangium) 으로나누어비교했는데성별에서의미있는차이를보였다 (p = ). 신생존의분석 Oxford 분류의측정항목중에서 T (p = ) 항목만이신생존율에의미있는차이가있었고 (Fig. 3), M (p =0.194) 항목, S (p = 0.185) 항목, E (p = 0.518) 항목은통계적인유의성을보여주지못했다. 또한전자현미경소견에따른신생존분석에서발돌기융합정도 (p = 0.553), 사구체기저막두께 (p = 0.483), 전자고밀도침착부위 (p = 0.631) 등은모두유의한차이를보여주지못하였다. Oxford 분류의 MSET 항목의점수 0 과 1 로 1차분류후에단백뇨 1 g/day 이상과미만으로나누어하위분석 (subanalysis) 을했을때신생존율에의미있는차이가모두없었다

신증후군소견을보인환자들의특징신증후군소견을보인환자는모두 25명 (11.7%) 이었으며평균연령 42.30 ± 17.79세, 남자 14명 (46.36 ± 19.48세 ), 여자 11명 (38.27 ± 15.03세 ) 이었다.

M, mesangial hypercellularity; S, segmental sclerosis; E, endocapillary hypercellularity; T, tubular atrophy/interstitial fibrosis. a p < 신증후군소견을보인환자들의특징신증후군소견을보인환자는모두 25명 (11.7%) 이었으며평균연령 ± 17.79세, 남자 14명 (46.36 ± 19.48세 ), 여자 11명 (38.27 ± 15.03세 ) 이었다. 평균단백뇨의양은 5,128 ± 2,686 mg/day를보였고혈청크레아티닌수치가두배로증가하거나말기신부전으로진행하여투석을한경우는모두 8명 (32.0%) 이었다. 진행을한환자군과진행을보이지않은환자군간에 WHO 및 Oxford 분류와전자현미경소견에유의한차이점은없었다. 단지진행된환자에서 1명만이면역억제제를투여받았고진행되지않은환자는 6명에서면역 억제제를투여받았다. 고찰 IgA 신증은무증상의미세혈뇨로부터심한단백뇨와신기능의저하에이르기까지질환의양상이다양하여임상적으로그예후를예측하는것은어려운점이있다. 불량한예후인자로는고령, 남성, 단백뇨, 진단당시의신기능저하, 고혈압등을보편적인예후인자로생각하고있다 [3-6]. 또한병리학적소견을근거로예후를평가하는시도들이있었는데

WHO 분류법에이어서 Oxford 분류법이새로이제안되었으 며 [13,14] 이를이용한연구결과들이보고되고있지만임상적유용성은아직검증단계에있다 [15-24].

WHO 분류법에이어서 Oxford 분류법이새로이제안되었으 며 [13,14] 이를이용한연구결과들이보고되고있지만임상적유용성은아직검증단계에있다 [15-24]. 특히신생존에관련하여 Oxford 분류법의평가항목은연구대상의환자수, 국가또는지역간의상이함, 인종등에의해약간의차이를보이고있어아직은일반화되지못하고있다. Yau 등 [19] 의연구에의하면 T 항목만이신기능의악화를예측할수있는인자로나타났으며 Shi 등 [23] 은신기능감소에 S 항목과 T 항목이독립적인예측인자라고하였다. Kang 등 [15] 은 M 항목, S 항목및 T 항목이단백뇨와연관이있고신기능의감소를예측할수있는것은 T 항목만이예측을하고 E 항목은연관이없다고발표를하였다. Lee 등 [16] 은신생존을예측할수있는항목으로 E 항목과 T 항목이

Renal outcome of IgA nephropathy - 연관이있다고하여국내발표연구에서약간의차이를보였다. 중국인을대상으로한연구조사에서는 M 항목과 T 항목이의미가있는것으로발표되었고 [17], Alamartine 등 [20] 은 T 항목, S 항목및 E 항목이신기능의감소를예측할수있다고하였다. 반면에 Lv 등 [21] 의연구에서 E 항목은신기능의감소와연관성이없는것으로나타났다. 최근의발표에서는 Hass 분류법과 Oxford 분류법을비교하면서 Oxford 항목의 T1과 T2가독립적으로혈청크레아티닌수치를증가시키는변수라고하였으며 [22], 신생존을독립적으로예측할수있는것은 M 항목, S 항목및 T 항목이었으며 M 항목과 E 항목은단백뇨가 1 또는 2 g/day 이상증가하는것과연관이있었고면역억제제의투여가 M 항목, S 항목, E 항목및 T 항목모두의수치를낮추는데기여를하는것으로 VALIGA 연구에서발표하였다 [24]. 본연구에서는 Oxford 분류법을통해기존에알려진예후인자와의상관성을비교했을때혈청크레아티닌은 E 항목과 T 항목에서의미가있었으며수치가올라갈수록의미있는차이를보였다. 반면에 egfr 로환산하여비교한것은 MSET 항목모두유의한연관성이있는것으로나타났다. 특히이번조사에서신기능의악화가진행되지않았던신증후군환자에서 6명이면역억제제를투여받은것으로조사가되었는데 VALIGA 연구에서발표했듯이면역억제제의투여로모든항목 (MSET) 의수치를낮출수있다면단백뇨의관해를유도하고나아가신기능의보존을기대할수있을것으로생각된다. 신증후군을보이는 IgA 신증환자는면역억제제를투여받고관해를보이는경우에는예후가좋은것으로알려졌다 [25]. 본연구에서는신기능이악화된신증후군환자중에 1명만이면역억제제가투여된것은지역적인조건, 시기적인여건등이관여하여적은인원이투여된것으로생각된다. 많은경우에있어서 RAS 억제제를사용하게되는데이번조사에서는혈압이유의한의미를갖지못했으며유럽의 86% [24] 에비해낮은것은환자들이보통증세가없어필요성을느끼지않은것과무관치않을것이며, 혈압약이라는것에거부감을갖는점도있을것으로생각된다. 신생존의예측에있어서는많은연구에서 T 항목이의미를가지는것으로조사되었고다음으로는 M 항목과 S 항목에서의미가있고 E 항목은거의없는것으로나타났다 [26]. 이러한내용은아직정립된것은아니지만 T 항목은다수의연구에서거론되는항목인것을고려한다면제일중요한항 목으로생각된다. 이번연구에서는 Oxford 분류항목중 T 항목만이유의한차이를보여 T 항목의점수가상승할수록예후는좋지않은것으로나타났으며다른항목은모두의미가없었다. 그러나이번연구에서 S 항목과 T 항목은모든예후인자인성별, 연령, 단백뇨, 혈청크레아티닌, egfr 과상관성이있는것을보여주었다. S 항목은사구체의경화증정도를평가하는것이고 T 항목은세뇨관의위축과간질의섬유화정도를보는것으로손상된조직의이차적인변화를보는항목들이라는점을감안하여 S, T 항목이가지는의미는큰것으로생각된다. IgA 신증이아닌다른신장질환의경우에는전자현미경소견과임상양상및예후의관계를보고한연구가몇몇있다 [27,28]. 그러나 IgA 신증의경우에는일부전자현미경소견에대한연구가있으나아직전자현미경소견을예후인자로확인한연구는없는상태이다 [29-31]. Oxford 분류법을포함한기존의 IgA 신증의분류법들은광학현미경소견만을근거로한분류법으로전자현미경적소견은고려하지않고있다. 따라서전자현미경검사의임상적가치는아직확립된것이없는상황이다. 본연구에서는기존의문헌에서신장질환과관련이있는것으로언급된전자현미경소견인발돌기융합, 사구체기저막두께, 전자밀도침착부위를근거로하여대상환자들을분석하였고임상적인예후인자들과의상관성을살펴보았다. 이번분석에서는신생존의예측에있어서는유의한결과를확인할수없었다. 결론적으로 IgA 신증에서전자현미경소견의임상적유용성을확인할수는없었으며따라서현재까지는기존의광학현미경소견에근거한분류법이우선으로고려되어야할것으로생각된다. Oxford 분류법의장점은다른기존의분류법과비교하여세뇨관의변화와간질의섬유화정도를추가하여좀더포괄적인등급을분류할수있는점이라고생각되지만, Oxford 분류법은각항목을총괄한총체적인중증도평가가없는상태로 Oxford 분류법에의한임상양상과예후의예측은각각의항목에따라개별적으로이루어져야하고이러한점은 Oxford 분류의제한점으로생각된다. 그러므로 Oxford 분류법만을이용하여신생존과같은지표를예측하는것보다 Oxford 분류의 MSET 항목과임상적인소견을종합하여분석하는것이필요하고 RAS 억제제의단일요법또는면역억제제의투여에지침이될수있는통합적인점수체계를이용한분류법이필요할것으로생각된다

8 - 대한내과학회지 : 제 88 권제 4 호통권제 656 호 요 목적 : 병리학적소견을바탕으로 IgA 신증환자의예후를예측하고자여러분류들이소개되었다. 최근에소개된 Oxford 분류법과전자현미경소견을이용하여예후인자와연관성및신생존의예측능력을알아보고자하였다. 방법 : 1997년부터 2007년까지 IgA 신증으로진단된환자 213명을후향적으로조사하여병리의사가 Oxford 분류법과전자현미경소견을바탕으로재분류하고연령, 성별, 단백뇨, 신기능저하, 고혈압등과의연관성및신생존율분석에적용하였다. 결과 : egfr은 Oxford 분류법의 M, S, E, T 모든항목과의미있는연관성을보여주었고 24시간소변단백 / 크레아티닌비는 S, E, T 모든항목과의미있는연관성을보여주었다. T 항목만이신생존율과통계적으로의미가있었으며전자현미경소견은신생존율을예측하는능력은보이지않았다. 결론 : 본연구에서는 Oxford 분류법의 T 항목인 tubular atrophy/interstitial fibrosis가단백뇨의정도를잘반영했을뿐만아니라신기능의악화를예측할수있는강력한평가항목임을보여주었다. 중심단어 : IgA 신증 ; 예후 ; Oxford 분류법 ; 전자현미경 약 REFERENCES 1.
