3 원로기고 참 ( 眞 ) 의사의길 ( 醫道 ) 을생각하며 김동집 ( 金東集 ) 가톨릭의대명예교수제 23 대대한혈액학회회장한국과학기술한림원종신회원대한민국의학한림원원로회원 우리학회가창립되던해 (1958년) 에나는의과대학을졸업하고의사가되었으니, 나의의사생활과학회는동갑 ( 同甲 ) 인셈이다. 불과 10여명의회원으로시작된우리학회는회원수가해마다증가하여이제는천여명에달하고, 국제혈액학회세계학술대회 (2002년) 를유치, 개최하는등, 학회의위상이세계적으로높아졌고, 학술분야의양과질에있어서도선진국으로인정받고있다. 진정자랑스럽고대견한일이아닐수없다. 이렇게훌륭한학회가되기까지노력하여주신선배, 동료, 후배회원님들께존경과찬사의마음을드리고싶다. 이와같이자랑스러운대한혈액학회회원의한사람으로내가걸어온과거를돌이켜볼때혈액학분야에서진료, 교육, 연구, 봉사를통해나름대로발전에일조를하였다고생각하게되어가슴뿌듯하고의사가되어혈액학을선택한것이최선의길이었다고자부하고있다. 그런데작금에이르러 醫道의倫理 ( 의도의윤리 ) 문제가빈번히비판의대상이되고있어, 이런화제가뜰때마다가슴이아프고내자신을다시돌아보게되곤한다. 의과대학을졸업하고의사가되어의성 ( 醫聖 ) 히포크라테스 선서를하는엄숙한의식을통해내가배운의술을통해서인류사회에성실하게봉사하겠다는다짐을하고의사의길 ( 醫道 ) 을걸어왔다. 현대화의물결을따라날로복잡, 다양해지는사회생활에따른의사의의식구조, 사고방식, 가치관등의변화로醫道의근본에도변화가올수도있다고생각되나, 양의東西와시대의고금을막론하고 히포크라테스의선서 를통해강조되고있는 생명의존엄성과의사의사명감 즉참다운의사의길의근본에는변함이있어서는안된다고생각한다. 참다운醫道란고매한철학적이론이나추상적인개념같은어려운것이아니고지극히평범하고일상생활속에서부여된천직으로서의진료임무를성실하게수행하는가운데서찾아볼수있는것이라고나는생각한다. 의사들은환자를질병에서구제한다는숭고한정신자세와기술을몸에익힌전문가라는것을무한한자랑으로여기며천직으로생각해야된다. 자기의지식과기술에의해환자의고통을제거해줄수있었을때의가슴벅찬기쁨이란진정이것을실제로체험하지못한사람은도저히이해할수없으리라생각한다. 4 e-newsletter 그러나이러한목적을달성하기위해서의사는평소무거운강박관념에짓눌리며노력하지않으면안된다. 주야를가리지않고때로는식음을잊으면서동분서주하는것은오직인명을구하기위한사명감때문이지, 그밖의어떠한보수를바라는것일수는없는것이다. 질병으로괴로워몸부림치는사람이남이아닌자신이라는생각에서, 정성스럽게환자의육체적고통을제거하고건강을회복시켜천수를누리게하는것을, 의사는자신의임무로삼아야한다. 이처럼정신적인면에서나사회적인면에서나변함없이환자를지키고구제한다는마음으로시종일관하는태도, 즉인간적인도리를철저히하는것이바로참醫道가아닌가하고나는생각한다. 올바른의도를걷기위하여항상새로운의학지식과기술을연수하여야하는것은물론인간성자체도수양이되어야하겠다. 옛말에 명의는나라의병도고친다. 는말이있다. 이말의뜻을나는 명의가되려면폭넓은사회적견식도아울러지녀야된다. 는것으로해석한다. 이러한醫道를구현하기위하여꾸준히노력하는의사가참다운 醫道의倫理 를이해하는의사라고생각된다. 아무리훌륭한醫道를지키는진료를시행하고있어도환자나주위사람들이이를바로받아들이지않는사회풍토가만연되고있는것이안타깝기도하다. 이런불행을미연에방지하기위해서무엇보다시급한것은의사와환자사이에가로놓인불신을씻고, 신뢰받는인간관계가이루어지도록한가지한가지해결해나아가야할의무도우리의사들의몫이라고생각한다 년 10 월 대한혈액학회회장김동집 4

5 춘계학술대회보고 2015 년제 56 차대한혈액학회춘계학술대회 - 일시 : 2015 년 5 월 29 일 ( 금 )~30 일 ( 토 ) - 장소 : 서울그랜드힐튼호텔 대한혈액학회 ( 회장서종진서울아산병원소아과, 이사장김선희삼성서울병원진단검사의학과김선희 ) 가 2015 년 5 월 29 일 ( 금 ) 부터 30 일 ( 토 ) 까지양일간서울그랜드힐튼호텔에서 800 여명의회원들이참석한가운데 40 여명의국내외혈액학분야석학의강의를통해혈액질환의진단과치료, 연구에관한최신지견을나누었고, 엄선된 70 여편의구연과 60 여편의포스터연제의발표를통해심도있는학술의장을열었다. 지난춘계학술대회는최근관심이집중되고있는 cell therapy ( 세포치료 ) 에대한집중적이고심화된학문을공유할뿐아니라기초학문과중계연구를활성화하기위한자리가마련되었고, 대한혈액학회산하연구회들의연구성과와최신지견을나누는시간이되었다. 1 Plenary session, 1 Special lecture, 4 Scientific Session, 3 Education session, 연구회 Working party session (6 sessions, 23 lectures), Oral presentation(9 sessions, 73 presentations), Poster presentation (66 presentations) 및 2 Luncheon symposium 으로진행되었다. 해외석학과의학문적교류 Plenary session 은이식학분야의최고권위자인 Wing Leung 교수 (St. Jude Children s Research Hospital, USA) 가 Recent advances in mismatched related transplantation and cellular therapy 에관한특강을, special lecture 는 Antonio Almeida 교수 (Instituto Português de Oncologia de Lisboa, Portugal) 가 Impact of recent genetic discoveries on MPN management 에대한강의가있었다. 6 e-newsletter 기초및임상의학문적교류의극대화 4 개의 scientific session 과 3 개의 education session 을통해기초부터최신임상연구에이르는 22 개의강의가열렸다. Scientific session 은 1) Basic science 세션에서중계연구의활성화를위해대한면역학회와함께기초의학자들과임상의학자들이교류의자리가마련되었고, 2) Thrombotic disorders( 혈전성질환 ) 세션에서최근이슈가되고있는새로운혈전치료제 (DOACs) 의치료와모니터링, 출혈관리를, 3) Immunology( 면역학 ) 세션에서혈액질환들의최신면역학적배경을, 4) Lymphoma 세션에서 extranodal NK/T cell lymphoma( 결절외 NT/T 세포림프종 ) 의생성과치료, 그리고미래에관한강의가열렸다. 대한혈액학회의산하연구회들의연구성과의공유다발성골수종연구회, 성인급성림프모구백혈병연구회, 혈우병연구회, 급성골수성백혈병 / 골수형성이상증후군연구회, 정맥혈전증연구회, 조직구증식증연구회등 6 개연구회에서 registry progress( 환자등록사업절차 ), study proposal( 연구제안 ), 최신지견및다양한연구성과를발표하였다. 대한혈액학회활성화및소통을위하여 ~ 2015 년 1 월새롭게취임한김선희이사장이하현집행부는학회의정책적상황, hot 이슈, 보험관련소식과더불어학회회원들과원활한의사소통위해 e-newsletter 를발행하기로하였다. 적극적인대외홍보를통해학회소식을알리고, 다양한내부의견수렴을통해학회의내실있는발전을도모하기로하였다. 학회산하연구회활성화에따른성과를현실적인제도및정책개발에도입하여급변하는의료환경에대처하기로하였다. 6

8 e-newsletter 초대의글 존경하는대한혈액학회회원여러분! 대한혈액학회회장서종진 대한혈액학회이사장김선희 제56차대한혈액학회추계학술대회가 2015년 11월 6일부터 7일까지양일간대구인터불고호텔에서개최됩니다. 이번추계학술대회는 presidential symposium과 KSH/EHA joint symposium을비롯해다양한학술프로그램에서혈액학분야의국내외석학을모시고, 혈액질환의임상및연구에대한최신지견을함께나누고자합니다. 추계학회에서처음으로열리는 presidential symposium에서는 hemophagocytic lymphohistiocytosis (HLH) 를주제로국내외석학들을모시고관련최신지견이나눠질예정이며, 유럽혈액학회와활발한교류를통해진행되고있는 KSH/EHA joint symposium에서는양학회의다발성골수종분야세계적권위자들이모여활발한강의와토론이이루어질예정으로, symposium 이후관심있는회원들과함께양학회의연구교류모임이개최될것입니다. Scientific session에서는 molecular approach of hematologic diseases, alternative donor HSCT, myelodysplastic syndrome, thrombohemorrhagic disease 등최근혈액학분야에서가장관심이모아지고있는분야에대한심도있는학술의장이열릴예정입니다. 또한급격히발전하고있는혈액질환의진단기법과흔하지는않지만임상에서언젠가접할수있는중요한만성혈액질환에대한 education session을마련하여, 임상적지식과이해의폭을넓힐수있는유용한내용의강의를준비하였습니다. 또한이번추계학술대회에서도대한혈액학회의자랑인산하연구회들의연구성과와관련최신지견을나누는시간을마련하였습니다. 12개연구회중재생불량빈혈연구회, 골수증식종양연구회, 유전용혈빈혈연구회, 림프종연구회, 만성골수성백혈병연구회, 소아급성림프모구백혈병연구회등 6개연구회에서각연구분야의최신지견과연구성과, 연구제안등의알차고유익한내용을담은세션을준비하였습니다. 많은관심과참석을부탁드립니다. 늦은가을, 한해를정리하며최신의학문적지식과함께반가운회원들간의즐거운만남이있는소중한시간을대구에서가져보시길바랍니다. 회원여러분들의많은관심과적극적인참여를통해대한혈액학회가더욱발전할수있는시간이될수있도록함께하여주시길바랍니다. 추계학술대회장에서뵙겠습니다 년 10 월 대한혈액학회회장서종진 이사장김선희 8

12 e-newsletter 보험소식 1. 항암제요양급여기준변경사항 (2015 년 3 월 12 일 년 9 월 12 일 ) 가. 골수형성이상증후군 azacitidine 및 decitabine 단독요법투여대상확대 고시일시 : 2015 년 5 월 1 일 azacitidine 주사제 ( 품명 : 비다자 100mg 현탁주사용분말 ) Bone marrow blasts 가 20% 이하인골수형성이상증후군 (MDS) 으로확진된환자로서다음의가, 나중한가지이상을만족하는경우 - 다음 - IPSS(International Prognostic Scoring System) Risk category 분류에따라 가. IPSS Low 또는 Intermediate-1 인환자로서다음중 1 가지이상에해당하는환자 (1) 빈혈이 erythropoietin 또는 antithymocyte globulin(atg) 에반응하지않는경우 (2) 절대호중구수 1,800/ mm3미만의호중구감소증이있는경우 (3) 혈소판수 100,000/ mm3미만의혈소판감소증이있는경우 나. IPSS Intermediate-2 또는 High 인환자로서다음중 1 가지이상에해당하여동종조혈모세포이식을시행할수없는경우 (1) 고령 (50 세이상 ) (2) 활동도가좋지않은경우 : ECOG 수행능력평가 (PS: Performance status) 2 이상 (3) 장기기능장애로인하여동종조혈모세포이식의금기증에해당하는경우 (4) 적절한공여자가없는경우 decitabine 주사제 ( 품명 : 다코젠주 ) Bone marrow blasts 가 20% 이하인골수형성이상증후군 (MDS) 으로확진된환자로서다음의가, 나중한가지이상을만족하는경우 - 다음 - NCCN Practice Guidelines 에의한 IPSS(International Prognostic Scoring System) Risk category 분류에따라 가. IPSS Intermediate-1 인환자로서다음중 1 가지이상에해당하는환자 (1) 빈혈이 erythropoietin 또는 antithymocyte globulin 에반응하지않는경우 (2) 절대호중구수 1,800/ mm3미만의호중구감소증이있는경우 (3) 혈소판수 100,000/ mm3미만의혈소판감소증이있는경우 나.
