항정신병약물병용요법 높은사망률 과의연관성도 제기되고있어 병용요법의이론 및임상적 근거와 실제 부작용에 대한 정확한 이해가 필요하다 따라서본고에서는 병용요법의 사용배경과 근거를살펴보고현재까지 임상연구를 통해 나타난 병용요법의 효과와 부작용을정리해 보고자한다 이를위해현재까지

This article reviews the evidence of antipsychotics polypharmacy and summarizes advantages and disadvantages shown in clinical trials. Clinicians choose antipsychotics polypharmacy to control the positive and negative symptoms more effectively especially in treatment resistant patients or to reduce adverse effects. There are some theoretical possibilities that antipsychotics polypharmacy affects a broader range of receptors, enhances D 2-receptor blockade and optimizes pharmacokinetic effects. Clinical evidence suggests that clozapine co-administered with risperidone, sulpiride, or amisulpride reduces psychotic symptoms in treatment-resistant patients and that aripiprazole with other antipsychotics reduces metabolic side effects. On the other hand, antipsychotics polypharmacy is associated with problems such as dose-dependent side effects, metabolic problems, increased mortality and treatment cost. Considering pros and cons, antipsychotics polypharmacy must be started after close scrutiny of the patient s medication history not just by clinical judgment. Also, changing the regimen from polypharmacy to monotherapy should be considered as a reasonable option to schizophrenic patients in stationary status. Received: July 15, 2013 Revised: August 28, 2013 Accepted: September 24, 2013 Correspondence author: Chan-Hyung Kim, MD, PhD Severance Mental Health Hospital, Yonsei University College of Medicine, 119 Gyeongchung-daero 1926beon-gil, Gwangju , Korea Tel: , Fax: , spr88@yuhs.ac 비정형항정신병약물의 등장이후조현병 치료에있어두 가지이상의 항정신병약물을 동시에처방 받는환자의 비율은점차증가하고 있다 임상에서 병용요법의 사용빈도는 조사시기와 방법그리고 지역에따라 다양하지만 미국의 경우대략 많게는 에이르고 유럽의 경우에도 에달한다 아시아에서는 개국을 조사한결과 평균적으로의 환자들이 병용요법을 처방받고 있다고 하며한국의 대학병원에서 실시된 연구현황에 따르면년 외래환자들의 병용요법처방비율은 이상이었다 또한 병용요법을사용하는 기간도짧지않아 등 은명의환자들을년 동안관찰한 결과의 환자들이일 이상병용요법을 처방받고있다고 보고했다 그러나임상 현장과는 달리임상치료 지침들은 여전히 항정신병약물의 병용요법의 사용에 대해소극적인 입장이다 한 국형조현병 치료지침 이나 의조현병 치료지침 에서는치료의 처음두단계에서 항정신병약물을 단독으로 사용하고 세번째단계에서- 을 사용한후에도 효과가부족한 경우마지막 방법으로병용요법을 시도해볼것을 권유한다 년- 의치료지침 은치료저항성환자들에게도 단독요법사용을 권유하고병용요법에대해서는 추가적인 논의가 필요하다고 언급하였다 다만 과 의병용에있어선 무작위대조군 연구의증거는없으나 경험적으로효과를 기대할수있는 치료방법 이라고제시하였다 또한- 치료지침 에서는 약물의 교체기간이외에는 두가지항정신병약물을 동시에사용하지 않을것을권고하고 있다 이처럼 조현병 환자들에게 병용요법의 처방근거는 아직 부족한수준이며 부작용 및치료비 증가 그리고 Copyright c 2013 대한정신약물학회 137

2 항정신병약물병용요법 높은사망률 과의연관성도 제기되고있어 병용요법의이론 및임상적 근거와 실제 부작용에 대한 정확한 이해가 필요하다 따라서본고에서는 병용요법의 사용배경과 근거를살펴보고현재까지 임상연구를 통해 나타난 병용요법의 효과와 부작용을정리해 보고자한다 이를위해현재까지 시행된항정신병약물 병용요법의 효과를 비교한 무작위 대조군 연구들을정리하였고 최근부각되고 있는병용요법 사용의문제점과추후의사용방안을 고찰해보았다 일반적으로 병용요법 이란한 환자에게 여러종류의약물을 함께사용하는 것을총칭하여 말한다 여러 논문과 문헌들에서 병합하는 방법이나 약물의 종류에 따라 병합요법 부가요법- 등다양한 용어들을 사용하여 병용요법을 불러왔다 본고에서는 한환자에게 한가지의항정신병약물을 사용하는 것을단독요법 으로 동시에 두 가지이상의 항정신병약물을 사용하는것을 항정신병약물 병용요법 으로통칭하여 쓰기로 한다 항정신병약물 병용요법이 사용되는첫 번째이유는 단독요법으로충분한 치료효과를얻지 못한치료저항성 조현병환자를치료하기 위함이다 항정신병약물 치료에충분한 효과가 없는경우 을사용해볼수있지만 환자의 은부작용으로 사용을중단하거나 충분한용량의 을 복용한후에도 양성증상이 관해되지 않는다 항정신병약물 병용요법은이러한 치료저항성환자들에게 한가지치료방법으로 선택될수있다 임상의들에게 항정신병약물병용요법을 사용하는이유에 대해질문한설문조사 에서임상의들은 양성 및음성증상 을 효과적으로 조절하기위해 병용요법을 선택한다고 응답하였다 또한영국의개 병원의 입원환자들의 처방에 대한조사 에서도 단독요법으로 증상조절에 실패한 경우대안으로 병용요법을 사용하는경우가 가장많았다 항정신병약물 병용요법은두 번째로폭력성을 보이는환자들에게 약물의효과를 빠르게얻을필요가있는 경우선택될 수있다 비정형항정신병약물로 치료받는환자들에게 약물의효과가 나타나기전 정형항정신병약물을 병용할 경우폭력적인 행동과 초조감을 조절할 수있다 특히 은공격적행동을 감소시키고 다른항정신병약물에 비해자살 예방효과가 뛰어나 자해및 타해의위험성이 높은환자들에게 을병용해볼수 있다환자의 젊은나이 조증심한 공격성이높은 병용요법 사용비율과 연관성이 있다는 점 과병용요법이 일반적인외래환자들보다 단기입원병원에 입원한환자나 수감중인 환자들에게 더많이 사용된다는 점은병용요법이 급성기의 폭력적 행동이나 초조감을 조절하기 위해사용되고 있음을 시사한다 세번째로병용요법은 한가지약물을 과량으로사용함으로써 발생하는 부작용을 줄이기 위해사용된다 앞서 언급된병용요법을 처방하는이유에 대한설문조사 에서임상의들은 정신병적 증상조절 이외에도 항정신병약물의총용량을줄이거나 추체외로계 부작용을줄이기 위해 병용요법을 선택한다고 응답하였다 약물부작용의 증가는환자들의 약물순응도를 낮추는가장중요한 요인중 하나이며 환자들의 삶의질을 낮춘다 항정신병약물의 부작용은 병용요법의 사용여부보다 한환자에게 투여되는 항정신병약물의 총용량과 더관련있을 수있다는 점 은병용요법을통해 부작용을줄일 수있는가능성을 제시한다병용요법을통해약물의 총용량을줄이는 것이외에도병용하는 항정신병약물의 특이한수용체 친화성을이용하여 부작용을 줄이기도 한다예를 들어 수용체부분 효현제인 은 과병용할 경우 고프로락틴혈증을 호전시킬수있다 네번째로정신병적 증상을제외한다른 부가적인증상을 조절하기위해 병용요법이선택될 수있다 은가장많은 빈도로병용되는 항정신병약물 중하나로 연구자들은 이항정신병효과 이외에 불면및불안감소 효과를위해 병용되었을 것으로판단하였다 마지막으로 첫번째항정신병약물로 만족할만한 효과를얻지못해다른 약물로교체시 일시적으로 두약물이 병용될 수있다 항정신병약물 병용요법의 사용근거로 가장많이 언급되는것은 수용체의 점유율을 높일수 있다는 것이다조현병의 병태생리를 설명하는 이론중 가설에서는양성 증상의원인으로 기능의이상조절을 제시하고있다 현재까지승인된 모든항정신병약물이 - 수용체를 차단하고 약물들의 상대적인 임상적강도가각약물의 수용체의 친화도와 거의비슷하다는 것 은 가설을 지지하는 강력한 임상적 근거가 될수 있다 이를고려할 때항정신병약물을 병용하여 선조체 수용체의 차단효과를 증가시키는 것은약물의 임상적 효과를증가시킬 수있는가능성이 있다양전자방출촬영- 연구결과 항정신병약물이 임상적효과를 갖기위해서는 이상의 수용체점유 138 대한정신약물학회지 2013;24:

3 장진구등 율이필요하다고 알려졌다 이를고려할 때낮은 수용체 점유율을갖는 혹은 과 을병용할경우 수용체 점유율을 높여 추가적인 효과를 기대할 수있다 두번째로 항정신병약물을 병용함으로써 수용체를제외한 여러수용체를 동시에차단하거나 항진시킬 수 있다 예를들어 이나 단독요법으로 치료받는환자들은 충분한 혹은 수용체차단 효과를갖지만 수용체 차단효과는부족하기 때문에 이나 을함께사용하여그 효과를보완할 수있다 수용체 효현 효과를 갖는 은세로토닌 재흡수를 막는항우울 효과가인정되고 있어다른 항정신병약물과 병용함으로써 추가적인 이득을 얻을수 있다 또한 부분효현제인 은 수용체를 차단하는 다른항정신병약물의 사용으로 인해발생한 고프로락틴혈증을 호전시킬 수있다 세번째로 병용하는 항정신병약물들이 서로의대사과정에 영향을미쳐약물의 혈중농도가변화되어 효과가나타날 수 있다 알려진 것처럼 대부분의 항정신병약물의 대사과정에 효소가관여하며 이나- 은 이의길항제 혹은효현제로 다른약물을 함께복용할경우그 약물의대사속도를 변화시킬수 있다 정형 항정신병약물 중 유도체인 역시 등의대사에 관여하는 에 길항효과가 있어 병용할경우 약물의농도가 변화할수 있다비정형항정신병약물의 경우현재사용되고 있는대부분의약물들은효소활성에영향을미치지는않는다 그러나인체에서 약물의대사과정은 완전히예측할 수 없어 과 을함께복용하였을 때- 의농도가 증가하였다는보고들 도있으며를 통해대사되는을복용하고있는환자에게- 길항효과를 가진 을병용했을때 알려지지 않은대사경로가 활성화되어 의청소율이 증가하는 경우도 있다 이처럼 항정신병약물의 병용이 약물 대사과정에 변화를줄 수있다는 것을고려할 때병용요법을 통한약동학적 변화가병용요법의 근거가될수 있다 앞에서기술한 바와같이임상의들은 다양한배경과 근거로항정신병약물의 병용요법을 사용하기 때문에 병용요법의 효용성을한 가지측면에서만 평가할수는 없다또한 항정신병약물 병용요법의 효과를 평가한 연구중 신뢰할 만한무 작위대조군연구는 그수가많지 않으며그 연구들도- 과다른 항정신병약물의 병용요법의효과를 관찰하였다는제한점이 있어병용요법의 효과를이분법적으로 규정하기는힘들다 등 은항정신병약물의 단독요법과 병용요법의효과를 비교한개 연구에대한 메타분석을 실시하여질병의급성악화기나 과의조합일 경우연구기간이주 이상이었던 경우 치료유지면에서 단독요법보다 병용요법이 유리할 수있음을 보고하였다 그러나 저자들은 긍정적인결과가 나온경우들은 특정국가에서 시행된연구들이많고병용요법의 부작용에대한 고려가부족하여 병용요법의효용성을 인정하기에는 한계가있다고 언급하였다- 등 은병용요법의 효과에대한 연구들을 고찰한결과각연구들의 대상환자 수가적고 비교하려는 결과가다양하며연구 설계의제한점으로 인해얻을 수있는 정보의질이 떨어져 병용요법의 효과를체계적으로 종합하여 설명하는 것은불가능하다고 결론지었다 따라서병용요법의 효과를 확인하기 위해서는 각조합의 연구결과들을 개별적으로 살펴보는것이필요하다 clozapine 임상의들은 단독요법으로 충분한효과를 얻지 못한치료저항성 환자들에게 주로 그리고 등을병용하며 이는앞서 이야기된것처럼 의 부족한 수용체점유율을 보완할수있다는 이론적 근거를 가지고 있다 현재까지 과다른 항정신병약물과의 조합의효과를 살펴보기위해 시행된무작위대조군 연구로는 - 그리고 과의조합에 관한연구들이 있다 무작위연구들의 결과는 과 에정리되어 있다 과 조합의 효과를 비교한 가지연구 의결과는각각 달랐다 등 은개월간 이상의 을사용하여도 정신병적 증상이 지속되는 환자명을 대상으로 과위약을무작위로 병용하여 주간관찰한결과을투여한환자들의 이하 총점과 양성증상점수가 유의하게 호전되었음을 관찰하였다 그러나 - 을 병용한환자들의 사용량이위약을 사용한 환자들보다 이상많았으며 병용요법을사용한 명의환자들 중명은 단독요법을 사용한과거력이없었다는 점은해석에 고려가필요하다 등 은 에부분반응 을보인명의 환자들을대상으로 위약과 을추가하는 연구를시행하였으나 오히려위약을 사용한 환자들의

6 항정신병약물병용요법 른항정신병약물의 병용요법을 사용하는 것은그효과가 충분히입증되지 않았다고보고하였다 그러나항정신병약물로 인한부작용을 줄이거나정신병적 증상외의 기타증상을 조절하기 위해병용요법을 사용하여 긍정적인결과를 얻은연구들 이있다 의경우독특한 작용기전으로 과병용하였을 때와같이 혈중프로락틴 수치와대사표지자를 호전시켰다 등 은 이나 을사용하는 명의환자들에게 을추가할경우 체질량지수나 콜레스테롤수치에는 변화가 없었지만 혈중프로락틴 농도가 감소한다고 보고하였다 등은 을복용하는명의 환자에게 을병용하여 체중과 가감소하였다 초기에 비정형항정신병약물 사용하는 환자들에게 - 을병용하는 것이정신병적 증상을 악화시킨다는 보고 도있었으나 최근실시된 무작위대조군 연구 에서 정신병적증상에는 차이가없었다 병용요법을 경계하는 첫번째 이유는 병용요법이 비정형 항정신병약물의 비정형성을없애고 결국부작용을 증가시킬 수있기때문이다 항정신병약물 병용요법이유발할 수있는 부작용에 대한임상 연구들은 비대조군 연구이거나 관찰연구 등이대부분이지만 부작용의 증가와 연관성이 있다는 연구 결과는일관적이다 등 은항정신병약물 병용요법과연관된 부작용에 대한연구들을 고찰하여 병용요법은 추체외로부작용 항콜린성약물 사용의증가및 혈중프로락틴농도 증가의 위험성이 있다고 언급하였다 항정신병약물 병용요법은 약물의과량사용을 유발할수 있으며이로 인해 고용량의항정신병약물이 유발할수 있는부작용을 증가시킬 수있다 연구에 참여한 명의환자들을 관찰할결과 정형항정신병약물을 사용하거나 항정신병약물 병용요법을사용한 환자들은 항콜린제제의 사용이많았다 병용요법으로 인한혈중 프로락틴 농도의 증가는 과 수용체결합력이 높은 이나 를병용할경우뿐만 아니라 이나 등혈중프로락틴농도에 큰영향을미치지 않는약물의조합에서도증가한다는 보고 가있다 이외에도 비정형항정신병약물의대사성 부작용과 연관되어 병용요법이 체중증가와 당뇨병의유병률을 높인다는보고도 있다 항정신병약물 병용요법은 또한환자들의 높은사망률과 연관성이있다는 연구도있다 조현병 환자들은 항정신병약물이 유발하는심혈관계 이상과식습관의 변화높은 물질사용자 살등으로일반인에 비해사망률이 높다 등 은명의 환자를년간 관찰한결과 항정신병약물 병용요법과생존율의 감소의 연관성을 확인하였으며 사용하는 항정신병약물의 개수가 늘어나는 것은높은 사망률과 연관이 있다는보고 도있다그러나 등 등 이보고한 대규모 코호트연구 결과에 따르면 항정신병약물을병용하여사용하는 것은사망률과 관계없었다 항정신병약물병용요법과 사망률에 대한연구결과는 다르지만 비정형항정신병약물이 대사질환을 증가시킬 수있다는 점을고려할때 특히심혈관계 질환및 대사증후군을 가진환자들에게병용요법의 사용은더욱 주의깊게선택되어야 한다 세번째로 항정신병약물의 병용요법을 사용할 때치료비가상승할 수있다는 점을고려해야 할필요가 있다병용요법은항정신병약물뿐만 아니라다른 종류의약물사용을 늘려 많은비용이 소모될수 있다 비용의측면에서 항정신병약물의효율성을평가하는 일은쉽지않다약제비뿐 아니라 환자가 적절한 약물치료를 받지못해 직업을 잃거나 응급실을방문혹은 입원하게되었을 경우소모되는 비용도고려해야하기 때문이다 등 은년간 항정신병약물 병용요법을사용하는 환자들과단독요법을 사용하는환자들의치료에 필요한총 금액을비교한 결과병용요법이 높은 치료비와연관되어 있다고보고하였다 우리나라의 경우국민건강보험 요양급여기준에 따라품목의 처방투여로는 치료효과를기대하기 어렵다고의학적으로 인정되는경우에 한하여병용 처방의보험 급여를인정하고 있어 병용요법 사용을통한의료비 증가를경계하고 있다 항정신병약물 병용요법의 사용에 있어문제가 되는것은 병용요법의선택이 계획적으로 이루어지지않는다는 점이다 등 이일본의개 병원에서 외래치료를 받는조현병환자들의 처방변화를 조사한 결과 약물을 변경한 환자의는 충분한 용량과 기간동안 항정신병약물을 사용해보지않았다 정신과 의사들이 임상치료지침에 대해가지는회의적인 태도가병용요법의 사용빈도를높이고 병용요법을많이 사용하는치료자들의 임상경험이 적게사용하는치료자들에 비해더 길었던점을 고려할때 병용요법이 치료자의 임상경험에 의존하여 사용되는 경우가 있음을 추측할수있다항정신병약물을 필요시마다 사용하는것도계획적이지 않은병용요법의 사용을증가시킨다 항정신병약물을으로 사용하는 것은처방 당시에는 단기간의 142 대한정신약물학회지 2013;24:

7 장진구등 행동조절을 위해사용되지만 치료효과를 유지하기 위해장기간의 사용으로 변경되는 경우가 있다 사용은 병동 간호진들의요구에 큰영향을받고 처방을시작할 당시이전에약물에 대한고찰이나 부작용 치료의 목표반응에 대한 평가가부족한 상태에서 사용되는경우가 많다또한 약물교체중환자의 상태가호전되면 환자가다시 악화될것을 걱정하여계획과 다르게병용요법을 유지하는 경우도적지 않다 항정신병약물 병용요법을 합리적으로 사용하기 위해서는사용하기 전그필요성뿐만 아니라약동학 약력학적 이론과임상적 효과 안전성에 대한고려가 필요하며 경험에만의존한 사용이나 비 계획적인 사용에 대한 경계가 필요하다 임상에서 이미항정신병약물 병용요법을 처방받고 있는 환자들의약물 치료방향에관한 문제또한논의가 필요한부분이다 치료자들은 항정신병약물 병용요법을 장기간사용하는환자에게서 다른한가지 약을중단하지 않는경향이 있다 그러나 항정신병약물 병용요법을 단독요법으로 바꾸는것의 효과를 관찰한 연구들을 살펴보면 등 이 명의환자들에게 병용요법에서 단일요법으로 바꾼결과 는증상이 악화되었지만 는변화가 없었으며에서는오히려증상이 호전되었다 또한최근항정신병약물 병용요법을사용하고 있는안정기 환자명을 대상으로개월간단독요법으로 변경하는무작위 대조군연구 에따르면 비록병용요법을 유지하는군이 한가지약물을 끊는군보다 치료가유지되는 비율이더 높았으나 는성공적으로 단독요법으로 교체하였고 정신병적 증상의악화 없이 체중이 감소하여 병용요법을 사용한 환자들의 처방을 단독요법으로 다시교환할 수있는 근거가 제시된 바있다 등 은병용요법을 단독요법으로변경한 연구결과들을 고찰한 결과 병용요법의 여러가지 부작용들을 고려하였을 때병용요법을 사용하는 안정기 환자들의 처방을 단독요법으로바꾸는시도가 긍정적일수 있다고언급하였다 임상에서항정신병약물 병용요법은 여러가지목적으로 사용되기때문에 효과에대한 평가또한다면적 개별적으로 이루어져야한다 치료에실패한 치료저항성환자들에게 나 을병용할 경우양성증상이 호전되었다는 보고들이 있으며 을추가하는것은 대사성부작용을 줄일수있어 다른항정신병약물과병용할 수있는가능성이 있다그러나 병용요법은 전반 적으로 추체외로 부작용 및 치료비 부담을 가중시키고 사망 률의 증가와도 연관되어 있어 선택에 주의를 기울여야 한다 병용요법은 치료자의 경험에만 의존하기 보다 단독요법의 효과에 대해 충분히 고찰한 후 환자의 개별적인 특성까지 고려하여 사용되어야 한다 또한 임상의들은 항정신병약물 병용요법의 단점을 염두에 두고 증상이 호전된 이후 단독요 법으로 바꾸려는 시도를 고려할 필요가 있겠다 항정신병약물 병용요법은 조현병 임상치료지침들의 소극 적 태도에도 불구하고 임상현장에서 높은 빈도로 사용되고 있다 이 종설에서는 항정신병약물 병용요법의 사용배경과 이론적 임상적 근거를 살펴보고 최근 병용요법 사용의 문제 점을 정리하였다 임상의들은 치료저항성 조현병 환자들의 증상을 조절하거나 한 가지 항정신병약물을 사용하며 발생하 는 부작용을 조절하기 위해 병용요법을 선택하는 경우가 많 다 병용요법의 이론적 근거로는 각 항정신병약물은 특징적 인 수용체 길항 및 효현 효과가 있어 병용요법을 통해 수 용체를 비롯한 여러 수용체 활성을 변화시킬 수 있다는 점과 약동학적 변화를 일으킬 수 있다는 점 등이 있다 몇몇 무작위 대조군 연구들에서 에 저항을 보인 환자들에게 - 과 혹은 를 병용하는 것의 긍정적 효과가 보고되었으며 의 병용을 통 해 대사성 부작용을 줄일 수 있는 가능성이 있다고 알려져 있다 그러나 병용요법은 항정신병약물의 총 용량 증가에 따 른 부작용이나 대사성 부작용을 증가시키고 사망률 증가와 관련이 있다는 보고도 있다 따라서 항정신병약물 병용요법 을 적절하게 사용하기 위해서는 환자의 과거 약물 사용력 등 을 신중히 검토하여 필요성을 다시 확인하고 명확한 사용 목표를 세워야 한다 또한 병용요법을 사용하고 있는 안정기 환자들에게서 단독요법으로 약물을 변경하는 것은 합리적인 시도일 수 있다 중심단어 항정신병약물 병용요법 조현병 Acknowledgments 본 논문의 요지는 년 월 일까지 인도네시아 발리에서 개최된 제 차 아시아조현병학회 - 의 기조심포지움 에서 발표되었음 REFERENCES 1.
