Methods : We searched pubmed for foreign journals with the title/abstract 'tinnitus, acupuncture' published in the last 10 years, and for domestic journals we searched NDSL, RISS, KISS, The National Assembly Library, The Journal of Korean Medicine Ophthalmology and Otolaryngology and Dermatology with the same conditions as foreign journals. Results : We selected total 10 papers: 7 RCTs, 1 RCT protocol, 1 clinical trial, 1 Systematic Review. In RCT, all studies showed positive effect, however in comparison with the control groups, only 2 studies showed meaningful effects. Conclusions : Positive effect of the acupuncture on tinnitus is expected, but because of several limits observed in many papers, we can't draw defined conclusions. Further well-designed researches are needed. 2 이규영외 1 인 : 이명의침치료에관한최근임상연구동향 Ⅰ. 서론 Ⅱ. 연구방법 한의학적으로이명은聲源이없이耳竅내에서자각적으로鳴聲하는증상을특징으로한다 1). 자각적이고울리는소리가다양하여風聲, 汽軸聲, 潮聲, 蟬聲, 鼓聲, 雷聲, 蚊噪聲, 蟻鬪聲등과같은여러명칭으로도불린다 1). 서양의학적으로이명은 Tinnire 라는라틴어에서유래한단어로 귀에서들리는소음에대한주관적느낌 을의미한다. 즉외부로부터의특이한청각적자극이없는상황에서도소리가들린다고느끼는상태이다. 모든사람의약 95% 가완전히방음된조용한방에서도 20 db이하의이명을느끼는데이런경우는임상적으로이명이라하지않고, 자신을괴롭히는정도의잡음일때이명이라한다 1). 최근복잡한사회환경과관련된스트레스, 산업발달에의한소음증가, 인구의노령화등으로인하여이명환자가증가하고있다 2-4). Yeo 5) 의연구에따르면전인구의 32% 정도가이명증상을가지고있으며 6% 는이러한이명으로고통을받고. 1% 는이러한이명때문에일상생활에지장을받는다고알려져있을만큼광범위한질환이다. 현재이명의발생기전과관련하여여러가설이제기되고있으나아직까지정해진바가없으며증상의정도, 진단및치료등에있어서도뚜렷하게제시된바가없다. 또한기존의침치료를통한이명에대한한방적접근은아직까지만족할만한성과를보여주지못하고있다 6-8). 따라서이명의침치료에관한최근 10년간의해외및국내논문을분석함으로써이명의침치료에관한객관적자료를얻고앞으로의연구방향을모색하고자하였다. 먼저해외에발표된논문은 Pubmed 를활용하여, 검색어는 'tinnitus, acupuncture' 로 Field는 Title/ Abstract 로, 제한조건에서 Article types는 Case Reports 또는 Clinical Trial 또는 Randomized Controlled Trial 또는 Meta-Analysis 또는 Review 또는 Systematic Review 로검색하였다. 또한 Text availability는 abstract avilable 로 Publication dates 는 Published in the last 10 years 로검색하여총 25 편의논문이검색되었다. 두침등을포함하여모든이명과관련된침치료를연구대상으로하였으며이중주제가이명의침치료와관련되지않은논문 17 편과원문을구할수없는논문 1편을제외하고총 7 편이검색되었다. 'Impact of Acupuncture on Otoacoustic Emissions in Patients with Tinnitus' 는검색된 The Effect of Acupuncture on Tinnitus: A Randomized Controlled Trial 의참고문헌중의하나로본논문의주제와관련되며검색조건과도부합하여검색어를논문제목으로직접입력하여검색하였으며이논문을포함하여총 8편이검색되었다. 국내논문은국가과학기술정보센터 (NDSL), 한국학술연구정보서비스 (RISS), 한국학술정보 (KISS), 국회도서관의학술데이터베이스와대한한방안이비인후피부과학회지홈페이지의기관자료통합검색을이용하여최근 10년이내에저술된이명의침치료와관련된논문을검색하였으며, 한약및기타치료를병행한경우 3편은제외하고총 2편의논문이검색되었다. 따라서총 10편의논문으로본연구를진행하였다. 3 한방안이비인후피부과학회지제 28 권제 3 호 (2015 년 8 월 ) Okada 등 6) 은 76명의성인이명환자 ( 여성 47 명, 남성 29명, 평균연령 56.9±12.0세 ) 를대상으로침치료가이명의주관적증상변화에미치는영향을평가하였다. 연구는전향적, 무작위배정, 이중맹검임상시험으로진행되었다. 침치료전이명의정도를 VAS(Visual Analogue Scale) 로평가하게하고이후 38명씩실험군과대조군으로무작위배정한후이명소리가더큰쪽귀에자침하였는데실험군은耳尖直上 6.5 cm에자침하고대조군은실험군보다 3 cm위의비경혈에자침하였으며침치료이후이명의정도를 VAS로재평가하도록하였다 mm의침으로두피에 45도각도로골막까지刺入하였고 15초간 2 Hz의속도로염전법을시행한후 1분간안정을취하게하였다. 침치료후실험군과대조군모두에서침치료전에비해이명의주관적증상에유의한완화를나타냈으며실험군에서더욱두드러지게나타났다. 또한치료종료 5일후전화로증상완화의지속기간과수면의질에미치는영향에대해서추적조사를실시하였는데완화된증상의지속기간은실험군에서평균 106.9시간, 대조군에서평균 72.3시간으로나타났고, 실험군 4명과대조군 4명에서수면의질향상이있었다고답했다. 결과적으로이연구는침치료가이명의증상완화에효과적임을보여준다. 2 Impact of Acupuncture on Otoacoustic Emissions in Patients with Tinnitus 이음향방사는정상와우기관에서자발적으로발생하거나음향자극으로인해발생하며, 이는 Table 1. 9 한방안이비인후피부과학회지제 28 권제 3 호 (2015 년 8 월 ) 와우및외모세포의기능을반영한다. 또한 medial olivocochlear tract는반대쪽외모세포를조정하므로한쪽귀에음향자극을가하면반대쪽귀의이음향방사가저하되는것으로알려져있다. Azevedo 등 9) 은 38명의성인이명환자 ( 여성 25 명, 남성 13명, 36~76 세 ) 들을대상으로이음향방사를측정하여침치료가와우기관의기능에미치는영향을평가하였다. 연구는전향적, 무작위배정, 이중맹검임상시험으로진행되었다. 실험군 (n=19) 은전정-와우영역에해당하는측두-두정엽지점 ( 耳尖直上 4.5 cm ) 에자침하였고, 대조군 (n=19) 은경혈점에해당하지않는耳尖直上 7.5 cm에자침하였다. 두군모두이명소리가더큰쪽귀에자침하였고 mm의침이사용되었으며두피에 45도각도로골막까지刺入하였고 15초간 2 Hz의속도로염전법을시행한후 1분간안정을취하게하였다. 이음향방사는실험군에서우측귀에침치료후좌측귀의이음향방사를평가한경우를제외하고는통계학적으로유의한차이를나타내었으며, 이음향방사의억제는좌측귀에침치료후좌측귀의이음향방사를평가한경우유의한차이를나타냈다. 또한대조군에서는이음향방사및이음향방사억제모두에서유의한차이가발생하지않았다. 이를통해측두-두정엽지점의자침이이명환자의이음향방사에의미있는효과를나타낸다고보고하였다. 3 Comparative study on therapeutic effects of acupuncture, Chinese herbs and western medicine on nervous tinnitus Tan 등 10) 은 90명의신경성이명환자 ( 여성 42 명, 남성 48명, 18~65 세 ) 를대상으로신경성이명에서경추夾脊穴자침, 한약, 양약의치료효과를비교하기위한연구를수행하였다. 이명이 1개월이상반복적으로재발하거나혹은 5일이 상지속적으로발생하는자를대상으로하였다. 침군 (n=30) 은상위경추위주의夾脊穴, 風池, 內關, 腦戶에자침하고捻轉補瀉法을사용하였으며 20분간유침하였다. 매일 1회씩치료하여총 30일간치료하였다. 한약투여군 (n=30) 은補中益氣湯加減方을매일 3회씩복용하여총 30 일간복용하게하였다. 양약투여군 (n=30) 은鎭痙劑, 혈관확장제, 영양제류를투여하였는데 bandazol 20 mg을매일 3회복용하게하고, dextran 250 ml를매일 1회정맥주사하였으며, 丹蔘片 3 片을매일 3회씩복용하게하였고, VitB μg를매일 1회정맥주사하였다. 치료는총 30일간이루어졌다. 치료결과는치유됨, 현저한효과, 유효함, 무효함으로나누어서평가하였는데치유됨은이명이소실되고청력이정상으로회복되며차후 1개월간재발하지않은경우를말하며, 현저한효과는이명이일과수면에영향을주던정도에서안정시나야간에출현하는정도로바뀌거나, 혹은지속적이명이경감되어간헐적으로발생하고, 뚜렷한청력장애가없는경우를말한다. 유효함은이명이일과수면에영향을주던정도에서시끄러운환경에서나타나는정도로바뀌거나, 혹은이명이시끄러운환경중에서나타나는것에서조용한환경에서출현하는것으로바뀌는경우, 혹은지속적이명이간헐적이명으로바뀌는경우를말한다. 무효함은이명이개선되지않거나가중되는경우를의미한다. 유효율은침군, 한약투여군, 양약투여군에서각각 73.3%, 40.0%, 33.3% 로나타났으며통계학적으로유의한차이가있었다. 침군에서치료효율과병정과의관계를분석하였을때, 병정이짧을수록치료효과가높았으며발병 1개월이내와 3개월이상의치료효율을비교하였을때유의한차이가있었다. 또한침군에서치료효율과발병연령과의관계도 56

10 이규영외 1 인 : 이명의침치료에관한최근임상연구동향 살펴보았는데 50세이하와 50세이상에서유의한차이가있었다. 또한침군에서청력이정상인경우가청력저하가있는경우에비해치료효율이높았으며그차이는현저한유의성이있었다. 세치료군중치유됨과, 현저한효과를나타낸경우에대한추적조사를치료종료후 4주간실시하였는데치료효과가유지되고양호한경과를나타내었다. 결론적으로경추夾脊穴위주의침치료는신경성이명에유효한효과가있으며한약이나양약치료에비하여우수한효과를나타냈다. 4 A randomized, placebo-controlled trial of manual and electrical acupuncture for the treatment of tinnitus Wang 등 11) 은 50명의성인이명환자 ( 여성 4명, 남성 46명, 30~70 세 ) 를대상으로침, 전침치료가이명에미치는영향을평가하였다. 연구대상자는 3개월이상의이명병력이있는자로하였다. 연구는무작위배정, 단일맹검, 위약효과통제임상시험으로진행되었다. 침치료군 (n=19), 전침치료군 (n=16), 플라시보군 (n=15) 의세군으로나누었으며치료는 1주에한번씩총 6회진행되었고모든치료에 mm의침이사용되었다. 전침치료는침치료군과같은穴位와깊이로자침했으며양측 GB-8과 TE-17 에 2/100 Hz의교대주파수로 3 초간격의전기자극을주었다. 플라시보군은물리적침습이없는 placebo needle을사용하였다. 이명발생의빈도, 이명강도, 삶의질은침치료전, 침치료후, 치료종료 1개월후에평가하였으며, audiogram 은침치료전, 침치료후에평가하였고, 전반적치료효율성의평가는침치료후, 치료종료 1개월후에이루어졌다. 또한이명발생의빈도, 이명강도의평가에는 waveform generator가사용되었는데이기계에서발생되는소리중이명과유사한정도의 소리를측정하여평가하였다. 이명발생의빈도와크기는전침치료군에서침치료후에침치료전에비해유의하게감소하였으며삶의질은침치료군과전침치료군에서침치료후와치료종료 1개월후에침치료전에비해유의하게향상되었다. 하지만각군간의의미있는차이는발견되지않았으며 audiogram 에도의미있는변화가나타나지않았다. 전반적치료효율성의평가는침치료군과전침치료군에서침치료후에침치료전에비해유의한효과를보였으며특히전침치료군에서뛰어난효과를나타냈다. 결론적으로이명치료효과에서침치료군과전침치료군간에통계학적으로유의한차이는없으나전침치료가보다양호한치료경과를나타냈다. 연구의제한점은첫째, 50명으로샘플수가적다는점, 둘째, 표준화된침치료방법을사용하였기때문에개개인의상태를고려하여맞춤식침치료를하는전통적인침술이론에부합하지않는다는점, 셋째, 침치료 1개월후까지만추적조사가이루어졌기때문에침치료의장기적효과에대해서는조사되지않았다는점등이있다. 5 Long-Term Effect of Acupuncture for Treatment of Tinnitus: A Randomized, Patientand Assesor Blind, Sham- Acupuncture- Conrolled, Pilot Trial Jeon 등 12) 은 33명의이명환자 ( 여성 12명, 남성 21명, 18~80 세 ) 를대상으로이명치료에서침의장기적효과를조사하였다. 연구는무작위배정, 피험자와평가자맹검의파일럿임상시험으로진행되었다. 연구대상자는 6개월이상의편측성이명을가지며, 중등도혹은심한난청이없는자로하였다. 침치료군 (n=17) 과가짜침치료군 (n=16) 의두그룹으로나누어가짜침치료군에는비경혈에得氣感없이자침하였으며두군모두 1주에 2 57

11 한방안이비인후피부과학회지제 28 권제 3 호 (2015 년 8 월 ) 회씩총 5주간치료를하였고모든치료에 mm의침을사용하였다. outcome 측정은치료전, 10회의치료후, 치료종료 3개월후에이루어졌다. 최종적으로 33명의참가자중침치료군 (n=13), 가짜침치료군 (n=10) 의총 23명이치료 session을완료하였다. 연구결과 VAS에서는각군간에연구기간에따른차이가뚜렷하였으나 THI(Tinnitus Handicap Inventory), PTA(Pure Tone Audiometry, 순음청력검사 ), SD(Speech Discrimination, 어음명료도검사 ) 에서는군별로유의한차이가없었다. THI, SD는침치료군에서치료전에비해치료후와치료종료 3개월후에유의한개선을나타냈으며, PTA 는치료종료 3개월후까지는개선효과가이어지지않았으나, 10회의치료후에는치료전에비해유의한효과를나타냈다. 결론적으로침치료군과가짜침치료군간에통계학적으로유의한차이가나타나지않았지만침치료전부터치료종료 3개월후까지의 VAS의평균 % 의변화는가짜침치료군에비해침치료군에서유의한차이를나타냈다. 또한이연구를통해침치료의장기적효과를입증할수있었다. 연구의한계점은적은샘플수이며또한가짜침대조군으로비경혈점을자극하는방법을사용하였는데이또한생리학적인효과를가져올수있으므로플라시보침에대한연구가필요하다. 또이연구에서는피험자와평가자맹검을실시하였으므로맹검이잘이루어졌는가에대한평가도필요하다. 6 The Effect of Acupuncture on Tinnitus: A Randomized Controlled Trial Jeon 등 8) 은 21명의이명환자 (18~60세) 를대상으로이명의침치료효과를연구하였다. 연구는무작위배정, 이중맹검임상시험으로진행되었으며중등도이상의난청과같은耳科的질환 없이 6개월이상만성적으로편측성단순이명이지속된환자들을대상으로하였다. 경혈점침치료군 (n=12) 과비경혈점침치료군 (n=9) 로나누어두군모두주 2회씩총 10회치료하였으며 mm의침을사용하였다. 침치료군은먼저목뒤및頭部 7개의경혈점을得氣感이느껴질정도로 10 mm가량直刺하고 10분간유침하였다. 발침한후앙와위에서환측귀주위 5개의경혈점과안면부 5개경혈점에같은방식으로 3~5 mm자침하고 10분간유침하였다. 대조군은환측귀주위 5개의비경혈점과체부 5개비경혈점에 3 mm이하로자침하고得氣등의조작을시행하지않고 10분동안유침하였다. 시험시작전과마지막치료후에주관적증상의변화 (THI, VAS), stress 상태의변화 (BDI- 2(Beck Depression Inventory-2), HRV(Heart Rate Variability)), 청력의변화 ( 청력검사, 어음명료도 ) 를비교분석하였다. 먼저주관적증상을나타내는항목중 THI 점수는실험군은유의한감소를보였으나, 대조군은유의한차이는없었다. VAS는실험군과대조군모두에서유의한차이가있었으며특히실험군에서보다강한호전을나타내었다. Stress 상태를나타내는 BDI-2, HRV에서실험군과대조군모두유의한차이가없었다. 청력검사와어음명료도는실험군에서유의한차이가있었으나대조군에서는유의한차이가없었다. 이상의연구결과, 이명의경혈점침치료가환자의주관적인증상및청력, 어음명료도개선에유의한효과를나타냈다. 연구의제한점으로는침시술위약대조군설정방법의한계와모집피험자수가적었다는점, 치료후추적조사가이루어지지않았다는점등이있다. 7 The comparative study on effect of manual acupuncture, periauricular electroacupuncture and distal electroacupuncture for idiopathic 58

12 이규영외 1 인 : 이명의침치료에관한최근임상연구동향 tinnitus Kim 등 13) 은 39명의성인이명환자 ( 여성 11명, 남성 28명, 20~75세 ) 를대상으로특발성이명환자에대한체침과전침복합치료효과를연구하기에앞서개별치료효과를비교하기위하여본연구를시행하였다. 연구는발병일이후 2주이상의지속적이명을호소하는자를대상으로하였다. 체침군 (n=12), 귀주위전침치료군 (n=14), 사지부원위전침치료군 (n=13) 으로나누어 1주일에 2회를원칙으로 4주간총 8회치료하였으며유침시간은 20분으로하였고 mm의침을사용하였다. 체침군은이명을호소하는방향 ( 일측혹은양측 ) 의귀주위 5개혈과원위부 4개혈에자침하였고, 귀주위전침치료군은이명을호소하는방향 ( 일측혹은양측 ) 의청궁 (SI-19), 예풍 (TE-17) 에자침후전기자극을주었다. 사지부원위전침치료군은이명을호소하는방향 ( 일측혹은양측 ) 의원위부 4개혈에전기자극을주었다. 주평가도구로는 THI, 부평가도구로는 VAS (loud, uncomfortable), 순음청력검사, 어음명료도검사를사용하였으며 1회차시술전초기평가, 5회차시술전중간평가, 8회차시술종료후종료평가. 8회차시술종료 4주후추적평가를시행하였다. 연구결과 THI score 는시간에따라유의하게감소하였으나각군간유의한차이는보이지않았다. VAS loud 와 VAS uncomfortable 는시간에따라유의하게감소하였으며, VAS loud 의경우각군간유의한차이를나타내지않았으나, VAS uncomfortable는체침군과사지부원위전침치료군이귀주위전침치료군에비해유의한호전을나타냈다. 또한순음청력검사와어음명료도검사에서각군은시간에따른호전양상을나타내지않았다. 결론적으로, 세군모두일정역역 에서시간에따른유의한차이는있었으나각군간유의한차이는보이지않았다. 하지만체침치료와전침치료가각각독립적으로일정의효과를나타내었으므로병용사용시더좋은효과를나타낼수있을것으로기대된다. 2) RCT protocol 1 Acupuncture at local and distant points for tinnitus: study protocol for a randomized controlled trial Shi 등 14) 은주관적이명을호소하는환자에게서근위취혈과원위취혈을동시에한경우에각각의치료를따로실시한경우에비해서얼마나더효과적인가를평가하기위한 protocol을발표하였다. 연구는무작위배정, 단일맹검임상시험으로설계되었다. 112명의환자를대상으로각각 28명씩 4군으로나누어 A군에서는경혈에근위, 원위취혈, B군에서는경혈에근위취혈, 비경혈에원위취혈, C군에서는비경혈에근위취혈, 경혈에원위취혈, D군에서는비경혈에근위, 원위취혈하도록설계하였다. 연구대상자는양측혹은편측에주관적이명을가진자로 18~65세의남녀, 최소 3주이상이명이지속된자로설정하였다. 치료는한주에 2회씩총 4주간진행하도록설계되었으며주된 outcome은 VAS로주관적이명의강도및불편감을치료시작전 (baseline) 과첫번째침치료후 4주, 8주로총 3회측정하도록하였고, 이차적 outcome 은 THI로 baseline과첫번째침치료후 4주, 8주에측정하고, 이명의톤과소음강도의변화를이명 matching 을사용하여 screening 과첫번째침치료후 8주차에평가하도록하였다. 또한침치료의신뢰성을평가하기위해서 4주간의침치료후에 TCS(Treatment Credibility Scale) 라는 5-point scale을사용하고, blinding의적절 59

13 한방안이비인후피부과학회지제 28 권제 3 호 (2015 년 8 월 ) 성을평가하기위하여 4주간의침치료후에 7-point scale 을사용하도록하였다. 연구의한계점은침치료연구에서대조군으로쓰이는비경혈점의위치, 자침깊이, 방향, 유침시간, 수기법의필요성에대한지침이아직까지확립되어있지않아본연구에서도대조군설정에어려움이있었으며, 단일기관연구로연구결과를일반화하는데한계점이있고, 시술자맹검이이루어지지않았기때문에 bias가발생할수있고샘플수가적다는점등이있다. 2. Clinical trial 논문 (1)Acupuncture for Tinnitus: A Series of Six n=1 Controlled Trials Jackson 등 15) 은 6명의성인이명환자 ( 여성 1명, 남성 5명, 평균연령 52세 ) 를대상으로 5일연속으로 2 주동안총 10회의침치료후, 침치료가이명에미치는영향을분석하였다. 연구의참여기준은 18세이상이며, 이명으로인한침치료의경험이없고영어를읽고쓸수있으며, 임신하지않은자로서, 연구에참여하기로동의한자를대상으로하였다. 연구설계는 n=1 study, AB design 으로하였는데, 이는대상자수가적고침치료와같이장기간에걸친효과를나타내는치료의효과를검증하는데효과적이다. 주된 outcome 은 Daily Dairy record 로침치료전 14일 (Phase A) 과침치료후 14일 (Phase B) 동안환자스스로매일기록하게하였으며이명의 4가지증상 ( 강도, 주파수, 하루중이명의영향을받는시간, 수면의질 ) 을각각강도와주파수, 수면의질은 0~10 점, 하루중이명의영향을받는시간은 0~24 h로표기하도록하였으며숫자가클수록심한상태를의미한다. 이차적 outcome 으로는 THI와 MYMOP (Measure Your Medical Outcome Profile) 를사용하여연구시작으로부터 1, 3, 5, 7, 11주차에기록하도록하였다. MYMOP는환자스스로가주증상을기 록하고 1~25 점사이로점수를매겨서변화를확인할수있도록되어있다. 결과는 Daily Dairy record 중강도의중앙값은 95% 신뢰구간에서 -2.49(-5.04, 0.02), 주파수의중앙값은 -1.39(-3.74, 0.89) 로감소되는결과를나타냈으며, 하루중이명의영향을받는시간, 수면의질은연구대상자들사이에서보다일치되는결과와중대한개선효과를나타냈는데이명의영향을받는시간은중앙값이 -2.76(-3.94, -1.63), 수면의질은중앙값이 -2.72(-3.45, -2.03) 의변화를나타냈다. 또한 THI 와 MYMOP도침치료후개선되는양상을나타냈다. 결론적으로침치료는이명에유익한효과를가져올것으로기대된다. 하지만이연구만으로는결과를일반화할수없기때문에충분한피험자수와침치료기간, 검증된 outcome measure와사후분석을갖춘추가적인연구가필요하다. 3. Review 논문 (1) Acupuncture for the treatment of tinnitus: a systematic review of randomized clinical trials16) 이연구는이명의침치료효과를평가하기위하여수행되었다. 총 9편의 RCT가평가되었으며침이단일치료로사용된경우만포함하였다. 연구에서실험군은침, 전침, 두침, 이침치료를받았고, 대조군은치료를받지않거나가짜침또는약물치료를받았다. outcome 평가도구로는증상의심한정도나완화된정도, 삶의질, response rate, adverse event 가있다. 국가별로는 3편이덴마크, 2편이중국, 2편이브라질, 1편이영국, 1편이한국에서연구되었다. 혈위선정은 8편이 TCM 이론을바탕으로하였고 1편은이전연구의 protocol을따랐다. 침시술자는 1편에서만언급되었는데, 10년이상의임상경험을가지는자였으며, adverse event도 1편에서만언급되었고, 자침중심한통증이 2건이었다. 60

14 이규영외 1 인 : 이명의침치료에관한최근임상연구동향 bias 평가를위해 Cochrane tool 의무작위배정순서생성방법, 배정순서은폐, 참여자, 연구자에대한눈가림법, 불완전한결과의양, 종류및처리, 선택적결과보고, 타당도를위협하는다른잠재적비뚤림의 7 가지기준을적용하였으며대부분높은 bias 위험성을나타냈다. 논문의방법론적인질은대부분낮았는데대부분샘플수가적었고 power calculation을적용한경우가없었다. 또한 5개의 RCT는 10개이하의 session으로이루어졌으며그중 2개는단일 session으로이루어졌다. 또한연구대상자의탈락에관한세부사항은 3 개연구에서만기록되었고침시술자나 adverse event의언급도 1개연구에서만나타났다. 5개의 RCT는침또는전침을가짜침과비교하였고, 2개의 RCT는두침을가짜침과비교하였으며주평가도구로 VAS를사용하였다. 연구결과두침은유의한효과를나타냈다. 또한 2개의 RCT는침치료와약물치료를비교하였으며연구결과침은신경성난청에는효과를보였으나노년난청에는뚜렷한효과를나타내지않았다. 결론적으로침치료가이명증상완화에유효하다는근거는부족하며평가된 RCT의수, 규모, 질이이명의침치료효과에대한확정적인결론을이끌어내기에는충분하지않으므로추가적인연구가필요하다. Ⅳ. 고찰한의학적으로이명의實證은肝火上搖, 痰火阻塞, 氣滯血瘀, 風熱로변증하며, 虛證은氣虛, 血虛, 腎陰虛, 腎陽虛, 心腎不交, 脾胃虛弱으로변증하여치료한다 1). 주관적인이명은외이도의귀지나이물, 삼출성중이염, 이명유발약제, 메니에르병, 개방성이관장애, 턱관절장애, 청신경종양등의원인으로발생하기도하며, 객관적인이명은경부동맥의협착, 경정맥구혈관종, 심장기형, 인공밸브, 동정맥단락, 중이나 구개의근육경련, 개방성이관, 자발이음향방사로인해발생한다. 하지만이명은검사로는명확한원인을찾을수없는경우가대부분이다 17,18). 이명의치료방법으로는원인제거, 국소마취제, 항경련제, 신경안정제등의약물요법, 수술, 전기자극, 차폐, 정신요법, 생체되먹임, 최면, 이명재훈련치료 (Tinnitus Retraining Therapy, TRT), 습관화를통한이명적응훈련법등이사용되고있으나아직까지그효과가미진하여어떠한단일치료방법도최선의치료법이라고확신하지못하고있는실정이다 18-21). 총 10편의논문중 RCT가 7편, RCT protocol 1 편, Clinical Trial이 1편, Systematic Review 가 1편이었으며. 해외에발표된논문은한국 2편, 중국 2편, 브라질 2편, 영국 1편, 덴마크 1편이었고, 연도별로는 2006년 2편, 2007년 2편, 2010년 1편, 2011년 2편, 2012년 2편, 2013년 1편으로매년비슷한정도로발표되고있다. 저널별로는 Brazilian Journal of Otorhinolaryngology 2편, Complementary Therapies in Medicine 2편, Complementary and Alternative Medicine 2편, Kyung-Hee University master's thesis 2편, Chinese Acupuncture and Moxibustion 1편, Trials 1편이있었다. RCT 논문중에서주로사용된경락은足少陽膽經 (GB) 이총 18회, 手少陽三焦經 (TE) 이 15회, 督脈 (GV) 이 10회, 足太陽膀胱經 (BL), 足陽明胃經 (ST), 手太陽小腸經 (SI), 經外奇穴 (EX) 이각각 3회, 手陽明大腸經 (LI) 과 Scalp acupuncture의측두-두정엽지점이각각 2회사용되었다. 또한다용된穴位는聽會 (GB-2) 가총 6회사용되었고, 翳風 (TE-17) 이 5회, 百會 (GV-20) 와耳門 (TE-21) 이각각 4회사용되었으며風池 (GB-20), 中渚 (TE-3) 가각각 3회씩사용되었다. 足少陽膽經은인체의측면을담당하므로耳病症과밀접한관련이있으며心과臟腑相通관계이고少陽相火와도관련되므로심인성으로발병하는이명과도관련이있다. 足少陽膽經의聽會 (GB-2) 는耳前에위치하여안으로耳와통하므로開竅利耳의효능이있고, 風池 61

15 한방안이비인후피부과학회지제 28 권제 3 호 (2015 년 8 월 ) (GB-20) 는疏散少陽風熱, 淸頭目, 利官竅의작용을한다. 手少陽三焦經은경락유주상其支者, 從耳後入耳中, 出走耳前하여耳와관련되며, 心包와表裏관계로정신, 순환기질환및少陽相火를다스려熱性病을치료할수있다. 翳風 (TE-17) 은手少陽과足少陽의交會穴로祛風淸熱, 開竅聰耳하며, 耳門 (TE-21) 은耳前上方에위치하여淸熱利竅하고, 中渚 (TE-3) 는淸瀉少陽風熱의작용이있다. 督脈은腦에屬하고人身의元氣와관련되며百會 (GV-20) 는督脈과足太陽의交會穴로消風散邪하여耳鳴을치료한다 22). 결과평가도구로는 VAS와 THI가각각 5편의논문에서사용되어가장많이사용되었으며 SD가 3편, PTA가 2편에서사용되었고, 그외개별연구특성에따른설문지가많이사용되었다. THI(Tinitus Handicap Inventory) 는총 25가지항목으로 17가지의심리상황에관한항목과 8가지의생활상황에관한항목으로구성되어있다. 각문항은 없다, 가끔있다, 자주있다 로표기하도록구성되었으며, 세가지의주요한내용들이평가가되는데, 첫번째는집중하는데어려움을느끼는것과같은이명에대한기능적반응이고, 두번째는화, 좌절, 짜증, 우울과같은감정적인반응이며, 세번째는절망, 심각한질환에대한두려움, 통제불능과협조불가능과같은이명에대한파국반응 (catastrophicreactions) 이다 23-24). SD(Speech Discrimination, 어음명료도검사 ) 는역치상에서가장잘들을수있는어음강도, 즉최적안정역치에서검사어음을얼마나정확히이해하는가를측정하여병변의부위, 보청기의적응및선택, 의사소통의장애정도, 언어치료계획등에필요한정보를제공하여준다. 특히청신경종양등의청신경병변에서가장현저한어음판별손실을보인다. PTA(Pure Tone Audiometry, 순음청력검사 ) 는음차에서발생되는것과같은순음을전기적으로발생시켜각주파수에따라음의강도를조절하여청력역치를측정하는방법이다. 순음청력검사는청력손실의정도, 청력도형, 난청형 ( 전음성또는감각신경성난청등 ) 에관한 정보를제공하여준다 25). 7편의 RCT 연구결과모든연구에서침치료가이명에양호한효과를나타냈지만대조군과의비교에서유의한효과를나타낸경우는 Azevedo 등 9), Tan 등 10) 의연구 2편뿐이었다. 수행된연구의질은낮은편으로, 샘플수의평균은대략 49.5명으로적었고, Tan 등 10) 의연구를제외한 6편이 10회이하의 session을가졌으며, 그중 2편 6, 9) 은단일 session으로서, 치료전후의비교로이루어져있었다. 치료기간은최대 6주 11) 였으며, 추적조사는최대치료종료후 3개월 12) 까지이루어졌고추적조사가없는논문도 2편 8,9) 있었다. 침시술자에대한언급은 3편 8,12,13) 에서있었고, adverse event는 2편 6,13) 에서언급되었다. 穴位선정은모두 TCM theory 에근거하였으며연구대상자의참여또는제외기준이언급되지않은논문이 2편 6,9) 있었고, 모든논문에서 blinding에대한평가는이루어지지않았다. 또한 Tan 등 10), Kim 등 13) 을제외한 5편의논문에서대조군으로 sham acupuncture를사용하였는데비경혈점자침도생리적인효과를가져올수있으며, 자침위치나깊이, 得氣感등에관한확립된기준이없어서많은연구에서침치료의대조군설정의문제를제기하였다. RCT protocol 14) 은침치료의신뢰성을평가하기위해서 TCS라는 5-point scale 을사용하였고 blinding의적절성을평가하기위하여 7-point scale 을사용하였으며 112명으로기존연구보다는샘플수가많고, 22년이상의임상경력을가지는침구사가침치료를시행하는등기존연구보다개선된 protocol 을제시하였다. Clinical trial 15) 은 6명의적은연구참여자를대상으로 2주동안총 10회의짧은연구기간으로진행되었으나 n=1 study, AB design 의설계로, 침치료전 14일 (Phase A) 과침치료후 14일 (Phase B) 동안개개인별로이명증상과관련된점수를스스로기록하도록하여전후비교를보다명확하게할수있도록하였다. 62

16 이규영외 1 인 : 이명의침치료에관한최근임상연구동향 Systematic Review 16) 에서는침치료가이명증상완화에유효하다는근거는부족하며평가된 RCT의수, 규모, 질이이명의침치료효과에대한확정적인결론을이끌어내기에는충분하지않으므로추가적인연구가필요하다고하였다. 이상의연구를통해침치료가이명에긍정적효과를가져오는것을기대해볼수있었으나, 각각의연구에서한계점으로지적했듯이적은샘플수와 session 수, 짧은연구기간, 단일기관에서의연구, 적절한 blinding 평가및확립된침대조군설정방법의부재등으로인하여명확한결론을이끌어내기어려웠다. 따라서이러한한계점을보완한체계적인연구가필요할것으로사료된다. Ⅴ. 결론본연구에서는최근 10년간해외및국내에발표된이명의침치료에관한논문을 Pubmed, 국가과학기술정보센터 (NDSL), 한국학술연구정보서비스 (RISS), 한국학술정보 (KISS), 국회도서관의학술데이터베이스와대한한방안이비인후학회지홈페이지의기관자료통합검색을통하여검색하여, 최종적으로총 10편의논문을정리, 분석하였다. 연구결과는다음과같다. 1. 총 10편의논문중 RCT와관련된논문이 8편, Clinical Trial가 1편, Systematic Review가 1편이었으며 RCT와관련된논문중 1편은 RCT protocol이었고나머지는 RCT 논문이었다. 2. 해외에발표된논문을국가별로분석하면한국 2 편, 중국 2편, 브라질 2편, 영국 1편, 덴마크 1편이었다. 3. 연도별로는 2006년 2편, 2007년 2편, 2010년 1편, 2011년 2편, 2012년 2편, 2013년 1편이있었고, 저널별로는 Brazilian Journal of Otorhinolaryngology 2편, Complementary Therapies in Medicine 2편, Complementary and Alternative Medicine 2편, Kyung-Hee University master's thesis 2편, Chinese Acupuncture and Moxibustion 1편, Trials 1편이있었다. 4. RCT 논문중에서다용된경락은足少陽膽經 (GB) 으로총 18회사용되었고手少陽三焦經 (TE) 이 15 회, 督脈 (GV) 이 10회였다. 또한다용된穴位는聽會 (GB-2) 로총 6회사용되었고, 翳風 (TE-17) 이 5 회, 百會 (GV-20) 와耳門 (TE-21) 이각각 4회사용되었으며風池 (GB-20), 中渚 (TE-3) 가각각 3회씩사용되었다. 5. 결과평가도구로는 VAS와 THI가각각 5번으로가장많이사용되었으며 SD가 3편, PTA 가 2편의논문에서사용되었고, 그밖에각각의연구에따른설문지가많이사용되었다. 6. 7편의 RCT 연구결과모든연구에서침치료가이명에긍정적인효과를가져왔지만대조군과유의한차이를나타낸경우는 2편뿐이었다. 7. 이상의연구들만으로는이명에대한침치료의유효성을평가하기어려우며, 향후한계점을보완한체계적인연구가필요하다. 감사의글이논문은 2013학년도상지대학교교내학술연구비지원에의해수행된연구임 Reference 1.
