Contrast galactography can often accomplish such localization and sometimes can suggest o r coniirm the nature of the pathologic process." Authors have experienced 077 cases of mammograms, and among them, 19 cases w ith nipple discharge have been at Yo undong Severance Hospital. Yonsei University since October 1, 1983 through June 10, The results were: 1. Incidence of nonlactating nipple discharge was 3.9% among 077 cases. Most frequent histopathologic disease among surgically confi rm ed 100 cases was papi lloma (3%), which was followed by papillomatosis (8%), fibrocystic disease (1%) and then breat carcinoma (13%) 3. Age distribution of patient of nonlactating nipple discharge showed no differences in comparing to the patients w ith the same histopathologic disease without nipple discharge. Bloody nipple discharge is the commonest finding (61 %) which was followed by serous (%), milk빼 (8%), and then serosanguinous (%) nipple discharge. In analyzing relationship between histopathologic diagnosis and nipple discharge, papilloma and papillomatosis had bloody nipple discharge (69%, 7%), frequently and serous nipple discharge (6%, %). Breat cancer had mostly bloody nipple discharge (8%), incontrast to the fibrocystic disease w hich showed 0% bloody an d 8% milkish nipple discharge. Galactographic characteristics of papilloma were single duct dilatation (63%) w ith single intraductal filling defect (66%) along with ductal obstruction (31%). Galactographic study was the method of choice to put exact diagnosis of the lesion in the lactiferous duct Diagnostic accuracy of papilloma 83%, papillomatosis 81 %, breast cancer 9%, breast abscess and fibrocystic disease 100, respectively 유두로부터바수유성유즙분비가있을애는자각증상이냐종괴촉지유무와판계없이도유선내외영밴을암시하는소견으로서유둔종 ( papill o ma ), 유두종증 ( papillomatosis ), 유방암 (carcinoma ), 섬유낭종성질환 (fi brocytic disease ), 연포성유선염 (comedomast iti s) 에서볼수있는데, 유방암의 1-% 에서도바수유성유즙분비의보고가있우므로, 3) 원안을추석하여야한마. 물론, 최근에유방암을조기에발견하기위한방법으로서필름유방촬영술, 초음파유방촬영술, 건조유땅촬영솔, 디지탈유방촬영술, 천산화유방촬영술과핵자기공명법에의한유방영상검사들이단독A로혹은뱅합으로이용되어서유선뱅변의진단률이증가되었으나 - 10 ) 이같은방엽 A로도촉지되지않거나, 증상없이혈성혹은장액성유즙분비가나타날애는유선파판계없는명벤의유우확인 11 ) t영변의위치, 범위및영소의성질을규명하기위하여유선조영술을행하여야만진단이될수있는경우라하겠다 6, 1 ) 닝 l 환자에서유선조영술을택하였다. 환자를투시촬영 기침대에사각앙오냐 1 로누인마음, 유두를베타단과 알콜로소독하고 vinca n ee dle 을이용하여유즙분비 가있는곳을선택석으로잡아마니면서조영제를주입 한다. 이때사용되는주사챔은 % Gauge 폴리에틸 렌끝 로싸인 plastic vinca needle 파 1cc 튜바콜린 주사기블이용하여유두끝을상하지않도록조심하였 으며, 조용제는 30% tele br ex 을사용하였고, 투시하 에서유선내에조영제가충분히차연, Senograph 00T 유방촬영기에서 상하 (C raniocaudad ) 와내외 ( mediolateral ) 측유땅촬영을행하고필요시에는 coneddown view 사위를촬영하기도한다. 외과의들이 원하연 methylene blue 냐 gentian violet 를주입하기 도하여, localization ) _ 국소에칭으로위치를정하기도한마 ( needle 결 과 유선조영술은비수유성유즙분비가있는유선에조 영제를선택적으로주입하여진단하는데, galactogr aphy 혹은 cluctography 라하며 1930 년에이마 R ies ) 가처음으로보고한이래로많이사용되고있는방법 이다. 저자는상기와같은비수유성유즙분비를주소로내 원한환자 19 명을대상으로하여유선조영술을행하 였고, 이중수술적으로병리조직학적진단을할수있 었던 100 예를중심으로비수유성유즙분비의딸생빈 도, 연령분포 ; 바수유성유즙분바의분석, 유선초영 술소견, 유선조영술과기타촬영검사업과의비교, 션조영술의확진율에대하여분석하였 A 여, 들의운헌과비교하여보았다. 검사방법 유 타저자 년 10 월 7 일이래로 1988 년 6 월 10 까지유땅 의동통, 불쾌감, 종괴촉지, 바수유성유즙분비나혹 은단순검진 (Sc reening tes t ) 을위하여연세대학교의 과대학영동세브란스영원에내원한총 077 명의환자 중에서비수유성유즙분비를주소로한 19 명에유션 조영술을행하였고, 이중확전된 100 예를분석하였는 데, 환자는모두여자환자이었고, 최소 11 세부터최 고 73 세였마 L 유즙분비질환의빈도 내원하여유선조영술을받은환자는 19 예로서 077 명의환자중 3_ 9% 에해당되였다. 뱅리조직학적으로확진펀 100 예의환자중에는유두 종이 3%, 유두종증이 8%, 유땅암 13 %, 섬유낭종 성질환 1%, 유선확장증 ( d uctectasis 혹은 secretory 198 년 8 월 1 일이래로총 1 9 예의비수유성유즙분 disease ) %, 유선상펴증식증 ( ductal epithelial 783 총

Hyperplasia 8 Abscess 8 6 Normal 11 Lymphadenitis Fibroadenoma Hematoma Total 19 cases 100 cases hyperplasia) 1 %, 유방영 렬종괴가 1 % 로분석되었다 ( Table 1). 및농양이 6% 였고, 유선내. 비수유성유즙분비환자의연령분포 확잔된총 1 00 예의환자중 36% 는 1-0 세사이에 분포되어있었고, 3 1 % 는 31-0 세사이에분포되어 있었다. 환자의질환밸로연령분류를행하여본결파유두종 환자는 0 매 ( 9% ) 와 0 대 ( 9% ) 에서고르게발생하 였고, 유두종증환자는 30 대 (9% ) 와 0 대 (3% ) 에서 호말하였다. 유땅암도 30 대 ( 3% ) 와 0 대 ( 9% ) 에서 호말하였고, 섬유낭종성질환은 0 대와 10 대에각기 ). 3. 비수유성유줌분비의분석 병리조직학적으로확진된 10 0 예의환자즙에서혈성 유즙분바 (6 1 % ), 혈장액성유즙분비 ( %), 장액성유 즙분비 ( % ) 가가장흔한유즙분바로서총환자의 88% 를차지하였고, 유액성유즙분비 (8%) 와수액성 유즙분바는 % 로서반도가많지않았다 ( Tabl e 3 ). 혈성과혈장액성유즙분배를보였던 66 예중에서는 유두종 ( 예 ), 유두종증 (1 9 예 ), 유방암 (1 예 ) 와섬유 낭종성질환 (7 예 ) 의반도를보였마. 장액성유즙분비를보였던 예중에서는유두종 (9 예 ), 유두종증 ( 7 예 ), 섬유낭종성질환 (3 예 ) 이않았으 냐유땅암은 l 예로서드문소견이었다. 조직학적으로확진된질환별로흔한유즙분비를분 류하였더니, 유땅암은대부분 (8% ) 이 혈성유즙비를 보였고, 유두종증의혈성유즙분비 (69% ) 가장액성유 즙분 tl]( % ) 보다흔한소견이었고, 유두종의혈성분 비 ( 7% ) 도장액성유즙분비 (%) 보다는많이나타났 A 나, 섬유낭종성질환과유땅농양은유액성분비가 않았다 (Ta ble 3 ).. 유선조영술소견 뱅리조직학석으로확진왼 100 예외환자들의유선조 영술소견을비교분석하였다. 유두종 (3 예 ) 의 유션조영술소견상에서는단일유선 확장 ( 63%), 유선내분엽양결손상 ( 66%), 그러고유 Table. 5 3 9ι- 大韓放射線醫學會註 : 第卷第號 선폐쇄 (3 1% ) 소견이가장흔하였고, 종괴내석회화소견은없었다 (F ig 1). 유두종증 (8예) 에서도단일유선확장 ( 6% ) 이전반적인유선확장 ( 1% ) 보다흔한소견이였고, 유선내에마수의소결손상 (79% ) 들이유선벽을불규칙한모양으로나타내며유선폐쇄 (% ), 구불거리거나 (% ), 낭종형성의소견을볼수있었마 (Fig.). 유방암 (1 3예 ) 의흔한소견은유선폐쇄 ( 69% ), 유선내에불규착한결손상 (% ), 유선주위종괴형성파둘러싸는소견 (e n c asem e nt ), 유선협착 ( Fig. 3, ) 파유선내석회화소견등이각기 38% 씩보였고 ( Fig 3,), 유선밴위 (31%), 폐쇄상부유선확장 ( 30% ) 파조영제가새어나가는소견 (extravasation, 3 % ) 등은반도가석어도유방암에서만보였던특정석인소견이었다 (Fig. ). 섬유낭종성질환 (1 예 ) 은대부분이정상소견 (79% ) (F ig. ) 을나타내었 A며, 유선의쿠불거럼 ( 9% ), 유선협착 (1% ), 유선확장 (1 %) 그러고유션의낭종형성소견 (1 %) 도보였다 ( Fi g 6). 만성유방염파농양 ( 6예 ) 에서는다수의유선확장 (0% ), 다수의유선내결손상 ( 67% ), 낭종형성 ( 67% ) 이주소견이었고, 유선협착유선폐쇄나유선의 구불거리는소견은드물었마 ( Tabl e, Fig. 7 ).. 유선조영술과기타촬영방법과의비교 확인펀 100 예의,,] 수유성 유즙분바환자에서는필 름유땅촬영술 (1 00여 ] ), 초음파유방촬영솔 (1 00예 ), 유 선조영술 (1 00 예 ) 를시행하였마. 상기각검사땅법을비교하였더니비수유성유즙분,,] 질환에서는필릎유방촬영술 (9% ) 보다는초음파유 방촬영술 (83% ) 파유선조영술 (98 % ) 이진단에도움을 주였고, 유선조영솔이가장진단적가치가있었다 (T able ). 6. 유선조영술에의한질환별정확도 유선조영술을행한,,] 수유성유즙분,,] 질환에서수 술전과수술후의명리조직진단을비교하여정확도를 비교하였다 ( Table 6 ). 수술전유두종으로진단되었고 38 례중 30 예에서수 술후에확진되어확진율이 83% 였고, 의양성 8 예 (1 8% ), 의음성이 예 (1 % ) 이었다. 수술전진단인 6 예의유두종증은수술후에 1 예에 Table. Total cases 서유두종증이확신되어 81% 의확진율을보였고, 으l 양성 예 (1 % ), 의음성 7예 (1% ) 였다. 그러므로유선상파의증식성종괴 (papillary neoplasm ) 인유두종파유두종증을합하연술전진단총 6 예중 9예에서확진할수있어서총 9% 의확진율을냐타내었마. 수술전유방암으로진단되였던 1예중수술후확진 된예는 11예로 9% 의확진율을보였고, 의양성 1 예 (7%), 의음성 예 (1 %) 였다. 수술천섬유낭종성질환 8예천예에서확진되었으나의음성이 6예 (3%) 이었고, 수숭천유방암과유방농양으로진단되었던 예모두도수술후에확진 (100% ) 되었으냐의음성이 l 예 (1 7%) 있었다
