9 국문요약 자궁경부상피내종양에서 p16 INK4a, cdk4, prb 의이상발현및 high risk HPV 감염여부, 재발과의상관관계 목적 : 자궁경부상피내종양에서세포주기조절단백인 p16 INK4a, cdk4, prb 의이상발현정도를살피고이들이고위험군 HPV 감염및재발등임 상적특성과연관이있는지알아보고자하였다. 연구방법 : 2001 년 1 월부터 2003 년 12 월까지본원산부인과에내원하여 자궁경부착공생검 (colposcopy directed punch biopsy) 및환상투열절제 술 (conization) 또는제 1 형확대형자궁적출술 (type I extended hysterectomy) 를시행받고조직병리학적으로자궁경부상피내종양을진 단받은 265 개의파라핀포매조직절편으로 tissue array 를제작하여면 1
10 역조직화학염색을시행하였다. 염색시약은 CINtec TM p16 INK4a Histology kit (DAKOCytomation, Carpinteria, CA, USA), cdk4 (DCS156) Mouse mab (Cell Signaling, Beverly, MA, USA) 및 retinoblastoma (Rb) Ab-1 (Neomarkers, Drive Fremont, CA, USA) 를사용하였다. 염색결과의판독 은두명의독립된관찰자에의해분석되었고 400 배광학현미경하시야에 서관찰하여전체관찰된상피세포수중염색된세포수를세어백분율 단위로표기하였다. HPV 감염검사로는자궁경부상피내종양진단당시 HPV microarray DNA chip ( 바이오메드, 서울, 한국, 이하 DNA chip) 을 시행하였다. 결과 : 자궁경부상피내종양의조직병리학적진행정도와 p16 INK4a, cdk4 의발현정도는통계적으로유의하게양의상관관계를보였으며반면 prb 의발현과는통계적으로유의하게음의상관관계를보였다. 저등급상 피내병변 (CIN1) 에서는고위험군 HPV 감염이있는경우유의하게 p16 INK4a 단백이상발현이증가하였으나, 고등급상피내병변 (CIN2, 2
11 CIN3, CIS) 에서는통계적으로유의한차이를보이지않았다. 또한 p16 INK4a 의경우자궁경부상피내종양병변에서재발군에서비재발군에비해통계 적으로유의하게발현률이낮았다. prb 는고등급자궁경부상피내종양 (CIN2, CIN3, CIS) 에서통계적으로유의하게재발군에서발현률이높았으 며비록통계적으로유의하지않았으나저등급상피내종양에서도재발군에 서 prb 의발현률이높은경향을보였다. 결론 : 본연구결과에따르면 p16 INK4a 의발현은재발군에서통계적으로 유의하게감소되었고 prb 의발현은재발군의고등급상피내병변에서증 가되는소견을보였다. 따라서 p16 INK4a 및 prb 의면역조직화학염색결과가 자궁경부상피내종양의예후인자로서치료방침을결정하는데도움을 줄수있을것으로사료된다. 핵심되는말 : 자궁경부상피내종양, HPV, 재발, p16 INK4a, cdk4, prb, 면역 조직화학염색 3
12 자궁경부상피내종양에서 p16 INK4a, cdk4, prb 의이상발현및 high risk HPV 감염여부, 재발과의상관관계 < 지도 ; 박용원교수 > 연세대학교대학원의학과 남은지 I. 서론 자궁경부암은특히한국여성에서발생률이높은중요한질환으로 5-15 년정도전구병소형태인자궁경부상피내종양단계를거쳐침윤성병변 으로진행된다. 인유두종바이러스 (human papilloma virus, 이하 HPV) 의 4
13 감염이자궁경부암의발생에가장중요한원인임은현재까지광범위한역 학조사및분자생물학적인연구에의해이미밝혀져있다. 1 HPV 는 double stranded DNA 바이러스로서현재까지약 80 여종이상이알려져있으며, 이중생식기에존재하는인유두종바이러스는크게고위험인유두종바이 러스형 (16, 18, 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, 66, 68, 69) 과, 저 위험인유두종바이러스형 (6, 11, 34, 40, 42, 43, 44) 으로나뉜다. 특히 고위험군 HPV 의지속적인감염이자궁경부암의발생과직접연관이있는 것으로보여진다. 2-4 HPV 가자궁경부암을유발하는기전은고위험군 HPV 의 E6 및 E7 단백 이각각 p53 및 prb 세포주기억제단백의기능을억제함으로써세포주기 G1 에서 S 로의이행을억제하지못하여발생된다고생각되고있다. 5-6 p16 INK4a 는 cdk4 및 cdk6 와작용하여세포증식촉진단백인 cyclin D/cdk4,6 complex 의형성을방해하여 cdk 에의한 prb 의인산화 (phosphorylation) 을억제하여 cyclin dependent kinase inhibitor 로서작 용한다. 7 종양억제단백인 prb 가인산화되면 prb 와결합하고있던전사조 5
14 절인자인 E2F 가유리되어세포주기가 G1 에서 S 기로이행되도록유도한 다. 8 그러나 HPV 감염으로인하여 E7 단백이 prb 와결합하게되면 E2F 는 prb 로부터유리되어 G1-S 세포주기이행에관련된유전자의발현을촉진 하게된다. 9 HPV E7 단백에의해 prb 의기능이불활성화되면 prb 와 p16 INK4a 의음성되먹임 (negative feedback) 기전으로인해상반적으로 p16 INK4a 는과발현 (overexpression) 하게된다 본연구에서는상기의문헌지견을바탕으로자궁경부상피내종양의암 발생 (tumoriogenesis) 에관계하는세포조절주기단백인 p16 INK4a, cdk4, prb 의발현을관찰하고이와아울러고위험군 HPV 감염및재발여부와 세포주기단백발현정도를비교하여 p16 INK4a, cdk4 및 prb 의임상적유용 성을밝히고자하였다. 6
15 II. 재료및방법 1. 연구대상 2001 년 1 월부터 2003 년 12 월까지본원산부인과에내원하여자궁경부 착공생검 (punch biopsy) 및자궁경부환상투열절제술 (conization) 또는 제 1 형확대형전자궁적출술 (type I extended hysterectomy) 을시행받 고조직병리학적으로자궁경부세포내종양 (cervical intraepithelial neoplasia, 이하 CIN) 을진단받은환자들의파라핀포매조직절편으로 tissue array 를제작하였다 (by Petagen Inc. Seoul, Korea). Tissue array 는 117 개의 CIN1 병변과 44 개의 CIN2, 60 개의 CIN3, 44 개의 CIS 로구성 되었다. 자궁근종등양성병변으로전자궁적출술을시행받고조직병리 학적으로자궁경부에종양성병변이없는 10 예를대조군으로사용하였다. 대상환자들중고등급상피내병변 (CIN2, CIN3 or carcinoma in situ of uterine cervix, 이하 CIS) 을진단받은환자들은모두치료로서자궁경부 환상투열절제술또는제 1 형확대형전자궁절제술을시행받았다. 저등급상 7
16 피내병변 (CIN I) 을진단받은환자들은냉동치료 (cryotherapy), 레이저 치료, 또는자궁경부환상투열절제술을시행받았다. 이후 3 개월에서 6 개월 사이의간격으로추적검사로서자궁경부세포도말검사또는질확대경검사 검사를시행하였으며의심되는병변이있는경우질확대경조준하자궁경 부착공생검을시행하였다. 2 년이상추적관찰기간중시행한생검결과 상다시자궁경부상피내종양병변이진단되는경우를재발로판정하였다. 2. 방법 가. 고위험군 HPV 의검출 고위험군 HPV 검출에는인유두종바이러스 oligonucleotide microarray DNA chip (Biomedlab Co., Seoul, Korea) 또는 HPV hybrid capture II kit (Digene/Abbott, Clopper Road, Gaithersburg, Maryland, USA) 를사용하여 검사하였다. 고위험군에속하는아형은 16,18, 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, 66, 68, 69 이며, 저위험군에속하는아형은 6, 11, 34, 40, 42, 43, 44 이었다. 나. 면역조직화학염색 (immunohistochemical staining) 및판독 8
17 (1) p16 INK4a, cdk4, prb 의면역조직화학염색 p16 INK4a 의염색은 CINtec TM p16 INK4a Histology kit (DAKOCytomation, Carpinteria, CA, USA) 를사용하였고제작회사의사용설명서에따라염색 을시행하였다. Cdk4 및 prb 의염색은 cdk4 (DCS156) Mouse mab (Cell Signaling, Beverly, MA, USA) 및 retinoblastoma (Rb) Ab-1 (Neomarkers, Drive Fremont, CA, USA) 을이용하여시행하였다. Tissue array 로제작된파라핀포매조직을섭씨 38 도의열판과섭씨 60 도의오 븐에서각 2 시간동안둔후파라핀제거와함께함수과정을거쳤다. Xylene 에슬라이드를넣은후 5 분씩 3 회반복하여탈파라핀을시행후 100%, 90%, 70% 알코올에슬라이드를 1 분씩두어 xylene 이빠지도록하 였다. 흐르는물에 5 분간씻은후 ckd4 와 prb 의경우 3%H 2 O 2 에 10 분간 담갔다. Epitope retrieval 를위해 citrate buffer 를넣고 10 분간예열후 slide 를넣고 15 분간데운다. 증류수에슬라이드를담궈서 20 분간식힌후 세척완충액 (wash buffer) 에 5 분간담갔다. 그후 peroxidase-blocking reagent 200μl 를떨어뜨리고 10 분간처리하고슬라이드를꺼내어 citrate 9
18 buffer 에 3 분간담근후 (3 회반복 ), 슬라이드를완충액에서꺼내어잘털 어준다음미리희석해둔일차항체와음성대조시약을 1:25 로희석하여 각각의검체에떨어뜨린다음습성배양기 (wet incubator box) 에마르지 않도록뚜껑을잘덮어서실온에서 1 시간반응시켰다. 슬라이드를꺼내어 세척완충액에 3 분간담근후발색시약 200μl 를각각의검체에떨어뜨린 다음습성배양기에마르지않도록뚜껑을잘덮어서 30 분간습성시킨후, 슬라이드를꺼내어세척완충액에 3 분간담갔다 (3 회반복 ). 슬라이드를완 충액에서꺼내어잘털어준후미리만들어놓은 substrate-chromogen solution 200μl 를각각의검체에떨어뜨린다음습성배양기에 10 분간습 성시켰다. 슬라이드를꺼내어증류수에헹구고, 슬라이드를 hematoxylin bath 에 1 분간담근후, 슬라이드를꺼내어다시증류수에헹궜다. 100%, 90%, 70% 알코올에 1 분씩둔후, xylene 에슬라이드를넣은후 10 분간 두었다. Histomount 를사용하여 coverslip 으로 mount 하였다. (2) 결과판독및통계처리 p16 INK4a, cdk4, prb 의조직발현의판독은 400 배광학현미경하시야에서 10
19 관찰하여전체관찰된상피세포중염색된세포의수를세어백분율단위 로표기하였다. 모든슬라이드는두명의독립된관찰자에의해분석되었다. 각조직학적유형에따른고위험군 HPV 감염여부, 재발여부, p16 INK4a, cdk4, prb 의발현정도의관계를 one-way ANOVA test, t-test, Pearson s correlation test (SPSS 12.0) 로분석하였고, p 값이 0.05 미만인 경우를통계적으로유의하다고판정하였다. 11
20 III. 결과 Table 1 은자궁경부상피내종양의병기에따른 p16 INK4a, cdk4, prb 의염색정도를나타낸표이다. Figure 1, 2, 3 는 p16 INK4a, cdk4, prb 의염 색상이다. 24 자궁경부상피내종양의조직병리학적진행정도와 p16 INK4a, cdk4, prb 의이상발현정도와는통계적으로유효하게차이가있었다 (Table 1). 저 등급자궁경부상피내종양에서고등급자궁경부상피내종양으로진행함 에따라 p16 INK4a 및 cdk4 의이상발현정도가통계적으로유의하게증가 하여양의상관관계를보였다. 또한 prb 의경우고등급자궁경부상피내 병변으로진행할수록이상발현정도가감소하여조직병리학적진행정도와 통계적으로유의하게음의상관관계를보였다 (Figure 4). p16 INK4a 와 cdk4 의발현정도는뚜렷한양의상관관계를보였고이들과 prb 와의관계는음 의상관관계를보였다. prb 와 p16 INK4a /cdk4 의연관성을분석해보았을때 이들은음의상관관계를가졌다. 이관계는모두통계적으로유의하였다 (Pearson s correlation test, p<0.05). 16

28 p16 INK4a 의경우저등급및고등급상피내종양병변모두에서재발군에서 비재발군에비해통계적으로유의하게발현률이낮았다. prb 는고등급 자궁경부상피내종양 (CIN2, CIN3, CIS) 에서통계적으로유의하게 재발군에서발현률이높았으며비록통계적으로유의하지않았으나저등급 상피내종양에서도재발군에서 prb 의발현률이높은경향을보였다 (Table 4). 20

30 IV. 고찰 HPV 감염은자궁경부암의발생에가장큰원인으로알려져있다. HPV 감 염에의한자궁경부의암화과정은 HPV 의 E6 및 E7 단백이세포조절주 기단백의기능을저하함으로생겨남이알려져있다. 본연구에서는자궁 경부상피내종양에서 HPV 의 E7 의주요타겟으로알려져있는 RB pathway (p16 INK4a -cdk4/cyclind1-prb pathway) 의변화를관찰하여고 위험군 HPV 감염과의연관성및예후와의관계를살펴보고자하였다. 이 전연구결과에서 p16 INK4a 의과발현 (overexpression) 이자궁경부암을 포함한대장암, 폐암, 식도암, 구강암등에서이미알려진바있다 p16 INK4a 의과발현의기전은현재까지명확하게밝혀지지않았으나 HPV E7 이 ubiquitin 의존성단백분해계에의해 prb 의분해를초래하여상보 적으로 p16 INK4a 발현이증가한다는의견이제시된바있다 또한자 궁경부이형성의등급이증가할수록 cdk4 의발현이증가하였는데이는 cdk 억제자인 p16 INK4a 의과발현에도불구하고 HPV 의증식이숙주세포의 증식을통해이루어지는과정에서증가되는것으로사료된다. 본연구에 22
31 서는자궁경부상피내종양의조직병리학적등급이증가할수록 p16 INK4a 와 cdk4 의발현이증가하였으며 prb 의발현은감소하여위가설과일치 된결과를보였다. 특히 p16 INK4a 의경우자궁근종으로전자궁적출술을시 행한 10 예를대상으로면역조직화학검사를시행한결과 p16 INK4a 의발현 이매우낮고 (0~10%), 자궁경부상피내종양의이형세포의경우뚜렷 이염색되어자궁경부상피내종양진단의재현성 (reproducibility) 을증 가시키는객관적인표지로서사용될수있을것으로사료된다. 본연구에서는고위험군 HPV 감염이있는경우 p16 INK4a 이상발현이증 가하였으며이는이전연구결과들과일치한다 이는고위험군 HPV 의 E7 단백에의해 prb 의발현이저하되어음성되먹이기전에의하여 p16 INK4a 의발현이더욱증가하기때문으로추정된다. Nakao 등은 p16 INK4a 의과발현이늦게악성화 (late malignant transformation) 과정에 관여하는것이아니라암발생초기불멸화과정 (immortalization) 에서 증가함을보고한바있다. 21 따라서 prb 의기능의불활성화및 16 INK4a 의 과발현은암발생 (carcinogenesis) 의비교적초기과정에이루어지는것 23
32 으로보이고특히 CIN1 병변에서통계적으로유의하게 p16 INK4a 의발현 이증가한본연구결과에부합하는것으로사료된다. Geradts 등은면역 조직학적방법에의한 p16 INK4a 의핵염색이 Western blot 의결과와일치 하였다고보고하여암화과정을연구하는데있어 p16 INK4a 의경우면역조 직화학검사가좋은연구방법이될수있음을시사한바있다. 22 다만 상피세포의기저층 (basal epithelium) 및가시층 (spinous epithelium) 에서는 p16 INK4a 가강하게염색된데반해 HPV 의감염에의해생겼다고 믿어지는원반세포 (koilocytes) 나 HPV 의복제 (viral replication) 이가 장활발히이루어지는과립층 (granular layer) 에서는 p16 INK4a 가거의음 성소견을보였다. 결론적으로 p16 INK4a 의과발현이고위험군 HPV 감염을 시사하는표지로서사용될수있는가능성을다시한번확인할수있었다. RB pathway 에관여하는세포주기조절단백의악성종양의통상적인임 상병리학적예후인자로서의역할에대해서는현재까지는논란의여지가 많다. P16 INK4a 의경우결과의불일치 (discrepancy) 가두드러진다. 난소 암 (ovarian carcinoma) 이나전립선암 (prostate cancer) 에서는 p16 INK4a 24
33 발현이증가하는것이나쁜예후와관계가있다는논문들이발표된바있 다 반대로악성흑색종 (melanoma) 이나췌장암 (pancreatic cancer) 에서는 p16 INK4a 의발현감소가나쁜예후와관련이있다는보고들이있 었다 하지만이러한상반되는결과들은암종의 prb status 와관련 지어설명될수있다. 즉주로 prb 가활성화된종양 (Rb positive tumor) 에서는 prb 를불활성화시킬정도의충분한 cdk4 를활성화하기위해 p16 INK4a 의활성이감소되는데반해주로 prb 가불활성화되어있는종양 (Rb negative tumor) 에서는 cyclind1 의과발현이필요치않으므로 p16 INK4a 발현이상보적으로증가된다. 27 본연구에서는 p16 INK4a 가재발군 에서비재발군에서보다통계적으로유의하게낮은발현을보였고 prb 와 음의상관관계를보여이전연구와일치된결과를보였다. 28 따라서 p16 INK4a 및 prb 의면역조직화학검사는자궁경부상피내종양의예후인 자로사용가능할뿐만아니라치료방침을정하는데보조적수단으로이 용가능할것으로사료된다. 25

42 Abstract Relationship between p16 INK4a, prb, cdk4 and High risk HPV infection and recurrence Eun Ji Nam Department of Medicine The Graduate School, Yonsei University (Directed by Professor Yong Won Park) Purpose: The p16 INK4A family specifically interferes with the formation of the cyclin D1-dependent kinase complex, CDK4. Loss of functional p16 INK4A may result in unregulated CDK4 activity, leading to persistent retinoblastoma protein (prb) phosphorylation and uncontrolled cellular 34

The purpose of this study was to evaluate the expressions of p16 INK4a, CDK4, and prb (RB pathway) in cervical intraepithelial neoplasia(cin), and then to determine the relationship between CIN grade and the status of RB pathway. Methods: The study group was comprised of 265 formalin-fixed, paraffin-embedded tissue array sections of the uterine cervix collected from women who underwent biopsy or conization or hysterectomy at Yonsei university hospital from Jan 2001 to December Immunohistochemical staining for p16 INK4A, CDK4 and prb was performed by using CINtec TM p16 INK4A Histology kit (DAKOCytomation, Carpinteria, CA, USA), CDK4 (DCS156) Mouse mab (Cell Signaling, Beverly, MA, USA) and retinoblastoma (Rb) Ab-1 (Neomarkers, Drive Fremont, CA, USA). Immunostaining of the sections was reviewed by two independent pathologists. Expression rates of RB pathway 35
44 components were calculated as the proportion of positive cells within total cells in the x400 microscopic field. HPV detection and genotyping was performed with HPV DNA Chip, a PCR-based DNA microarray system provided by Microarray Center, Biomedlab Co (Seoul, Korea). Statistical analysis was carried out by ANOVA test, Student s t test, Pearson s correlation test. Results: The mean expression rate of p16 INK4A was 27% in CIN Ⅰ, 36% in CIN Ⅱ, and 61% in CIN Ⅲ. There was positive correlation between p16 INK4A expression rate and grade of cervical lesion (p<0.001). The mean expression rate of Cdk4 was 23% in CIN I, 28% in CIN Ⅱ, and 35% in CIN Ⅲ. There was positive correlation between CDK4 expression rate and grade of cervical lesion (p<0.001). The mean expression rate of prb was 47% in CIN I, 41% in CIN Ⅱ, and 19% in CIN Ⅲ. There was reverse correlation between prb expression rate 36
45 and grade of cervical lesion (p<0.001). The expression rate of p16 INK4A was higher (33%) in CIN I with high risk HPV infection, than in CIN I without high risk HPV infection (18%) (p=0.007). In all CIN lesions, the expression rate of p16 INK4A was lower in recurred group than without than in those which did not recur. (p<0.05) The expression rates of prb was higher in CIN2 and CIN3 that later recurred than in those which did not recur. (p<0.05) Conclusions: With increasing CIN grade, p16 INK4A and CDK4 expression was also increased, but prb expression was decreased. Relatively decreased p16 INK4a expressions and increased prb expressions significantly cooperate to predict a recurrence of the CIN lesions.
