We analyzed p16 INK4a expression in ASC-H liquid-based cytology specimens (LBCS) to develop more effective methods for the management of ASC-H patients. Methods: We carried out p16 INK4a immunostaining with 57 LBCS of ASC-H diagnostic categories, all of which were histologically cofirmed and 43 cases of which were compared with the results of a human papillomavirus (HPV) chip test. Results: p16 INK4a immunostaining with ASC-H LBCS was positive in 20% (3/15) of cervicitis, 25.0% (3/12) of tissue-low-grade squamous intraepithelial lesion, 75.0% (18/24) of tissuehigh grade squamous intraepithelial lesion (HSIL), and 100% (6/6) of invasive cancer cases. The positivity of p16 INK4a in LBCS was correlated with higher grade of histologic diagnosis (r= 0.578, p= 0.000). The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of p16 INK4a immunostaining for the prediction of tissue-hsil+ were 80.0%, 77.8%, 80.0%, and 77.8%, respectively. The sensitivity, specificity, PPV, and NPV of p16 INK4a immunostaining plus HPV chip test for predicting tissue-hsil+ were 71.2%, 86.4%, 84.2%, and 79.2%. Conclusions: p16 INK4a immunostaining as well as HPV chip testing with remaining LBCS with ASC-H are useful objective markers for the prediction of tissue-hsil+. Key Words: Cervical cancer; Cervical intraepithelial neoplasia; p16 INK4a ; DNA probes, HPV 자궁경부암은 15세에서 44세까지의여성에게발생하는암중, 전세계적으로두번째로많은암이며, 암으로인한사망원인에있어서도두번째를차지한다. 1 Pap 검사는자궁경부암을진단하는데있어가장성공적인선별검사중하나로, 이검사가도입된이래자궁경부암의발생률과사망률이감소하였다. 2 그러나, 세포학적검사는주관적성격을완전히배제할수없으며, 특히진단이애매한경계영역의증례를진단할때, 진단의재현성이크게떨어질수있다. 3,4 이러한경계영역을 Bethesada system에서는 atypical squamous cells of undetermined significance (ASC-US) 와 atypical squamous cells, cannot exclude HSIL (ASC-H) 로분류하였다. 5 이중, ASC-US 에서 high grade squamous intraepithelial lesion (HSIL) 이상으로진단되는비율은 11.6% 인데반해, ASC-H에서진단되는비율은 % 이다. 6,7 ASC-US는인유두종바이러스 (human papillomavirus, HPV) 검 사, Pap 검사의반복등상대적으로느슨한추적관찰을, ASC-H의경우는먼저질확대경검사와생검을시행하고, HSIL 이상의진단을찾지못한경우에는밀착추적관찰과 HPV 검사를시행하게된다. 8 이렇게경계영역에속한증례들을해결하기위한여러가지노력들이있어왔지만아직도많은문제점들이완전히해결되지않아시간손실이심하며, 의료비용또한증가하는원인이되고있다. 그러나최근자궁경부암의발생과정에서 p16 INK4a 의중요성이부각되면서, 자궁경부액상세포검사 (liquid-based cervical cytology) 검체에 p16 INK4a 면역세포화학염색을시행한연구들이있었고, 이연구들모두 cervical intraepithelial neoplasia (CIN)2 이상의증례에서 p16 INK4a 의과발현을보고하였으며 p16 INK4a 면역화학염색이 Pap 검사의경계영역에속하는증례를판독하는데도움이될것이라고제시하였다 그러나실제로 ASC-H 등의경계영역에대한연구는그증례수가충분치않고일치된결과를보이지않으며, ASC-H만을중점적으로연구한보고도거의없는실정이다. 이에 290

2 291 p16ink4a Immunocytochemical Staining in ASC-H 본 저자들은 ASC-H로 진단되었던 액상 자궁경부 세포진 검체에 포의 숫자 및 염색의 강도 두 가지를 기준으로 판독하였다. 즉 핵과 p16 면역세포화학염색을 시행하여, 이 염색이 Pap 검사의 경계 세포질에서 짙은 갈색으로 염색되는 상피세포를 세어 그 수가 10개 영역을 진단하는 데 실제적인 도움을 줄 수 있는지 알아보기로 하 이상일 때를 p16ink4a 과발현으로 간주하여 양성으로 판독하고(Fig. 였다. 1A), 염색된 세포가 전혀 없거나, 염색이 되더라도 염색 강도가 약 INK4a 한 세포만이 관찰되거나 혹은 짙은 갈색으로 염색된 상피세포가 10개 미만일 때를 음성으로 판독하였다. p16ink4a 면역세포화학염색 재료 및 방법 된 액상세포 슬라이드는 조직 결과를 모르는 상태에서 2명의 병리 의사가 판독하였고, 진단이 불일치한 증례의 경우에는 다인용 현미 연구 대상 경 하에서 협의하여 재판독하였다. 2008년 10월부터 2010년 11월까지 가톨릭대학교 의과대학 서울 자궁경부 조직 p16ink4a 면역화학염색 성모병원에서 Pap 검사를 시행하여 ASC-H를 진단받고, 이후 생검 이나, 원추조직절제술, 단순 또는 근치적 자궁절제술을 시행하여 자궁경부 조직 슬라이드는 모두 재검하여 대표 블록을 선정한 조직 검체를 얻은 환자 57명을 대상으로 하였다. 또한 연구에 앞서 후 4 μm 절편을 만들어 p16ink4a 면역화학염색에 사용하였다. 이때 환자의 임상 및 병리 기록지를 참고하여 진단 당시의 나이 및 HPV 검체는 탈파라핀 및 함수 과정을 거친 후 내인성 효소 제거를 위하 chip 검사 결과를 검색하였으며, 각 환자 검체의 실험적 사용을 위 여 0.3%의 H2O2로 처리하였다. 그리고 0.01 M citrate 완충액(pH 한 임상 윤리 위원회의 허가를 얻었다. 6.0)에 담가 microwave vacuum histoprocessor (RHS-1, Milestone, Bergamo, Italy)를 사용, 121 C에서 15분간 열을 가하여 항원복구를 자궁경부 액상세포 p16ink4a 면역염색 진행하고, p16ink4a 일차 항체(1:50, E6H4, Dako) 처리 후 실온에서 30분간 방치하였다. 그 후 Dako ChemMateTM EnVision kit (Dako) 자궁경부 액상세포검사 슬라이드(Suerpath, Becton-DickinTM son/tripath, Burlington, NC, USA)는 자일렌에 담가 커버슬라이드 를 이용하여 발색과정을 거친 후 헤마톡실린으로 대조 염색하고 현미경으로 관찰하였다. 를 제거한 후 100% 알코올로 자일렌을 제거하여 함수하였으며, 염 자궁경부 조직에서 시행한 p16ink4a 면역화학염색은 병변의 평편 색은 CINtec p16ink4a cytology kit (Dako Cytomation AS, Glostrup, 세포상피 전체 두께 아래 2/3 이상에서 미만성으로 세포의 핵과 세 Denmark)를 사용하여 제작자의 지침서에 준하여 시행하였다. 이 포질에 강하게 염색되는 경우를 p16ink4a 과발현으로 간주하여 양 를 간단히 기술하면, 98 C 온수에 10분간 담가 항원복구 과정을 거 성으로 판독하였다(Fig. 1B). 친 후 내인성 효소 제거를 위하여 3%의 H2O2로 처리하고 p16ink4a 일차항체(clone E6H4TM, ready-to-use) 처리 후 실온에서 30분간 방 HPV DNA chip 검사 치하였으며, 발색과정을 거친 후 헤마톡실린으로 대조 염색하였다. 자궁경부 액상세포 검체에서 시행한 p16ink4a 면역염색은 양성 세 A 병리과 전산프로그램을 통하여 환자가 자궁경부 액상세포진검사 B Fig. 1. 3 292 임광일 강여주 김태은외 6 인 후잔여검체로 HPV DNA chip 검사 (MyHPV DNA chip, MyGene, Seoul, Korea) 를시행받은경우그결과를확인하였다. 통계 통계는 SPSS ver (SPSS Inc., Chicago, IL, USA) 을사용, 카이 제곱검정및스피어만상관분석법으로분석하였으며, p< 0.05 를통 계적으로유의하다고정의하였다. 환자요약 결과 자궁경부액상세포검사에서 ASC-H 진단후조직검사를통하여 조직검체를얻은 57 명의환자를대상으로하였다. 환자는 24 세에서 79 세로평균나이는 47 세였다. 자궁경부조직검사상자궁경부염이 15 예, 원반세포증 (koilocytosis) 9 예, CIN1 이 3 예, CIN2 이 4 예, CIN3 이 19 예, 침윤암이 6 예로진단되었다. 이중원반세포증과 CIN1 을 조직 -low-grade squamous intraepithelial lesion (LSIL) 군, CIN2 와 CIN3 를조직 -HSIL 군으로분류하였다. 전체 57 명의 ASC-H 환자중 43 명에게잔여액상세포검체를이용 하여 HPV DNA chip 검사를진행하였는데, 이중 10 명 (23.3%) 이 HPV 음성, 33 명 (76.7%) 은 HPV 양성이었다. 33 명의 HPV 양성환자 중 15 명 (45.5%) 은 HPV 유형 16 또는 18 의감염이었으며나머지 18 명 (54.5%) 은그외유형의 HPV 감염이었다. 자궁경부액상세포및조직 p16 INK4a 면역염색결과 ASC-H 자궁경부액상세포검체에서 p16 INK4a 면역염색을진행한 결과, p16 INK4a 양성률은총 57 예중 30 예 (52.6%) 였다. 환자의조직 Table 1. Result of cytology p16 INK4a immunostaining according to clinicopathological characteristics Age (yr) Histologic results n Cytology p16 INK4a Negative (%) Positive (%) p-value (40.9) 13 (59.1) > (51.4) 17 (48.6) Cervicitis (80.0) 3 (20.0) < LSIL 12 9 (75.0) 3 (25.0) HSIL 24 6 (25.0) 18 (75.0) Cancer 6 0 (0) 6 (100) Negative (71.4) 8 (28.6) Postive 29 7 (24.1) 22 (75.9) < LSIL, low-grade squamous intraepithelial lesion; HSIL, high grade squamous intraepithelial lesion. 진단별로는자궁경부염총 15 예중 3 예 (20%), 조직 -LSIL 총 12 예중 3 예 (25.0%), 조직 -HSIL 총 24 예중 18 예 (75.0%), 침윤암총 6 예중 6 예 (100%) 에서양성소견을보였다 (Table 1). 조직진단의등급이높 아질수록자궁경부액상세포 p16 INK4a 면역염색의양성률은증가하 였다 (r= 0.578, p= 0.000). 자궁경부조직검체에서 p16 INK4a 면역염색을진행한결과 p16 INK4a 양성률은총 57 예중 29 예 (50.9%) 였다. 조직진단별로는자궁경부 염및조직 -LSIL 총 27 예중 2 예 (7.4%), 조직 -HSIL 총 24 예중 21 예 (87.5%), 침윤암총 6 예중 6 예 (100%) 에서양성소견을보였으며, 조 직진단의등급이높아질수록자궁경부조직검체의 p16 INK4a 면역 염색양성률이증가하였다 (r= 0.777, p= 0.000). 자궁경부조직검체의 p16 INK4a 면역염색과 ASC-H 자궁경부액상 세포검체의 p16 INK4a 면역염색간에결과일치율은총 57 예중 42 예 (73.7%) (p= 0.000, kappa value= 0.473) 였으며, 환자의나이에따른 자궁경부액상세포 p16 INK4a 면역염색양성률과의유의한차이는없 었다 (p= 0.439) (Table 1). 자궁경부액상세포 p16 INK4a 면역염색및 HPV DNA chip 의임상적유용성 ASC-H 자궁경부액상세포검체에서 p16 INK4a 염색검사의임상적 유용성을알아보기위하여조직 -LSIL 이상, 조직 -HSIL 이상, 조직 p16 INK4a 양성을기준으로민감도, 특이도, 양성예측률, 음성예측률 등을비교하였을때, 액상세포 p16 INK4a 염색은조직 -HSIL 이상을기 준으로하였을때가장좋은민감도, 특이도, 양성예측률및음성 예측률 ( 각각 80.0%, 77.8%, 80.0%, 77.8%) 을보였다 (Table 2). Correlation between cytology p16 INK4a immunostaining results and HPV infection status Cytology p16 INK4a Negative (%) Positive (%) 장좋은민감도, 특이도, 양성예측률및음성예측률 ( 각각 84.4%, 45.5%, 81.8%, 50.0%) 을보였다 (Table 2). 자궁경부액상세포 p16 INK4a 면역염색및 HPV DNA chip 검사를모두시행한총 43 예중, 자궁 경부액상세포 p16 INK4a 면역염색과 HPV DNA chip 검사결과가모 두양성인증례를양성으로, 어느하나라도음성일경우에는음성 으로보고, 민감도, 특이도, 양성예측률, 음성예측률을비교하였으 며, 조직 -HSIL 이상을기준으로하였을때민감도 71.2%, 특이도 86.4%, 양성예측률 84.2% 및음성예측률 79.2% 를보였다 (Table 2). 자궁경부액상세포 p16 INK4a 면역염색과 HPV DNA chip 결과의비교 ASC-H 자궁경부액상세포검체 p16 INK4a 면역염색결과와 HPV 감염여부, HPV 유형과의관계를알아보았을때, HPV DNA chip 결 과에따른액상세포 p16 INK4a 발현율의유의한차이는보이지않았 다 (p= 0.162). 그리고 HPV 양성환자중 HPV 유형 (HPV 유형 16,18 vs 비 -16,18) (p= 0.271) 에따른액상세포 p16 INK4a 발현율의유의한 차이또한보이지않았다 (p= 0.482) (Table 3). 고찰 Total HPV test Negative 7 (70.0) 3 (30.0) 10 Positive 14 (42.4) 19 (57.6) 33 Total p-value HPV type 16, 18 a 5 (33.3) 10 (66.7) 15 Others b 9 (50.0) 9 (50.0) 18 Total p-value a HPV 16 or HPV18 infection; b HPV non-16,18 infection. HPV 와자궁경부암이깊은관계가있다는것은이미널리알려 진사실이다 특히고위험군 HPV 는 99% 이상의자궁경부암에 서발견된다. 21 고위험군 HPV 는숙주세포의유전자에끼어들어가 서 E6, E7 발암단백질을발현하는데, E6 는 p53 종양억제단백질을 분해하며, E7 은 retinoblastoma (Rb) 단백질을비활성화시켜, 세포 가무한증식하게만든다. 22,23 정상상태에서 p16 INK4a 은 cyclin-dependent kinase 의억제단백질로, Rb 단백질을인산화시켜서세포 분열을억제하는음성되먹임작용을한다. 24,25 그런데 HPV 의 E7 발 암단백질에의해음성되먹임이작동되지않게되면그반작용으로 p16 INK4a 은증가되고, 면역조직화학염색으로증가된 p16 INK4a 을찾아낼수있게된다. 26 따라서 p16 INK4a 이발현된것은 HPV가단순히존재한다는것을넘어서활동적인상태에있다는것을의미한다. 27 본연구에서는자궁경부세포검사에서경계영역에속하여자궁경부염에서침윤암까지의가능성을모두배제할수없어환자치료방향결정에어려움이있는 ASC-H 환자를세분하고자액상세포 p16 INK4a 염색을시도하였으며, 자궁경부조직학적진단의등급이높아질수록자궁경부세포진 p16 INK4a 양성률이증가함을확인하였다 (r= 0.578, p= 0.000). 또한 ASC-H 환자의자궁경부세포진 p16 INK4a 면역조직화학염색결과는조직진단 HSIL 이상인환자를민감도 80.0%, 특이도 77.8%, 양성예측률 80.0%, 음성예측률 77.8% 로구별할수있었으며, 자궁경부세포진 p16 INK4a 면역조직화학염색결과와 HPV DNA chip 결과를종합하여판독할경우 HSIL 이상인환자를민감도 71.2%, 특이도 86.4%, 양성예측률 84.2%, 음성예측률 79.2% 로구별할수있었다. 이전 ASCUS/ASC-H 를대상으로 p16 INK4a 염색결과로 CIN2 이상을구별하고자한연구에서 Duncan 등 14 은민감도 48.4%, 특이도 67.2%, Guo 등 15 은민감도 54%, 특이도 52% 로진단적유용성이떨어진다고보고한반면, Nieh 등 16 은민감도 95%, 특이도 96%, Sung 등 17 은민감도 80%, 특이도 77.3% 로 CIN2 이상을구별하는데유용하다고보고하였다. Sung 등 17 의보고에서는 ASC-H 검체에서의민감도및특이도가각각 100%, 80% 로본연구의민감도 80.0%, 특이도 77.8% 와비교해볼때민감도에서차이를보였다. 그이유로대상군의차이, 액상세포검체 (Thin Prep vs SurePath TM ) 의차이및양성판정기준의차이를들수있겠다. Sung 등 17 은양성판정기준을 p16 INK4a 염색강도가약하여도분명히핵또는세포질의양성을보이는세포가관찰될때로정의하였다. 반면저자들은 Duncan 등 14 의보고와같이약하게염색되는세포들에반응성세포가다수포함되는것을관찰하여짙은갈색으로염색되는세포들만을양성세포로그수가 10 이상일때양성으로판독하였다. 자궁경부 CIN 조직진단시 p16 INK4a 면역조직화학검사를같이시행하면진단의정확도를높일수있다고보고되고있다. 28,29 본연구에서는자궁경부조직진단이 LSIL 이하인병소총 27예중 2예 (7.4%) 에서조직 p16 INK4a 가양성이었고, 조직진단이 HSIL 이상의병소총 30예중 3예 (10%) 에서조직 p16 INK4a 가음성으로나타났는데, 이는이전발표와유사한소견이었다. 또한 ASC-H 환자의자궁경부세포진 p16 INK4a 결과로조직 p16 INK4a 양성환자를구별하고자시도하였으며, 그민감도및특이도는각각 75.9% 및 71.4% 로조직-HSIL 이상을구별하고자할때의민감도, 특이도와유사하지만조금낮은결과를보였다. 한편 p16 INK4a 면역화학염색은비정형세포에염색되는이외에도, 배경세포에비특이적으로염색되거나, 세균, 다핵거대세포와난관화생에교차반응할수있다고보고되어있다. 9-11,30 본연구에서나타난낮은특이도는이런반응성세포들때문이라생각된다. 그외에, 자궁전절제를시행하지않은경우, 실제로는 HSIL 이

5 294 임광일 강여주 김태은외 6 인 상의병변이있을지라도조직검사에서그런병변을찾지못한경우나, HPV의감염이있고, p16 INK4a 의발현이있지만, 아직자궁이형성이나타나지않았거나 LSIL에머무는경우도생각해봐야하겠다. 일부연구결과에서는위양성세포들을형태학적으로구분하여음성세포로판독하고, 비정형세포에염색된경우만양성으로판독하기도하였다. 10,11 그러나본연구에서는 ASC-H를대상으로하였기에비정형세포에염색된경우와세포에비특이적으로혹은교차반응하여염색된경우를형태학적으로구분하기매우어려웠고, 구분하여양성세포를판독하였을때, 두병리의사간의의견이일치하지않았다. 때문에저자들은형태학적으로비정형세포와반응성세포를구분하지않고염색된모든세포의숫자를기록하였다. p16 INK4a 은 HPV 감염과연관된지표이지만, 자궁경부액상세포 p16 INK4a 결과와 HPV DNA chip 결과및유형 HPV 16, 18 여부는유의한상관관계를보이지않았다 (p> 0.05). 이는 p16 INK4a 이 HPV가단순히감염된경우는발현되지않고, 감염이활발히진행되어숙주세포가생리적반응을보이는경우에만발현되기때문으로사료된다. 결론적으로, 본연구에서는 ASC-H 환자에게 p16 INK4a 염색을시행하여조직-HSIL 이상의병소를민감도 80.0%, 특이도 77.8% 로구별할수있었으며, HPV DNA chip 검사와종합할경우조직-HSIL 이상의병소를민감도 71.2%, 특이도 86.4% 로구별할수있었다. 본결과에따르면, ASC-H 환자에게서잔여액상세포검체를이용한 HPV 고위험군감염여부검사및 p16 INK4a 을통하여 HSIL 이상의병소유무를예측하는데유용하리라사료된다. 그러나액상세포 p16 INK4a 염색검사가 ASC-H 환자의치료방침결정을위한기본검사로정착하기위하여는광범위한 ASC-H 환자를대상으로 p16 INK4a 염색결과에따른자궁경부조직진단의차이및추적관찰경과연구가필수적이라하겠다. 참고문헌 1.
