1 대한안과학회지 2009년제 50 권제 4 호 J Korean Ophthalmol Soc 2009;50(4): DOI : /jkos 접수번호 : 프로스타글란딘제제의장기점안이각막두께에미치는영향 박명희 조경진 문정일 가톨릭대학교의과대학안과및시과학교실 목적 : 개방각녹내장또는정상안압녹내장환자에서프로스타글란딘계열의점안안압하강제의사용이각막두께에미치는영향을알아보고자하였다. 대상과방법 : 개방각녹내장또는정상안압녹내장으로진단받은환자 65 명의 130 안을대상으로하였다. 안내수술의과거력또는각막에병변을가지고있는환자는대상에서제외되었다. 대상안을프로스타글란딘제제또는기타안압하강제를사용한 2 개의군으로나누어안압하강제를사용하기전과 24 개월이상 ( 평균 27 개월 ) 사용후각막두께, 최대교정시력, 굴절력, 전방내염증도등을측정하여비교하였다. 결과 : 안압하강제사용후프로스타글란딘제제중 Travoprost 와 Latanoprost 를사용한군에서중심각막두께의유의한감소를보였고, Bimatoprost 를사용한군에서는유의한변화를보이지않았다. 최대교정시력, 굴절력, 전방내염증도는두군모두에서유의한변화를보이지않았다. 결론 : 안압을효과적으로낮출수있는프로스타글란딘제제중 Travotrost 와 Latanoprost 의안압하강제는장기간사용시각막의두께를유의하게감소시켰다. < 대한안과학회지 2009;50(4): > 프로스타글란딘계열의안압하강제는고안압증, 원발개방각녹내장, 정상안압녹내장등의환자에서안압을안전하고효과적으로낮추는것으로증명되었다. 1 하지만결막충혈, 각막미란, 홍채및눈주위피부의색소침착, 속눈썹의길어짐과두꺼워짐, 안와지방의위축, 낭포황반부종, 단순포진각막염등의부작용이생길수있다. 2-4 또한혈액- 방수장벽을약화시켜전방내염증을일으킨다는보고도있다. 5 최근에는프로스타글란딘에의한각막두께의변화에대해서도발표가되어지고있는데, Lass et al 6 은프로스타글란딘제제중하나인 Latanoprost % (Xalatan ; Pfizer Inc., New York, NY, USA) 를 1년간점안한군을베타차단제인 Timolol 0.5% (Timoptic XE; Apotex Inc., USA) 을 1년간점안한군과비교했을때각막에미치는영향에차이가없다고했지만 Arcieri et al 7 은프로스타글란딘제제로여겨지는 Bimatoprost 0.03% (Lumigan ; Allergan Inc., Irvine, CA, USA) 를 4주간점안한군에서유의한각막두께의감소를보였다고하였다. 이러한각막두께의변화는안 접수일 : 2008 년 5 월 20 일 심사통과일 : 2008 년 12 월 9 일 통신저자 : 문정일서울시영등포구여의도동 62 가톨릭대학교성모병원안과 Tel: , Fax: jimoon@catholic.ac.kr * 본논문의요지는 2008 년대한안과학회제 99 회춘계학술대회에서구연으로발표되었음. 압의측정값에영향을줄수있기때문에정확한안압의변화를알아야하는녹내장환자에게있어서의미있는부작용일수있다. 8 따라서본연구에서는안압하강제중프로스타글란딘제제를장기간점안했을때각막두께에미치는영향에대해서알아보고자하였다. 대상과방법 2003년 1월부터 2008년 2월까지성모병원안센터에서원발개방각녹내장또는정상안압녹내장으로진단받은환자를대상으로하였고이전에안내수술을받은경력이있거나각막외상및각막질환등의과거력이있는경우는대상에서제외하였다. 이들은모두프로스타글란딘제제및알파길항제, 또는베타차단제의안압하강안약중 1가지만을진단시부터최소 24개월이상사용하였으며경구용약제등기타다른안압하강제를사용하지않았다. 이들을프로스타글란딘제제의안압하강제를사용한군과그외의안압하강제를사용한군으로나누어안압하강제를사용하기전그리고안압하강제사용후최소 24개월이후에각막두께, 최대교정시력, 굴절력, 전방내섬광을측정하여비교하였다. 각막의두께는 Orbscan (Orbtek Ic., USA) 을사용하여 Topography로측정하였으며각막의중간, 위쪽, 아래, 안 565

CANON) 를이용하여측정후최적구면도수 (manifest spherical equivalent) 로환산하였다. 전방내섬광은세극등현미경을이용하여관찰하여단위면적당섬광의개수에따라 0에서 4로나누었다. 사용된프로스타글란딘제제는 Latanoprost 0.005% (Xalatan ; Pfizer Inc., New York, NY, USA), Travoprost 0.004% (Travatan ; Alcon Inc., Ft Worth, TX, USA), Bimatoprost 0.03% (Lumigan ; Allergan Inc., Irvine, CA, USA) 였으며, 대조군에서는 Brimonidine 0.2% (Alphagan P; Allergan Inc., Irvine, CA, USA), Timolol 0.5% (Timoptic XE; Apotex Inc., USA), Betaxolol 0.5% (Betoptic-S; Alcon Inc., USA) 을사용하였다. 통계는양군간의나이, 각막두께, 교정시력, 그리고굴절력의비교는 Windows SPSS version 13.0을사용하여 independent t-test 방법을사용하였으며, 실험군및대조군의각약물별투약전후의각막두께의변화비교는비모수검정방법인 Wilcoxon signed rank test를사용하였다. 결과 대상안은원발개방각녹내장 59안과정상안압녹내장 71안이었으며이중프로스타글란딘제제를사용한 군이 70안, 대조군이 60안이었다. 프로스타글란딘제제중 Latanoprost, Travoprost, Bimatoprost가각각 36안, 26안, 8안에서사용되었으며대조군에서는 Brimonidine, Timolol, Betaxolol 이각각 22안, 19안, 19안에서사용되었다. 평균연령은프로스타글란딘제제를사용한군이 56.25±15.07 (25~78) 세, 대조군이 54.04±11.73 (38~78) 세로유의한차이를보이지않았다 (p-value 0.581). 성별은프로스타글란딘을사용한군에서여자 : 남자 =38:32, 대조군에서여자 : 남자 =34:26이었으며두군간에성별의유의한차이를보이지않았다 (p-value 0.494). 안압하강점안제의평균사용기간은프로스타글란딘제제를사용한군이 27.11±13.88 (24.7~45.3) 개월, 대조군이 28.14±12.14 (24.3~43.8) 개월로통계적으로유의한차이를보이지않았다 (p-value 0.793)(Table 1). 안압하강제사용전에측정된각막의두께는각막의중간부분에서프로스타글란딘제제를사용한군과대조군이각각 ±37.49 µm, ±28.42 µm (p-value 0.604), 위쪽부분에서 ±32.17 µm, ±34.55 µm (pvalue 0.611), 아래쪽부분에서 ±48.27 µm, ±39.48 µm (p-value 0.635), 안쪽부분에서 ±38.38 µm, ±44.29 µm (p-value 0.455), 바깥쪽부분에서 ±43.56 µm, ±41.29 µm (p-value 0.594) 으로모두통계적으로유의한차이를보이지않았다 (Table 2). 프로스타글란딘을사용한군에서좌안이 33안, 우안이 566

( p =0.057, p <0.001) 37이었으며이들의중심각막두께의평균은각각 ± µm, ±31.08 µm (p-value 0.843) 으로통계적으로유의한차이를보이지않았다. 또한대조군에서좌안이 29안, 우안이 31안이었으며이들의중심각막두께의평균은각각 ±27.14 µm, ±24.89 µm (p-value 0.791) 으로통계적으로유의한차이를보이지않았다. 안압하강점안액의사용후측정된각막의두께및안압하강점안액사용전과의차이를보면프로스타글란딘제제를사용한군에서가운데부분이 ±34.29 µm ( 변화량 µm, 변화율 2.9%, p-value<0.001) (Fig. 1), 위쪽 ±45.07 µm( 변화량 µm, 변화율 2.6%, p-value<0.001) (Fig. 2), 아래쪽 ±32.48 µm( 변화 량 µm, 변화율 1.1%, p-value 0.203) (Fig. 3), 안쪽 ±41.28 µm( 변화량 µm, 변화율 3.5%, p- value<0.001) (Fig. 4), 바깥쪽 ±48.55 µm( 변화량 µm, 변화율 3.1%, p-value 0.001) (Fig. 5) 으로측정되었으며대조군에서는가운데부분이 ±47.57 µm ( 변화량 µm, 변화율 1.3%, p-value 0.055) (Fig. 1), 위쪽 ±33.53 µm( 변화량 µm, 변화율 0.0%, p-value 0.919) (Fig. 2), 아래쪽 ±47.28 µm( 변화량 -4.7 µm, 변화율 0.8%, p-value 0.269) (Fig. 3), 안쪽 ±44.11 µm( 변화량 µm, 변화율 1.7%, p- value 0.057) (Fig. 4), 바깥쪽 ±28.89 µm( 변화량 µm, 변화율 1.7%, p-value 0.270) (Fig. 5) 로측정 567

( p =0.270, p =0.001) 되어두군의모든부분에서각막두께의감소를보였고특히프로스타글란딘제제를사용한군에서, 그리고가운데, 위쪽, 안쪽, 바깥쪽에서통계적으로유의한각막두께의감소를보였다 (Table 3). 프로스타글란딘제제를사용한군에서각각의점안안압하강제에대해사용전과사용후의중심각막두께를비교해보면 Latanoprost 0.005% 를사용한군에서각각 ± µm, ±51.92 µm (p-value<0.001), Travoprost 0.004% 를사용한군에서각각 ±31.71 µm, ± µm (p-value<0.001), Bimatoprost 0.03% 를사용한군에서각각 ±49.67 µm, ±17.95 µm (p-value 0.691) 으로 Latanoprost 를사용한군과 Travo prost를사용한군에서는각막두께의유의한감소를보였으나 Bimatoprost를사용한군에서는유의한변화를보이지않았다. 또한대조군에서각각의점안안압하강제에대해사용전과사용후의중심각막두께를비교해보면 Brimonidine 0.2% 를사용한군에서각각 ±27.89 µm, ±40.28 µm (p-value 0.562), Timolol 0.5% 를사용한군에서각각 ±30.30 µm, ±32.51 µm (p-value 0.514), Betaxolol 0.5% 를사용한군에서각각 ±37.42 µm, ±41.70 µm (p-value 0.012) 으로 Brimonidine를사용한군과 Timolol 를사용한군에서는각막두께의유의한변화를보이지않았으나 Betaxolol 를사용한군에서는유의한감소를보였다. 최대교정시력은점안안압하강제를사용하기전과사용후프로스타글란딘제제를사용한군에서각각 0.11±0.14 logmar, 0.10±0.18 logmar (p-value 0.377), 대조군에서각각 0.13±0.11 logmar, 0.11±0.27 logmar (p-value 0.308) 로두군에서모두유의한차이를보이지않았다. 굴절력은점안안압하강제를사용하기전과사용후프로스타글란딘제제를사용한군에서각각 -2.29±0.28 Dioptor, -2.27±0.21 Dioptor (p-value 0.582), 대조군에서각각 -1.79±0.21 dioptor, -1.91±0.31 Dioptor (p-value 0.323) 로두군에서모두유의한차이를보이지않았다. 전방내염증정도를측정하는전방내염증도는점안안압하강제사용전과사용후두군에서모두 0으로측정되었다. 568

5 - 박명희외 : 프로스타글란딘제제와각막두께 - 고찰 안압하강제로사용되는프로스타글린딘제제 (Prostaglandin F 2α analogue; PGF 2α analogue) 는모양체근을이완시키며 9,10 모양체근과공막의기질금속단백분해효소 (Matrix metalloproteinases; MMPs) 를증가시키게되고이 MMPs 에의해콜라겐 (Collagen) 의분해가일어나게되어세포외간질의재구성이일어나포도막공막유출이증가하게된다 또한 PGF 2α analogue는각막에흡수되어가수분해되고활성화되어 MMPs 를증가시키게되는데각막의 MMPs 증가는원추각막환자의 MMPs 증가로인한각막의얇아짐 14,15 과같은기전으로각막두께에변화를주지않을까생각된다. 하지만이전연구들에서 PGF 2α analogue로인한각막두께의변화에대해논란이많은데, Lass et al 6 의연구에서는 Latanoprost를점안한군과 Timolol 을점안한군간에각막의변화에차이가없다고했으나, Harasymowycz et al 16 의연구에서는 Travoprost를 6주동안사용했을때각막의두께가 6.9 µm 얇아졌다 (p-value<0.001). Brandet et al 17 의연구에서도 PGF 2α analogue만을사용한군이베타차단제를사용한군에비해빠른각막두께의감소속도를보였다고했다. 본연구에서는 PGF 2α analogue를평균약 27개월점안한군에서중심및위쪽, 안쪽, 바깥쪽의각막두께의유의한감소가나타났고알파길항제및베타차단제를비슷한기간동안점안한대조군에서는각막두께의유의한변화를보이지않았다. 하지만원추각막에서는각막의얇아짐이아래쪽부위에서많이일어나게되는데원추각막에서는프로스타글란딘제제로인한것과는달리추가적으로각막세포의자연사멸및다른기계적효과가작용할것으로생각된다. 프로스타글란딘제제의약제별각막두께의변화에서는 Latanoprost 및 Travoprost를사용한군에서는유의한각막두께의감소를보였으나 Bimatoprost를사용한군에서는유의한변화를보이지않았다. 사실 Bimatoprosts 는 PGF 2α analogue라는점에있어논란이있어왔는데, Bimatoprost의 C1기에붙은아마이드 (amide) 기때문에 PGF 2α analogue가아닌 prostamide라고분류되기도하였다. 18,19 또한 Latanoprost 및 Travoprost는각막에서가수분해되면서활성화되는데 Bimatoprost는각막에서가수분해되어활성화되는양이상대적으로적다고한다. 20 이러한차이로인해 Bimatoprost가각막의두께에미치는영향이적을것으로생각이되지만본연구에서 Bimatoprost 를점안한대상이 8안으로적었기때문에앞으로더많은대상안에대한연구가있어야하겠다. 또한대조군에대해서 Brimonidine 및 Timolol 를사용한 군에서는각막두께의유의한변화를보이지않았으나 Betaxolol 을사용한군에서는각막두께의유의한감소를보였다. 이는 Latanoprost를사용한군에서비슷한기간동안각막두께가평균 µm, 그리고 Travoprost를사용한군에서 µm 감소한것에비하면적은양 ( µm) 이지만통계적으로유의한감소를보였으므로더많은연구가필요할것으로생각된다. 그외에본연구에서는언급되지않았으나각막두께에일중변동이있는지도규명해볼필요가있을것이며, 정상적인노화과정에서각막두께의변화가있는지도연구해보아야할것이다. 이전에 von Bahr, 21 Hansen, 22 Wolfs et al 23 의발표들에서는 1년간각막두께에유의한변화가없다고했지만, Brandt et al, 24 Hahn et al 25 의연구에서는각각 1년에 µm, µm의각막두께의변화를보고했다. 하지만본연구에서보인점안안압하강제사용에따른감소량에비해적은양임을알수있다. 본연구에서사용된각막두께측정법은 Orbscan을이용한것으로초음파측정계 (Ultrasonic pachymeter) 를이용한결과와는차이가있을수있다. 하지만 Kang et al 26 은 Orbscan 이초음파측정계와같이재현성이높은장치이며임상에서각막두께측정에활용될수있을것이라고했다. 결론적으로본연구에서는프로스타글란딘제제중 Latanoprost 및 Travoprost를약 27개월간점안했을때각막두께의유의한감소를보였으나안압측정시의오차를일으킬수있는정도의임상적으로의미있는변화는아닐수있다. 하지만평생약을투여해야하는녹내장의특성상더장기간에걸친각막두께의변화, 장기간사용에의한점안안압하강제의각막누적효과, 그리고원추각막으로각막이식을시행한후에안압상승이발생하는경우프로스타글란딘계열점안안압하강제의원추각막재발에미치는영향등에대해서도연구가필요할것으로생각된다. 참고문헌 1) Alexander CL, Miller SJ, Abel SR. 7 - 박명희외 : 프로스타글란딘제제와각막두께 - =ABSTRACT= The Effects of Prostaglandin Analogues on the Corneal Thickness Myoung Hee Park, MD, Kyongjin Cho MD, Jung-Il Moon, MD Department of Ophthalmology, St. Mary s Hospital, The Catholic University of Korea, Seoul, Korea Purpose: To evaluate the effects of prostaglandin analogues on the corneal thickness of patients with primary open-angle glaucoma (POAG) or normal tension glaucoma (NTG). Methods: This study included 130 eyes of 65 patients who were diagnosed with POAG or NTG. All patients were divided into two groups; one group received prostaglandin analogues, while the other group received alternative ocular hypotensive eyedrops. Corneal thickness, best corrected visual acuity, and flare in the anterior chamber were measured and compared before treatment and at least 24 months (mean: 27 months) after treatment. Results: The mean decrease in corneal thickness was statistically significant in the group using prostaglandin analogues, but not in the control group. Among the various prostaglandin analogues used, travoprost and latanoprost decreased mean corneal thickness, but bimatoprost had no effect. Best corrected visual acuity, refraction power, and flare in the anterior chamber did not change significantly in either group of patients when ocular hypotensive eyedrops were used.
