Due to its potent anti-inflammatory property, thalidomide is being re-evaluated in several clinical fields. Thus, we examined whether thalidomide affects neointimal overgrowth. Materials and Methods:Male Sprague-Dawley rats, pretreated with thalidomide (100 mg/kg qd) for 3 days, underwent carotid artery angioplasty. administration was then continued for 2 weeks after injury. Results:Compared with the control rats, the systemic inflammatory marker (serum TNF-α) reduced significantly in the thalidomide-treated rats at 3 and 14 days after injury (856±213 vs 449±68 pg/ml, p=0.001, day 3;129±34 vs 63±18 pg/ml, p=0.001, day 14). This effect was accompanied by marked decreases in the arterial macrophage infiltration and by attenuated expressions of TNF-α and bfgf in the arteries, which were measured as local tissue inflammatory indicators. The anti-proliferative effect of thalidomide was confirmed by a reduced number of PCN-positive vascular smooth muscle cells in the arteries (43.1±2.9 vs 7.4±1.7 %, p<0.001, day14). Morphometric analysis 2 weeks after injury revealed that gains in the luminal area of the thalidomide-treated rats (0.17±0.04 vs 0.05± 0.02 mm 2, p=0.001) were due to the suppression of neointimal hyperplasia (neointima-to-media[n/m] ratio, 0.35±0.13 vs 1.26±0.29, p<0.001). Moreover, a strong positive correlation was observed between the serum TNF-α and the N/M ratio. Conclusion:Through its anti-inflammatory and anti-proliferative effect, thalidomide significantly inhibits neointimal hyperplasia. Therefore, thalidomide can be applied in various ways, for instance, as a new drug-eluting stent or as a systemic oral drug against restenosis. 2 서론 관동맥재협착예방을위한최근의눈부신발전에도불구하고이는여전히경피적관동맥중재술의주요한한계로남아있다. 재협착의병리기전이아직완전히규명되지는않았지만염증반응이중추적역할을하고있다는증거들이계속쌓여가고있다. 1) 게다가염증반응이강할수록재협착도심하게발생한다. 2) 혈관성형술로인한염증반응이재협착을유발한다는증거들은세포, 분자, 유전자수준의여러단계에서제시되고있다. 풍선성형술이나스텐트로혈관에손상을가하면혈관평활근세포 (vascular smooth muscle cells, VSMCs) 와염증세포는활성화되고 3) 이들로부터다양한종류의사이토카인과성장인자 (growth factor, GF) 들이분비되어 4) VSMCs의유주 (migration) 와분열 (proliferation) 을촉진하여결국혈관신생내막증식 (neointimal hyperplasia) 을초래하게된다. 5) 더군다나 CD 18, IL-1 receptor antagonist, matrix metalloproteinases 와 6)7) 같이염증반응에서주요한역할을담당하고있는분자들의유전형질과재협착과의연관성이유전자다형성 (polymorphism) 연구를통해밝혀지고있다. 한편처음에진정제로개발되었던탈리도마이드는이후강력한항염증, 면역조절효과가있음이알려지게되어 8) 최근에는루푸스, 크론씨병, 류마티스관절염, 다발성골수염, 이식편대숙주반응등과같은다양한범위의질환들에서다시사용되고있다. 8) 이러한항염증효과에근거하여만성심부전환자들에게탈리도마이드를 6개월간투여하여종양괴사인자 (tumor necrosis factor, TNF-α) 가감소하고좌심실의구혈률이호전되었음을보고하기도하였다. 9) 탈리도마이드는또한세포수준에서섬유아세포성장인자 (basic fibroblast growth factor, bfgf) 의분비를억제하는데 8) bfgf 는염증반응을매개하는동시에 VS- MCs 의활발한분열을조장한다. 5) 세포부착분자의발현또한탈리도마이드로감소되고 8)10)11) 따라서염증세포의유주능도저하된다. 게다가탈리도마이드는염증반응과세포분열의핵심적전사인자 (transcription factor) 인 NF-κB의활성화를억제한다. 12) 따라서본연구는상기한사실에근거하여탈리도마이드가혈관성형술후혈관신생내막증식을억제하 는지를그리고이약제가혈관재협착예방을위한치료제로서의가능성을갖고있는지를규명하고자하였다. 재료및방법 재료및전처치모든실험절차는서울대학교병원, 임상의학연구소, 전임상실험실의 동물실험가이드라인 을준수하였다. 탈리도마이드 (lan Pharmaceutical, London, U.K.) 를 corn oil(sigma-ldrich Korea) 에녹여 50 mg/ml 농도의혼탁액을만들었다. 혈관성형술 3일전부터 17 주령의수컷 Sprague-Dawley 쥐 (417±11 g, 대한바이오링크사, 충북 ) 에 100 mg/kg qd의탈리도마이드를경구투여하였다. 혈관손상모델과후처치전처치에대해 blind 한시술자가쥐경동맥에풍선손상을가하였다. Xylazine(5 mg/kg, 유한양행, Bayer Korea) 과 ketamine hydrochloride(50 mg/kg, 유한양행, Bayer Korea) 를복강내로주사하여마취하였다. 우측경동맥을노출, 절개한후 2 F 풍선카테터 (Baxter Healthcare Corp) 를삽입하였다. 카테터를총경동맥원위부까지삽입한후풍선을확장시킨상태로잡아당기면서좌측총경동맥전체에일정한손상을가하였다. 이과정을 5회정도반복하여혈관내피를탈락시키고혈관벽에압력손상을가하였다. 혈관성형술후 2주동안시술전과동량의탈리도마이드투여를지속하였다 (100 mg/kg qd). 경동맥수집과형태학적분석혈관성형술후 3일과 14일째형태학적분석을위해시술받은쥐에게과량의 pentobarbital 투여후희생시켰다 ( 각군과각시점당 8마리씩 ). 양측의총경동맥을약 1 cm 정도절제한후 10% formalin 으로관류고정한다음파라핀에포매 (embedding) 하고일정한간격을두고네곳에서 4 μm 두께의절편을 4~6 개얻었다. 절편을 hematoxylin-eosin이나 Verhoeff-van Gieson 으로염색하고혈관내강 (lumen), 신생내막 (neointima), 중막 (media) 의면적을 Image-Pro Plus

를이용하여구하였다. 각각의면적은한혈관의다른네부위에서측정하여평균을구하였다 ( 각군당, 각시점당 8마리씩 ). 염증반응의측정탈리도마이드의기전을규명하기위하여성형술을받은혈관의염증반응과분열정도를검사하였다.

ELIS 를이용한혈청 TNF-α 측정 TNF-α 는염증반응의중추적매개체로서혈관성형술후활성화된대식세포나 VSMCs 에서분비된다. 3)4) 탈리도마이드가 TNF-α 발현을억제하는지확인하기위해시술후각각 3일과 14일째혈청 TNF-α 의농도를측정하였다. 경동맥채취시 18 G 정맥카테터를복부대동맥에삽입시켜칼슘튜브에동맥혈을모으고 4 에서혈청을원심분리한후 -70 에보관하였다. 혈청을녹인후효소면역측정법키트 (enzyme-linked immunosorbent assay kit, ELIS kit, BD PharMingen, C, US) 를사용하여 TNF-α 농도를 duplicate 으로측정하여평균을구하였다. TNF-α ELIS 의민감도는 13 pg/ml 였다. 4 염증반응도감소시키는지를확인하기위하여 Western blot 으로시술받은경동맥의 TNF-α 와 bfgf 의발현을분석하였다. 혈관조직을 lysis buffer 에서용해시킨후 Micro BC Protein ssay kit(pierce) 를사용해단백질을정량화하였다. 20 μg씩의단백질을 SDSpolyacrylamide gel 에서전기영동으로분리하고 nitrocellulose membrane 에옮긴후이들단백질에특이적으로반응하는항체를사용하였다. 항 TNF-α 항체 (N- Serum TNF-α (pg/ml) ; sc-1350) 와항 bfgf 항체 (147; sc-79) 는 Santa Cruz Biotechnology에서구입하였다. 면역조직화학염색법을통한국소조직의염증반응측정여러사이토카인과성장인자의원천인대식세포는손상된혈관에침윤하는것으로보고된다. 2)13) 면역조직화학염색으로대식세포의혈관침윤과 VSMCs 에서 bfgf 의발현을확인하기위해다음과같은특이항체를사용하였다. 대식세포침윤 (ED-1, Serotec), bfgf 발현 (147; sc-79, Santa Cruz Biotechnology). 슬라이드에서파라핀을제거한후 microwave 로항원을노출시키고표준화된 peroxidase-antiperoxidase 염색과정을거쳤다. 결과 탈리도마이드를통한혈관신생내막증식의억제시술 3일째혈관내막과중막은두그룹사이에의미있는형태학적차이를보이지않았다. 아직양쪽그룹에서신생내막은관찰되지않았다 (Fig. 1C, D, n=16). 그러나시술 2주후대조군에서는현저한신생내막의증식이관찰되었고 (Fig.

29 대 0.35±0.13, p<0.001, Fig. 1F, H, I, J). 따라서혈관내경은탈리도마이드투여로보다넓게유지가되었다 (0.24±0.04 대 0.36±0.06 mm 2, p<0.001). Fig. 1I, J에결과를요약하였다.

탈리도마이드는전신염증반응의표지자인혈청 TNF-α 를시술후 3일과 14일째각각 48% 와 51% 씩감소시켰다 (856±213 대 449±68 pg/ml, n=16, p=0.001, 3일 ;129±34 대 63±18 pg/ml, n=16, p=0.001, 14일, Fig. 2).

VSMCs 의세포분열을정량화하기위해혈관절편당네부위에서 PCN 로염색된세포를전체세포수로나누어 PCN Labeling Index 를구하였다 ( 각시점과각그룹당 n=3). 통계분석모든 data 는평균 ± 표준편차로표시하였다. 탈리도마이드군과컨트롤군을 unpaired, 2-tailed t test로비교하였고 p<0.05 를통계적으로유의한것으로정의하였다. 모든통계적인계산은 SPSS 11.0 프로그램을사용하였다. 결과 탈리도마이드를통한혈관신생내막증식의억제시술 3일째혈관내막과중막은두그룹사이에의미있는형태학적차이를보이지않았다. 아직양쪽그룹에서신생내막은관찰되지않았다 (Fig. 1C, D, n=16). 그러나시술 2주후대조군에서는현저한신생내막의증식이관찰되었고 (Fig. 1E, G) 이에반해탈리도마이드군에서는신생내막의면적이 71% 감소하였으며 (0.17± 0.04 대 0.05±0.02 mm 2, n=16, p<0.001), 신생내막 / 중막비 (neointima/media ratio, N/M ratio) 는 72% 감소하였다 (1.26±0.29 대 0.35±0.13, p<0.001, Fig. 1F, H, I, J). 따라서혈관내경은탈리도마이드투여로보다넓게유지가되었다 (0.24±0.04 대 0.36±0.06 mm 2, p<0.001). Fig. 1I, J에결과를요약하였다. 혈관성형술을받지않은경동맥은대조군과탈리도마이드군에서차이를보이지않았다 (Fig. 1, B). 탈리도마이드를통한전신염증반응의감소탈리도마이드투여로인한내막형성의억제는염증반응의감소와밀접히연관되어있었다. 탈리도마이드는전신염증반응의표지자인혈청 TNF-α 를시술후 3일과 14일째각각 48% 와 51% 씩감소시켰다 (856±213 대 449±68 pg/ml, n=16, p=0.001, 3일 ;129±34 대 63±18 pg/ml, n=16, p=0.001, 14일, Fig. 2). 더욱이 14일째혈청 TNF-α 와 N/M ratio 는강한양의상관관계를보여 (Fig. 2B) 혈청 TNF-α 가억제될수록 N/M ratio 도감소하였다 (Pearson correlation coefficient 0.908, n=16, p<0.001, two-tailed). 손상혈관에서탈리도마이드를통한국소염증반응의감소탈리도마이드투여로손상받은혈관에서 TNF-α 와 bfgf 의국소발현이억제됨을 Western blotting 으로확인하였다. 혈관성형술을시행하지않은혈관에서는 TNF-α 와 bfgf 의발현이미미하였으나시술후 3 일과 14일째대조군에서는이들의발현이뚜렷이증가하였다.

전체혈관세포의핵중 bfgf 양성핵의수를 bfgf 양성지수로정의하였을때탈리도마이드는 bfgf 양성지수를혈관성형술후 3일과 14일째현저히감소시켰다 (43.3±6.9 대 9.0±2.8%, n=3, p<0.001, 3일 ;39.8± 4.2 대 15.0±5.8%, n=3, p<0.001, 14일, Fig. 5G).

이러한모든결과는탈리도마이드가전신염증반응뿐만아니라국소염증반응도억제함을보여주는것이다. 탈리도마이드를통한혈관평활근세포분열능의감소억제된염증반응은 VSMCs 의분열능감소와동반하여나타났다. 혈관성형술후 3일째대조군에비해서탈리도마이드를투여한혈관의중막에서는 PCN 양성세포수가현저하게감소하였다 (Fig. 6C, D).

6 손상혈관에서탈리도마이드를통한국소염증반응의감소탈리도마이드투여로손상받은혈관에서 TNF-α 와 bfgf 의국소발현이억제됨을 Western blotting 으로확인하였다. 혈관성형술을시행하지않은혈관에서는 TNF-α 와 bfgf 의발현이미미하였으나시술후 3 일과 14일째대조군에서는이들의발현이뚜렷이증가하였다. 하지만탈리도마이드를투여한혈관에서는두시점에서모두그발현이현격히감소하였다 (Fig. 3, B). 탈리도마이드가 bfgf 를억제함은면역조직화학염색에서도확인되었다. 대조군에서는 bfgf 염색이강하게되었으나 (Fig. 4C, E) 탈리도마이드를투여한혈관의핵과세포질에서는유의하게약화되었다 (Fig. 4D, F). 전체혈관세포의핵중 bfgf 양성핵의수를 bfgf 양성지수로정의하였을때탈리도마이드는 bfgf 양성지수를혈관성형술후 3일과 14일째현저히감소시켰다 (43.3±6.9 대 9.0±2.8%, n=3, p<0.001, 3일 ;39.8± 4.2 대 15.0±5.8%, n=3, p<0.001, 14일, Fig. 5G). 조직에침투한대식세포의용해소체의막 (lysosomal membrane) 에주로존재하는 110 kd의 glycoprotein ED-1 을면역조직화학법으로염색한결과탈리도마이드는혈관성형술 2주후대식세포의혈관침윤을대조군에비해유의하게감소시켰다 (19.2±3.5 대 4.0±2.1%, p=0.021, Fig. 5). 이러한모든결과는탈리도마이드가전신염증반응뿐만아니라국소염증반응도억제함을보여주는것이다. 탈리도마이드를통한혈관평활근세포분열능의감소억제된염증반응은 VSMCs 의분열능감소와동반하여나타났다. 혈관성형술후 3일째대조군에비해서탈리도마이드를투여한혈관의중막에서는 PCN 양성세포수가현저하게감소하였다 (Fig. 6C, D). 더군다나이러한탈리도마이드의분열억제효과는신생내막과중 Uninjured injured day 14 B injured day ED-1 positive cell (%) C D 00 Fig. 5. PCN 양성세포수는대조군에서 3일과 14일째각각 31.0± 7.3% 와 43.1±2.9% 였으나탈리도마이드군에서는 9.4± 1.9% 와 7.4±1.7% 로줄어들었다 (p<0.001 unpaired, 2-tailed t test). 고찰 탈리도마이드는 1950년대진정제로개발된후동물실험결과 10 g/kg 의용량에서도독성이없어오심과구토의조절을위해 over-the-count drug으로판매되기시작하였다. 한때독일에서는연간판매량이 15톤에이르기도하였으나 dysmelia syndrome 과같은부작용이알려지면서사용이중단되었다. 8) 그러나최근에는이약제의강력한항염증, 항혈관형성, 면역조절효과가보고되면서탈리도마이드에관한관심이다시일고있다. 이제탈리도마이드는루프스, 류마티스관절염, 크론씨병, 베체씨병, 다발성골수염과같이염증반응이중요한역할을하고있는많은임상분야에서사용이확대되고있다. 8)9) 지난 40 여년간의연구에도불구하고탈리도마이드 의작용기전은아직완전히규명되지않았다. 이약제의항염증효과는주로사이토카인과성장인자의생산과분비에변화를일으키기때문으로생각되고있다. 이중최근까지가장잘알려진것이 TNF-α 억제효과이다. 8)14) 일부 in vitro 연구들은탈리도마이드가단핵세포의 TNFα mrn 의반감기를줄인다고보고하였다. 14) TNF-α 는염증반응의주요한매개체로서혈관성형술후활성화된백혈구와 VSMCs 에서발현된다. 3)4) 더욱이혈관성형술후 10 일까지대부분의신생내막의 VS- MCs 은 TNF-α 와그 mrn 를발현하며지속적으로분열하였으며, 3) TNF-α 항체코팅스텐트를사용하여 TNF-α 의효과를억제하였을때 VSMCs 의증식을억제할수있었다. 15) 또한 rabbit 을이용한다른동물실험에서는 TNF-α 의주요한원천이되는대식세포의혈관침윤이혈관성형술후시술받은혈관에서 2주에서 4주까지지속됨을보고하고있다. 2)13) 이러한기존의사실들과잘부합하게이번실험에서도, 혈관성형술후대조군혈관조직의 TNF-α 의발현은 2주까지지속적으로높아져있음을확인할수있었고 (Fig. 3) 이는전신염증반응을반영하는혈청 TNF-α 의변화와는차이를보였다 (Fig. 2). 한편다른실험에서 Zhou 등 16) 352 Korean Circulation J 2004;34(4):

8 은 TNF-α 항체로혈관조직의 TNF-α 활성을억제하였음에도불구하고신생내막증식억제에실패하였다고보고하여신생내막형성에있어이매개물의중요성에의문을보이기도하였다. 그러나이전의다른결과들을고려할때 2)13) 전체실험기간 3주중초기 1주간만 TNF-α 의활성을억제한것은불충분하였을것이다. 따라서좀더오랜기간충분히 TNF-α 의효과를중화시켰다면보다뚜렷하게신생내막의증식을억제할수있었을것이다. 본연구에서는 ELIS 와 Western blot 으로확증했듯이탈리도마이드의투여로혈관성형술후 14 일까지 TNFα의생성과대식세포의혈관침윤을유의하게억제할수있었다. 대식세포의혈관침윤은약 80%(Fig. 5D), 혈청 TNF-α 는약 50% 정도 (Fig. 2) 감소하였고이는다른연구들의결과와도견줄만하다. 13) 더군다나본실험결과혈청 TNF-α 와혈관신생내막증식량사이에는강한양의상관관계가존재하여 (Fig. 2B) 혈청 TNFα가억제될수록 N/M ratio 도감소함을알수있었다. 즉염증반응이억제될수록신생내막의증식도약화됨을관찰하였다. 게다가탈리도마이드는 TNF-α 의전신적인분비를억제하였을뿐만아니라손상혈관에서의국소발현또한감소시켰다 (Fig. 3). 이러한결과들은탈리도마이드가전신염증반응과국소염증반응을함께줄임으로써신생내막의과증식을억제함을암시한다. 이러한결과들은 rabbit 을이용한 stent 나 balloon 손상모델에서 LPS 로비특이적인전신염증반응을유발하였을때신생내막의발생이더욱현저히증가한다는다른실험의결과와도잘부합한다. 2) 따라서 Danenberg 등 2) 은관동맥중재술을시행할때가능한전신적또는국소적염증을최소화해야한다고주장하고있다. bfgf 와그 mrn 는혈관성형술후분열하는 VSMC 과내피세포의핵과세포질에서발현이증가한다. 5) bfgf 는분비된후세포막의수용체를통해세포질과핵내로이동한후축적된다. 17)18) 이러한세포내이동은 protein kinase CKII 를자극하여세포주기의진행과세포분열을유도한다. 따라서 VSMCs 이 bfgf 의자극을받게되면신생내막이매우증가하게되며 5) 반면에 bfgf 항체로 bfgf 의활성을막게되면 VSMCs 의분열이억제된다. 19) 탈리도마이드는단독치료만으로도다발성골수종, 골 수이형성증후군, 조직구증 (histiocytosis) 환자에서혈청 bfgf 를유의하게줄였다. 20) 탈리도마이드가 bfgf 를억제하는기전으로제시된것중한가지는이약제가 DN 에결합한다는것으로 10) 많은증거들이이를뒷받침하고있다. 21) bfgf 유전자의 promoter region 에는전형적인 TT box 가없고그대신다수의 GGG- CGG sequence(gc boxes) 가존재하는것으로알려져있다. 12)22) 그런데탈리도마이드는구조적으로 adenine 보다는 guanine 과더욱유사하여 GC promotor site 에보다특이적으로결합하는것으로보고되고있다. 22) 그러므로탈리도마이드가이 GC box 에결합하여 bfgf 유전자의전사를억제하게되므로 bfgf 는그발현이억제되는것이다. TT box 나 CCT box 대신에 GC box 의조절을받는단백질로는 bfgf 외에도 Insulinlike growth factor-1(igf-1), IGF-1 수용체, bfgf 수용체, αv β3 integrin 등이있다. 이들모두는재협착의병리기전과매우밀접하게관련되어있는분자들이다. 23) 본연구에서는이러한사실들과잘부합하게탈리도마이드를투여한동맥에서 bfgf 의발현이현저히감소하였음을 Western blotting 결과확인할수있었다 (Fig. 3B). 또한면역조직화학염색결과대조군에비하여 (Fig. 4C, E) 탈리도마이드군의핵과세포질에서 bfgf 염색이 3일과 (Fig. 4D) 14일째 (Fig. 4F) 모두유의한감소를보였다. 아마도탈리도마이드의투여는대식세포의침윤을줄이고 TNF-α 와 bfgf 의발현을억제하였으며이는 VS- MCs 의분열의저하로이어지고내막증식의감소를초래했을것이다. 위에서이미언급한것들뿐만아니라탈리도마이드는 IL-1, IL-6, IL-8과같은 pro-inflammatory 사이토카인들은억제하는반면 8)10) IL-10 과같은 antiinflammatory 사이토카인의생성은높인다. 9) 또한 integrin, ICM, VCM 8)11) 과같은부착분자들의발현을억제하여결과적으로혈관내피세포와백혈구의접촉도저해한다. 게다가탈리도마이드는 I-κB의분해를억제하여염증반응, 세포분열, 재협착과정에서중요한역할을하는 NF-κB의활성을막는다. 12)24) 게다가동맥경화반이나재협착병변의진행에있어 vasa vasorum 같은신생혈관의형성이또한중요한역할을하는것으로보고되고있는데탈리도마이드는강력한 anti- 353

9 angiogenic 특성을보이고있어이점도역시재협착형성억제에도움이될것으로생각된다. 이번실험에서는실제임상상황에비해서다소많은양의탈리도마이드를사용하여연구의제한점으로거론될수는있겠으나양군간에부작용이나몸무게의변화, 사망률의차이는뚜렷하지않았다. 또한탈리도마이드가임신중기형을유발하는것은사실이다. 하지만다른경우에는매우안전한것으로알려져있다. 가장흔한부작용으로는말초신경염, 진정작용, 피로등이다. 8) 하지만이러한부작용들은대체로경미하며이로인해약제를끊는경우는드물다. 최근증상이있는심부전환자들에게매일 100~200 mg 의용량을 6개월간투여하면서 2 주간격으로일반혈액검사, 일반화학검사, 간기능검사등을실시하였으나치료전과큰변화는없었다. 9) 더군다나관상동맥질환은대부분고령의환자들에서발생하는것으로이들은임신의가능성이매우희박하므로기형아출산은큰문제가되지않을것이확실하다. 한편몇몇임상시험결과 sirolimus 코팅스텐트가이미재협착을뚜렷하게감소시킨것은사실이다. 25)26) 하지만이임상시험들은추적기간이너무짧고등록된환자의수가적다는문제점을안고있어지연된부작용 (delayed complication) 의가능성이아직배제되지않았다 ) 게다가비용효과문제또한주요한제한점으로남아있다. 30) 이러한문제점들에대해 Lemos 등 30) 은다른많은약물코팅스텐트들의개발로가격경쟁이발생해야이의가격이떨어질것이라고지적했다. 따라서탈리도마이드와같이재협착을억제하는새로운약물의개발과발견을위한지속적인노력이매우필요한실정이다. 본연구에서우리는처음으로탈리도마이드의투여가신생내막의생성을유의하게억제하였음을보고한다. 이는 TNF-α 와 bfgf 의발현저하, 대식세포의침윤억제그리고그와동반된 VSMCs 의분열감소를통하여이루어졌다. 본연구는 in-stent restenosis의모델을사용하지않고 de-novo 신생내막증식모델을사용하였으므로앞으로큰동물을이용한 in-stent restenosis 모델에서약효의확인과혈관평활근세포를사용한 in vitro 실험이추후필요한상태이지만탈리도마이드가재협착의주요기전인신생내막의성장을유의하게억제함을분명히보여주었다. 따라서탈리도마이드는재협착 방지를위한새로운약물코팅스텐트의개발이나경구투여약물로응용가능성을보인다고하겠다. 요약 배경및목적 : 염증반응은혈관신생내막증식에중추적역할을하고있다. 한편탈리도마이드는강력한항염증, 면역조절효과로인해최근여러임상분야에서다시사용되고있다. 이에저자들은탈리도마이드가혈관신생내막증식을억제할수있는지를알아보고자하였다. 방법 : 수컷 Sprague-Dawley 쥐를경동맥혈관성형술 3 일전부터 100 mg/kg qd의용량으로전처치를한다. 경동맥손상후 2주간동일한용량으로투여를지속한다. 결과 : 경동맥성형술후 3일과 14일째전신염증반응을나타내는혈청 TNF-α 를 ELIS 로측정하였을때대조군에비하여탈리도마이드군에서현저한감소를보였다 (856±213 대 449±68 pg/ml, p=0.001, 3일째 ; 129±34 대 63±18 pg/ml, p=0.001, 14일째 ). 그리고혈관성형술을받은조직의국소염증반응정도를평가하기위하여면역조직화학염색법과 Western blotting 으로측정한 TNF-α 와 bfgf 의발현, 대식세포의혈관침윤정도는탈리도마이드로뚜렷이약화되었다. 이는또한혈관평활근세포의분열능저하와연관되어나타났다 (43.1±2.9 대 7.4±1.7%, p<0.001, 14일째 ). 혈관성형술 2주후형태학적분석결과탈리도마이드를투여한혈관은신생내막의성장이현저히억제되어대조군에비해보다넓은내강을확보할수있었으며 (0.17±0.04 대 0.05±0.02 mm 2, p=0.001) 더욱이혈청 TNF-α 와신생내막 / 중막비 (Neointima/Media ratio) 는강한양의상관관계를보여 TNF-α 가감소할수록신생내막의증식도억제되었다. 결론 : 강력한항염증, 항분열효과를통해탈리도마이드는혈관신생내막의증식을유의하게억제할수있었다. 이에탈리도마이드는재협착방지를위한새로운약물로의가능성을보였다. 354 Korean Circulation J 2004;34(4):
