Streptococcus pneumoniae, Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydophila pneumoniae, Legionella pneumophila, and Bordetella pertussis are common pathogens of CAP; however, the conventional methods used to detect these agents, including culturing, lack sensitivity and are time-consuming. We evaluated a recently developed multiplex PCR assay which can test these agents simultaneously. Methods: One hundred patients with pneumonia and 99 healthy adults were tested using the Seeplex Pneumobacter ACE Detection assay (Seegene, Inc., Seoul, Korea). Culture and urinary antigen tests were also performed. Results: In patients with pneumonia, the positive detection rates of PCR for S. pneumoniae and H. influenzae were 52.0% (52/100) and 30.0% (30/100), respectively, those of M. pneumoniae and L. pneumophila were 2.0% (2/100) and 1.0% (1/100), respectively, and B. pertussis and C. pneumoniae were not detected. In healthy adults, the detection rates of S. pneumoniae and H. influenzae revealed similar results, 53.5% (53/101) and 40.4% (40/101), respectively, and the other four pathogens were not detected. The sensitivity and specificity of PCR for S. pneumoniae in pneumonia patients were 100% (95% confidence interval [CI], %) and 65.7% (95% CI, %), respectively, according to the urinary antigen test and cultures of the respiratory samples and blood. Conclusion: Differentiating S. pneumoniae and H. influenzae colonization from infection was difficult using the PCR assay. Therefore, the use of this assay is inappropriate for the diagnosis of pneumonia due to these agents, although multiplex PCR assay would be useful for the detection of M. pneumoniae and L. pneumophila. (Tel) , (Fax) , ( ) miae@ewha.ac.kr 론 아니라진단까지의시간이오래소요된다. 이러한단점을극복하기위해새로운검사법들이계속개발되고있으며, 그중에서도중합효소연쇄반응 (polymerase chain reaction, PCR) 원리를이용한분자진단법이대표적으로민감도가높고보다신속하고정확한진단이가능하여사용이기대되고있다 [6]. 최근에 Streptococcus pneumoniae, Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydophila pneumoniae, Legionella pneumophila 및 Bordetella pertussis 등의 6가지균주를한꺼번에검사할수있는다중 PCR 검사가개발되었다. 이들균주들은지역사회획득폐렴의가장흔한원인균들이다 [1-3,7]. 그러나일반적인배양검사로검출하기어려워민감도가높은분자진단방법이유용할것으로기대된다 [8]. 국내에서는이제까지폐렴의원인균을주로배양등의전통적인방법으로검출해왔으며, 실제폐렴환자를분자진단법으로검사한연구는드물었다. 이에본연구에서는폐렴환자 40

2 Chae Lim Jung, et al. : Multiplex PCR in Bacterial Pneumonia 41 의호흡기검체에서다중 PCR 검사로원인균들을검출해보고자하였다. 또한 PCR 검사는민감도가높아 S. pneumoniae 같은집락을형성하는균에서는유용성이떨어질수있으므로 [5] 호흡기질환이없는성인의인후검체에서다중 PCR 검사를시행해결과를비교하고자하였다. 대상및방법 1. 연구대상 2008년 10월에서 2009년 4월까지본원을방문하여다중 PCR 시스템인 Seeplex Pneumobacter ACE Detection assay (Seegene Inc., Seoul, Korea) 를시행한폐렴환자들중 100명을무작위로선정하였다. 폐렴은발열, 기침, 객담, 호흡곤란및청진상수포음등의특징적인임상증상이있고흉부방사선학적소견상폐침윤이있는경우로정의하였다. 지역사회획득폐렴진단시는입원후 3일이상경과한경우와내원 1주전까지입원한과거력이있는경우는제외하였다 [2,3]. 대조군으로호흡기질환의증상이나징후가없는정상인 99 명을선정하였다. 공고를통해피험자를모집하여모두동의서를받았고본원임상시험심사위원회 (Institutional Review Board, IRB) 의심의를통과하였다. 폐렴환자군의평균연령은 65.4±16.5세 (26 91세) 였고, 남자 60명, 여자 40명이었다. 지역사회획득폐렴환자가 92명으로대부분이었고, 나머지 8명은입원중폐렴이발생한경우였다. 정상인군에서의평균연령은 30.6±9.4세 (19 58세) 였고, 남자 39명과여자 60명이었다. 2. 연구방법폐렴환자 100명모두에서호흡기검체와혈액배양검사, S. pneumoniae와 L. pneumophila에대한소변항원검사를시행하였다. 호흡기검체종류는객담 84예, 경기관흡인액 12예, 기관지폐포세척액 2예, 기관지흡인액 1예, 흉수 1예순이었고, 객담은모두저배율에서상피세포가 25개이하이고호중구가 25 개이상인양질의객담이었다 [9]. 정상인 99명에서는인후면봉검사를시행하여얻은인후검체로다중 PCR 검사와배양검사를동시에시행하였다. 폐렴군과정상인군에서다중 PCR 검사, 소변항원검사및배양검사에대한각원인균들에대한양성률을산출하여서로비교하였다. 또한폐렴군에서소변항원검사와배양검사를기준으로 S. pneumoniae에대한다중 PCR의민감도, 특이도, 양성예측도, 음성예측도등을산출하여다중 PCR의정확도를평가하였다. 1) 배양 : 환자의호흡기검체와정상인의인후검체를 5% 면양혈액을포함한혈액한천배지와초콜릿배지에접종하여 o C, 5% CO 2 배양기에서배양한후 24시간과 48시간에 판독하였다. 특징적인집락이관찰되면그람염색을하였고 S. pneumoniae의경우 optochin 감수성검사, H. influenzae 경우 Ⅹ, Ⅴ 인자요구시험등을시행하여확인하였고최종적으로 S. pneumoniae 는 Vitek II ID-GP Card (biomérieux, Durham, NC, USA), H. influenzae는 Vitek II ID-NH Card (biomérieux) 를통해균주를동정하였다. 2) 소변항원검사 : 면역크로마토그래피법원리를이용한 BinaxNOW 키트 (Binax Inc., Scarborough, ME, USA) 를사용하여검사하였다. 3) 분자진단법 : 다중 PCR을포함한전과정은 Seeplex PneumoBacter ACE Detection assay (Seegene Inc.) 를이용하여시행하였다. 이진단법은 dual priming oligonucleotide (DPO) 기술을사용해서비특이적인 priming 을차단하여민감도와특이도를높이는원리를이용하며, S. pneumoniae, H. influenzae, M. pneumoniae, C. pneumoniae, L. pneumophila 및 B. pertussis 의 6가지균주를동시에검출할수있는다중 PCR 검사방법이다. 제조회사에따르면 S. pneumoniae 는 ply 유전자, H. influenzae는 P6 유전자, M. pneumoniae는 ITS1 유전자, C. pneumonia는 ompa 유전자, L. pneumophila는 mip 유전자, B. pertussis 는 prn 유전자를표적으로한다. 검출한계는 10 copy/reaction (10 copy/3μl DNA) 이다. (1) 검체전처리 : 객담을포함한호흡기검체는 4% NaOH와 1:1로혼합하여 1분간 vortex mixing 한후실온에서 15분간정치시켰다. 1.5 ml를새용기에옮겨 16,000 g에서 10분간원심분리한후상청액을버리고 PBS 1 ml를첨가하여잘혼합하였다. 10분간 16,000 g에서원심분리한후상청액을버리고, PBS 150μL를첨가후 vortexing하였다. (2) 다중 PCR: 모든과정은제조사의지침대로시행하였다. 핵산추출은 Viral Gene-Spin, viral DNA/RNA 적출키트 (Intron, Seoul, Korea) 를이용하여시행하였다. 0.2 ml PCR 시험관에 PCR mastermix 17μL를준비하고추출한검체의핵산 3μL를첨가하였다. GeneAmp PCR system 9700 (Applied Biosystems, Singapore, Singapore) 에서 94 o C에서 15분간반응시킨후 94 o C에서 0.5분, 60 o C에서 1.5분, 72 o C에서 1.5분반응시키는과정을 40회반복하였고그후 72 o C에서 10분간반응시켰다. 증폭산물 5μL를 etidium bromide를포함한 2% agarose gel 에서전기영동후자외선을조사하여나타난증폭산물을키트내에포함되어있는표지자와비교하여해석하였다. 각각순서대로 583 bp, 472 bp, 350 bp, 257 bp, 200 bp, 146 bp는 M. pneumoniae, L. pneumophila, S. pneumoniae, H. influenzae, B. pertussis, C. pneumoniae 를나타내었다 (Fig. 1). 3. 통계분석통계프로그램인 SPSS 12.0 for Windows (SPSS Inc.,

P value가 0.05 이하인경우통계적으로유의한것으로간주하였다. 결과 1. 폐렴군과정상인군의검사별양성률다중 PCR 검사결과폐렴군은 S. pneumoniae 가 52.0% (52/100), H. influenzae가 30.0% (30/100) 에서양성이었다. 정상인군도 S. pneumoniae가 53.5% (53/99), H. influenzae 가 40.4% (40/99) 에서양성으로비슷한결과를보였고, 두군에서 S. pneumoniae와 H. influenzae의양성률에는유의한차이가없었다 (P>0.05). 폐렴군에서 M. pneumoniae와 L. pneumophila는 PCR 검사의각각 2.0% (2/100), 1.0% (1/100) 양성률을나타냈고, C. pneumoniae와 B. pertussis는검출되지않았다. 정상인에 서 M. pneumoniae, C. pneumoniae, L. pneumophila 및 B. pertussis는검출된경우가없었다 (Table 1). S. pneumoniae에대한소변항원검사결과폐렴군의 19.0% (19/100) 에서양성으로나타났다. 폐렴군에서호흡기검체배양결과 S. pneumoniae는 12.0% (12/100), H. influenzae는 2.0% (2/100) 에서검출되었다. 혈액배양검사에서는 S. pneumoniae 가 4.0% (4/99) 에서양성이었다. 정상인군은인후배양검사에서 S. pneumoniae 와 H. influenzae 가검출된경우가없었다 (Table 2). 2. 폐렴군에서다중 PCR 검사의민감도, 특이도, 양성및음성예측도폐렴군에서 S. pneumoniae의다중 PCR 검사에대한민감도, 특이도, 양성예측도, 음성예측도는호흡기검체배양검사를기준으로했을때는 100% (95% 신뢰구간, %), 54.5% (95% 신뢰구간, %), 23.1%, 100%, 소변항원검사, 호흡기검체및혈액배양검사를모두고려했을때는 100% (95% 신뢰구간, %), 66.7% (95% 신뢰구간, %), 53.8%, 100% 였다 (Table 3). 3. 폐렴군에서검체별 S. pneumoniae의검사법비교객담검체인경우다중 PCR 검사와소변항원검사의일치율은 64.3%, 다중 PCR과배양결과의일치율은 61.9% 였고, 경기관지흡인액등다른호흡기검체인경우각각 81.3%, 75.0% 로더높은일치율을보였다 (Table 4). 고 찰 Fig. 1. PCR 검사는특히 L. pneumophila, M. pneumoniae 및 C. pneumoniae 등사람호흡기에집락을잘형성하지않고배양이어려운균의검출에유용할것으로여겨진다 [5,8]. 본연구결과, S. pneumoniae는폐렴환자의 52.0% (52/100) 에서, H. influenzae 는 30.0% (30/100) 에서다중 PCR 양성이었고, 정상인군의인후검체에서시행한다중 PCR 결과도비슷하게나타났다. Murdoch 등 [12] 의연구에서도폐렴환자와정상인의인후검체에대한 PCR 검사결과각각 55% 와 58% 로거의차이가나지않았다. 또한 Strålin 등 [13] 도폐렴이아닌대조군의기관지폐포세척액에서 S. pneumoniae와 H. influenzae 의양성률이각각 17% 와 39% 로나타났다고하였다. 따라서, 정상인의 PCR 양성결과가감염보다는집락형성을더반영하는것으로추정된다. S. pneumoniae 와 H. influenzae 의보균율은성인에서대략 3 36% 정도로보고되고있다 [14-16]. 본연구에서는다중 PCR 검사로호흡기질환이없는성인에서도 % 검출되는것을확인하였고이는기존에배양검사로보고한것보다매우높은비율이다. Brugger 등 [17] 의연 구에서도 S. pneumoniae의집락형성비율이배양검사로는 40.0% 였는데 PCR 검사를시행했을때는 51.6% 로높아진것을볼수있다. 본연구에서정상인의인후배양검사에서는 S. pneumoniae와 H. influenzae 가동정되지않았는데이는배양조건이까다롭고다른상재균이많이존재하여검출되지못했을가능성이있다. 본연구와다른연구들 [12,13] 에서폐렴이아닌대조군에서도 S. pneumoniae와 H. influenzae 의양성률이높았던것과대조적으로, 다른연구들은감염질환의증거가없는성인의비인두검체에서 S. pneumoniae가 PCR 검사결과 3 8% 에서만검출되었다고보고하였고, H. influenzae 에대한 PCR 결과도마찬가지로폐렴군보다정상군에서적게검출된다고하였다 [18,19]. 이와같이 PCR 검사에의한 S. pneumoniae 보균율결과가연구에따라다른것은, 연구대상의특성이나위험요인, 진단기준, 검사방법, 검체종류등에따라결과가큰영향을받기때문에각연구마다결과가매우다양할수있을것으로생각한다. 폐렴군에서혈액배양검사를기준으로했을때 S. pneumoniae에대한 PCR 검사의민감도는 %, 객담배양결과기준시민감도는 26 88% 정도로보고되고있다 [5,12]. 본연구에서호흡기검체와혈액에대한배양검사를기준으로했을때, 다중 PCR 검사의민감도는모두 100% 로매우높았고, 그에비해특이도는 % 로낮았다. 소변항원검사, 호흡

5 44 Korean J Clin Microbiol 2010;13(1):40-46 기검체및혈액배양검사를모두기준으로했을때민감도는역시 100% 였고, 특이도는 66.7% 로상승하였다. PCR 검사가매우예민하므로결과가음성인경우 S. pneumoniae 감염을배제할수있으나, 양성일경우에는위양성의가능성도고려하여해석에주의할필요가있다. 위양성의원인으로는앞에언급한집락형성을생각해볼수있다. PCR 검사는매우민감하기때문에, 현성호흡기질환이없는정상인에서질환을유발하지는않을정도의적은양이존재하는경우에도검출될가능성이있다. 집락형성과감염과는양에서차이가나기때문에대안으로정량 PCR 검사법을들수있다 [20,21]. 다른가능성으로는 PCR 검사의표적유전자에따른특이도의차이를생각해볼수있다. 본검사법은 S. pneumoniae는 ply 유전자를, H. influenzae 는 P6 유전자를표지자로사용하였다. ply 유전자를표지자로이용했을때, PCR 결과가양성인경우의 29% 정도가 S. pneumoniae 외다른병원균에감염되어있었다는연구결과가있다 [17]. 이를방지하기위해보다특이적인 PCR 탐색자가필요할것으로생각한다. 최근 lyta 표지자를사용해서특이도를향상시켰다는보고가있다 [22]. H. influenzae 검사시는 P6 유전자를단독으로사용하는것보다 P6 유전자와 16S rrna 유전자를함께사용하는것을추천한연구가있다 [8]. 한편, 본연구에서와같은키트를사용한 Park 등 [23] 의연구에서는다른균주와의교차반응은없는것으로보고하여, 표지자의특이도문제보다는집락형성의문제가위양성의주된원인일것으로생각한다. 본연구에서 L. pneumophila는폐렴환자 100명중 1명에서 PCR 양성이었다. 환자는유방암으로수술후항암화학치료중이던분으로, 소변항원검사는음성이었으나배양에서다른세균이검출되지않았고 1주일만에폐렴으로사망한경과를보여원인균으로배제할수는없는경우였다. M. pneumoniae 는폐렴환자 100명중 2명에서 PCR 양성이었는데, 2명다 M. pneumoniae IgM 항체가는음성이었다. 그러나 1예는첫검사시보다 1주일후시행한검사시혈청 IgG 항체가가 277 U/mL 에서 2,333 U/mL 로 8배상승하여임상적으로의심되는경우였고 azithromycin 으로치료하였다. 다른 1예는 M. pneumoniae 항체검사에서혈청 IgG 항체가가 524 U/mL로양성이었는데, 추적검사를시행하지못하였다. C. pneumoniae와 B. pertussis 는검출된경우가없었다. 4균주모두정상인에서는검출된경우가없었고, Kumar 등 [18] 의연구에서도감염질환의증거가없는성인의비인두검체에서이 4가지균주가모두검출되지않아다중 PCR 검사가진단에도움이될수있을것으로생각한다. 본연구에서다중 PCR 검사와다른검사결과를비교했을때검체가객담인경우의일치율은 % 인데비해경기관지흡인액등다른호흡기검체일경우의일치율은 % 로더높았다. 이로보아결과의정확성을높이기위해서 는객담보다는상대적으로오염이적은침습적검체가더유용할것으로생각한다. 결론적으로, S. pneumoniae와 H. influenzae에대한 PCR 검사는민감도가우수하여결과가음성인경우감염을배제할수있지만정상성인에서도검출되어결과가양성인경우집락형성과감별이어려워서분자유전검사를적용하기적합하지않다고생각한다. 7 46 Korean J Clin Microbiol 2010;13(1):40-46 = 국문초록 = 호흡기검체에서폐렴의세균성원인균검출을위한다중 PCR 검사의임상적평가 이화여자대학교의학전문대학원진단검사의학교실정채림, 이미애, 정화순 배경 : 지역사회획득폐렴은전세계적으로유병률과사망률이높은주요한감염질환중의하나이다. Streptococcus pneumoniae, Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydophila pneumoniae, Legionella pneumophila, Bordetella pertussis 등은지역사회획득폐렴의흔한원인균들이나, 배양등의전통적인방법은민감도가낮고시간이많이소요된다. 최근이균주들을한번에검사할수있는다중 PCR 검사가개발되어, 본연구에서는폐렴환자의호흡기검체에서다중 PCR 검사를평가하고자하였다. 방법 : 폐렴환자 100명과정상인 99명에대해 Seeplex Pneumobacter ACE Detection assay (Seegene Inc., Seoul, Korea) 를시행하였다. 소변항원검사와배양검사도함께실시하였다. 결과 : 다중 PCR 결과 S. pneumoniae와 H. influenzae가각각폐렴환자의 52.5% (53/101), 30.7% (31/101) 에서양성이었고, M. pneumoniae와 L. pneumophila는각각 2.0% (2/101), 1.0% (1/101) 에서양성이었으며, B. pertussis 및 C. pneumoniae는검출되지않았다. 정상인은 S. pneumoniae와 H. influenzae의양성률이각각 53.5% (53/99), 40.4% (40/99) 로폐렴군과비슷하게나타났고, 나머지 4균주는검출되지않았다. 폐렴군에서 S. pneumoniae의다중 PCR 검사에대한민감도와특이도는소변항원검사, 호흡기검체및혈액배양검사를모두기준으로했을때각각 100% (95% 신뢰구간, %) 와 66.7% (95% 신뢰구간, %) 였다. 결론 : S. pneumoniae와 H. influenzae 균주의다중 PCR 검사는집락형성과현성감염의감별이어려워분자유전검사를적용하기적합하지않다.
