There is little information on the recent antimicrobial susceptibility patterns of S. pneumoniae in Busan and Gyeongnam of Korea. The aim of this study was to investigate the distribution and antimicrobial resistance of S. pneumoniae at 4 university hospitals in Busan and Gyeongnam. Methods: We collected and analyzed the antimicrobial susceptibility results of 850 S. pneumoniae strains isolated from regional 4 university hospitals during the last 2 years from July 2013 through June Results: Among 850 S. pneumoniae strains, 635 strains were isolated from respiratory specimens, followed by blood (N=121), CSF (N=13), and others (N=81). Antimicrobial susceptibility rates to penicillin, cefotaxime and ceftriaxone were 79.4%, 76.6% and 83.6%, respectively. The resistant rates to erythromycin and clindamycin were 80.9% and 68.2%, respectively. The resistant rates to levofloxacin were 9.2%. There were some differences in resistant rates by age groups, years, and specimen types. Conclusion: We found the changes of antimicrobial resistance of S. pneumoniae during the last 2 years. It is necessary to monitor the antimicrobial susceptibility of S. pneumoniae regularly for empirical therapy and for early detection of the changes of resistance. (Ann Clin Microbiol 2016;19:48-53) Key Words: Drug resistance, Multiple, Streptococcus pneumoniae INTRODUCTION 폐렴알균 (Streptococcus pneumoniae) 은지역사회획득폐렴의가장중요한원인균으로 [1,2], 특히침습성폐렴알균감염의경우영유아, 노인, 만성질환자및면역저하자에게매우치명적인결과를야기할수있다 [3-6]. 폐렴알균의항균제내성은 1980년대이후꾸준히증가하였고다제내성폐렴알균이세계적으로확산된것으로보고되었다 [7,8]. 국내의경우 beta-lactam계와 macrolide계항균제에대한내성률이매우높고다제내성도많아지역사회감염의문제점으로대두되기도하였다. 하지만, 2008년 Clinical and Laboratory Standards Institute (CLSI) 의항균제감수성검사지침에서 penicillin 내성기준에 대한 breakpoint가개정되었고, 변경된기준에따라기존의내성으로보고한다수가감수성으로해석되어실제치료가능범주내에들게되고이에따라 penicillin 내성률이급격하게감소되었다 [9]. 이러한이유로폐렴알균의항균제내성률에대한관심이떨어지고, 최근이와관련된보고는찾아보기어렵다. 비록 CLSI 지침의변경에따라페니실린의내성률이급격히감소하기는하였지만, macrolide계내성률은여전히매우높으며, 폐렴알균감염증의주요치료항균제인 3세대세팔로스포린과퀴놀론계항균제의내성도적지않아, 폐렴알균감염증의적절한치료와내성의전파방지를위해국내항균제내성실태및변화추이를주기적으로파악하는것은매우중요하다. 이에저자들은최근 2년간부산, 경남지역에위치한 4개대 Received 23 March, 2016, Revised 23 May, 2016, Accepted 7 June, 2016 Correspondence: Jeong Hwan Shin, Department of Laboratory Medicine, Busan Paik Hospital, Inje University College of Medicine, 75 Bokji-ro, Busanjin-gu, Busan 47392, Korea. MATERIALS AND METHODS 2013년 7월부터 2015년 6월까지총 2년간부산경남지역 4 개의대학병원에서분리된폐렴알균의항균제감수성검사결과, 검체종류및분리시기등의자료를수집하여분석, 조사하였다. 폐렴알균의동정은 4개의대학병원모두 VITEK2 System (biomérieux, Marcy l Étiole, France) 의 VITEK2 GP 카드를이용하였다. 항균제감수성검사는 2곳의대학병원에서는 Micro- Scan (Siemens Healthcare Diagnostics, Sacramento, CA, USA) 의 MICroSTREP을이용하였고, 다른 2곳의대학병원에서는 VITEK2 System (biomérieux) 의 AST-ST01와함께 penicillin 감수성검사를위해 Etest 를추가로이용하였다. 따라서전체항균제중 chloramphenicol, amoxicillin-clavulanic acid, meropenem, cefuroxime, cefepime, cefaclor는 MicroSacn (Siemens) 을사용한 2개의병원에서그결과를수집하였고 linezolid는 VITEK2 를사용한 2개의병원에서그결과를수집하였다. 항균제감수성결과는 CLSI기준에따라판정하였고, 특히뇌척수액에서폐렴알균이분리된경우 CSLI기준에따라뇌수막염 (meningitis) 기준으로판정하였다 [10]. 수집된자료를바탕으로항균제내성률을병원별, 시기별, 나이별, 및검체종류별로구분하여카이제곱검정 (chi-square test) 을시행하였고, 시간변화에따른내성률의변 화는선형성검정 (Cochran-Armitage test) 을시행하였다. 통계분석은 SPSS (version 22, IBM Corp, Armonk, NY) 프로그램을이용하였다. 폐렴알균의다약제내성은미국 Food and Drug Administration (FDA) 의기준 [11] 에따라 erythromycin, cefuroxime, trimethoprime-sulfamethoxazole, penicillin, 및 tetracycline의 5계열중 2 개이상의계열에내성인경우로정의하는데본연구에서는이중 cefuroxime 대신 cefotaxime 또는 ceftriaxone에내성인경우로변경하여조사하였다. RESULTS 연구기간동안 4개의병원으로부터각각 139주, 300주, 234 주, 및 177주총 850주에대한폐렴알균자료가수집되었다. 폐렴알균이분리된환자의나이는 0세부터 103세까지였고 5세이하가 4.5% (38주), 6세부터 64세까지가 44.9% (382주), 65세이상이 50.6% (430주) 를차지했다. 폐렴알균이분리된검체는호흡기검체가 74.7% (635주) 로가장많았으며, 그다음으로혈액이 14.2% (121주), 뇌척수액 1.5% (13주), 기타 9.5% (81 주 ) 순이었다. 총 4개병원을대상으로분석한항균제감수성결과중 clindamycin (68.2%), erythromycin (80.9%), tetracycline (79.0%) 에서내성률이높았고, 2개병원만을대상으로분석된항균제중에서는 cefuroxime (71.8%), cefaclor (77.4%) 에서항균제내성률이높았다. 항균제내성률은병원별로차이를보였는데, pen- Table 1. 3 50 Ann Clin Microbiol 2016;19(2):48-53 icillin에대한내성률은 8.3% 였고각병원별로는 6.5%, 12.7%, 6.2%, 및 4.7% 였다 (P<0.001). 중등도내성을포함한비감수성률은 20.6% 였고, 각병원별로는 25.2%, 26.0%, 15.6%, 및 14.0% 였다. Cefotaxime에대한내성률및비감수성률은 8.2% 와 23.4% 였는데, 각병원별로는 6.5% 와 13.7%, 9.0% 와 19.3%, 11.6% 와 30.9% 및 3.5% 와 28.3% 로병원간내성률에차이가있었다 (P<0.001). Trimethoprim-sulfamethoxazole의내성률은 40.3% 였고, 각병원별로는 37.4%, 38.0%, 49.1% 및 34.5% 였다. Levofloxacin의내성률은 9.2% 였고각병원별로는 7.2%, 10.7%, 12.8%, 및 3.4% 였다 (P<0.018). Meropenem의경우내성률이 44.4% 였고 vancomycin과 linezolid에는모두감수성이었다 (Table 1). 연령대별항균제내성률은다음과같다. Penicillin 내성률및비감수성률은 5세이하에서 13.5% 와 24.3%, 6-64세에서 6.4% 와 17.4% 였고 65세이상에서 9.4% 와 23.0% 였다. Cefotaxime 에대해서는내성률과비감수성률이 5세이하에서 2.8% 와 33.3%, 6-64세에서 9.2% 와 25.1% 였고 65세이상에서 7.7% 와 21.2% 였다. Clindamycin 과 erythromycin 에대한내성률이 5세이하에서각각 89.5% 와 94.7%, 6-64세에서각각 64.0% 와 78.9% 이고 65세이상에서 70.0% 와 81.4% 였다. 이밖에 tetracycline, meropenem, cefepime에대해 5세이하에서내성률이각각 94.6%, 60.0%, 및 30.0% 였다. Levofloxacin과 chloramphenicol 에대해서는 5세이하에서모두 100% 감수성인반면 6-64세에서각각 7.6% 와 25.4%, 65세이상에서 11.4% 와 28.5% 에서내성이었다 (Fig. 1). 연도별항균제내성률의변화를살펴보면, penicillin에대해 2013년에는 6.2%, 2014년 7.3%, 2015년 10.8% 로증가하는추세를보였으나통계적유의성은없었고 (P=0.062), 중등도내성률을포함한비감수성률은각각 21.3%, 21.0%, 19.5% 였다. Cefotaxime 에대해서는내성률과비감수성률이 2013년 10.6% 와 27.2%, 2014년 6.6% 와 20.3%, 2015년 8.8% 와 25.3% 였다. Levofloxacin에대해 2013년 10.6%, 2014년 8.4%, 2015년 9.5% 였다 (Fig. 2). 검체종류에따른내성률을분석하였다. Penicillin에대한내성률과비감수성률이뇌척수액검체 (84.6% 와 84.6%) 에서높은반면혈액배양검체 (8.5% 와 18.8%) 와호흡기검체 (6.5% 와 19.1%) 에서는낮았다 (P<0.001). Cefotaxime에대해서는내성률과비감수성률이뇌척수액검체 7.7% 와 53.8%, 혈액배양검체 6.0% 와 10.3% 였고호흡기검체는 9.0% 와 25.2% 였다. 하지만이와달리 levofloxacin은뇌척수액검체에서는모두감수성이었고혈액배양검체 (7.6%) 와호흡기검체 (9.4%) 에서내성률이높았다 (Table 2). 총 850개의균주중 666개 (78.4%) 의균주가 5계열중 2계열이상에내성을보이는다약제내성이었다. 다약제내성중 2개, 3개, 4개의항균제에내성인균주가각각 37.6%, 30.9% 및 5.8% 였고, 5개의항균제에모두내성인균주가 4.0% 였다. 본연구에서조사한 4개의병원에서다약제내성균주의분포는각각 77.7%, 78.3%, 79.1% 및 78.0% 로유사하였다. 다약제내성균주의연령별분포는 5세이하, 6-64세, 65세이상에서각각 92.1%, 74.6% 및 80.5% 였고검체별분포는혈액검체, 뇌척수액검체, 호흡기검체에서각각 76.0%, 76.9% 및 77.8% 였으며 Fig. 1. DISCUSSION 일반적으로폐렴알균감염의치료시일차선택약제로 β- lactam계, macrolide계혹은 fluoroquinolone계열의항균제를단독으로사용하거나, β-lactam계항균제와 macrolide계항균제의병용요법을시행하게된다 [12,13]. 따라서지역사회감염의주요원인인폐렴알균의항균제내성양상을파악하는것은경험적항균제사용을위한중요한지침이된다 [14]. 2013년국가항균제내성정보자료에따르면국내종합병원의비수막염폐렴알균의 penicillin의내성률은 2010년 27.0% 로나타난이후차츰감소하여 2013년 7.5% 로보고되었으며 [15] 이는본연구에서의 8.3% 와유사한결과였다. 연도별로 penicillin 내성률은 2013년 6.2% 에비해 2015년 10.8% 로조금증가하였으나통계적유의성은없었다 (P=0.062). 그러나중등도내성률이 12.3% 로높아향후내성률이더증가할것으로예상된다. 이것은 2008년 CLSI 지침서가개정된이후감소한 penicillin내성이다시조금씩증가하고있다는것을시사하며따라서지속적인 penicillin 내성조사가필요함을뒷받침해주는결과이다. 또한 penicillin 내성률이 4개의병원간에도 4.7% 에서 12.7% 까지많은차이 (P<0.001) 를보임을확인할수있었다. Cefotaxime 의경우내성률은 8.2% 인반면, 중등도내성률이 15.3% 로높았는데특히한개의병원에서는내성률은 3.5% 이지만중등도내성률이 24.9% 에달하였고, meropenem 도중등 도내성률이 24.4% 에달해향후내성률증가로이어질가능성이있음을시사하고있다. Erythromycin의경우내성률이 2008 년 70.9%, 2013년에는 78.1% 로보고되었는데, 본연구에서도 80.9% 로내성률이지속적으로높음을알수있다. 2013년부터 2015년까지 amoxicillin-clavulanic acid는매년내성률이조금씩감소하였고 (P=0.002), trimethoprim-sulfamethoxazole 및 cefepime은내성률에차이가없었다. 본조사에서 2013년과 2015 년은각각 6개월씩의데이터만포함되어 3년간의연간데이터비교는차이를보일수있으며, 향후추가적으로확인할필요가있다고생각한다. 연령대별비교에서 clindamycin (P=0.003), tetracycline (P= 0.032), amoxicillin-clavulanic acid (P=0.003) 는 5세이하의소아에서내성률이더높았고, erythromycin (P=0.056) 은높긴하지만통계정유의성은없었으며, penicillin, meropenem, cefepime 등은연령대에따른내성률의차이는없었다. Cefotaxime 의경우 5세이하에서내성률은 2.8% 로낮지만중등도내성률을포함한비감수성률은오히려더높아향후내성률증가의가능성을염두에두어야할것이다. 반면 levofloxacin과 chloramphenicol에서는모두감수성을보였는데, levofloxacin의경우소아에서 fluoroquinolone계의사용이제한적이기때문일것으로생각된다. 검체별로내성률의차이가있음을확인하였는데, 뇌척수액에서검출된폐렴알균의경우다른검체에서검출된폐렴알균에비해 penicillin 내성률이 84.6% 로매우높았고 (P<0.001) 이것은기존의보고와유사한수치였다 [16]. 뇌척수액검체의경

5 52 Ann Clin Microbiol 2016;19(2):48-53 우 cefotaxime에대해내성률은 7.7% 로다른검체와큰차이가없지만, 중등도내성률을포함한비감수성률이 53.8% 로매우높아향후내성률의증가와더불어치료실패의가능성을짐작할수있다. 하지만 levofloxacin의경우전반적으로는내성률이증가하였지만뇌척수액검체는모두감수성을보여상이한결과를보였다. 본연구에서 2계열이상의항균제에내성을보이는다제내성폐렴알균은 78.4% (666주) 였다. 이중 2개, 3개, 4개의항균제에내성을보이는균주는각각 37.6% (320주), 30.9% (263 주 ), 및 5.8% (49주) 였다. 4% (34주) 는 5계열의항균제모두에내성이었고이것은환자의치료실패가능성과밀접한연관이있을것으로생각된다. Erythromycin과 tetracycline 두계열모두에내성인균주가 76.5% (650주) 로다약제내성의주요원인이었다. 다제내성균의분포조사에서병원별발생빈도는차이가없었고, 연령대빈도는 5세이하에서더높았으며 (92.1%, P=0.014), 검체종류에따른차이는없었다. 본연구에서는 2013년도부터 2015년도까지부산경남의 4개병원의폐렴알균항균제감수성현황을조사하여주요항균제의내성률과연령대별, 연도별, 검체별항균제내성률의차이를확인할수있었다. 경험적항균제의선택과향후내성률증가가능성을고려할때주기적인항균제내성률조사가필요할것으로생각한다.
