We investigated the Antimicrobial susceptibility patterns of A. baumannii and P. aeruginosa isolated from sputum, urine, wound, blood, nasal swab, body fluid. The antimicrobial resistance of A. baumannii were shown 96% for piperacillin, carbenicillin 82%. cefotaxime 78%, ciprofloxacin 77%, sulfamethoxazole/trimethoprime 76%, ceftazidime 75%, tobramycin 72%. For P. aeruginosa, the resistance of cefotaxime and sulfamethoxazole/ trimethoprime were 100%, carbenicillin 49%, piperacillin 47%, ticarcillin 45%, ticarcillin/ clavulanic acid 40%. Key Words : Acinetobacter baumannii, Pseudomonas aeruginosa, Antimicrobial susceptibility, β-lactams I. 서론 Acinetobacter baumannii와 Pseudomonas aeruginosa는포도당비발효성그람음성간균으로폐렴, 요도염, 균혈증을포함한원내감염의중요원인균으로알려져있다 (Yoshihara 등 1991; 오등, 2006). 포도당비발효그람음성간균의내성경로는세포외막 교신저자 : 김수정, ( 우 ) , 대구시북구태전동산 7 번지 TEL : , sjkim@mail.dhc.ac.kr porin 과염색체성 β-lactamase 생성으로인해이루어진다 ( 오등, 2006; Jiang 등, 2006). 최근에 extened-spectrum β-lactamase(esbl) 등의생성균주의출현으로여러항균제에대해다제내성을가지는 A. baumannii와 P. aeruginosa의출현빈도도증가하고있는추세이다 (Gootz 와 Marra, 2008; Sinha 등, 2007). P. aeruginosa에서 ESBLs은 monobactam 계열인 aztreonam(atm), 광범위 cephalosporine 계열의 ceftriaxone, cefotaxime(ctx) 에대해서도내성빈도가점차증가하였다. 따라서 P. aeruginosa에서도 ESBL생성균주가보고되고있다 (Jiang 등, 75

2 대한임상검사학회지 40(2):75-79, ). 김등 (2004) 에의하면녹농균이중환자실에서포도알구균과함께가장흔한원내감염균으로써많은항생제에대해자연내성과획득내성을가지며최근에는 imipenem 내성녹농균이증가하고있다고보고하였다. Imipenem과 meropenem 등은 carbapenem 계열의항균제로서 penicillin-binding protein(pbp) 에친화력이매우우수하며대부분의 β-lactamase에안정하여다른 β-lactam 제에비해 β-lactam 내성인그람음성간균의감염증치료에유용하게사용되어왔다. 그러나김등 (2004) 에의하면 carbapenem 계열내성녹농균의분리율이 17.4% 로보고되었고내성원인으로는분자적분류에의해 class B(metallo-β-lacatamases) 유전자를가지고있으며그비율이총 116 균주중 20 균주로 17.2% 를차지한다고보고되었다. 본연구에서는대구지역에서의 A. baumannii와 P. aeruginosa의검체별, 연령별분리율및항균제내성추이를살펴보고궁극적으로는항균제내성을보이는 A. baumannii와 P. aeruginosa의증가추세를둔화시킬수있는기본자료를제시하고자대구지역의한대학병원에서 2007년 6, 7, 8월에분리동정된 A. baumannii 86 균주와 P. aeruginosa 116 균주를이용하여항생제내성및감수성현황, 검체별분리빈도및연령별분리율을조사하고자하였다. II. 재료및방법 1. 균주수집 2007년 6월에서 8월사이 3 개월간대구지역 3차의료기관의환자로부터분리된 A. baumannii와 P. aeruginosa 를대상으로하였다. 분리된균주는생화학적방법과미생물동정기기인 Vitek2 system(biomerleux Vitek Inc., Hazelwood, Mo., USA) 을이용하여동정확인하였다. 2. 항균제감수성시험항균제감수성시험은 Vitek2 system을이용하여감수성검사를실시하였다. 항균제종류로는 amikacin, carbenicillin, ceftazidime, ciprofloxacin, cefepime, cefotaxime, gentamycin, imipenem, meropenem, piperacillin, ampicillin/ sulbactam, sulfamethoxazole/trimethoprime, ticarcillin, tobramycin, ticarcillin/clavulanic acid 등을이용하였다. 정도관리는표준균주를사용하여항균제감수성검사를실시하고 CLSI(Clincial and Laboratory Standards Institute) 에의해규정하였다. 그람음성표준균주는 Escherichia coli ATCC25922, Klebsiella oxytoca ATCC700324, Pseudomonas aeruginosa ATCC27853이며, 그람양성표준균주는 Staphylococcus aureus ATCC29213를사용하여검사를실시하였다. III. 결 과 1. Acinetobacter baumannii와 Pseudomonas aeruginosa의검체별분리율 대구시지역 3차의료기관에서 3 개월간분리된 A. baumannii 86 균주와 P. aeruginosa 116 균주를이용하여검체별분리분포를조사결과 A. baumannii 86 균주에서는객담, 창상, 소변등의순으로나타났고, P. aeruginosa 116 균주에서는객담, 소변, 창상등의순서로나타났다. 일반적으로객담에서의분리빈도가가장높은것으로나타났다 (Table 1, Table 2). Acinetobacter baumannii와 Pseudomonas aeruginosa의연령별분리율 A. baumannii와 P. aeruginosa는기회감염및원내감염의원인균으로알려져있고면역기전이저하된환자에서분리됨으로연령별분리율을조사하였다. A. baumannii 의분리율은 71~80, 61~70, 41~50 세순으로높게나타냈으며 P. aeruginosa에서는 61~70, 71~80, 51~60, 81~90 세의순으로높게나타났다 (Table 3). 3. Acinetobacter baumannii의항균제감수성양상 A. baumannii에서는 piperacillin은 96%, carbenicillin 82%, cefotaxime 78%, ciprofloxacin 77%, gentamicin과 sulfamethoxazole/trimethoprime은 76%, ceftazidime 75%, tobramycin 72%, amikacin 64% 의내성률을보였으며, 반면 ampicillin/sulbactam, imipenem 및 meropenem은각각 100%, 99%, 96% 의감수성이었다 (Table 4). 4. Pseudomonas aeruginosa 항균제감수성양상 P. aeruginosa의항균제감수성은 cefotaxime과 sulfamethoxazole/trimethoprime에서 100% 의내성률을, carbenicillin은 49%, piperacillin 47%, ticarcillin 45%, ticarcillin/ clavulanic acid 40% 등의내성률으로보인반면, 감수성률은 meropenem 92%, imipenem 90%, amikacin, cefepime 85%, gentamicin 79%, ciprofloxacin 64%, ceftazidime 63% 를나타냈다 (Table 5). 4 대한임상검사학회지 40(2):75-79, 2008 IV. 고찰 세포벽합성에관여하는항균제로서가장많이이용되는 β-lacatam 계열항균제는현재약 150여가지가임상에사용되고있으며현재까지그람음성간균에광범위하게이용되어져왔다 ( 김등, 2004; 류등, 2004; 육, 2007). Broad-spectrum β-lactamase를생성하여이효소에안전한제3세대 cephalosporin, monobactam 등이개발되어임상에널리사용되어왔다. 다양한항균제의사용은여러내성균주의출현을유발하였으며특히 P. aeruginosa와 A. baumannii에서다제내성균주는병원환경에서문제가되고있는병원감염균이되었다 (Gootz와 Marra, 2008). 파키스탄에서조사된 P. aeruginosa의항생제감수성검사에서는가장내성률이낮은항생제로 amikacin(24%) 을나타냈으며우리나라는 imipenem(6%), meropenem(8%) 을들수있으며프랑스의병원에서는 amikacin(13.3%), ceftazidime(16.1%) 으로보고되었고우리의조사에서는 amikacin(15%), ceftazidime(20%) 로약간의차이를보였다 (Khan 등, 2008, Bert와 Lambert-zechovsky, 1997). 본조사지역의 P. aeruginosa 항균제내성현황이조사된다른나라보다현저히낮은수준임을알수있었다. 2007년도신등의대전지역의조사결과와비교할때, P. aeruginosa의항균제내성정도는 gentamicin 내성은높고 ( 대전 : 8.5%; 대구 : 21%), imipenem과 meropenem의내성은낮았다 ( 대전 : 16.1%, 14.7%; 대구 : 6%, 8%). 다제내성균의지표로알려진 carbapenem계항균제의내성이전국적으로급증하고있는상태에서본조사의결과는대구지역의항생제관리및병원감염관리가다른지역보다효율적으로이루워지고있다고사료되었다. 검체에따른분포를비교해보면신등 (2007) 은 P. aeruginosa 분리시 secretion와 sputum, wound, urine의순으로높게나타났으며본논문의조사에서는 sputum, urine, wound 등의순서로나타나호흡기감염이나창상감염에서주로분리되는경향이일치하였다. 이러한검체의성향은일반적인병원감염균분리검체의성향과유사하며, P. aeruginosa의감염은병원감염이주된경로라는간접적증거로보여진다. A. baumannii는 2006년오등의국내항균제감수성조사결과로부터본조사결과가 carbapenem계항균제 (<2%) 를제외하고는대부분의항균제에서높은내성률을보였다. 2007년 Sinha 등에의한국외의항균제감수성조사결과와도유사한결과를얻어대구지역의독특한항균제내성환경이존재할가능성을제기하였다. 2008년 Aggarwal 등의조사결과는 A. baumannii의 imipenem 내성이 0% 로본조사결과 (6%) 보다오히려낮았는데이러한지역적특성이항균제내성균의연구에중요할것으로사료되었다. 본조사가대구지역에서광범위한조사를통한결과가아니어서대구지역전체를나타낼수는없지만세균의내성 pattern이지역적항생제사용환경과밀접한관계가있을것이라는주장을뒷받침하는결과를보였다고판단이되었다. 본결과로대구지역에서의분리된 A. baumannii와 P. aeruginosa에대한 β-lacatam 계열항균제에대한내성빈도의비교, 연령별분포율을확인하였으며더나아가미생물검사실에서 β-lacatam 계열항균제내성유전자를검출하여보다정확한내성경로를파악함으로서항균제내성증가추세를조절할수있을것으로사료된다. 참고문헌 1.
