Methods : We tested 224 clinical isolates of E. coli, K. pneumoniae, K. oxytoca and P. mirabilis during May 2006 to March These isolates were examined by the Phoenix and the CLSI ESBL tests simultaneously. For the isolates showing discordant results between the two tests, boronic acid disk test was performed to differentiate AmpC β-lactamase and ESBL. Results : Among the 224 clinical isolates, 75 and 79 isolates were positive for ESBL by CLSI ESBL test and Phoenix test, respectively. Having detected 4 more isolates as ESBL-producers, Phoenix test showed a 98.2% agreement with a 100% sensitivity and 97.3% specificity compared with CLSI ESBL test. Among the four false positive isolates, three were AmpC-positive but ESBL-negative. Conclusions : The BD Phoenix ESBL test was sensitive and specific, and can be used as a rapid and reliable method to detect ESBL production in E. coli, Klebsiella species, and P. mirabilis. ESBL은기존의 β-lactam 항균제뿐만아 185

2 186 이교관 김성태 홍기숙외 2 인 니라 oxyimino-cephalosporins (ceftazidime, cefotaxime) 과 monobactam에내성을나타내며 clavulanic acid (CA) 에의해억제되는성질이있다 [2]. 대다수의 ESBL은기존의 TEM- 1, TEM-2 및 SHV-1을발현하는유전자에돌연변이가생긴것으로다양한항균제에내성을나타낸다 [3]. 국내에서는 Escherichia coli와 Klebsiella pneumoniae에서 TEM-52, SHV- 2a, SHV-12형 ESBL이흔하였으나 [4] 2001년 CTX-M-14를생성하는 K. pneumonia, E. coli 및 Shigella sonnei가보고된이후 CTX-M-15, CTX-M-14, CTX-M-3 등 CTX-M형의 ESBL 분리빈도와유형이증가하고있으며 [5, 6] 또한 VEB-1, GES-5, PER-1, OXA-30형등새로운 ESBL이보고되었다 [7, 8]. Clinical and Laboratory Standards Institute (CLSI) 에서 E. coli, K. pneumoniae, Klebsiella oxytoca 및 Proteus mirabilis의 ESBL 검출을위한디스크확산법과액체배지희석법지침에따르면 ESBL 생성균은모든 penicillins, cephalosporins과 aztreonam에내성으로보고한다 [9]. 그러나일부 ESBL 생성균에서 AmpC-type β-lactamase (AmpC) 를동시에생성하거나 [10, 11] porin 변화에의한막투과성의감소등이있을때표현형검사에서위음성이있을수있다 [11]. 또, TEM-1과 SHV-1 β-lactamase를생성하는 K. pneumoniae 의외막단백소실이동반된경우에는 ESBL 생성균으로판독하여위양성결과가나올수있다 [12]. ESBL 생성균을검출하는자동화된항균제감수성검사장비로 VITEK System (biomerieux, Marcy l Etoile, France [Vitek]) 과 MicroScan WalkAway-96 System (Dade Behring, Inc., West Sacramento, CA, USA [Microscan]) 이임상검사실에서사용되어검사시간을단축하였고최근에는 Phoenix Automated Microbiology System (Beckton Dickinson Diagnostic Systems, Sparks, MD, USA [Phoenix]) 이국내에도입되어이용되고있다. 자동화된항균제감수성검사장비를이용하는경우수기법보다쉽고신속하게 ESBL 생성균을검출할수있지만, CLSI ESBL 확인법에서처럼위음성과위양성등이있을수있다 [13-16]. ESBL을놓치거나위양성결과를보고하면환자치료에심각한영향을줄수있기때문에검사실에서는자동화된장비의정확도와제한점에대해서알고있어야한다. 이에, 본연구에서는 E. coli, Klebsiella spp. 및 P. mirabilis 를대상으로새로개발된 Phoenix system의 ESBL 시험을 CLSI ESBL 확인법과비교하여 Phoenix system의 ESBL 검출능을평가하였다. 대상및방법 1. 균주의수집및 ESBL 생성균검출법 2006년 5월부터 2007년 3월까지동국대학교일산병원에서 Phoenix NMIC/ID-108 combo panel (Beckton Dickinson Diagnostic Systems) 에서동정된 E. coli 103주, K. pneumonia 61주, K. oxytoca 13주, P. mirabilis 17주등194균주와이화여자대학교동대문병원 Vitek 1 장비에서 ESBL로확인된 E. coli 8주와 K. pneumonia 24주등총 226주를대상으로하였다. 분리된균주의동정은전통적인생화학적방법으로확인후균주동정의결과가전통적인법과다를경우재시험하였다. 균주의확인동정에서 Citrobacter brackii와 Enterobacter aerogenes로동정된두균주를제외한 224주모두를 Phoenix ESBL 시험과 CLSI ESBL 확인법을동시에시행하여 ESBL 생성균을검출하였고, 두검사법에서불일치를보인균주는 boronic acid 디스크법을추가로시행하였다. 2. Phoenix ESBL 검사및항균제감수성시험자동화된 ESBL 생성균의검출은 Phoenix (software version 5.15A/V4.11B) 에 Phoenix NMIC/ID-108 combo panel 로시험하였다 McFarland 탁도의 ID broth에서 25 μl 를취하여 antimicrobial susceptibility test (AST) indicator 를한방울첨가한 AST broth로옮긴후 NMIC/ID combo panel에분주하고장비에장착하였다. Cefepime, cefpodoxime, ceftazidime (CAZ), CAZ plus CA, cefotaxime (CTX) plus CA 및 ceftriaxone plus CA의 6개의항균제가함유된 well에서균의성장유무를검출한후알고리듬에따라 ESBL 생성여부를판독하였다. 알고리듬에따라 E. coli, K. pneumoniae, K. oxytoca에서 ESBL 판독기준에합당하면 rule No.1505 에의해 ESBL 생성균으로판정하였다. rule No.1505 알고리듬에음성이지만, carbapenem 감수성이고 piperacillin에내성이며 3세대 cephems이나 cefpodoxime 또는 aztreonam에내성일경우 rule No.1502 에의해 ESBL 양성으로판독하였다. E. coli의 AmpC나 K. oxytoca의염색체매개성 β-lactamase (K1) 와같이 ESBL과유사한내성기전을가지고있으면양성으로판독될수있다. 선택적으로 CLSI의 ESBL 선별검사기준으로양성일경우 rule No.106 에의해 ESBL 생성균일가능성이있으므로확인검사를하도록추천하였다. ESBL 검출에는약 8.5시간 (6-11시간) 이소요되었다. 3 Evaluation of the Phoenix for Detecting ESBL 187 험과 CLSI ESBL 확인법의결과에서차이를보일경우반복검사및 boronic acid 디스크법을병행하였다. 3. CLSI 디스크확산법에의한 ESBL 확인법 McFarland 0.5탁도의세균부유액을면봉으로 Mueller- Hinton 한천배지에고르게접종한후 CTX (BBL Microbiology Systems, Cockeysville, MD, USA, 30 μg), CTX-CA (30/10 μg), CAZ (30 μg) 과 CAZ-CA (30/10 μg) 디스크를놓고 35 C 에서 16-18시간배양후억제대를측정하여 CTX-CA 또는 CAZ-CA의억제대가 CTX과 CAZ보다 5 mm 이상클경우 ESBL 양성으로판정하였다. 그리고정도관리를위하여양성대조로 K. pneumoniae ATCC , 음성대조로 E. coli ATCC 25922를사용하였다. 4. Boronic acid 디스크법에의한 AmpC 와 ESBL 검출법 CLSI ESBL 확인법에서 ESBL 음성이었으나 Phoenix ESBL 시험에서는양성으로불일치를보인 4균주에대해 AmpC와 ESBL 생성여부를확인하기위하여 boronic acid (BA) 디스크법을이용하였다 [17, 18]. 120 mg의 phenylboronic acid (benzeneboronic acid; Sigma-Aldrich, St. Louis, MO, USA) 를 3 ml의 dimethyl sulfoxide (DMSO) 에녹인뒤에 3 ml 의증 Table 1. Abbreviations: ESBL, extended-spectrum β-lactamase; CLSI, Clinical and Laboratory Standards Institute. 류수를넣었다. 이용액 20 μl 를 cefoxitin (FOX), cefotetan (CTT), CAZ, CTX, CAZ-CA과 CTX-CA 디스크에넣고공기중에 60분간건조시킨후즉시사용하거나 4 C 에냉장보관하였다. 시험세균을 Mueller-Hinton 배지에접종하고디스크를놓은후 35 C 에서하룻밤배양하였다. BA첨가 FOX, CTT 디스크와 FOX, CTT의억제대차이가 5 mm 이상이면 AmpC 양성으로, BA첨가 CAZ-CA, CTX-CA디스크와 BA첨가 CAZ, CTX의억제대차이가 3 mm 이상이면 ESBL 양성으로판단하였다 [18]. 결과 1. Phoenix ESBL 시험의평가총 224분리주의 ESBL 생성균검출을위한검사에서 CLSI ESBL 확인법에서 75균주가 ESBL 양성이었고 Phoenix ESBL 시험에서는두검사법모두에서양성인 75주이외에 4균주에서추가로양성을보여위음성 0예, 위양성 4예였다. CLSI ESBL 확인법과비교시 Phoenix ESBL 시험은예민도 100%, 특이도 97.3% 였고, 일치도는 98.2% 였다 (Table 1). 두검사법에서불일치균주는 E. coli 3주와 K. pneumoniae 1주였으며 K. oxytoca와 P. mirabilis는모두일치하였다. Phoenix ESBL 시험양성인 79주중 15주는 rule No.1502 에의해 ESBL로판정하였으며 rule No.106 균주는없었다. 2. Boronic acid 디스크법에의한 AmpC와 ESBL 검출 CLSI ESBL 확인법과 Phoenix ESBL 시험에서불일치를보인 4균주에대해 AmpC와 ESBL 검출을위해 BA 디스크법을실시한결과 E. coli 3균주중 2균주와 K. pneumoniae 1균주에서 AmpC 양성이었으나 ESBL은모두음성으로 AmpC와 ESBL을동시에생성하는균주는검출되지않았다 (Table 2). 4 188 이교관 김성태 홍기숙외 2 인 고찰본연구에서는총 224균주중 75 주가 ESBL 생성균으로확인되었다. Phoenix ESBL 시험과 CLSI ESBL 확인법을동시에시행하였을때 Phoenix ESBL 시험에서위음성은없었고, 위양성 4예를보여예민도는 100%, 특이도는 97.3% 였다. 그리고 CLSI ESBL 확인법과일치율은 98.2% 였다. 불일치균주는 E. coli 3주와 K. pneumonia 1주였으며 K. oxytoca와 P. mirabilis는모두일치하였다. 국내에도입되어이용되고있는 Phoenix의 ESBL 시험은 cefepime, cefpodoxime, CAZ, CAZ plus CA, ceftriaxone plus CA, CTX plus CA의 6개의항균제가함유된 well을사용하여알고리듬에따라판독한다. 유럽에서는 cefepime을제외한 5가지항균제 well을사용하고있다. Phoenix를이용해 Enterobacteriaceae 510균주의 ESBL 생성연구에서 double-disk synergy시험및분자유전학적방법과비교하여 100% 의예민도와 98.9% 의특이도를보였고, K1을과잉생성하는두균주의 K. oxytoca와 ESBL을감별하지못하였다고보고하였다 [13]. 다제내성 E. coli와 Klebsiella spp. 91균주에서의 ESBL 검출방법비교연구에서 E-test 및분자유전학적방법과비교하였을때 Phoenix는예민도 92%, 특이도 82% 를보였고정확도는 89% 로 Vitek 1 (83%), Vitek 2 (78%) 보다우수하다고보고하였는데앞의연구보다예민도가낮은이유는 rule No.1502 인균주를 ESBL 음성으로판단했기때문이다 [14]. Enterobacteriaceae 147균주에서 ESBL 생성균검출을위한연구에서분자유전학적방법과비교했을때 Phoenix는 99% 의예민도를보여 VITEK 2의 86% 와 Microscan 의 84% 보다높았지만특이도는 52% 로 VITEK 2의 78% 보다낮았는데이는 K1을과잉생성하는 K. oxytoca와 ESBL을감별하지못하였기때문이었다 [15]. 본연구에서와같이 6가지항균제 well이있는 Phoenix NMIC/ID-108 combo panel을사용한 Thomson 등 [16] 은 ESBL 생성여부가확인된 E. coli와 Klebsiella species 102주를대상으로 ESBL 검출능을조사하였을때 Phoenix ESBL 시험은 96% 의예민도와 81% 의특이도를보였고 Vitek 2는 89% 의예민도와 85% 의특이도를보였다고보고하였다. 전체적으로 Phoenix는 Vitek이나 Microscan 보다예민도가높았으나 K1을과잉생성하는 K. oxytoca와 ESBL을감별하지못하는경향이있다. 본연구에서 Pheonix는 100% 의예민도를보였고위음성은없었다. ESBL 생성균검출을위한 CLSI 표현형검사에서위음성의원인으로는 K. pneumoniae에서 ESBL과함께 AmpC를동시에생성하거나 [10, 11, 16] E. coli에서 TEM-28을생성할 때 [16] porin변화에의한막투과성의감소등이있다 [11]. ESBL 과함께 AmpC를동시에생성할때 AmpC가 CA 에의해억제되지않기때문에표현형검사위음성을보이는것으로설명된다 [10]. 본연구에서는 ESBL과 AmpC를동시에생성하는균주가없었기때문에위음성이없었을가능성이있다. 또, TEM- 28을생성하는 E. coli나 porin변화에의해막투과성이감소된균주도포함되지않았기때문에예민도에대한연구로는충분하지않다. 국내에서도 ESBL을생성하는 K. pneumonia 75주중 40주 (53.5%) 에서 plasmid 매개성 AmpC를동시에생성하였고모두 cefoxitin에내성을보였다고보고하였다 [19]. 따라서 ESBL과 AmpC를동시에생성하는균주가많은검사실에서는 Phoenix ESBL 검출에위음성이증가할위험이있기때문에향후이부분에대한연구가더필요하다. 다른자동화된항균제감수성검사장비인 Vitek 1과 2의 ESBL 시험에서음성이고 cefoxitin에내성을보인 E. coli와 K. pneumoniae의경우 ESBL과 AmpC를동시에생성하는균주일가능성이있으므로위음성을줄이기위해추가적인검사가필요하다고보고되고있다 [20-22]. 하지만, Phoenix NMIC/ID-108 combo panel 에는 cephamycin계항균제가포함되어있지않아서이와같은알고리듬을적용하기어려운점이있다. 반면에, ESBL을검출하는 rule No.1505 는 cefepime, cefpodoxime, CAZ, CAZ plus CA, CTX plus CA 및 ceftriaxone plus CA을포함하는 6개의 well에서균의성장유무를종합하는알고리듬으로서 Vitek이나 Microscan보다많은 ESBL 확인검사용 well을사용하고있고 [13-16], rule No.1505 에음성일때 carbapenem 감수성이고 piperacillin에내성이며 3세대 cephems이나 cefpodoxime 또는 aztreonam에내성일경우 rule No.1502 에의해 ESBL 양성으로판독하기때문에예민도를높일수있다. 본연구에서도 rule No.1502 에의해 15균주가 ESBL로검출되었고, 이중 1주만위양성이어서 rule No.1502 가예민도를높이는데기여함을알수있다. Phoenix ESBL 시험위양성의원인으로는 K. oxytoca에서 K1을과잉생성할때 [13-16], E. coli에서염색체매개성 AmpC 의과잉생성, 그리고 K. pneumoniae에서 SHV-1 과잉생성이나 plasmid 매개성 AmpC를생성할때등이보고되었다 [15, 16]. 본연구에서위양성 4균주중 BA시험결과 3균주에서 AmpC 양성으로나와서 E. coli에서염색체매개성 AmpC를과잉생성하거나 K. pneumoniae에서 plasmid 매개성 AmpC를생성할때 Phoenix에서 ESBL로잘못판정할가능성이있다. AmpC 생성에의한위양성을선별하는방법으로 Phoenix ESBL 시험양성인균주중에 cefoxitin 내성을확인하는방법이있으나

5 Evaluation of the Phoenix for Detecting ESBL 189 Phoenix NMIC/ID-108 combo panel에는 cephamycin계항균제인 cefoxtin이나 cefotetan이포함되어있지않아서어려움이있다. 본연구에서 rule No 균주중 1주가 AmpC 에의한위양성이었고 rule No.1505 균주중 2주가 AmpC에의한위양성으로나와서 Phoenix의알고리듬만으로는 AmpC 에의한위양성을예측할수없고 Phoenix ESBL 시험양성인경우 AmpC에의한위양성을검출하기위해서는 CLSI ESBL 확인법이나 BA 디스크법등을추가로시행해야한다. 결론적으로 BD Phoenix ESBL 시험은검출시간이빠르고 CLSI ESBL 확인법과비교하여높은예민도와특이도를보여 E. coli, Klebsiella species와 P. mirabilis에서 ESBL 비생성균주로판독되면부가검사없이보고할수있을것이다. ESBL 생성균주로판독되면 AmpC 생성을확인하기위해 CLSI ESBL 확인법이나 BA 디스크법을고려할수있다. 요약배경 : 본연구에서는 Escherichia coli, Klebsiella pneumoniae, Klebsiella oxytoca, Proteus mirabilis에서새로개발된 BD Phoenix system (Beckton Dickinson Diagnostic Systems, USA) 의 extended-spectrum β-lactamase (ESBL) 시험을디스크확산법에의한 ESBL 표현형확인시험 (Clinical and Laboratory Standards Institute ESBL 확인법, CLSI ESBL 확인법 ) 과비교하여 Phoenix system의 ESBL 검출능을평가하였다. 방법 : 2006년 5월부터 2007년 3월까지임상검체에서분리된 E. coli, K. pneumoniae, K. oxytoca와 P. mirabilis 224 균주를대상으로하였다. 224균주에대해 Phoenix ESBL 시험과 CLSI ESBL 확인법을동시에시행하였다. 불일치균주에대해 AmpC β-lactamase와 ESBL 생성여부를확인하기위하여 boronic acid 디스크법을이용하였다. 결과 : 총 224균주중 CLSI ESBL 확인법에서는 75균주에서 ESBL양성이었다. Phoenix ESBL 시험은 4균주에서추가로양성을보여예민도 100%, 특이도 97.3% 였고 CLSI ESBL 확인법과비교시일치도는 98.2% 였다. 위양성 4균주중에서 3균주는 AmpC를생성하였지만 ESBL을생성하는균주는없었다. 결론 : BD Phoenix ESBL 자동화시험은예민하고특이적이며임상검체에서분리되는 E. coli, Klebsiella species와 P. mirabilis에서 ESBL 생성을검출하는데빠르고유용할것으로사료되었다. 참고문헌 1.
