The purposes of this study were to ascertain the clinical significance of viridans streptococcal bacteremia and to determine the epidemiological, clinical, and microbiological features. Methods : All cases of viridans streptococcal bacteremia in the period from January 1998 to December 2002 were reviewed. Bacterial identification was done with the Vitek system, and antimicrobial susceptibility tests for penicillin, cefotaxime, chloramphenicol, clindamycin, and erythromycin by the disk diffusion method or E-test were carried out. Results : Of 154 cases of viridans streptococcal bacteremia, 56 cases were identified to the species level such as S. salivarius (19), S. mitis (16), S. intermedius (7), S. sanguis (7), S. uberis (3), S. oralis (2), S. bovis (1), and S. mutans (1). The most frequent diagnostic category was infectious diseases (54), followed by solid organ tumors (45), hematologic malignancies (29), and liver diseases (8). Seven patients (4.5%) had subacute endocarditis and all of them had underlying heart disease. Seventy six (49.3%) patients showed positive blood culture from two or more blood samples. Thirty three (21.4%) patients had an absolute neutrophil count of less than 500 cells/ L. Thirty four cases of bacteremia were polymicrobial (22%). When the diagnosis of viridans streptococcal bacteremia was established by the presence of two or more positive blood cultures or presence of neutropenia or signs of sepsis, 29.9% was interpreted as contamination. Two patients died of septic shock caused by viridans streptococcal infection alone. Sixty-eight percent of isolates were susceptible to penicillin. Conclusions : About 70% of viridans streptococcal bacteremia was clinically significant. Most viridans streptococcal bacteremia was found in patients with cancer or neutropenia and these infections were associated with limited morbidity. Viridans streptococi는구강, 비인두, 여성비뇨생식관등을포함한신체의점막에존재하는정상세균총의일종이다. 과거 viridans streptococci는낮은독성을가지고있어감염의주원인균으로관심이덜하였으나, 근래 viridans streptococci 균주들에의한다양하고심각한감염증의보고가증가되어감염의원인균주로인식되고있다 [1-3]. 특히아급성심내막염의경우 viridans streptococci는가장흔한원인균중의하 246

2 Viridans Streptococci 균혈증 247 나이며, 이는류마티스성열, 승모판탈출, 선천성결손, 인공판막등으로인한심장판막손상이나결손이있는사람에서주로발생한다 [4]. 최근에는혈액종양이나고형암또는골수이식등의치료에사용된항암제에따른심각한호중구감소증, 구강점막의궤양등과연관되어 viridans streptococci가균혈증의중요한원인균으로인식되고있으며, penicillin과기타항균제에대한내성도증가하고있어문제가되고있다 [5, 6]. 하지만 viridans streptococci 는피부에상재할수있기때문에혈액배양에서이균이동정되면병원균보다는오염균으로잘못해석될수있다. 지금까지국내에서는혈액에서분리되는 viridans streptococci의항균제감수성에대한연구가일부이루어졌으나, viridans streptococci에의한균혈증의임상적인의미에대한보고는거의없었다. 이에본연구에서는최근 5년간본원에서혈액배양검사를시행하여 viridans streptococci가검출된환자들을대상으로역학적, 임상적인특성을분석하여 viridans streptococci에의한균혈증의임상적인의미와항균제감수성양상을살펴보고자하였다. 대상및방법 1. 대상 1998년 1월부터 2002년 12월까지본원미생물검사실에서혈액배양검사를시행하여 viridans streptococci가동정된환자 154명을대상으로하여의무기록을후향적으로분석하였는데, 환자의성별과나이, 진단명, 증상, 혈액배양양성횟수, 절대호중구수치, 다균혈증여부, 사망원인등을조사하였다. 이중에서혈액배양검사에서두번이상양성반응을나타낸경우와절대호중구수치가 500/ L 이하로감소한경우, 균혈증의증상을보인경우를임상적으로의미가있는것으로판정하였다. 2. 혈액배양본원에서는 30분에서 1시간간격으로 2-3 회혈액을채취하여각각호기성과혐기성혈액배양병에분주하여배양한다. BACTEC Standard 10 Aerobic/F와 BACTEC Standard Anaerobic/F 배양병을사용하여 BACTEC 9240 (Becton Dickinson, Sparks, MD, USA) 혈액자동화장비에서 5일간배양하였다. 3. 세균동정및항균제감수성검사세균의동정은 Vitek system (biomerieux, Inc., Hazelwood, MO, USA) 을이용하였다. 항균제감수성검사는 NCCLS의 Streptococcus pneumoniae 이외의연쇄구균에대한디스크확산법을사용하여 penicillin, cefotaxime, chloramphenicol, clindamycin, erythromycin, vancomycin에대하여시행하였다. Penicillin에 대한항균제감수성시험은 Mueller-Hinton 한천에 5% 면양혈액을첨가한배지에균액을골고루바른후 penicillin E-test strip (AB Biodisk, Solna, Sweden) 을올려놓고 24시간배양한뒤최소억제농도 (minimum inhibitory concentration, MIC) 를측정한다음 NCCLS의기준 [7] 을적용하여해석하였다. 1. 임상특성 결 환자의남녀비는 1.5:1의비율이었으며평균연령은 44.7세였다. 혈액배양검사결과 viridans streptococci가검출된환자의최종진단명을크게분류하면감염성질환이 54명 (35.0%) 으로가장흔한진단군이었으며고형종양 45명 (29.2%), 혈액종양 29명 (18.8%), 간경화를포함한간질환이 8명 (5.2%) 이었다. 7명의환자가아급성심내막염환자로이들은모두기저심질환을가지고있었으며, 동일시행한모든혈액배양검사에서 viridans streptococci가검출되었다. 76명 (49.3%) 의환자가동일시행한혈액배양검사에서두번이상균이검출되었다. 33명 (21.4%) 의환자가절대호중구수치가 500/ L 이하였다. 34명 (22.1%) 의환자는다균혈증환자였다. 10명의환자가균혈증으로사망하였는데, 이중 8명은다균혈증환자였으며, 2명은 viridans streptococci에만의한균혈증으로사망하였다. 총 154 명중동일시행한혈액배양검사에서두번이상양성반응을나타낸 76명의환자와절대호중구수치가 500/ L 이하로감소된환자와균혈증의증상을보인환자에서의 viridans streptococci의동정을오염균이아닌감염균으로해석하면, 70.1% (108/ 154) 에서임상적으로의미가있는균이었다 (Table 1). Identification of viridans streptococci isolated from blood cultures Organisms No. (%) S. salivarius S. mitis S. intermedius S. sanguis S. uberis S. oralis S. bovis S. mutans Total 세균동정 154명의환자중 56명에서분리된균주는종단계까지동정되었으며, 이들균주에서균종별분리율은 S. salivarius가 19 (33.9%) 로가장흔히분리되었고, 그다음으로는 S. mitis 16 (28.6%), S. intermedius 7 (12.5%), S. sanguis 7 (12.5%) 의순이었다 (Table 2). 3. 항균제감수성 혈액에서분리된 viridans streptococci 154 균주의항균제별내성률은 erythromycin과 clindamycin이 31.8%, 20.1% 로가장높았고, penicillin 내성률은중간내성이 24%, 내성이 7.8% 이었다. Vancomycin에는모두감수성이었다 (Table 3). 고 Viridans streptococci는사람의구강내에존재하는상재균으로흔히관찰되고대부분은병원균이아니며, 병원성이약한균종이지만최근호중구감소증환자에서균혈증의주요원인균으로대두되고있는데, 이는 norfloxacin과같은 fluoroquinolone 계통의약제를예방적으로사용함에따라 viridans streptococci의비율이증가했기때문이다 [6]. 이후 viridans streptococci에의한균혈증을예방하기위하여예방적항균제에 penicillin을추가함으로써 -lactam제제에내성인 viridans streptococci의출현을야기했으며, -lactam 제제에내성일뿐아니라 macrolides, tetracyclines, trimethoprim-sulfamethoxazole에대한내성도획득하게되어문제가되고있다 [8, 9]. Viridans streptococci의균종별분리빈도를살펴보면국내에서는 2002년어등 [10] 이혈액에서분리된 45균주의 viridans streptococci 중 API Strep 32를이용하여 S. mitis (31.1%), S. oralis (17.8%), S. constellatus (11.1%), S. anginosus (8.9%) 의순으로보고하였다. 본원에서는 S. salivarius (33.9%), S. mitis (28.6- %), S. intermedius (12.5%), S. sanguis (12.5%) 의순이었는데, 찰 Table 3. Antimicrobial susceptibilities of viridans streptococci by disk diffusion method or E-test Antimicrobial agent No. (%) of isolates Susceptible Intermediate Resistant Penicillin 105 (68.2) 37 (24.0) 12 (7.8) Erythromycin 101 (65.6) 2 (2.6) 49 (31.8) Chloramphenicol 148 (96.1) 1 (0.6) 5 (3.3) Clindamycin 123 (79.9) 0 (0.0) 31 (20.1) Vancomycin 154 (100) 0 (0.0) 0 (0.0) Cefotaxime 145 (94.2) 3 (1.9) 6 (3.9) 이를해석할때 Vitek장비에서는모든 viridans streptococci를균종수준까지동정하지못하고 36.4% 만특정균종으로동정한제한점을고려하여야할것이다. 혈액배양에서 S. intermedius가동정되었을때오염균인경우는드물고, 대부분의경우임상적으로의미가있다고알려져있다 [11, 12]. 본연구에서는 S. intermedius에의한균혈증이 7명의환자에서관찰되었는데, 이중 5명의환자는고형암환자였고, 1명의환자는다균혈증에의한패혈성쇼크로사망하였다. 나머지한명의환자는 Escherichia coli에의한요로감염으로치료받고호전되어 S. intermedius는오염균인것으로판단되었다. 호중구감소증이있는암환자의혈액배양검사에서 viridans streptococci의동정은최근십수년사이에증가하는추세이며 [13], 스위스의한연구에서는입원한호중구감소증환자의균혈증중약 30% 가 viridans streptococci에의한것으로보고한바있다 [14]. 본원에서혈액배양에서 viridans streptococci가검출된환자의 21% 가절대호중구수치가 500/ L 이하로, 호중구감소증이 viridans streptococci에의한균혈증의중요한위험인자임을확인하였다. Viridans streptococci는피부에도상재할수있기때문에혈액배양검사에서분리된경우에오염균으로오판할수있으며, 혈액에서분리된균이감염균인지오염균인지는정확하게판단하기는어렵다. 하지만일정한시간간격을두고시행한혈액배양검사에서두번이상같은균이검출될경우감염균으로해석할수있다. 일시적인균혈증은정상인에서는대부분특별한부작용없이회복되지만, 특히항암치료를받고있는호중구감소증환자에서의균혈증은심각한합병증을초래할수있으므로 [15], 절대호중구수치가 500/ L 이하인환자에서의동정은임상적인의미가있는것으로판단하였다. 따라서동일시행한혈액배양검사에서두번이상양성반응을나타낸 76명의환자와절대호중구수치가 500/ L 이하로감소된환자 (33명) 와균혈증의증상을보인환자 (18명) 에서의 viridans streptococci의동정을오염균이아닌감염균으로해석하였으며, 70.1% 에서임상적인의미가있었다. NCCLS에서는혈액등정상적으로무균적인신체부위에서동정된 viridans streptococci는 penicillin 또는 ampicillin의최소억제농도를측정하도록하고있는데 [7], viridans streptococci에의한아급성감염성심내막염의경우, 측정된최소억제농도에따라치료약의종류와용량이달라진다 [16]. 본연구에서는 penicillin의최소

4 Viridans Streptococci 균혈증 249 억제농도를 E-test를시행하여구하였으며, penicillin 감수성률은 68.2% 로, 이는어등이보고한 penicillin 감수성률 64.4% 와비슷하였다. 3세대 cephalosporin인 cefotaxime은 94.2% 에서감수성이었는데, 어등은 ceftriaxone에대한감수성률을 89% 로보고하였다 [10]. 스페인의 1,000 병상규모의한대학병원에서는 ceftriaxone, ceftazidime, cefpirome과 cefepime의내성률을각각 22%, 53%, 14% 와 34% 로보고하였으며 [17], 스페인과같이 penicillin 내성폐렴구균의유행지역에서두연쇄상구균종간에 penicillin과 cephalosporin에대한내성이전달되어 viridans streptococci의 cephalosporin 계통의약물에대한내성률도높은것으로추측되고있다 [15]. 따라서국내에서는아직까지 cephalosporin계약물에대한내성률이외국보고에비해약간낮은편이긴하지만, penicillin 내성폐렴구균의비율이 80% 에달하므로 [18], cephalosporin계약물에대한내성률의증가를잘감시하여야할것이다. 아직까지 vancomycin에내성인 viridans streptococci는보고된적이없으며, 본연구에서도모든균주가 vancomycin에감수성이었다. 외국의몇몇암센터에서는호중구감소증환자에서 viridans streptococci에의한균혈증의예방약제로 penicillin 투여를시험적으로 vancomycin으로대체하고있으나 [1, 13], 이는 Enterococcus의 vancomycin 내성률을높일수있으므로, Enterococcus의 vancomycin 내성률이높은국내에서 vancomycin의예방적사용은신중하게고려되어야할것이다. Penicillin에과민성이거나내성균주에의한감염일경우에는 erythromycin이나 clindamycin 계열약물을대체약제로사용할수있는데, 이들에대한내성률은중간내성을포함하여각각 34.4% 와 20.1% 로나타나어등의연구 [10] 와비슷한내성률을보였다. Chloramphenicol은아직까지 S. bovis의단일균종을제외하고는 [19] 항생제에주로감수성으로보고되었는데 [8], 본연구에서는 96.1% 가감수성으로나타났다. 요약배경 : Viridans streptococci는여러균종을포함하는명칭으로, 주로구강상재균으로존재하며임상검체에서이균의동정은오염균으로잘못해석될수도있다. 본연구에서는 viridans streptococci에의한균혈증의임상적의미를분석하고, 역학적, 임상적인특징과항생제감수성양상을살펴보고자하였다. 방법 : 1998년 1월부터 2002년 12월까지혈액배양에서 viridans streptococci가분리된환자들을대상으로의무기록을후향적으로분석하였다. 균의동정은 Vitek 장비를이용하였으며, penicillin, cefotaxime, chloramphenicol, clindamycin, erythromycin에대해서는디스크확산법으로항균제감수성검사를시행하였으며, penicillin에대해서는 E-test를실시하여최소억제농도를구하였다. 결과 : 154명의 Streptococcus viridans 균혈증환자중 56명에서종단위까지동정되었는데, S. salivarius (19), S. mitis (16), S intermedius (7), S. sanguis (7), S. uberis (3), S. oralis (2), S. bovis (1), S. mutans (1) 순이었다. 가장흔한진단군은감염성질환 (54명 ) 이었으며, 고형종양 (45명), 혈액종양 (29명), 간질환 (8명) 순이었다. 7명의환자는아급성심내막염 (4.5%) 이였으며이들모두는기저심질환을가지고있었다. 76명 (49.3%) 의환자가혈액배양검사에서 2회이상분리되었다. 33명 (21.4%) 의환자는절대호중구수치가 500/ L 이하였다. 34명 (22%) 은다균혈증환자였다. 혈액배양검사에서두번이상양성소견을보인환자와절대호중구수치가 500/ L 이하로감소된환자와균혈증의증상을보인환자에서의 viridans streptococci의동정을감염균으로해석했을때, 29.9% 는오염균에의한것으로판단되었다. 2명의환자가 viridans streptococci 단독에의한균혈증으로사망하였다. 68% 의 viridans streptococci는 penicillin에감수성이었다. 결론 : 약 70% 의 viridans streptococci에의한균혈증은임상적인의미가있었다. 대부분의 viridans streptococci에의한균혈증은암환자나호중구감소증환자에서발생하지만, 치사율은높지않았다. 참고문헌 1.
