We report the first case of a large cell neuroendocrine carcinoma in the hilar bile duct. A 73-year-old woman presented with continuous dull pain in the epigastrium. We diagnosed a Klatskin s tumor, Bismuth type II by computed tomography. An initial endoscopic cytology of the common hepatic duct showed a poorly differentiated adenocarcinoma. She underwent an extended right hepatectomy. Histologically, the tumor was diagnosed as a large cell neuroendocrine carcinoma. She received adjuvant radiotherapy for tumor involvement of the resected margin. At 12 months after surgery, lymph node metastases were present in the retroperitoneum, but she was still alive at 23 months after surgery. The role of additional therapy, such as radiotherapy, must be investigated to improve patient prognoses. (Korean J Med 2011;80:S137-S141) Keywords: Neuroendocrine carcinoma; Klatskin s tumor 서론담관암은간내담관암과간외담관암으로나뉘며, 간외담관암은다시간문부담관암과원위부담관암으로나뉜다. 양쪽주간관의합류부에서발생한종양을간문부담관암이라고하며, 간문부는원위부의간외담관에비해담관벽이얇고주위혈관이인접하며, 상당부분이간실질에둘러싸여 있어암이주위연조직, 혈관, 간실질로쉽게침습하여완치율이불량하다 [1]. 간외담관암의 90% 이상은선암이고, 신경내분비종양은 0.4% 미만으로극히드물다 [2]. 신경내분비종양중대세포신경내분비종양은고도로공격적인암으로폐에서처음기술된후 [3] 폐외의여러기관에서보고되었다 [4,5]. 담도계에서는담낭, 바터팽대부에서수례발표되었고 [6-11], 국내 Received: Revised: Accepted: Correspondence to Sang Hyub Lee, M.D. Department of Internal Medicine, Seoul National University Bundang Hospital, 300 Gumi-dong, Bundang-gu, Seongnam , Korea Tel: , Fax: , gidoctor@snubh.org -S137-

2 - The Korean Journal of Medicine: Vol. 80, Suppl. 2, 에서는담낭에서 1예보고된바있다 [12]. 간외담관에서발생한경우는 Sato 등이발표한총담관의대세포신경내분비종양과선암이혼합된 1예 [13] 외에는발표된적이없다. 저자들은상복부의둔통으로내원한 73세여자환자에서간문부담관암으로진단하여수술시행하였으며술후조직소견에서대세포신경내분비종양으로진단된예가있어문헌고찰과함께보고하는바이다. 증례환자 : 73세, 여자주소 : 상복부의둔통현병력 : 환자는내원 1달전부터시작된상복부의지속적인둔통과소화불량증세로내원하였다. 과거력및사회력 : 3년전건강검진으로시행한위내시경에서만성위염및헬리코박터균양성으로치료받았으며, 2년전고혈압, 고지혈증, 골다공증진단받고혈압강하제와칼슘제복용중이었다. 사회력에서특이사항은없었다. 이학적소견 : 내원당시혈압 138/68 mmhg, 맥박수 68회 / 분, 호흡수 18회 / 분, 체온 36.5 였고, 신체검사에서의식은명료하였으나만성병색이있었고, 결막은창백하지않았고, 공막에황달은없었다. 흉부및복부진찰소견은정상이었다. 검사소견 : 말초혈액검사에서백혈구 6,000/mm 3, 혈색소 13.2 g/dl, 헤마토크릿 39.3%, 혈소판 254,000/mm 3 이었고, 생 화학검사에서 AST 22 IU/L, ALT 25 IU/L, 알칼리포스파타제 51 IU/L, r-gtp 158 IU/L, 총빌리루빈 1.4 mg/dl, 총단백 7.5 g/dl, 알부민 4.6 g/dl로 r-gtp 와총빌리루빈의상승이외에는특이소견없었다. 혈액응고검사는프로트롬빈시간이 13.1초 (103%) 로정상이었고, 간염표지자검사는 HBsAg 과 anti-hbs, anti-hcv 모두음성이었다. 종양표지자검사는 CA U/mL, CEA 1.3 ng/ml로정상범위였다. 췌담도전산화단층촬영에서간문부및총간관의벽이두꺼워져있고, 이는근위부로는왼쪽주간관까지원위부로는총담관까지이어져있었다. 총간관및총담관주변으로여러개의뭉쳐진림프절비대가보였다 (Fig. 1A). 병변의근위부로간내담도가확장된소견관찰되었으며간전이는없었다 (Fig. 1B). 이에간문부담관암 ( 클라스킨종양, Bismuth 분류 2형 ) 으로진단하였다. 내시경적역행성췌담관조영술에서도간문부에조영제가차지않아간문부종괴로생각되었고, 이로인해왼쪽주간관이조영이되지않았다 (Fig. 2). 총간관에서세포진검사및간문부에서조직검사를하였고, 내시경적비담관배액술을시행하였다. 간문부조직검사에서는상피밑섬유화외에이상소견없었으나세포진검사에서분화도가나쁜선암종의심되는소견이었다. 치료및경과 : 상기소견으로수술을결정하였고, 확대오른간절제술및간공장문합술을시행하였다. 수술후조직소견에서병변은양쪽주간관및합류부, 총간관, 낭성관, 근위부총담관에걸쳐있었고, cm 크기로섬유 A B Figure 1. 수술 20개월 후 다시 황달 발생하였고, 역시 피 부간경유 담관배액술 및 풍선확장술 시행 후 호전되어 배액 관 제거하였으며, 수술 23개월 후인 현재까지 추적 중이다. 고 찰 신경내분비 세포는 신체에 광범위하게 분포하여 있고 따 라서 신경내분비종양은 신체의 여러 부위에서 발생할 수 있 다.

Immunohistochemical staining demonstrates expression of the neuroendocrine marker chromogranin (chromogranin stain, 400). 황달 발생하였고, 췌담도 전산화단층촬영에서 문합 부위의 부종으로 인해 간내 담도들이 확장되어 있었고, 왼쪽 후복강 에 림프절들이 커져 있어 전이로 생각하였다. 환자는 보존적 인 치료로 피부간경유 담관배액술 및 풍선확장술 시행 받고 호전되었다. 수술 20개월 후 다시 황달 발생하였고, 역시 피 부간경유 담관배액술 및 풍선확장술 시행 후 호전되어 배액 관 제거하였으며, 수술 23개월 후인 현재까지 추적 중이다. 고 찰 신경내분비 세포는 신체에 광범위하게 분포하여 있고 따 라서 신경내분비종양은 신체의 여러 부위에서 발생할 수 있 다. 2000년 발표된 위장 및 췌장 신경내분비종양의 WHO 분 Figure 3. Large tumor cells with abundant cytoplasm and prominent nucleoli (hematoxylin and eosin stain, 400). 류에 따르면 한 범주는 poorly-differentiated neuroendocrine carcinomas 그룹으로, 고도의 악성을 보이며 소세포암, 대세포 신경내분비종양이 이에 속해 있다. 다른 범주는 well-defined 근육주변층까지 침범하였다(T2). 맥관계 및 신경주위 침범 adult neuroendocrine tumors로 점진적인 경과를 보이며 주로 이 있었고, 림프절이나 간, 담낭의 침범은 없었으나 왼쪽 주 위장관계에서 발생하며 카르시노이드 종양, 췌장 섬세포 간관 절제 변연의 침범이 있었다(T2N0M0). 현미경 소견에서 종양 등이 있다. 이는 WHO 분류에 따라 well-differentiated 는 풍부한 세포질을 가지는 큰 종양세포로 뚜렷한 핵인을 neuroendocrine tumors와 well-differentiated neuroendocrine car- 보였으며(Fig. 3), 면역조직화학염색에서 CD56, chromogranin, cinomas로 다시 나눌 수 있다[14]. synaptophysin에 양성반응을 보여(Fig. 4), 대세포 신경내분비 위장관계에서 발생하는 종양 전체에서 보면 신경내분비 종양으로 진단하였다. 환자는 절제 변연의 침범으로 28일간 종양은 드물며, 그 중 담도계에서 발생하는 비율은 0.2-2% 총 50.4 Gy의 방사선 치료를 시행 받았다. 수술 12개월 후 정도로 매우 낮다[15]. 담도계의 대세포 신경내분비종양은 - S 139 -

Treatment and prognosis of large cell neuroendocrine carcinoma in the biliary system Authors Year Location Age/sex Histology Treatment Prognosis Cavazza [6] 2003 AoV 74/F LCNEC PD D 8 mo AS Cheng [7] 2004 AoV 55/F LCNEC Palliative gastrojejunostomy D 6 mo AD Kang [12] 2004 GB 78/F LCNEC Palliative cholecystectomy D 1 mo AS Sato [13] 2006 CBD 68/M LCNEC+AC PD + CC (cisplatin + etoposide) D 3 mo AS Jun [8] 2006 GB 67/F LCNEC CC D 10 mo AD 2006 GB 55/M LCNEC CC D 1 mo AD 2006 AoV 56/M LCNEC PPPD 2 mo AD, liver meta Huang [9] 2006 AoV 59/M LCNEC PD+CC (cisplatin + cyclophosphamide) D 10 mo AS Selvakumar [10] 2006 AoV 48/M LCNEC Whipple s operation 6 mo AS, liver meta Shimono [11] 2009 GB 64/F LCNEC CC + RT + operation + gamma knife D 69 mo AD Present case 2010 Hilar BD 73/F LCNEC Extended right hepatectomy + RT 12 mo AS, LN meta AoV, ampulla of Vater; GB, gall bladder; CBD, common bile duct; BD, bile duct; LCNEC, large cell neuroendocrine carcinoma; AC, adenocarcinoma; PD, pancreaticoduodenectomy; CC, chemotherapy; PPPD, pylorus preserving pancreaticoduodenectomy; RT, radiotherapy; D, death; mo, month; AS, after surgery; AD, after diagnosis; meta, metastasis; LN, lymph node. 수례증례보고되었으며표 1과같다. 간외담관에서소세포암은수례발표된적이있으나 [16,17] 대세포신경내분비종양은총담관의대세포신경내분비종양과선암이혼합된 1예외에는발표된적이없으며, 금번증례는대세포신경내분비종양만으로이루어진간문부담관암의첫증례이다. 신경내분비종양은면역조직화학염색에서신경내분비성표지자인 chromogranin A, synaptophysin, neuron-specific enolase (NSE) 등이하나이상양성소견을보이는경우진단하게된다. 또한전자현미경에서신경내분비과립을관찰할수있다. 대세포신경내분비종양은광학현미경에서신경내분비종양의특징을보이고카르시노이드와감별되는특징으로고도의악성 (10 high-power field 에서 10개이상의유사분열을보이며광범위한괴사를동반함 ) 을보인다. 또한소세포암과감별되는특징으로세포가크고낮은핵핵질간비율을보이며, 결이거친핵안의염색질이있으며인의빈도가높다 [6]. 또한 Jun 등이발표한바에따르면전산화단층촬영에서담낭및담관의암이간으로의공격적인침범및광범위한림프절침범이있을때대세포신경내분비종양같은고도의신경내분비종양을의심해볼수있다고한다 [8]. 이증례는간문부담관에서발생한대세포신경내분비종양으로 T2N0M0 로비교적초기에발견하여근치적절제수술시행하였고, 절제변연의침범이있어방사선치료도시 행하였으나수술약 1년후복강내림프절로전이되었다. 이전발표된담도계의대세포신경내분비종양의증례들을보면 (Table 1) 바터팽대부의대세포신경내분비종양으로근치적수술시행하였으나수술 5개월후간으로전이되었고, 수술 8개월후사망하였으며 [6] 다른증례들도근치적수술 2개월, 6개월후각각간으로전이되었다 [8,10]. 즉, 근치적수술후에도예후가좋지못하였다. 또한바터팽대부종양으로근치적수술 5개월후간전이가있어항암화학요법을시행하였으나 5개월만에사망한증례가있었고 [9], 총담관에대세포신경내분비종양와선암이혼합되어나타난증례에서도근치적수술 2개월후간전이를보여항암화학요법시행하였으나 3개월만에사망하여 [13] 전이이후에는항암화학요법도별효과를보이지못한것으로보인다. 담낭의대세포신경내분비종양으로근치적수술이불가능하였으나보존적수술, 항암화학요법, 방사선치료, 뇌전이에대한감마나이프치료까지적절히시행하여진단후 69개월까지생존한증례가있었고 [11], 이를토대로다각적인치료가환자의생존기간연장에도움이될수있음을알수있다. 이증례에서도절제변연의침범이있어방사선치료를하였고, 다른발표된증례들보다병의재발이비교적늦고, 생존기간이긴편으로방사선치료의효과가있었을것으로생각된다. 환자는전신상태고려하여항암화학요법 -S140-
