Even though many drugs has been developed, PONV still remains unsolved problem. Ondansetron is a commonly used 5-HT 3 receptor antagonist. It acts through specific binding to the 5-HT 3A, 5-HT 3B receptor complex. We hypothesized that patients with genetic variation in 5-HT 3A receptor might have variable incidence of PONV and respond differently to ondansetron. Methods: We included 204 patients undergoing gynecologic laparoscopic surgery. PONV were documented during 24 hours after operation. Ondansetron was injected to every patient who had PONV at PACU and PONV reassessed after 15 minutes. DNA was extracted from blood and 5-HT 3A Pro16Ser missense mutation was analyzed by using real-time PCR. Results: The incidence of PONV were 50% for wild type, 53% for heterozygote and 0% for homozygote. There were no significant differences between wild type and heterozygote in VAS of nausea and VAS change after ondansetron. (Anesth Pain Med 2008; 3: ) Key Words: 5-HT 3A receptor, ondansetron, polymorphism, PONV, Pro16Ser. 서 마취의발달과많은약제들의개발에도불구하고수술후발생하는구역및구토 (postoperative nausea and vomiting, PONV) 는아직도해결되지않는문제점이다. 많은환자들이술후통증보다 PONV를더참기괴로운증상으로생각하고있다. 1) PONV는전신마취를시행받는환자에서매우흔하여발생률이 20 40% 에이르며, 2) 고위험환자의경우 80% 까지보고되고있다. 3) 예방적항구토제사용에대해서는논란이많지만, 고위험군에속하는환자는 PONV의예방을위해항구토제를미리투여하는것이환자의만족도를높이는데기여하는것으로알려져있다. 4) PONV의예방과치료를위해 metoclopramide, droperidol, dexamethasone, 5-hydroxytrptamine (5-HT 3) 수용체길항제등의약제들이사용되는데, 5-9) 이중 5-HT 3 수용체길항제는다른항구토제와달리추체외로증상이나진정, 저혈압, 불쾌감같은부작용이적고, 효과가우수하여주술기에많이사용된다. 10,11) 하지만 5-HT 3 수용체길항제인 ondansetron을예방적으로사용하여도산부인과골반경수술을받는환자의 5 25% 에서여전히 PONV가나타났다고한다. 12) PONV의발생기전이아직정확히밝혀지지는않았으나, 동일한약제에대해개개인의반응이다른것은약물대사능력이다르거나, 약물이작용하는수용체반응차이등의원인을들수있다. 이제까지연구된 5-HT 3 수용체의아형으로는 5-HT 3A,B,C,D,E 등이있으며 ondansetron의작용은주로 5-HT 3A, 5-HT 3B 에의해나타난다 ) Kaiser 등에 17) 의하면항암치료시유발된구역및구토 (chemotherapy-induced nausea and vomiting: CINV) 의치료실패와 HT 3B 수용체의 AAG 결손형돌연변이 (AAG deletion mutation) 가연관이있다고하였으나, PONV 론 255

2 256 Anesth Pain Med Vol. 3, No. 4, 2008 의치료시오심구토의발생률과 HT3B 수용체의 AAG 결손형돌연변이와는통계적으로차이가없다고하였다. 18) 5-HT 3A 수용체의경우, 아직까지 PONV와수용체유전적다형성에관한연구가전무하다. 따라서본연구에서는한국인에서 5-HT 3A 수용체 Pro16Ser 부위의유전자형의빈도를파악하고유전자형의변이가수술후구역및구토발생빈도와 ondansetron의효과에미치는영향을알아보고자하였다. Primer Sequences for Searching HTR 3A Pro16Ser (rs ) Left primer 1) Oligo for G 5'-GCCTCCGAGTGCTCAGTG-3' 2) Oligo for A 5'-GCCTCCGAGTGCTCATGA-3' Right primer 5'-GCAGCCTCAGAAGGTGTGA-3' (common primer) 대상및방법병원윤리위원회의승인후, 전신마취하에복강경을이용하여부인과수술을받는미국마취과학회신체등급분류 1, 2에해당하는환자중수술전연구의목적과방법을설명듣고피험자동의서에동의한환자 204명을대상으로하였다. 알코올 ( 일일평균알코올 50g 이상섭취 ) 또는약물남용의과거력이있거나, 카페인과다섭취자 ( 하루커피 5 잔이상등 ), 약물의흡수, 대사또는배설에영향을줄수있는만성질환자, 기타연구자의판단에의해부적합하다고판단된자는대상에서제외하였다. 마취전처치제로서수술장입구에서 0.05 mg/kg의 midazolam을정주하였다. 마취유도는 thiopental sodium 5 mg/kg, rocuronium 0.6 mg/kg, remifentanil 0.5 mcg/kg를사용하여기관내삽관을실시하였다. 마취유지는산소 1 L/min, 아산화질소 1 L/min, desflurane 4 vol% 흡입과 remifentanil 목표효과처농도 2 6 ng/ml 의지속정주로하였고호기말이산화탄소의분압이 mmhg가되도록조절호흡을통해유지하였다. 수술후통증조절을위해 30 mg의 ketorolac을초기정주한후, 정맥자가통증조절장치 (Acemedical, Korea) 를이용하여 3 mg/cc 농도의 ketorolac용액이 1 ml/hr의속도로지속정주되고, 10분의잠금시간을두고환자가단추를누르면 3 mg의약이추가주입되도록하였다. 수술이끝나면근이완의회복을위하여 glycopyrrolate 0.4 mg와 neostigmine 1.5 mg를투여하고발관하였다. 환자가회복실에도착후, 45분간의구역정도를 VAS (visual analog scale, 100 mm) 척도를사용하여측정하고, 구토여부를관찰하였다. 구역과구토의구분은구토의동작이없으면구역, 있으면구토로간주하였다. PONV를호소하는환자에게는 ondansetron 4 mg을정주하였으며, 약물투여 15분후환자의구역정도를 VAS를사용하여다시측정하였다. 추가통증조절이필요한경우 ketorolac 30 mg을다시정주하였다. 생체징후가안정되고의식이충분히회복된것을확인한후환자를병실로이동시켰다. 연구에참여한모든환자에서유전자형을얻기위하여 EDTA 튜브에 4 cc의혈액을담아서 DNA를추출한후실시간중합효소연쇄반응 (real-time polymerase chain reaction, realtime PCR) 방법으로유전자형을분석하였다. 분석은 SYBR Green TM supermix (Bio-Rad Laborateries Inc, USA) 와대립유전자특이적인시동체 (allele-specific primer) 를이용하여 ABI 7500 Real Time PCR System (Applied Biosystems, USA) 으로 PCR을시행하였다. 사용된시동체는 Table 1과같다. 다형성부위옆의뉴클레오티드 (nucleotide) 는 wild type, variant type 대립유전자의중합효소연쇄반응 (polymerase chain reaction, PCR) 환경을분화시켜구분지었다. 대립유전자특이적실시간중합효소연쇄반응 (allele-specific real-time PCR) 은 95 o C 에서 10초간, 60 o C에서 60초간 45주기를시행하였다. PCR을위한모든시동체는 Bioneer (Daegeon, Republic of Korea) 에서구입하였다. PCR 용액은총 25μl이며, 조성은 DNA 1.5μl, SYBR Green Supermix TM 12.5μl, former primer 0.2μl (200 nm), reverse primer 0.2μl (200 nm) 와증류수 10.6μl이다. 10 nm DNA를 wild type 시동체또는 varient type 시동체를포함한 PCR 시약과각각증폭시켜, wild type 시동체를첨가한시약에서만증폭이되면 wild type, variant type 시동체를첨가한시약에서만증폭이되면동형접합체 (homozygote type), 양쪽모두에서증폭이되면이형접합체 (heterozygote type) 라고판단하였다. 모든결과는평균 ± 표준편차와백분율 (%) 로표시하였으며각유전자형간의환자의나이, 몸무게, 수술시간, remifentanil 사용량, 구역의 VAS값은 one way ANOVA를이용하여비교하였다. 수술후오심및구토발생률은카이제곱검정으로분석하였으며, P < 0.05인경우통계적으로유의하다고판단하였다. 통계처리는 SPSS 15.0 for Windows (SPSS Inc, USA) 를사용하였다. 결과대상환자의 Pro16Ser 유전자형별역학적자료는 Table 2 와같으며각유전자형간의통계적유의한차이는없었다. HT 3A 수용체의 Pro16Ser 유전적다형성빈도는 wild type이 82.8%, 이형접합체 (heterozygote) 가 16.7%, 동형접합체 (homozygote) 가 0.5% 이었다. 5-HT 3A 수용체유전자형에따른수술후 1일까지 PONV 발생은 wild type 85명 (50.3%), 이형접합체 18명 (52.9%) 이었으며, 구토발생은 wild type 8명 (4.7%),

There is no significant difference between genotypes. 이형접합체 1명 (2.9%) 으로통계적인차이는없었다 (Table 3). 수술후오심의 100 mm VAS 값은 wild type이이형접합체보다작아보였으나통계적으로차이는없었다. Ondansetron 투여후오심의 VAS 값역시 wild가이형접합체보다더작아보였으나 (wild type 13.9 ± 24.9, 이형접합체 20.0 ± 30.8) 의미있는차이는없었고, 감소된 VAS 값도두군에서비슷하였다 (p = 0.24) (Table 4). 환자의나이, 몸무게, remifentanil 사용량에따른오심의 VAS 값은차이가없었지만, 수술시간이길수록환자들에게오심이심하게발생한것으로나타났다 (P = 0.02). 고 본연구에서는한국인에서 HT 3A 수용체의 Pro16Ser 과오돌연변이 (missense mutation) 를이용하여 5-HT 3A 수용체유전자형의빈도를파악하고유전자형의변이가수술후구역및구토발생빈도와 ondansetron의효과에미치는영향을알아보고자하였다. 그결과대상환자의 17% 에서 Pro16Ser 염기변이가있었다. 유전적다형성에따른 PONV의빈도및정도는유의한차이가없었으며, ondansetron 투여후반응또한차이가없었다. 전신마취후 PONV의발생률은 20 40% 로알려져있으나, 고위험군환자에서는 80% 까지도증가하는것으로알려져있다. 2,3) PONV에영향을미치는요인으로는환자의특성, 수술의종류, 마취적위험인자, 수술후관련인자등이복합적으로작용한다. 환자의요인으로는여성, 소아, 편두통의과거력이있는환자, PONV나멀미의과거력이있던 찰 Table 4. There is no statistical difference between genotypes. 환자, 비흡연자등에서 PONV 발생위험이높다고알려져있다. 마취적위험인자로는마취전투약, 흡입마취제의종류및아산화질소의사용, 아편유사제의사용등이관련되며, 부인과수술, 안과의사시수술이나, 이비인후과수술, 복강경및복강수술, 유방수술, 갑상선수술등에서 PONV 발생빈도가높다고하였다. 수술후요인으로는통증조절을위한아편유사제의사용, 빠른보행, 저혈압, 음식물의섭취등이관련이있다 ) 이번연구에서는부인과복강경수술을받는여성환자를대상으로하였으며, 수술중아산화질소와아편유사제인 remifentanil을사용하였다. 또한예방적항구토제사용이유전자형에따른 PONV의발생에영향을미칠수있기때문에사용하지않았다. 그결과대상환자의 50% 정도에서 PONV가나타났고, 통상적인전신마취후 PONV의발생률보다높았는데이는연구대상환자가 PONV의고위험군환자였기때문이라판단된다. 한편 PONV 는연령에따라발생률이달라서, 소아에서 PONV의발생률이 2배높고성인의경우 55세미만의연령에서더높은 PONV의발생률을보인다고하였다. 23) 그러나본연구에서유전자형에따른연령의통계적차이는없었기때문에연령에의한 PONV 발생차이는배제할수있었다. 수술시간이길어질수록수술후오심의 VAS 값이더큰것으로나타난것은, 수술시간에따라사용된마취제의양이늘어나고복부조작의기회가증가하기때문이라판단된다. 수술중마약사용량이많을수록 PONV가심해질수있다고알려져있으나, 23) 본연구에서사용한 remifentanil은아편유사제중가장최근에상용화된약제로, 약효의발현시간이약 1.5분내외로빠르고다른아편유사제가간에서의대사를거치는것과달리혈중의비특이적인에스터라제에의해대사되기때문에, 중증의간질환, 신질환및노인환자의경우에도일반환자와마찬가지로 5분이내의반감기를가짐으로써약제의체내축적에따른부작용이적은것으로알려져있다. 24) 또한 morphine이나 hydromorphone 등에비해 fentanyl 같은아편양제제는인간 5-HT 3A 수용체에특별한작용을나타내지않는경향이있다고하였다. 25,26) 따라서이 0

4 258 Anesth Pain Med Vol. 3, No. 4, 2008 번연구에서수술중 remifentanil의사용량과 PONV에는큰영향이없을것으로사료되지만이에따른연구가더필요할것이다. 5-HT 3 수용체는위장관운동, 내장성통증, 오심및구토등다양한증상에생리적, 병리적으로직, 간접적으로관여한다. 27) 5-HT 3 수용체는중추에서최후야 (area postrema) 의화학수용체방아쇠영역 (chemoreceptor trigger zone:ctz) 에위치하고말초적으로는미주신경말단에분포한다. 28) 5-HT 3 수용체길항제는항암치료시유발된구역및구토 (chemotherapy-induced nausea and vomiting:cinv) 치료에효과가있는것으로처음기술되었고, 29) 이러한항구역효과는방사선치료로유발되었거나, 30) 수술후유발된구역및구토치료에이용되었다. 31) 또한최근에는 5-HT 3 수용체가과민성대장증후군이나 32) 다양한정신과질환의치료에잠재적인목표로서주목받고있다. 32) 5-HT 3 수용체는 5개로구성된아형에따라 5-HT 3A-E 까지다섯종류가존재하며, 항구역구토작용은주로 5-HT 3A, 5-HT 3B 수용체에의해나타난다 ) 5-HT 3A 수용체의경우, 자연적으로 5가지의개체간단일염기변이 (Single Nucleotide Polymorphism, SNP) 가발견되었으며이것은사람에서 5-HT 3A 유전자의아미노산변화를초래한다고하였다. 이중 3가지의 SNP (A33T, M257I, R344H) 에서는 wild type의 5-HT 3A 나 5-HT 3B 수용체가발현되어도, 정상적인 5-HT 3A 의기능을복원시키지못했다고한다. 33) Kaiser 등은 17) 242명의항암치료를받는환자에게 ondansetron을투여하고, 5-HT 3A 수용체유전자의다형성과이에따른 CINV의연관성에관해연구하였다. 이들은총 21가지의 HTR 3A 유전적다형성을발견하였는데그중두가지는아미노산치환 (Ala33Thr, Met257Ile) 이었으며, HTR 3A 의유전적다형성과 CINV는연관이없다고하였다. 본연구는이들이시행하지않았던 Pro16Ser 유전자형과 PONV와의연관성을조사하였다. Pro16Ser 변이는 HTR3A의 178번째염기 C가 T로치환되는변이로서, 그결과 proline이 serine 바뀌는아미노산치환이일어나고, 이것이 HTR 3A 의성질변화를일으켜수용체기능에변화를줄수있을것이라기대되었다. Pro16Ser 과오돌연변이를이용한기존의연구결과를보면이부위유전자의차이에의해 5-HT 3A 수용체가기능적으로다양해지고, 양극성장애 (bipolar disorder) 의감수성에도영향을미칠수있다고하였다. 34) 그러나선택적세로토닌재흡수저해제 (Selective serotonin reuptake inhibitors) 인 fluvoxamine 사용시나타나는위장관부작용과, Pro16Ser 변이와는관련이없었다. 35) 본연구에서도 Pro16Ser 변이에의한 HTR3A 유전자형과수술후구역및구토의발생률은차이가없었음을알수있었다. 5-HT 3A 수용체유전자의 Pro16Ser 과오돌연변이는중추신경계에존재하는 5-HT 3A 수용체의기능과관련이있을수있지만, 위장관에분포한 5-HT 3A 수용체와는큰관련이없을것이라고여겨진다. 결론적으로한국인에서 5-HT 3A 수용체의 Pro16Ser 유전적다형성은수술후오심, 구토의발생과 ondansetron 투여에따른치료효과와의미있는연관성이없다. 참고문헌 1.
