Anaplastic large cell lymphoma (ALCL) of the stomach is very rare and regarded to have a poor prognosis. A 66-year-old man complained of dyspepsia for 3 days. A esophagogastroduodenoscopic examination revealed a flat, depressed ulcer at the distal antrum. Biopsy of the lesion showed diffuse infiltration of large pleomorphic lymphoid cells that were positive for CD3 and CD30 and negative for cytokeratin and anaplastic lymphoma kinase (ALK). We diagnosed the patient with ALK-negative primary gastric ALCL with multiple lymphadenopathies, and he received systemic chemotherapy. CHOP-like regimens followed by consolidation therapy have been widely used for ALK-negative ALCL. However, the patient maintained complete remission for 36 months with only induction chemotherapy. This case suggests that heavy treatment for ALK-negative ALCL is not required for all patients. (Korean J Med 2012;83: ) Keywords: Lymphoma, large-cell, anaplastic; Anaplastic lymphoma kinase 서론위의원발성악성림프종은전체위악성종양의 1-7% 를차지하며대부분 B세포림프종이다. 역형성큰세포림프종 (anaplastic large cell lymphoma, ALCL) 은비호지킨성림프종의 2-8% 를차지할정도로매우드물다. ALK negative ALCL - prognostic index (IPI) 4점의고위험군의환자에서 CHOP (cyclophosphamide, doxorubicin, vincristine, prednisolone) 항암화학요법으로완전관해를유지하고있는 1예를경험하였다. 증례환자 : 남자, 66세주소 : 소화불량현병력 : 고혈압이외특이병력없었던환자로, 3일간의소화불량으로개인의원에서시행한내시경상위의날문방 (antrum) 에협착을동반한덩어리가관찰되어본원으로전원되었다. 야간발한과발열및체중감소소견은보이지않았다. 가족력 : 특이소견없음. 이학적소견 : 입원당시혈압은 130/80 mmhg, 체온 36.5, 맥박 75회 / 분, 호흡 20회 / 분이었다. 전신상태는양호하였으며의식은명료하였다. 복부진찰상압통및반사압통은없었으며약간팽만된상태였다. 검사소견 : 말초혈액검사상백혈구 6,310/µL, 혈색소 15.1 g/dl, 적혈구용적률 45.1%, 혈소판 185,000/µL, 적혈구침강속도 15 mm/hr였다. 혈청생화학검사에서는총단백 6.5 g/dl, 알부민 3.9 g/dl, AST/ALT 22/20 IU/L, LDH 599 U/L, Ca/P 8.9/4.8 mg/dl, AFP 2.31 ng/ml, CEA 1.77 ng/ml, CA U/mL, HBs Ag 양성, HBs Ab 음성이었고, 대변검사및소변검사에서는이상소견보이지않았다. 방사선소견 : 복부전산화단층촬영상위의날문방앞부위에원형의위벽비후소견이관찰되었고, 다수의위주위임파선이다양한크기로관찰되었다 (Fig. 1). PET-CT에서좌측경부임파선, 양측폐문부임파선, 종격동임파선, 작은그물막임파선, 위주위임파선에서 fluorodeoxyglucose (FDG) 섭취증가소견을보였으며, 흉골, 우측갈비뼈, 좌측흉막, 좌측장골에서도 FDG 섭취증가소견을보였다. 내시경소견 : 상부소화관내시경에서위의원위부날문방의대만부에서비교적경계가분명한함몰궤양이관찰되었으며 (Fig. 2), Borrmann 2형의형태로, 날문부협착소견이관찰되었다. 상부내시경초음파에서병변은위의전층을침윤하며, 저에코소견을보였으며일부장막층의침윤소견및대동맥임파선도관찰되었다. 병리조직소견 : 위병변에서시행한조직검사상크기가다양한다형성의종양세포가관찰되었으며, 유사분열이많이관찰되었다 (Fig. 3). CD3와 CD30, leukocyte common antigen (LCA), epithelial membrane antigen (EMA) 양성소견보였으며, CD15, CD20, cytokeratin, anaplastic lymphoma kinase (ALK), EBV IHC 음성소견을보였다 (Figs. 4 and 5). 골수천자및생검소견 : 골수천자검사상림프종의침윤은관찰되지않았고, 골수생검상세포충실도는 30-40% 이었다. 치료및경과 : 역형성큰세포림프종진단후 CHOP (cyclophosphamide, doxorubicin, vincristine, prednisolone) 복합항암 Figure 1. CD30 (ALK stain, 200, A; CD30, 200, B). 화학요법을시행하였고, 소화불량증세와날문부협착소견및다발부위의임파선비대및뼈전이소견이사라지고, 특별한재발소견없이 36개월째외래추적관찰중이다.

ALCL 은비호지킨성림프종의 2-8% 를차 지하며특히위에서원발한경우는매우드물다. ALCL은 1985년 Stein 등이처음으로보고하였고역형성세포모양과응집되어자라고림프동굴을침범하는경향, CD30 이발현됨을기초로하였다 [3].

ALCL 은비호지킨성림프종의 2-8% 를차 지하며특히위에서원발한경우는매우드물다. ALCL은 1985년 Stein 등이처음으로보고하였고역형성세포모양과응집되어자라고림프동굴을침범하는경향, CD30 이발현됨을기초로하였다 [3]. 면역표현형및세포유전학적, 임상적정보가추가되면서 ALCL은 systemic ALCL 과 primary cutaneous ALCL 로구분하고, systemic ALCL 은 ALK 양성과 ALK 음성으로구분하고있다 [2]. Kiel의분류에따르면 ALCL은 T, B, null 세포성이포함되어있으나, WHO 분류에의하면 T 세포성과 null 세포성만을 ALCL로보고 B 세포성은미만성거대 B 세포림프종의 anaplastic large cell

4 - 김형석외 6 인. ALK 음성역형성큰세포림프종 - type으로구별하고있다. ALCL의종양세포는세포크기가크고다형성이면서다발성의핵을가진다. 그래서육종, 미분화암, 조직구종양등으로오인되기도한다. CD30 은종양괴사인자수용체가족의경막사이토카인수용체이다. 이는 ALCL 외에도호지킨성림프종에서발현되며다른종양에서도관찰될수있다. 따라서 ALCL로진단하기위해서 CD30의발현과함께다른면역조직화학적검사가필요하며경우에따라 CD3, CD20, EMA, cytokeratin, CEA, vimentin, ALK 등이사용된다. EMA 의경우 ALCL의다수에서양성으로나타나므로상피세포암종과의감별에는유용하지못하며 cytokeratin이감별에도움이될수있다 [4]. 본환자는 CD3, CD30, EMA 가양성이었고 CD20, cytokeratin, ALK 는음성이었다. ALK 단백질은 systemic ALCL의 50-85% 에서발현된다. ALK 양성 ALCL의 70-80% 는전형적인염색체이상인 t (2;5) (p23;q35) 를보이는데, 2번염색체에위치한 ALK 유전자와 5번염색체에위치한 nucleophosmin (NPM) 유전자가융합하여 80-kDa의 NPM-ALK 융합단백질을코딩하게되며이는 tyrosine kinase 활성을가진다. NPM-ALK 융합유전자는기본적으로종양유전자 (oncogene) 로작용하는것으로알려져있다 [2,4]. ALK 음성 ALCL에서는전형적인염색체이상이아직규명되지않은상태이며이림프종의병인을파악하기위해분자유전학적접근을하고있다. Lamant 등 [5] 은 ALK 양성과 ALK 음성 ALCL이다른 gene expression profile을가짐을보여주어이두질환이다른질환군임을확인하였다. Thompson 등 [6] 에의한다른연구에서는 ALK 양성과 ALK 음성 ALCL 두군모두에서높게발현되는유전자를확인하였고두군이공통적인병리유전학적기전을가질가능성을제안하였다. 임상양상과예후면에서도 ALK 음성 ALCL은 ALK 양성 ALCL과다른것으로알려져있다. ALK 음성 ALCL 은 대의고령환자에서보고되며 B 증상을동반한진행된병기로흔히진단된다. 림프절외침범을보이며골수침범은약 30% 에서관찰된다. ALK 음성의경우양성에비해치료성적이나쁜데, anthracycline을포함한항암화학요법을시행하였을때 ALK 양성 ALCL의약 90% 에서완전관해를획득하였지만 ALK 음성 ALCL은 30-50% 만이완전관해를획득하였다 [1]. International peripheral T-cell lymphoma project 의보고에의하면 5년무병생존율 60% 와 36% (p = 0.015), 5년생존율 70% 와 49% (p = 0.016) 로예후에서명백한차이를보여주었다 [2]. 원발성위림프종의대부분은 B 세포기원으로, Iwamizu- Watanabe 등은 369예의원발성위림프종중에 14예 (3.8%) 가 non-b 세포성비호지킨성림프종이었고, ALCL 은 3예 (0.8%) 로굉장히드물게발견됨을보고하였다 [7]. 따라서위에서원발한 ALCL의예후를명확히추정하기는어려우나, Nakamura 등이 updated Kiel 분류로 B 세포성림프종의 anaplastic large cell type 환자를포함하여시행한생존분석에서 1년, 3년, 5년생존율이각각 58%, 22%, 13% 를보여림프절이나피부에서기원한 ALCL에비해예후가나쁠것으로예상된다 [8]. 또다른예후인자로 Gallamini 등이제시한 ALCL을포함한달리분류되지않은말초 T 세포림프종 (peripheral T cell lymphoma unspecified) 의예후인자 (PIT, Prognostic Index for PTCL-U) 를살펴보면위환자는 4점중 2점으로 group 3에해당하는데이는 5년생존율 32.9% 로나쁜예후를보인다 [9]. 위에서발생한 ALCL의표준치료는아직명확히규명되지않았으며여러증례보고에따르면국소적병변인경우외과적절제를시행하고일부에서는추가적으로항암화학요법또는방사선치료를시행하였다. 진행된병기인경우는 anthracycline을기초로한항암화학요법이시행되었다 [8]. 예후가불량할것으로예상되는 T 세포림프종에서 CHOP 이후공고요법으로 GDP (gemcitabine, dexamethasone, cisplatin), GemOX (gemcitabine, oxaliplatin), ICE (ifosfamide, carboplatin, etoposide) 등이사용되기도한다. 자가조혈모세포이식이 ALCL의초치료혹은재발시의치료에이용되고있으며 ALK 음성환자에서도이식후지속적으로관해를유지하였다는보고가있어고위험군에속하는 ALCL 환자에서는적극적으로자가조혈모세포이식또는동종조혈모세포이식을고려해볼수있겠다 [10]. 본증례의경우예후가불량할것으로예상되는고령에서진행된병기로진단된 ALK 음성원발성위 ALCL 환자에서 CHOP 항암화학요법단독시행후에도 36개월간완전관해를유지하고있다. 본증례와같이 T 세포림프종의아형인 ALK 음성 ALCL에서는전체 T 세포림프종에서사용하는예후인자인 PIT가높아나쁜예후가예상될지라도, 활동도 (performance status) 가좋고, 그외이식의대상 (65세이하 ) 은아니지만비교적젊은나이인환자에서반드시예후가불

5 - The Korean Journal of Medicine: Vol. 83, No. 5, 량한그룹에맞추어치료를할필요가없을것으로생각된다. 이를검증하기위해추후 T 세포림프종, 특히 ALK 음성 ALCL 그룹에서사용가능한새로운예후인자의발견이필요하다. 요 원발성림프절외림프종은림프절이아닌조직이나기관에서유래한림프종으로위장관에서가장호발한다. 역형성큰세포림프종은비호지킨성림프종의아형으로여러면역조직화학적검사가진단과예후의판정에이용되며이중 ALK 단백질은 50-80% 에서발현되는것으로양성인경우에비해음성인경우가예후가나쁜경향을보인다. 저자들은고령이며진행된병기로진단된 ALK 음성원발성위역형성큰세포림프종환자에서복합항암화학요법으로 36개월간완전관해를유지하고있는 1예를경험하였기에문헌고찰과함께보고하는바이다. 중심단어 : 역형성큰세포림프종 ; Anaplastic lymphoma kinase 약 REFERENCES 1.
