The purpose of this study was to investigate clinical presentations of in children with acute respiratory infections. Methods: We examined the medical records of pediatric patients (age < 18 years) admitted with acute respiratory infections of C. pneumoniae to Gachon University Gil Medical Center between March 1, 2011 and August 31, We compared the clinical features of infection with that of Mycoplasma pneumoniae infection. Results: We confirmed acute respiratory infections of in 110 patients out of 2,156 patients (5.1%) admitted with acute respiratory infections. The mean age was 37.2 ± 30.1 months. More than half of them (54.5%) had coinfection. infection had mild and subacute courses. The mean duration of symptoms prior to admission was 8.5± 13.8 days. There were remarkable seasonal variations and prevalence was higher in December and April (P= 0.03 and P= 0.02, respectively). Although rhinorrhea and pharyngeal injection were more common in infection (P< 0.05), clinical signs and symptoms were similar between and. Extrapulmonary manifestations such as skin lesion, Gastrointestinal symptoms, hepatitis, and neurologic symptoms were common (41.0%) in infection and, had similar incidence in infection. Conclusion: is an important infectious agent of acute respiratory infections in children. Clinical pictures of are similar to, even in extrapulmonary manifestations. should be taken into consideration in differential diagnosis of acute respiratory infection in children. 2 성재진외 소아환자에서 Chlamydia pneumoniae 의임상양상 로존재하며지역적으로꾸준히발병, 유행하여때로는대규모로발병하기도한다. 2) 계절적변동은겨울직전이나직후에주로발병하고, 6,7) 가을에드물다고알려져있으나 8) 계절적차이가없다는보고도있다. 9) 폐렴의진료에원인균을아는것이중요한데, 폐렴의흔한원인인폐구균 (Streptococcus pneumoniae), 비정형폐렴의원인인 M. pneumonia 등다양한바이러스가원인이되며, 이러한다양한원인균은계절적특성이나임상양상, 검사소견에차이가있고, 이소견들은진단과치료에도움이될수있다. 10) 는호흡기감염의중요성에비하여비교적최근에분류되었기때문에이에대한연구가많지않고, 특히소아환자의임상에관련한국내자료는부족한실정이다. 저자는급성호흡기감염으로입원한환아중에 로진단되었던환아의연령및성별, 계절적변동의역학적특성을조사하였고, 비정형폐렴의대표적인원인균으로알려진 와비교를통해임상증상과이학적소견, 검사실소견, 임상경과를분석하였다. 또한 도다형홍반같은피부질환이나, 11) 뇌염같은신경학적질환을동반하는경우가있는데, 12) 이와같은폐외증상의종류와빈도차이를 와비교하여알아보고자하였다. 대상및방법 1. 연구대상본연구는 2011년 3월부터 2014년 8월까지가천대학교의과대학길병원소아청소년과에급성호흡기감염 ( 폐렴, 기관지염, 모세기관지염, 인두염 ) 으로입원한 18세이하의 감염으로확인된환아를후향적으로조사하였다. 대상환아중에병원내감염으로확인된경우, 호흡기증상이없었던경우, 자의퇴원하여경과를확인할수없었던경우는제외하였다. 의무기록을통해성별, 연령, 임상증상및이학적소견, 검사실소견, 영상의학적소견, 입원경과를추출하였다. 의임상양상을분석하는데동반감염이영향을주는것을배제하기위해서, 호흡기바이러스감염이있는경우는제외하여 단독양성군을 단독양성군과비교하였다. 의계절적차이는개월별검사에서양성인결과를추출하여각월별의양성률을얻고, 다른개월의양성률과차이를비교하였다. 폐외증상은구토나설사가있었던경우는장관계이상으로분류하였고, 간기능검사에서혈청아미노전이효소 (aspartate aminotransferase/alanine aminotransferase, AST/ALT) 증가가있었던경우는간염으로, 발진이동반되었던경우는피부질환으로, 수막자극증상이나경련이동반되었던경우를신경학적이상으로분류하여연구하였다. 2. 연구방법 1) 검사실진단대상환아의입원당일에채취한혈액에서효소면역분석법 (enzyme-linked immuneosorbent assay, ELISA) 을이용해항클라미디아면역글로불린 M항체 (antichlamydia pneumoniae immunoglobulin M antibody) 를검출하여양성인경우를 감염으로진단하였다. 기준은 negative, <9.00 index; equivocal, index; positive, >11.00 index 로본연구에서는 positive 결과만양성으로하였다. 효소면역분석법은 ELI- SA (Vircell, Granada, Spain) 를이용하였다. 감염은입원당일에채취한혈액에서화학발광면역분석법을이용해항마이코플라즈마면역글로불린 M항체 (antimycoplasma IgM antibody) 를검출하여양성인경우로진단하였다. 기준은 negative, <10.00 index; positive, index로면역분석법은 chemiluminescent immunoassay (Sanofi diagnostic pasteur, Marnes la coquette, France) 를이용하였다. 코인두면봉법 (nasopharyngeal swab) 을통해바이러스검체를수집하여다중역전사중합효소연쇄반응 (multiplex reverse transcription polymerase chain reaction) 을이용하여호흡기바이러스감염을확인하였으며다중역전사중합효소연쇄반응은 Seeplex RV12 ACE Detection Kit (Seegen, Seoul, Korea) 를이용하였고, A 형, B형 influenza virus, A형, B형 respiratory syncytial virus, 1,2,3 형 parainfluenza virus, adenovirus, metapneumovirus, bocavirus 의 10가지호흡기바이러스를확인할수있었다. 2) 통계분석통계분석은 MedCalc (MedCalc, Ostend, Belgium) 을사용하였다. 각집단의평균값의비교는 Student t-test를이용하였고각값은평균값 ± 표준편차로표시하였다. 표준편차가큰평균값의비교는 Mann-Whitney test를이용하였다. 각군간의빈도는 chi-square test로비교하였고, 각대상수가 5미만일경우에는 Fisher exact test를시행하였다. 통계적유의수준은 P<0.05로하였다. 결과 1. 대상환아의특성및계절적변동연구기간동안에전체 2,156명을대상으로검사가시행되었고, IgM 양성인환아는 113명이었으며, 병원내감염 2 명, 호흡기증상을동반하지않은환아 1명을제외하여 110명이대상이되어, 검출률은 5.1% 였다. 평균연령은 37.2±30.1개월이었고, 성별은남아가 51명으로 46.4% 였으며, 여아는 59명으로 53.6% 였다. 양성인경우에전체동반감염은 60명으로 54.5% 였고, 바이러스와동반된감염은검사를시행한 95명중 32명으로 347

3 Sung JJ, et al Clinical presentations of Chlamydia pneumoniae in children 29.1% 였다. 각각의동반감염률을보면 와동반감염된경우가가장많아서 28명 (25.5%) 이었고, 다음으로 adenovirus 13명 (11.8%), metapneumovirus 7명 (6.4%), bocavirus 6명 (5.5%), respiratory syncytial virus 5명 (4.5%), influenza virus 4명 (3.6%) 순이었다. 임상양상을조사하기위해 단독감염군 50 명과다른바이러스감염이없는 IgM 양성인 94명과비교하였는데, 단독감염군의평균연령은 24개월 ( 개월 ) 로, 단독감염군의 50 개월 ( 개월 ) 에비해어렸지만 (P<0.01), 성별과비만, 천식, 미숙아, 선천성심장질환, 알레르기비염등기저질환의비율은통계적으로유의하지않았다 (Table 1). 단독감염군의임상적진단은폐렴 41명 (82.0%), 기관지염 2명 (4.0%), 모세기관지염 3명 (6.0%), 인두염 4명 (8.0%) 이었고, 단독감염군의임상적진단은폐렴 90명 (95.7%), 기관지염 1명 (1.1%), 모세기관지염 2명 (2.1%), 인두염 1명 (1.1%) 이었다. 감염의계절적변동을조사하였을때에 4월에 7.9%, 12월에 10.0% 로 에의한호흡기감염의비율이다소높았고, 2월에 0%, 3월에 1.2% 로호흡기감염의비율이다소낮았다 (Fig. 1). 2. 임상소견임상양상은 양성군을 양성군과비교하였을때에 양성군의주요임상증상은기침, 발열, 콧물순서였고, 양성군은기침, 발열, 가래순서였다 (Table 2). 임상증상으로발열, 기침, 가래, 호흡곤란, 빈호흡, 구토, 설사등의빈도는각군간에의미있는차이가없었고, 이학적소견의수포음과천명음도통계적으로유의하지않았다. 다 Table 1. 4 성재진외 소아환자에서 Chlamydia pneumoniae 의임상양상 만 단독감염군에서상기도감염의증상인콧물과 인후발적의빈도는 양성군에비하여유의하게높 았다 (P<0.05). 3. 임상경과및중증도 증상발현후에입원까지기간은 양성군에서 4 일 ( 일 ) 로 양성군의 7 일 ( 일 ) 에비해 짧았고, 입원기간은두군에서유의한차이는없었다 (Table 3). 발 열이있는경우총발열기간은 양성군에서 M. pneumoniae 양성군보다하루정도짧은것으로나타났고 (P = 0.019), 입원후발열기간도 양성군에서짧은것으 로나타났다 (P = 0.007). 흉부방사선검사에서폐렴이있었던경우 는 양성군은 34 명 (68.0%) 으로, 양 성군의 77 명 (81.9%) 에비하여적었지만통계적으로유의하지않았 고 (P = 0.092) 기관지폐렴, 대엽성폐렴, 부폐렴삼출액등세부적 으로도차이는없었다. 중증도를확인하기위해, 중환자실에입실하여기관삽관을받 은환아를평가하였을때 양성군에서 1 명 (2.0%) 의 중환자실입원이있어기관삽관을통한기계환기를받았으며, 그 환아는출생주수 24 주, 670 gm 미숙아로, 출생당시계면활성제투 여및 56 일간의기계환기의치료를받았고, 기관지폐이형성증이있 는환아였으며치료후에호전되었다. 양성환아중 에사망한환자는없었다. 4. 검사실소견 양성군과 양성군에서총백혈 구수및림프구, 중성구비율, 염증지표를확인하였는데, 양성군에서말초혈액백혈구수는 12,500/mm³, 림프구비 율은 39.1% 로 양성군에비해높았고, 중성구비율 은 48.2% 로낮았다 (P < 0.05). 혈소판수치와 C- 반응성단백 (C-reactive protein, CRP) 은유의한관계가없었다 (P = 0.57, P = 0.50) (Table 4). WBC, white blood cell; CRP, C-reactive protein. 5. 폐외증상 양성군에서폐외증상은 21 명 (42.0%) 에서동반 되었고, 양성군과비교하여차이가없었다 (Table 5). 세부적으로간기능검사에서혈청아미노전이효소 (AST/ALT) 증가가 22.0%, 피부병변이 16.0% 로 양성군에비하 여더많았으나통계적으로유의한차이는없었다. 그외의장관계 이상이나신경학적이상도유의한차이는없었다. 고찰 는 1989 년 Campbell 등 1) 에의해새로운종으로 밝혀졌으며구성은감염성기본소체 (infective elementary bodies) 와생식성망상체 (reproductive reticulate bodies) 로되어있다. 13) 사 람사이에서만감염이되며감염경로는명확하지않지만, 호흡기 에서분무된액적 (droplet) 상태에서생존하여전파될수있는것으 로알려져있다. 2) 혈청학적연구에서미국을비롯한여러나라에서 성인의반이상에서 특이 IgG 를가지며, 대개 5 세 에서 15 세사이에급격히항체를형성하여이시기에감염이처음 발생하고그이후로재감염이흔한것으로보고되고있다. 13) 항체 양성률이나유병률은지역적인차이를보여개발도상국에서더 높은양성률을보이며일본을비롯한아시아가풍토적인지역으로 더어린연령에서감염이발생하는것으로알려져있으며, 집단생 활을하는곳에서유행으로발생하기도한다. 2,14) Lee 등 15) 의연구에 따르면성인에서대조군과호흡기증상이있는환자들간에 C. pneumoniae IgM 항체양성률은유의한차이가없었다. 본연구에서 는 와비교하여임 상증상과이학적소견이유사하였으며, 발열기간이짧았다. C. pneumoniae 는전형적폐렴균인폐구균에비하여발열의정도가 낮고호흡곤란, 저혈압의빈도가적으며, 혈액검사에서총백혈구 수및 C- 반응성단백등도낮은것으로알려져있다. 16,17) 비정형폐 렴균인 에비해서발열의정도가낮다는보고가 Table 5. 5 Sung JJ, et al Clinical presentations of Chlamydia pneumoniae in children 있으나 14) 임상증상과이학적소견이유사하여 자체의고유한특성은없는것으로생각된다. 9,16) 다만입원까지증상기간이 1주이상으로천천히진행하고경과가경한것으로알려져있다. 18) 본연구는폐구균에서입원까지증상기간이 4일이라는보고 16) 와비슷하며, 보다짧았고사망이나중환자실집중치료가거의없는경한경과를나타내었다. 그러나경과도집단이나지역에따라달라서겸상적혈구성빈혈이있는경우에심한폐렴으로이행할수있고, 19) 태국에서입원환자의 16.7% 가심한폐렴소견을보이며 20.8% 의늑막삼출을보였다는보고도있다. 20) 또한노령환자에서심한폐렴을보이는경우가많은데이들은환자의기저질환에연관되는것으로추정된다. 21) 의계절적변동은겨울이시작하기전이나겨울직후에호발하는것으로알려져있는데독일에서유치원과초등학교소아대상의연구 6) 에서겨울의시작인 12월과직후인 4월에서발생률이가장높았고, 입원환아대상의연구 7) 는 11월에발생률이높았다. 하지만 의발생은계절적차이가없다는보고도있다. 9) 본연구도겨울의시작인 12월과겨울직후인 4월의발생률이높았다. 는다형홍반같은피부질환이나뇌염같은신경학적이상을동반한다는보고가있고, 11,12) 핀란드에서성인을대상으로한연구에서신경학적이상으로두통이 54.2% 에서발생하여폐구균에비하여흔하였고, 17) 일본의연구에서혈청아미노산전이효소의수치의상승을보이는간염이폐구균에비하여증가되고, 와비슷한정도로발생하였다. 16) 본연구도 M. pneumoniae와비슷한정도로폐외증상이흔하게동반됨을확인할수있었다. 본연구는후향적으로이루어졌기때문에정확한임상증상과이학적소견특히, 폐외증상의수집에제한이있을수있으며, 입원한환자만연구에포함하여외래에서치료가능한경한 감염에의한결과를연구에반영할수없었다. 는배양검사에서일반배지에자랄수없고, 세포주를감염시켜배양하는데실제임상의사용은제한적이어서진단은주로혈청검사가사용된다. 2) 본연구의진단은효소면역분석법을사용하였는데, microimmunofluorescence법에비하여취급이쉬워많은환자의결과를판독할수있고, 주관적판단이배제되는장점이있으나감염후특이 IgM이나타나는데최소 1주이상걸리기때문에음성일가능성이있고, 지속기간도보통수주이상으로길기때문에초기검사에서양성이라하더라도급성감염이아니라최근감염일가능성이있어이에대한한계점이있다. 초감염인경우에특이 IgM의증가이후특이 IgG가증가하며진단은특이 IgM 양성이나특이 IgG 항체의 4배상승으로하게되는데재감염인경우특이 IgG 와특이 IgA가증가하지만, 특이 IgM이양전되지않아특이 IgM 양성으로초감염만진단할수있어본연구는실제발생률보 다과소진단이이루어졌다. 감염의진단은특이 IgM 뿐만아니라특이 IgG 도고려해야하지만, 현실적으로 3 주이 상의간격을두고특이 IgG 를반복측정하는데실질적인어려움이 있어특이 IgM 만검사를시행하였다. Choi 등 22) 에의하면우리나 라소아의항체양성률이 2 5 세에 11%, 6 10 세에 22%, 세에 44% 로, 외국의항체양성률과비슷하여, 우리나라소아에서도이 에의한감염증이발생하고있을것으로추측되지만, 본연구에서 는이전의연구들과는달리 감염의초감염의진단 만이루어져서 감염에비해발생연령이더낮고 발생률이낮은결과를보인것으로생각된다. 그러한과소진단에 도불구하고이전연구들과비슷하게급성호흡기감염에중요한 원인으로생각해볼수있었다. 는천천히진행하며비교적경한경과를보이며 임상증상이나이학적소견이 와유사하며특히, 폐외증상의동반이흔하였다. 급성호흡기감염의주요원인으로 빈도가흔하였으며어린연령에도빈발하였다. 는 호흡기감염에중요한역할을하므로, 임상양상과경과를아는것 이중요하지만그에비하여충분한주의를받지못하고있다. 특히 페니실린, 베타락탐의기존항생제에반응하지않고 23) 어린연령에 도흔하기때문에원인균을진단하지못하고경험적인치료가필요 한경우가많은소아의호흡기감염의치료에충분히고려되어야 할것이다.
