It is a real-time imaging modality, and in experienced hands using state-of the art equipment, sonography can provide rapid and accurate results. The availability of portable ultrasound equipment has expanded the use of ultrasound in the operating room as an invaluable tool for surgeons. Intraoperative ultrasound (IOUS) has been used as an important tool in breast and thyroid surgery. In breast cancer surgery, IOUS offers many benefits, including localization of small palpable or non-palpable lesions for patients desiring breast conserving surgery, ensuring negative resection margins, and confirming excision of lesions by ultrasound for intraoperative specimens. IOUS is also useful in detection of very small thyroid lesions and sentinel lymph node biopsy for thyroid carcinoma patients. In addition to classical use of ultrasound in diagnosis and follow-up of thyroid carcinoma patients, IOUS facilitates tumor localization and accurate excision of metastatic nodes and recurrence in patients requiring re-operative neck surgery. In this article, we will review the previous studies regarding IOUS and discuss the usefulness of IOUS in breast and thyroid surgery. Keywords: Breast, Thyroid, Operative, Ultrasonography 서론 1942년오스트리아의신경과의사인 Karl Theo Dussik 이뇌종양환자에서뇌실의위치를알기위해초음파를환자에게처음사용한이후,(1) 놀라운발전을거듭하여현재초음파는인체의다양한기관들을조사하는매우유용한영상학적진단도구가되었다. 또한초음파는기술의발달로최근에는손에들고다닐수있을정도로작게개발되었으며 21세기의시각청진기 라고불릴정도로환자진료에있어필수적인장비로여겨지고있다.(2) 검사방법이편리하고이동이쉽다는초음파의큰장점에따라수술실에서직접사용하는수술중초음파 (intraoperative ultrasound; IOUS) 가자연스럽게도입되었다. Cook과 Lytton (3) 이신장에서결석을찾기위해수술중에초음파를이용한것이 IOUS의첫보고이며, 이후 Makuuchi 등 (4) 은간종양환자에서수술중간내종괴를찾는데초음파를유용하게사용할수있음을보고하였다. IOUS의역사는크게세기간으로나누어볼수있다.(5) 1960년대에 A-mode 와비실시간 B-mode를이용한 IOUS 가처음소개되었는데판독의어려움때문에많이사 Journal of Surgical Ultrasound is an Open Access Journal. 2 J Surg Ultrasound Vol. 3, No. 1, 2016 용되지않았다. 이후 년대에고주파의실시간 B-mode 초음파가보급되면서 IOUS가간담췌외과, 신경외과, 흉부외과등에서널리사용되기시작하였고, 1990 년대복강경초음파, 칼라도플러이미지 (color Doppler image) 등진보한초음파들이접목되면서 IOUS 는현재다양한수술에서수술방법의결정및수술술기의지침에꼭필요한항목이되었다. 유방암과갑상선암은국내여성에서가장호발하는암으로건강검진의확대및진단방법의발전에힘입어조기에발견되는비율또한증가하고있다.(6) 유방과갑상선은모두표면기관 (surface organ) 으로초음파를통한병변의진단이효과적이며, 수술후추적관찰및수술중처치에도초음파가매우유용하게사용된다. 이에저자들은문헌고찰을통해유방과갑상선질환특히암수술에있어 IOUS의유용성을알아봄으로써실제임상적용에도움이되고자한다. 본론 1. 유방암의수술중초음파유방암은국내여성암중발생률 2위를차지하는암으로해가갈수록발생률이증가하고있다.(6) 유방암의가장흔한증상은무통성의종괴이지만특별한증상없이건강검진에서우연히발견되는경우도 33.4% 에이른다.(7) 서양과는달리국내유방암의호발연령은 40대후반에서 50대초반으로유방의치밀도가높은시기이며, 동일한연령에서도 50세미만에서는서양에비해유방치밀도가상대적으로높아유방질환의진단에있어초음파의역할이더욱중요하다.(8) 유방암환자에서 IOUS를시행할때탐촉자 (probe) 는일반적인유방초음파와동일하게 MHz의선형탐촉자가적합하다. 유방암환자에서 IOUS 의장점은크게촉지되지않는작은유방병변의위치결정 (localization) 과유방보존술시적절한음성절제연의확보두가지로요약할수있다. 1) 유방병변의위치결정촉지되지않는작은크기의유방종괴또는미세석회화를절제하기위한수술전위치결정방법으로피부위에표시 (skin marking), 생체염료 (vital dye), 탄소용액 (carbon solution), 방사성-유도하위치결정 (radio-guided localization) 등이있으나가장많이이용되는방법은바늘-유도하위치결정 (wire-guided localization; WGL) 이다 년 Frank 등 (9) 이유방촬영술에서관찰되는비촉지성유방병변에 hook tip을이용한 WGL 을처음소개한이후현재까지 WGL 은표준치료 (standard of care) 로자리잡고있다. 그러나 WGL은수술전추가적인처치로인하여환자의통증과불안이증가하고바늘이빠지거나이동하는단점이있으며수술실패율이 4-9% 까지높게보고된다.(10) 유방수술에서 IOUS 의적용은 1988 년 Schwartz 등 (11) 이처음기술하였는데석회화를제외한비촉지성유방병변들의정확한절제를위해선택된환자들에서 WGL 대신초음파가효과적이라고보고하였다. 이후여러연구들에서유방수술에서 IOUS의장점에관하여보고하였는데가장큰장점은수술실에서실시간영상을통하여병변을관찰할수있으므로절제조직을최소화할수있다는것이다.(10,12,13) 다른위치결정방법과는달리 IOUS 는전신마취상태에서시행하므로환자의불안을최소화할수있고바늘삽입에따른합병증 ( 바늘이동, 절단, 전기소작에의한열손상, 기흉등 ) 이없으며적절한절개부위의선택도가능하다. 또한 IOUS는일반적으로수술을집도하는외과의사가직접시행하기때문에술전영상의학과와의시간조율이필요없으며절제된조직에서병변을실시간으로확인할수있고학습곡선 (learning curve) 이짧다는장점도있다. 이렇게많은장점을가진 IOUS에의한유방병변의수술정확도는 % 까지보고되고있다.(13,14) 유방의크기가작은경우에는병변의위치를확인하기위해단순히피부위에표시만으로도가능하지만유방의크기가크거나병변의위치가깊은경우에는 IOUS 를통한생체염료주입또는바늘삽입을통해병변의위치를결정한후수술을진행할수도있다 (Fig. 1). 병변의절제후 IOUS를통한표본 (specimen) 확인은매우중요한데이를통해목표로한병변이완전히잘절제되었는지확인가능할뿐아니라악성종양의경우절제면까지의거리를측정함으로서음성절제연의확보에도큰도움이된다 (Fig. 2).(15-17) 2) 음성절제연의확보유방암환자에서유방보존술후방사선치료를병행하면유방절제술과생존율에서차이가없다는보고가있은 2

이 연구에서는 재수술률과 최 소한으로 적절하게 제거된 조직의 양에서도 IOUS가 좀 더 우수한 결과를 보였으나 통계학적인 의미는 보이지 않았 다(P = 0.07, P = 0.14).

(14) 작고 국소적인 병변에서 IOUS의 음성 절제연 확보는 매우 우수하지만 반면에 종괴를 동반하지 않는 광범위한 미만성 석회화를 보이는 상피내암의 경우에 IOUS는 한계 를 갖고 있다. Fig. 2.

(B) The needle tip (arrow) was inserted into the 0.5 cm daughter nodule located in right 1 o clock direction. 0.52, 95% CI ). 이 연구에서는 재수술률과 최 소한으로 적절하게 제거된 조직의 양에서도 IOUS가 좀 더 우수한 결과를 보였으나 통계학적인 의미는 보이지 않았 다(P = 0.07, P = 0.14). 또한 촉지성과 비촉지성 유방암 환자를 모두 포함한 13개 연구들의 메타 분석에서도 전향 적, 후향적 연구 모두에서 IOUS가 다른 위치 결정 방법들 보다 우수한 음성 절제연 확보율을 보였다.(14) 작고 국소적인 병변에서 IOUS의 음성 절제연 확보는 매우 우수하지만 반면에 종괴를 동반하지 않는 광범위한 미만성 석회화를 보이는 상피내암의 경우에 IOUS는 한계 를 갖고 있다. Confirmation of target lesion in specimen by intraoperative ultrasound in patient with breast cancer. 그러나 최근 상피내암 환자에서 IOUS와 WGL을 비교한 James 등(21)의 연구에 따르면 음성 절제연 확보와 재수 술률에서 두 방법간에 차이를 보이지 않아 상피내암 환자 후,(18) 현재 국내 유방암 환자의 67.2%가 유방보존술을 에서도 선택적으로 IOUS가 유용하게 사용될 수 있음을 증 시행 받고 있다.(7) 유방보존술을 시행하는 경우 재발률 명하였다. 향후 유방보존술 시 음성 절제연 확보를 위한 을 낮추는 가장 중요한 인자는 음성 절제연(negative re- IOUS의 적용은 더욱 늘어날 것으로 예상된다. section margins)을 확보하는 것으로 대부분의 보고에서 즉, 유방암 환자에서 IOUS는 비촉지성 병변의 위치 결 종양과 절제연의 거리를 2-10 mm로 권고하고 있다.(19,20) 정과 음성 절제연 확보에 매우 우수하며 실시간으로 표본 음성 절제연 확보 측면에서 IOUS가 WGL보다 더 우수 초음파를 통해 절제술의 정확도를 향상시킬 수 있다. 또한 하다는 여러 보고가 있었는데 1개의 무작위 비교연구와 9 의료진들간의 직접적인 소통을 통해 절제 범위를 결정할 개의 코호트 연구가 포함된 18개의 기존의 보고들에서 수 있으며, 환자 측면에서도 마취하에 시행하는 비침습적 1,328명의 환자를 메타 분석한 Ahmed와 Douek (13)의 인 도구로 불편감을 최소화하고 재수술률을 낮춤으로서 보고에 따르면 IOUS가 WGL보다 통계적으로 의미있는 음 의료 비용도 절감할 수 있다는 장점이 있다. 성 절제연 확보율을 보였다(P ; Odds ratio 3

4 J Surg Ultrasound Vol. 3, No. 1, 갑상선암의수술중초음파갑상선암은국내암중발생률 1위를차지하는암으로특별한증상이없어초음파를통한건강검진을통해우연히발견되는경우가많다.(6) 국내갑상선암은약 95% 이상이유두상갑상선암으로경부림프절전이가많은특징이있으며수술환자의약 20-50% 에서경부림프절전이가발견된다.(22) 경부림프절전이는갑상선암의재발과밀접한관련이있으며조직학적형태, 갑상선외침범, 수술시절제범위등에따라달라지지만전체갑상선암의약 10-45% 가재발을경험한다.(23) IOUS는갑상선암수술에서도매우유용한데탐촉자는 7-12 MHz의선형탐촉자가적합하다. 갑상선암환자에서 IOUS는수술의완결성을향상시키고특히경부림프절재발병변의위치결정에탁월한장점이있다. 1) 수술완결성향상수술전종양의효과적인위치결정을위한조건으로 4S 가있는데한단계 (single stage) 로외과의사가행하며 (surgeon performed) 단순하고빠르면서 (simple and fast) 안전하고효과적인 (safe and effective) 방법을찾는것이다.(24) 그런면에서 IOUS는갑상선암환자에게매우유용하게사용될수있다. 분화갑상선암환자의수술시 IOUS의유용성을살펴본연구에따르면 IOUS 를시행한군에서시행하지않은군에비해통계학적으로의미있게낮은재발률을보였다 (1.9% vs. 12.5%, P < 0.05).(25) 따라서갑상선암환자의진단및추적관찰뿐아니라수술시에도초음파를사용함으로서수술성공률을증가시켜매우유용하게사용될수있음을보고하였다. 또한 Agcaoglu 등 (26) 은갑상선암환자중변형근치경부절제술을시행하는 25명의환자를대상으로 IOUS를적용하였을때 4예 (16%) 에서 IOUS 를통해 II, IV와 V 구역에서잔존하는림프절을찾아절제함으로써수술의완결성을높일수있었다고보고하였다. IOUS 적용에따른예상되는제한점으로수술시간지연이있는데갑상선과부갑상선종양환자에서외과의사가 IOUS를이용하여생체염료주입후수술을시행하였을때 5-10분의추가만으로모든예에서신경손상없이수술을성공적으로마쳤다는보고가있었다.(27) 2) 재발병변의위치결정임상적으로림프절전이가명백하지않은분화갑상선 암환자에서예방적중앙림프절절제술은재발률은낮출수있지만생존율을증가시키는지에대해서는아직까지명확하게입증된바없다.(28,29) 따라서분화갑상선암의고위험환자즉, T3 혹은 T4 병변, 15세미만또는 45세이상, 다발성병변, 갑상선외침범, 임상적으로측경부림프절전이가있는경우를제외하고예방적중앙림프절절제술은권고되지않는다.(30) 또한분화갑상선암환자에서예방적측경부림프절절제술도시행하지않는다. 이에전체갑상선암환자의약 1/3이중앙과측경부림프절재발을포함한국소, 구역및전신전이를경험하게된다.(23) 갑상선암의경부림프절 ( 특히 IV 구역 ) 전이시수술적절제가표준치료인데이전수술에의한유착및반흔 (scar) 조직에의해병변의제거가어려운경우가있다. 따라서재수술시에는첫수술보다후두신경과부갑상선의손상등의합병증발생률이높으며수술시간도더오래걸리는것으로알려져있다.(31) 재발갑상선암환자에서 IOUS 는매우유용하며중앙경부림프절전이와병변의크기가 2 cm 미만인경우그리고이전에외부방사선조사를받은경우에특히효과적이다.(32) Harari 등 (33) 은재발갑상선암환자의재수술시 IOUS 유도하생체염료주입의유용성에대해연구하였는데 53건의수술중 91% (48건) 에서성공적인절제술을시행하였으며단 2예의반회후두신경손상과 1예의저칼슘혈증만을보여 IOUS 유도하생체염료주입은매우안전하고효과적인술기라고보고하였다. 재발갑상선암환자에서병변의 IOUS 유도하생체염료주입은수술실에서외과의사가직접시행하는경우수술시목표병변의위치를가늠하는데매우효과적이며 (Fig. 3),(24) 또한절제된병변의동결절편검사를시행하지않아도되기때문에수술시간및의료비용을절약할수있는장점도있다. 경부림프절전이병변의절제시에는목표병변과함께주변림프절까지함께절제해야 (en-bloc resection) 수술의완결성을높이는것으로알려져있다 (Fig. 4). 따라서갑상선암환자에서 IOUS는수술의완결성을향상시켜재발을줄일수있으며재발병변에 IOUS 유도하에생체염료를주입하여위치결정하는방법은매우안전하고효과적인술기라고할수있다. 4

IOUS는 환자들이 마취된 상태에서 시행하므로 편리하고 실시간 이미지를 통해 수술 중 의료진들간의 소통이 가능하다는 장점이 있으며, 특히 유방과 갑상선암 환자들의 수술에서 IOUS는 매우 안전하고 효과적인 것으로 판단된다. 따라 서 향후 유방과 갑상선 질환의 치료에 있어 IOUS의 적용 1. Park IY.

IOUS는 환자들이 마취된 상태에서 시행하므로 편리하고 실시간 이미지를 통해 수술 중 의료진들간의 소통이 가능하다는 장점이 있으며, 특히 유방과 갑상선암 환자들의 수술에서 IOUS는 매우 안전하고 효과적인 것으로 판단된다. 따라 서 향후 유방과 갑상선 질환의 치료에 있어 IOUS의 적용 1. Park IY.
