7 < 국문요약 > 뇌하수체선종환자에서술전호르몬변화와 정맥마취제요구량의상관관계 서론 : 뇌하수체선종을앓고있는환자들은시상하부-뇌하수체-말단기관으로연결되며인지능력과의연관성이있는것으로알려진부신피질호르몬, 성호르몬그리고뇌에서합성, 분비되는신경스테로이드호르몬이나대사성호르몬인갑상선호르몬들의다양한변화를보이는경우가많다. 마취제는이런호르몬의분비에영향을미치는것으로잘알려져있지만, 호르몬들의분비상태와분비능력의변화가마취제요구량에미치는영향은매우제한적으로연구된바있으며결과도일관되지않다. 따라서뇌하수체선종으로수술을받게되는환자들의호르몬분비상태와분비능력은마취제요구량의상관관계를연구하기에좋은모델이될것으로생각된다. 본연구는뇌하수체종양으로경접형동접근수술 (transsphenoidal approach, TSA) 을받게되는여자환자들에서술전호르몬상태에따른정맥마취제요구량의상관관계를알아보고자하였다. 재료및방법 : TSA가계획된 20세부터 60세이하의성인여성환자 51명을대상으로 propofol과 remifentanil의약역학적모델에따라목표농도조절주입기 (Target controlled infusion, TCI) 를사용하여전정맥마취를시행하였다. 마취유도는 propofol 효과처농도 (Ce) 를 2.0 μg/ml로부터시작하여 0.5 μg/ml 단위로증가시키면서목표농도에도달 15초후에구두명령과가벼운움직임에반응이없을때를마취유도가된것으로정의하고, BIS (Bispectral index) 40 이하에서 remifentanil (Ce, 3-7 ng/ml) 과 rocuronium 0.8 mg/kg 을정주하여기관내삽관을시행하였다. 수술중마 - 1 -

8 취유지는 propofol을이용하여 BIS 사이로유지하였으며, remifentanil의용량을조절하여혈압과맥박이마취전기준치의 30% 범위내로유지하였다. 수술종료후마취유도시 propofol Ce와수술전과정에서소요된 propofol과 remifentanil의용량을총마취시간과체중으로나눈값을기록하고, 이들각각과수술전에측정된호르몬들 (T3,fT4, TSH, LH, FSH, PRL, GH, ACTH, cortisol, somatomedin) 의기저치와뇌하수체자극반응검사의최대값 (TSH, LH, FSH, PRL, GH, ACTH, cortisol: 0-120분 ) 간의 Pearson correlation을시행하였다. 결과 : 마취유도시 propofol Ce는 estradiol, ACTH, cortisol 기저치와양의상관관계를나타났으며, remifentanil 총소요량은 T3 기처치와음의상관관계를, ACTH 기저치와양의상관관계를보였으나 propofol 총소요량은어떠한호르몬과도유의한상관관계를보이지않았다. 또한측정된호르몬들의분비능은마취제의요구량과아무런상관관계를보이지않았다. 결론 : 스트레스호르몬인 ACTH, cortisol 기저치의변화는 propofol의마취유도량과, cortisol과 T3 기저치는 remifentanil 총소요량과연관이있었으나이들호르몬들의술전분비능력은마취제의요구량과연관이없었다. 그러나각각호르몬변화를보인대상환자들의수가제한적이어서많은수의뇌하수체선종환자나술전스트레스호르몬이나갑상선호르몬의변화를보이는환자들을통한보완연구가필요할것으로생각한다. 핵심되는말 : 뇌하수체종양, 호르몬, 목표농도조절주입기, propofol, remifentanil - 2 -

9 뇌하수체선종환자에서술전호르몬변화와 정맥마취제요구량의상관관계 < 지도교수민경태 > 연세대학교대학원의학과 김은미 Ⅰ. 서론 환자의연령, 성별, 체중, 신장등신체의특성은마취제요구량을결정 하는중요한인자이며, 이외에도환자의생리적여건, 특히심혈관능력이 나, 어떤호르몬의분비상태는흡입마취제요구량에영향을미친다. 지능력과연관성이있는것으로알려진부신피질호르몬 1 이나성호르몬 2 인 그 리고뇌에서합성, 분비되는신경스테로이드호르몬 3 등은흡입마취제요 구량과연관성이있는것으로보고되고있다. 마취제가이들호르몬의분 비에영향을미치는사실은잘알려져있지만, 4 이들호르몬의분비상태에 따라마취제요구량이변하는지에대한연구는그다지많지않으며연구결 과도일관적이지않다. 일례로, 여성의생리주기중 progesterone 이증가 - 3 -

10 하는황체기에는마취제요구량이감소되며 5 progesterone 은신경스테로 이드인 alloprenanolone 을합성하고분비시킴으로써 benzodiazepine 과유 사하게 GABA 수용체에작용하여진정작용을가진다고한다. 6 그러나성 호르몬인 estrogen 은 GABAA 수용체의수를증가시킬뿐마취효과는없 으며 2 Yavuz 등 7 에의하면 estradiol 의혈중농도증가시마취제요구량 이감소되었지만중추신경계에미치는영향은 estradiol 이아니라황체기 여성에서증가된 progesterone 에기인된다고하였다. 8 또한스트레스호 르몬으로알려진 catecholamine, cortisol 9 의증가가둔화된척추손상환자에서정상인에비해흡입마취제인 sevoflurane 마취요구량이 20-39% 정도더많이증가된다고하였다. 부신피질자극호르몬 (adrenocorticotropic hormone, ACTH) 의증가로인해상승된 allopregnanolone은 GABAA 수 용체의기능을조절하며 10 prolactin(prl) 도시상하부 - 뇌하수체 - 부신피질 축과감정을조절하는중요한인자로작용하며 11 PRL 의분비를억제하는 dopamine의 12 농도가높을수록마취제요구량이많아진다. 13 갑상선자극호르몬 (thyroid-stimulating hormone, TSH) 을분비하는뇌하수체선종환자의경우 propofol 제거율이연장되어약물동력학적변화가초래된다는보고도있다. 14 최근의연구에 15 의하면술전 triiodothyronine(t3) 를복용한심장수술환자들은대조군에비해수술중 T3가감소하지않음으로써혈역학적관리에도움이되었다고하였다. 흡입마취제와정맥마취제는의식소실과진통이라는공통적인임상효 - 4 -

11 과를나타내고있으나마취기전에중요한역할을하는것으로알려진신경 전달물질들 (GABA, acetylcholine, glutamate) 의신경연접부위에서의작용 기전은각각다른것으로알려져있다 Propofol 과 remifentanil 을이 용한전정맥마취는신경외과환자의수술시뇌대사량과뇌압의감소, 빠른회복등의특징으로널리사용되고있다. 19 정맥마취제의경우도흡입 마취제와마찬가지로호르몬과연관이있을것으로예상할수있으나정맥마취제요구량과호르몬분비상태나분비능력과의연관성에대하여보고된연구는거의없다. 뇌하수체전엽종양으로수술을받는환자의약 25% 는뇌하수체기능이정상이지만나머지대부분의종양은호르몬의과잉분비를보이는선종을형성하며, 시상하부-뇌하수체-말단기관 ( 부신, 갑상선, 생식기관등 ) 의축을통하여양성또는음성의상호영향을미치는복잡한연결고리를통해호르몬변화에따라뇌하수체선종은다양한임상양상을나타낸다. 뇌하수체전엽의분비가항진된선종은분화된세포종류에따라 PRL, ACTH, GH(growth hormone), TSH, FSH(follicle-stimulating hormone), LH(luteinizing hormone) 등의합성과분비가달라지며쿠싱질환 (Cushing disease), 고티록신혈증 (hyperthyroxinemia), 말단비대증 (acromegaly), 고프로락틴혈증 (hyperprolactinemia), 생식샘저하증 (hypogonadism) 등이초래된다. 20 또한뇌하수체의기능이정상이더라도 뇌하수체줄기 (stalk) 를압박함으로써이차적인호르몬의변화가초래될수 - 5 -

12 있다. 뇌하수체선종환자들은수술전에뇌하수체로부터분비되는호르 몬뿐아니라시상하부와말단기관으로부터분비되는호르몬의상태와분비능력을검사하므로, 다양하게변화된호르몬상태와마취제요구량의상관관계를연구하기에좋은모델을제공할것으로생각된다. 본연구는뇌하수체종양으로경접형동접근수술 (transsphenoidal approach, TSA) 을받게되는여자환자들에서수술전측정된호르몬들의변화가마취제요구량에영향을미칠것이라는가설을통하여이들호르몬들과마취제요구량의상관관계를알아보고자하였다. Ⅱ. 재료및방법 본연구는임상연구심의위원회의승인을얻고환자로부터연구동의서를취득한후 TSA가계획된 20세부터 60세이하의여성환자 51명을대상으로하였다. 수술전임상증상에따라환자들의기저치호르몬 (T3, thyroxine:t4, free T4, TSH, LH, FSH, PRL, GH, ACTH, cortisol, somatomedin) 과 LH-RH(luteinizing hormone release hormone), TSH, insulin 을이용하여 2 시간동안 복합뇌하수체자극검사를시행하여 TSH, LH, FSH, PRL, GH, ACTH, cortisol 의반응을측정하였다. 마취전처치는시행하지않았으며, 심전도표준전극 Ⅱ, 맥박산소포 - 6 -

13 화도계측기, 요골동맥압, 체온, 호기말이산화탄소분압, Bispectral index monitor(a-2000 BIS monitor, Aspect Medical System, Norwood, MA, USA) 등의지속적인감시아래마취방법을표준화시켰다. 환자의성별, 나이, 신장, 체중의변수를보정하는목표농도조절주입 (Target controlled infusion: TCI) 장비인 Orchestra Base Primea (Fresenius Vial, Brezins, France) 를사용하여 propofol (Marsh 약역학 model) 과 remifentanil (Minto 약역학 model) 을이용한전정맥마취를시행하였다. 마취유도는 propofol 효과처농도 (Effect site concentration, Ce) 를 2.0 μg/ml로부터시작하여 0.5 μg/ml 단위로증가시키면서목표농도에도달하면 15초후에구두명령과가벼운움직임에반응이없을때를마취유도가된것으로정의하고, BIS(Bispectral index) 40 이하에서 remifentanil (Ce, 3-7 ng/ml) 과 rocuronium 0.8 mg/kg을정주하여기관내삽관을시행하였다. 수술중마취유지는 BIS 35-55를목표로하여 propofol 용량을조절하였으며, 혈압과맥박이마취전기준치의 30% 범위를이탈할경우일차적으로 remifentanil 용량을조절하였으며필요시수액과승압제를 사용하였다. 산소와공기를 2 L/min 의유량 ( 흡입산소분압 0.5) 으로호 기말이산화탄소분압을 mmhg로유지하면서조절환기시켰다. 능동적대류가온기 (Bair Hugger, Arizant Healthcare Inc. Eden Prairie, MN, USA) 를장착하여액와체온이 C를유지하도록하였다. 수술종료후마취유도가되었을때의 propofol Ce와수술전과정에서 - 7 -

14 소요된 propofol과 remifentanil의용량을총마취시간과체중으로나눈값을기록하고, 이들각각과수술전에얻은호르몬들의기저치와자극반응검사 2시간동안의최대치에대한 Pearson correlation(spss Inc., Chicago, IL, USA) 을시행하였다. 유의수준 0.05 이하를통계적으로의의있는것으로간주하였으며각측정값들의기술은환자수, 평균 ± 표준편차또는정중값 ( 최소 - 최대 ) 으로표시하였다. 그결과를통하여마취유도시 propofol Ce와마취중 remifentanil의총소모량에연관이있는것으로밝혀진스트레스호르몬과신체대사과정에중요한역할을하는갑상선호르몬과마취제요구량의관계를보안조사해보았다. ft4 기저치에따라서감소군 (<0.73 ng/dl), 정상군 ( ng/dl), 증가군 (>1.953 ng/dl) 으로나누고, 자극반응에따른 cortisol 의분비능력을무반응 (<100 ng/ml), 정상하반응 ( ng/ml), 정상반응 (>180 ng/ml) 으로나누어다시 propofol Ce, propofol과 remifentanil의총소모량을 oneway ANOVA를이용하여비교하였다. Ⅲ. 결과 환자의평균나이는 39.9 ± 세였으며 (20 대 15 명, 30 대 13 명, 40 대 10 명, 50 대 13 명 ), 신장은 ± 4.7 cm, 체중은 62.2 ± 11.0 kg 이 - 8 -

15 었다. 수술당시임상증세는쿠싱질환, 말단비대증, 고프로락틴혈증등의호르몬과잉분비증 33명, 종괴효과로인한신경압박증세 13명, 비특이적임상증세 3명, 우연발견 2명이었다. 혈색소 (13.1 ± 1.0 mg%) 와혈중전해질 (Na ± 2.0 meq/l, K ± 2.0 meq/l) 은모두정상범위에속하였다. 환자들의수술전호르몬의변화는 Table 1에나타내었다. T4는검사자의수가많지않아분석에서제외되었다. 마취유도시 propofol Ce는 4.7 ± 1.2 μg/ml, 정중값 4.5 μg/ml ( μg/ml) 이었으며 BIS 는 49.6 ± 11.8 이었다. 총소요된평균마취시간은 ± 50.1 분이며정중값 245분 ( 분 ) 이었다. 마취기간동안 propofol과 remifentanil은각각 ± mg (9.0 ± 3.1 mg/kg/hr) 과 ± μg (8.4 ± 2.7 μg/kg/hr) 이소요되었다. 마취유도시 propofol Ce는 estradiol, ACTH, cortisol 기저치와양의상관관계를나타났으며, 마취전과정에서소요된 remifentanil 량은 T3 기처치와음의상관관계를, ACTH 기저치와는양의상관관계를보였으나 propofol 총소요량은어떠한호르몬과도유의한상관관계를보이지않았다 (Table 2 and Figure 1). 또한 ft4 기저치에따른감소, 정상, 증가군과 cortisol 분비능력에따른무반응, 정상하반응, 정상반응군 propofol Ce, propofol 및 remifentanil의총소모량은통계적으로차이가나지않았다

19 Ⅳ. 고찰 마취는어떤약물을사용하던지간에의식소실과진통이라는공통적인 임상현상을수반하지만, 마취기전에관여하는중추신경계의신경전달물질 과이들고유의수용체에대한흡입마취제와정맥마취제의작용기전은서 로상이하다 일반적으로전신마취제의요구량에영향을미치는요인 으로는환자의나이, 성별, 체중, 신장, 혈압, 체온, 혈중전해질, 동맥혈산소분압, 동맥혈이산화탄소분압, 임신여부, 간, 신장의기능등이있다. 이외에도일부호르몬상태의변화또한신경전달물질의변화를초래하여흡입마취제의요구량이변할수있으므로용량선택에고려할것을시사한바있다. 5,7,9,21,22 많은뇌하수체전엽종양은여러가지호르몬의과잉분비를보이는선종을형성한다. 부신피질자극호르몬의다량분비로인해부신피질의코티졸의과잉분비에의한 Cushing s disease는복부비만, 고혈당, 피부선 (striae) 등의신체적변화를일으킨다. 성장호르몬과다분비종양은말단비대증, 심장비대등의증상을일으키며, 젖분비호르몬분비종양은유즙 분비, 무월경및불임등을유발한다. 이런신체적인큰변화를일으키는 뇌하수체종양환자의호르몬들의변화는정맥마취제인 propofol 이나 remifentanil 요구량의상관관계를규명하는데좋은연구모델이될수있다. 본연구에서마취유도시 propofol Ce는 estradiol, ACTH, cortisol의

20 기저치와양의상관관계를나타났으며, 마취전과정에서소요된 remifentanil 량은 T3와 ACTH의기저치와각각음과양의상관관계를보였으나 propofol 총소요량은어떠한호르몬과도상관관계를보이지않았다 ( 표 2). Estradiol의경우, 1595 pg/ml의매우높은수치를보인한명의환자로인해통계적으로유의한연관성에영향을미친것으로보여이환자를제외하면통계적인유의성은나타나지않았으므로 propofol과의상관관계는명확하지않을수있다. Yavuz 등 7 에의하면부인과수술을받는 44 명의환자에서수술전 estradiol 수치가높을수록마취제소모량이감소하였는데 estradiol 자체가마취효과를나타내기보다는 GABAA 수용체의전사 (transcription) 를통한수용체수를증가시킨다고하였다. 2,17 본연구결과와달리스트레스호르몬의증가가마취제소모량을감소시킨다는연구가보고된바있는데, Torres 등은쥐에게외부에서 ACTH를투여한후혈장과뇌의 allopregnanolone이증가하는것을관찰하였고, 10 allopregnanolone은 GABAA 수용체에작용하여진통작용을나타내는것 으로보고한바있다. 6 그러나내재적으로스트레스호르몬이증가된경 우 GABAA 수용체의수가증가하므로 23 오히려많은량의 propofol 이필 요할가능성이있다. GABAA 수용체에선택적항진기능이뚜렷한 neurosteroids 는 6 약동력변화가커서심한스트레스와같은환경에서역 할이뚜렷해지는경향이있을뿐아니라 3 성호르몬 24 이나스트레스호르 몬, 10 prolactin 11 등에의해분비가영향을받게되는복잡한과정이관련되

21 므로마취제소모량에영향을미칠수있는것같다. 스트레스호르몬이 증가된척추손상환자는 9 정상군환자에비해 sevoflurane 의요구량이 20-39% 정도감소할뿐아니라교감신경반응과코티졸의분비도감소하였다. 그러나 ACTH와같은스트레스호르몬인 cortisol 의분비능력에따라편평, 정상하및정상반응을보이는환자들간에마취제요구량은차이가나지않았다. PRL 은항불안효과가있는것으로보고되기도하고 11 PRL 분비종양에 서치료제로사용되는 dopamine 계열의약물은 12 사용량이증가함에따라 25 선조체 (striate) 에서의 dopamine 함량이증가하여흡입마취제인 halothane 의요구량을 49% 나낮추었다는보고도있다. 26 하지만본연구에서고프 로락틴혈증으로무월경증상을보이는환자 18 명중 12 명은정상수치 (0-15 ng/ml) 의 3-20 배증가한 ng/ml 이상을보였는데이렇 게큰차이에도불구하고 PRL 은 propofol 의마취유도 Ce 와상관관계를 보이지못하였다. Hong 등은 27 PRL 의혈중농도가높을수록 propofol 의마 취유도량이증가하였다고하였으나그이유는밝히지않았다. 생식선에서유리되는성호르몬들과흡입마취제요구량에대한연구는 서로상반된결과로나타나는데, 동양인의경우여성이남성보다흡입마취 제요구량이 26% 적었다는보고가있는가하면 28 생리주기에따라통증의 역치나 29 행동의변화가발생함에도 24 불구하고생리주기는마취제요구량 과상관관계가없었다는결과도있다. 30 본연구에서 LH, FSH 는정맥마취

22 제인 propofol과상관관계가없었으며 progesterone은난소뿐아니라뇌에서도합성과분비가이루어질수있으므로명확한관계를설명하기에부족한면이있다. 갑상선자극호르몬 (TSH) 에의한갑상선기능항진증환자는 propofol의제거율과 14 분포구획량 (distribution volume) 이 31 증가하여마취유도시 propofol의요구량이증가할수있다고하여마취제소요량과의연관성을 보고한바있다. 최근의연구에 15 의하면술전 T3 를복용한심장수술환 자에서대조군에비해수술중혈역학적관리가유리하였다고하였으나마 취제소요량은비교하지않아아쉬웠다. 갑상선기능이변하면 propofol 의 단백질결합또한변화되므로 32 마취제소모량이영향을받을수있음을 시사하였다. 그러나본연구에서 T3, ft4, TSH는 propofol의소모량과연관성이없었다. T4는수술전측정한환자의수가많지않아서비교할수없었으며, ft4의수술전수치에따라감소, 정상및증가군간마취요구량의차이또한나타나지않았다.( 자료보이지않음 ) 비특이적인유사에스트라제 (nonspecific pseudoesterase) 에의해대사되는 remifentanil의총소모량은 T3 기저치와음의상관관계를보여예상밖의결과를얻었다. 이는아마도수술과정은뇌종양의존재상태와제거된상태모두를포함하므로수술중매우심한호르몬의변화가나타남을예상할수있다. 따라서 propofol과 remifentanil 소모량과여러가지호르몬들과의상관관계를구하는것은매우어려울수있으므로본연구에서마취전과정동안

23 remifentanil의소요량이 T3, ACTH, cortisol과상관관계를보였다고단정할수는없는것같다. 또한마취유도과정이나마취전과정에서소요된 propofol이나 remifentanil은호르몬의최대분비능과는직접적인상관관계를찾을수없었으므로, 알려진대로마취제에따라호르몬의분비가영향을받을수는있어도분비능력자체가마취제요구량에영향을미치지는않는것같다. 본연구는여자환자만을대상으로하였음에도불구하고검사된호르몬들의다양한기능적상태를보이는환자수가충분하지못한문제점을내포하고있다. 또한이들호르몬의합성과분비는시상하부-뇌하수체-말단기관에이르는음, 양의조절효과를통하여복잡하게서로영향을받을수있으므로다변량회귀분석이나요인분석의적용이어려울것으로생각되어각각호르몬들과마취제요구량간의상관관계만을시행하였다. 그러나본연구에서마취제요구량이환자의연령과상관관계가없었던결과는성별, 체중, 신장의변수들이보정되었음을의미하고비록소수이지만다양한호르몬혈중농도의환자들이포함되었기에임상적의미가있을것으로생각된다

24 Ⅴ. 결론 결론적으로본연구는호르몬의변화가심한뇌하수체선종환자들을대상으로수술전호르몬의상태와마취유도과정및마취전과정에서사용되었던정맥마취제인 propofol과 remifentanil의요구량의상관관계를조사한바, propofol의마취유도 Ce는 ACTH, cortisol와연관이있는것같으며, 추후연구에서는좀더다양한호르몬의상태를보이는다수의환자들을대상으로마취전, 마취후및종양제거전과후의과정에서마취제요구량에미치는영향을조사함으로써본연구의문제점들을보완할수있으리라생각한다

29 Abstract Correlations of preoperative hormones with intravenous anesthetic requirements in pituitary adenoma patients Eun Mi Kim Department of Medicine The Graduate School, Yonsei University (Directed by Professor Kyeong Tae Min) Backgrounds: Volatile anesthetic requirement is reported to have a close relation to certain hormones such as adrenocortical hormones, sex hormones or neurosteroid hormones which are synthesized and secreted in brain although there are still controversies. Most functional pituitary adenomas represent hyper-secretary status of various hormones according to differentiated cell types of pituitary gland which gets positive and negative feedback from hypothalamus and pituitary and target organs. Therefore pituitary adenoma patients may provide a good model for the study of the relation between hormones and anesthetic requirements. This study was focused on the correlation of hormones and intravenous anesthetic requirements in pituitary adenoma patients based on hypothesis that any preoperative hormonal changes may affect the anesthetic requirements. Methods: 51 adult female patients, aged yr, undergoing TSA were enrolled in this study. General anesthesia was induced with i.v. propofol and remifentanil using TCI. An infusion of propofol with a preset effect site concentration (Ce) of 2.0 μg/ml was started and increased with a step of 0.5 μg/ml until the patient had reached the anesthetic induction status, not responded on verbal commands or mild shakings. When BIS had reached the

30 score 40, trachea was intubated with intravenous injection of remifentanil (Ce, 3-7 ng/ml) and rocuronium 0.8 mg/kg. Anesthesia was maintained with remifentanil and propofol. The BIS values were maintained at 45 ± 10, by propofol. Hemodynamic change was controlled within 30% of baseline blood pressure and heart rate, by remifentanil. The effect site concentration of propofol for anesthetic induction and total requirements of remifentanil (μg/kg/hr) and propofol (mg/kg/hr) during anesthesia were recorded. Preoperative basal values of hormones (T3, ft4, TSH, LH, FSH, PRL, GH, ACTH, cortisol, somatomedin) and the maximal value of hormones on combined pituitary function test (TSH, LH, FSH, PRL, GH, ACTH, cortisol: 0-2 hours) were also recorded. Pearson correlation was analyzed for the relationships between these hormones and propofol Ce, total propofol and remifentanil dose. Results: Propofol Ce for anesthetic induction had a positive relation with basal estradiol, ACTH, cortisol. The amount of remifentanil during the total period of anesthesia had negative relation with basal T3 and positive relation with basal ACTH. Maximal values of any hormones did not correlate with either anesthetic induction or maintenance. Conclusions: This study suggests that preoperative basal ACTH or cortisol might affect propofol Ce for anesthetic induction but not maximal capacities of hormones. But total requirement of propofol and remifentanil may lack the relationship to hormonal status even though remifentanil related to basal T3 and ACTH.
