We aimed to assess the frequency and clinical factors for pneumothorax occurring during NIPPV application in a tertiary children s hospital. Methods: We selected cases of pneumothorax related to NIPPV by keyword searching in our institution's clinical data warehouse, and their medical records were retrospectively reviewed. Results: During a period of 17 years, 15 cases undergoing NIPPV developed pneumothorax, which was an incidence of 0.64% (15 of 2,343). There were 9 neonates and 6 adolescents. In 9 neonates, pneumothorax was caused by the continuous positive airway pressure (CPAP) ventilator, and occurred on 2 days after birth (median, range 1 3 days). In neonates, pneumothorax developed within 36 hours after CPAP application. One neonate underwent tracheal intubation and 3 neonates underwent chest tube insertion. In the postteenager group, pneumothorax developed 23 months (median, range 5 days to 47 months) after NIPPV application with a mask. All of the 6 patients had underlying neuromuscular disorders and one had superimposed interstitial lung disease. One of the 7 cases underwent surgical treatment and 4 cases were intubated. One case died from the deterioration of underlying interstitial lung disease. Conclusion: Although it rarely happens, the NIPPV can result in pneumothorax. In most cases, it can be resolved by supportive cares with oxygen or chest tube with or without tracheostomy. The prognosis is related to the type of underlying disease and its progression. Pediatric pneumothorax in noninvasive positive pressure ventilation 거나힘주는행위등에따라압력이부족하거나혹은과도하게전 달될수있다. 13 압력이부족하게전달되는경우산소포화도감소나 이산화탄소축적등이유발될수있으며, 압력이과도하게전달되는 경우, 기흉이발생할우려가있다. 4,5,14 비록 NIPPV 가어느정도의 공기누출을허용하기에다른침습적호흡보조법보다기흉의발생 위험이낮을것이라생각할수있으나산소포화도확보를위해순 응도가감소한폐에압력을높게설정한경우, 기침등으로인해급 격히센압력이주어질때기흉이발생할우려는사라지지않는다. 15 기흉은양압환기와관련된주요합병증중하나로, 침습적기계 환기를적용중인환자에서기흉발생률은 0.5% 40% 에이른다. 15 그러나 NIPPV 적용과관련된기흉의발생은드물다고만알려져 있고 16 정확한발생률에대한보고는부족하다. 또한 NIPPV 를적 용하다가실제로기흉이발생환환자들의기저질환및적용기간에 대해분석한연구는아직없는실정이다. 따라서향후보다안전한 진료와효과적대처를위해 NIPPV 를적용하면서기흉이발생한 사례들을검토하여, NIPPV 와연관된기흉발생의임상요인과환 자군의양상을파악할필요가있다. 대상및방법 본기관에최근도입된임상정보데이터웨어하우스 (clinical data warehouse) 를통해 NIPPV 관련기흉이발생한대상을추출하여 의무기록을검토하였다. NIPPV 의범주로는이상성양압호흡기 (bilevel positive airway pressure, BiPAP) 와지속성기도양압 (continuous positive airway pressure, CPAP) 을포함하였으며, high flow nasal cannula 는포함시키지않았다. 대상기간은 2000 년부터 2018 년 8 월까지로본기관에전자의무기록시스템이도입된이래 모든환자에대해검색을적용하였다. 검색대상은어린이병원에 Table 1. The original Korean phrases were translated into English ones. 입원한환자로한정하였으며, NIPPV 적용여부와기흉발생여부 는 Table 1 에제시된검색어를각각간호진술문과진단명에적용하 여이둘이모두존재하는경우를일차검색대상으로확정하였다. 이들가운데, 개별의무기록을검토하여시간적선후관계와인과 성이확인된대상을최종증례로확정하였다. 침습적기계환기를 적용하다가기흉이발생한경우, NIPPV 적용시점과동떨어진시 기에기흉이발생한경우, 개흉수술혹은비디오보조흉강경시술 후기흉이관찰된경우는포함시키지않았다. 외과적수술직후기 흉이발생한경우는대상에서제외하였다. 해당증례의의무기록을검토하여인구학적정보, 적용된호흡 보조의종류와설정, 기흉발생당시임상정보를수집, 분석하였다. 통계소프트웨어는 IBM SPSS Statistics ver (IBM Co., Armonk, NY, USA) 을이용하여중앙값과범위, 빈도등을계산하였다. 군간 비교는 Wilcoxon rank sum test 를시행하였다. 이연구는서울대학 교병원의학연구윤리심의위원회의승인을받았으며, 대상자로부 터의동의획득은면제되었다 ( 승인번호 : ). 결과 연구대상자선정흐름도는 Fig. 1 과같다. 기흉관련진단명중 traumatic pneumothorax 를제외한진단명을넣은결과총 611 명 2,343 Patients under the NIPPV support 9 Patients diagnosed with pneumothorax during NICU admission 76 Screened patients associated with both conditions 15 Patients diagnosed with pneumothorax during NIPPV 611 Patients diagnosed with pneumothorax 51 Patients with pneumothoraces related to invasive ventilator 8 Patients with spontaneous pneumothoraces unrelated to ventilator 2 Patients with pneumothoraces related to thoracic surgery 6 Patients diagnosed with pneumothorax outside NICU Fig. 1. 3 이보라외 비침습적양압기계환기중발생하는기흉 (4,933 건 ) 이검색되었고, 간호진술문중 NIPPV 관련검색어를넣 은결과총 2,343 명 (402,933 건 ) 이검색되었다. 이중 1 세미만의신 생아가총 1,995 명이었고, 1 세이상 12 세미만소아가 188 명, 12 세 이상환자가 160 명이었다. NIPPV 와기흉의두조건을동시에만족 하는대상자는총 76 명이었으며, 이 76 명의의무기록을검토하여 NIPPV 적용과관련하여발생한기흉사례를검토한결과최종적 으로 15 명에서 16 예가발생하였다. 즉, NIPPV 사용과관련한기흉 의발생률은 0.64% (15/2,343) 였다. 15 명의대상중 1 세미만의신생 아가총 9 명이었고, 1 세이상 12 세미만의소아는없었으며, 12 세이 상환자가 6 명이었다. 따라서연령대별발생률은 1 세미만영아에서 0.45% (9/1,995), 1 세이상 12 세미만소아에서 0%, 12 세이상환자 에서는 3.75% (6/160) 였다. 12 세이상환자중 1 명에서기흉이두차 례발생하였다. 기흉이발생할당시환자의임상적특성은 Table 2 와같다. 신생 아중환자실에서기흉이발생한환자들은재태주수중앙값 34 주 4 일 ( 범위 : 28 주 3 일 37 주 3 일 ) 로출생하였고출생시몸무게는중앙 값 1.94 kg ( 범위 : kg) 이었다. 이들 9 명대상자중 3 명은출 생직후신생아실로입실한이후에호흡부전증상을보여신생아 중환자실로전동되었는데, 2 명은일과성신생아빈호흡의심하에곧 바로 NIPPV 를적용하였고, 1 명은신생아호흡곤란증후군의심하 에폐계면활성제 (pulmonary surfactant) 투약후 NIPPV 를적용하 였다. 나머지 6 명중 5 명은출생직후신생아중환자실로입실하여 NIPPV 를적용하였고, 1 명은출생직후분만장에서기관삽관을시 Table 2. NA, not available; TTN, transient tachypnea of newborn; RDS, respiratory distress syndrome; RD, respiratory distress; ILD, interstitial lung disease. 행한채입실하여기계호흡보조를받다생후이틀째발관하고 NIPPV 를적용하였다. 기흉이발생한나이는중앙값 2 일 ( 범위 : 1 3 일 ) 이었고, 모두 NIPPV 적용 36 시간내에기흉이발생하였다. 신생 아를제외하고소아청소년과에서추적관찰중인환자는 6 명으로 총 7 예의기흉이발생하였다. 기흉이발생한나이는중앙값 22 세 ( 범위 : 세 ) 였고남자가 5 명으로많았다. 몸무게는중앙값 29.4 kg ( 범위 : kg) 이었고, 체질량지수는중앙값 11.2 kg/m 2 ( 범위 : kg/m 2 ) 이었다. 여섯명모두신경 - 근질환을앓고있 었고, 한명은이차성간질성폐손상이함께있었다. 기흉은 NIPPV 를시작한지중앙값 23 개월 ( 범위 : 5 일 3 년 11 개월 ) 후에발생하였 다. 7 예중 4 예 (57.1%) 는선행하는호흡기감염이나질병악화의선행 징후없이기흉이발생한반면나머지 3 예중 2 예는중환자실에서 각각선행한폐렴과간질성폐질환의악화로치료받던중, 1 예는병 동에서폐렴으로치료받던중기흉이발생하였다. 기침이나흡기 시악화되는흉통이나주관적호흡곤란이기흉발생과연관된선 행증상이었다. 기흉이발생할당시환자에게적용된 NIPPV 의종류와설정값 은 Table 3 과같다. NIPPV 를적용했던환자군중 1 세미만신생아 들은 318 명 (15.94%) 에서 BiPAP 을적용하였으나, 1 세이상의연령 에서는 338 명 (97.13%) 에서호흡보조로 BiPAP 을적용하였다. Bi- PAP 적용여부와기흉발생의유의한연관성은없었다 (P = 0.385). 신생아의실제기흉발생사례는, 모두중환자실에서 CPAP 를코에 마스크로연결하여적용하던중발생하였다. 당시적용된호기말 양압 (positive end expiratory pressure, PEEP) 은중앙값 5 cmh 2O ( 범위 : 4 7 cmh 2O) 였다. 한편소아 ( 혹은그이상연령대 ) 기흉발생 환자 6 명은 BiPAP 을얼굴혹은코마스크로적용하던중기흉이 발생하였다. 이들 6 명중 1 명은기저폐질환의악화로입원치료를 받던중양압환기를처음적용한지 5 일만에기흉이발생하였고, 5 명 (83.3%) 은평소에도 NIPPV 를적용하고있었다. 5 명모두양압환 기를 24 시간적용하지않고, 밤에잘때는규칙적으로사용하는대 Table 3. Expired. 신낮에는간헐적으로만사용하였다. 특정기계회사에서더많이 발생하지는않았고, 6예가압력모드로사용한반면, 1예는용적모 드로사용하였다. 대상자의 PEEP 혹은호기양압 (expiratory positive airway presssure, EPAP) 은중앙값 6 cmh 2O ( 범위 5 8 cmh 2O) 였다. 기흉발생당시의 PEEP 혹은 EPAP은소아 ( 혹은그이후연령 ) 환자군과그외의군사이에유의한차이를보이지않았다 (P = 0.063). 기침유발기는단 2예에서만규칙적으로사용하고있었다. 발생한기흉에대한조치와예후는 Table 4와같다. 신생아중환 자실에서발생한 9예중 5예 (55.5%) 에서 100% 산소흡입등보존적 치료만으로호전된반면, 4예는흉관삽입이나기관삽관의조치가 필요하였다. 이들중각각 3예에서흉관삽입및음압흡인과기관삽 관을시행하였는데, 이중 2예는흉관삽입과기관삽관을모두시 행하였다. 이들에서기흉치료를위한호흡보조나흉관삽입이필요 한시간은중앙값 3일 ( 범위 : 2 8일 ) 이었고, 모든환자에서퇴원시 기흉과관련된호흡기합병증은발생하지않았고호흡보조없이퇴 원할수있었다. 한편 12 25세환자에서발생한총 7예중 1예만흉 관삽입없이, 산소적용등보존적치료만으로호전된반면, 5예에 서중환자실에내려가기관삽관을받았으며, 이중 3예는기관절개 시행후절개관을통해가정용양압환기기계를적용하였다. 기흉 에대해수술적치료가필요한경우는 1예였다. 사망환자를제외하 고, 기흉발생후퇴원까지소요된시간은중앙값 18.5일 ( 범위 : 일 ) 이었다. 사망은 1예였는데, 기저질환인간질성폐질환의악화 가원인이었다. 고찰이연구는어린이병원에입원한환자를대상으로 NIPPV와관련된기흉의발생양상, 위험과경과를기술한첫보고이다. 17년간의기록을검토한결과, NIPPV 적용과관련하여발생한기흉은총 17 예로매우드물었으며신생아기와학동기이후발생한사례들로확연히구분되었다. 신생아기에발생한사례들은출생직후의호흡부전에대해 CPAP을적용하는과정에서주로발생하였고대부분보존적치료만으로해결된반면, 학동기이후의발생한사례들은기저질환과연관된만성호흡부전상태에서장기간 NIPPV를적용하는과정에서발생하였고다수에서절개관확보후보존적치료를시행함으로써해결되었다. NIPPV 적용의원인이된기저질환과임상상황이특정기계나특정호흡보조방식및설정값보다기흉발생과예후의중요한인자로작용하였다. 발생연령으로구분할때, NIPPV와관련된기흉은뚜렷하게구분되는두개의연령군, 즉신생아군과청소년및이른청년환자군으로구분되었다. 전연령대를통해볼때, NIPPV의적용과무관하게기흉은신생아시기에발생빈도가가장높다고알려져있는데, 17,18 이연구에서 NIPPV 적용과관련된기흉도출생직후신생아가전체대상자의 60% (9/15) 로가장많았다. 신생아들에게적용된 NIPPV는 CPAP으로이시기급성호흡곤란의치료로이용이보편화되어있으며, 19 발관후재삽관을피하기위해일시적호흡보조의방법으로도사용되고있다. 이연구에서는 8예가전자의목적으로, 1예가후자의목적으로적용되던중기흉이발생하였다. 신생아에서 CPAP은침습적기계환기의합병증을줄이는장점이있으나이전연구에서산소만공급한군이나조기기관삽관을시행받은군에비해기흉의발생빈도가높았기때문에 CPAP을적용할때는기흉발생에항상주의할필요가있다. 더구나 CPAP으로호흡부전이해결되지않아기관삽관으로이행한신생아에서도 CPAP 만으로호흡부전이해결된군보다기흉발생의위험이더높았기에, 23 침습적기계환기로전환한뒤에도여전히기흉발생을주의깊게감시해야한다. 신생아시기에 CPAP 사용과관련하여기흉이잘발생하는이유가과호흡으로인한공기누출때문인지, 타연령대보다경직된특징을보이는신생아의폐에 24 과도한양압이가해진결과로초래된현상인지는아직밝혀지지않았다. 한편, 신생아기를지나면한동안 NIPPV 적용과연관하여발생한기흉은발생하지않았으며, 기저질환을가진환자에서긴기간 NIPPV를적용뒤에야다시기흉이발생하였다. 유아기부터초기학동기까지 NIPPV 관련기흉발생사례가드문이유는잘알려지지않았으나, NIPPV를적용할만한환자수자체가적었거나이시기에이르러공기누출에대한방어인자가충분히발달했기때문으로추정할수있다. 한편, 소아기이후에 NIPPV와연관된기흉이발생한환자들도해당시기에는호흡부전으로진행하지않아, 8,15,

NIPPV 적용과관련하여기흉이발생한환자들의임상특성을보면, 특정군이기흉에취약함을알수있다. 이들은청소년및이른청년환자군으로서, 신경-근질환을진단받았고, 기저질환의진행으로인해만성호흡부전에이르렀으며, 수면중저환기등호흡부전의징후가보여호흡부전의진행을늦추고생존확률을높이고자 NIPPV를적용하고있었다. 8,26,27 해당환자군은호흡근의기능부전과동반하여폐순응도가감소하기에외부압력에의한폐실질손상에취약한상태에이르렀을것으로추정된다. 이연구의청소년및이른청년환자들도장기간매일 BiPAP을적용하고있었는데기흉발생전중앙값기준 23개월에다다르는비교적긴기간동안 NIPPV를적용하고있었고, 과반수에서선행하는호흡기감염이나질병악화의징후없이단독으로기흉이발생하였다. 이는해당연령대의기흉이, 호흡부전이장기간지속되어순응도가많이떨어진폐가양압에장기간노출된결과로폐포에작은압력손상이발생하고이것이누적된결과일것으로추정된다. 28,29 이연구에서 NIPPV의종류와설정변수의영향에대해서는대상수가적어충분한결론을내릴수없었다. 폐포에이르는지나친압력이기흉의발생기전이라한다면, NIPPV의적용방식, 즉용적모드와압력모드중압력모드가, 낮은 PEEP에큰일회용적을적용하는경우가, 연결관이압력전달에충실한반면호흡주기동기화에불리한활성모드 (active circuit) 인경우가발생사례의대부분일것으로예상했으나, 15,30 실제기흉이발생한사례의수자체가적어이를통계적으로검정할수없었다. 본기관의경우동원되는폐포를늘리고폐를보호하는것을더중요하게여기기에일반적으로용적모드보다는압력모드로, 가능한일회용적을낮추는방향으로인공호흡기를적용하는경향이있었다. 따라서, 추후연구에서용적모드에큰일회용적으로충분한환기량을확보하는방식으르선호하는기관의자료를취합하여비교할수있다면보다흥미로운결론을도출할수있을것으로생각한다. 한가지주목할점은기흉이발생한사례중단 2예만기침유발기 (mechanical in-exsufflator) 를적용하고있었다는사실이다. 이는기흉발생의위험이기도에걸리는압력의크기보다는일정량이상의압력이적용된기간에의해, 그리고기저질환의진행정도에의해결정될가능성을시사한다. 끝으로어느경우에해당하든이연령대의모든사례가 BiPAP을적용한가운데발생하였던점을고려할때신생아기를지난환자에게적용하는 CPAP은기흉발생의측면에서매우안전한방법으로추정된다. 신생아에서발생한사례의대부분은흉관삽입과산소치료등보존적치료만으로대부분완전히회복되어퇴원할수있었던반면, 소아기이후발생한사례들은보존적치료외에도흉강경을이용한폐엽절제나기관절개등수술적치료를받아야했다. 이는소 아기이후에기흉이발생한환자들이이미만성호흡부전상태에서 다다랐기에양압환기를지속할수밖에없었고, 앞으로기흉이추가 로발생할상황에대해고려해야했기에, 낮은압력으로보다정교 하게환자에게적용하기위해선택적기관절개를권유한것과관련 이있다. 다만, 호흡부전상태에서발생한기흉이실제로생명을위 협한사례는 1 예뿐이었고, 이또한기저질환으로간질성폐손상이 동반되었던점을고려할때, 환자가가지는기저질환자체가 NIPPV 와연관된기흉의경과와결과를결정하는주요인자로작 용할것으로생각한다. 이연구는단일기관에서이루어졌기에결과를일반화하기어렵 다. 그러나신생아를제외한소아기이후발생사례의경우, 만성호 흡부전으로 NIPPV 적용이필요한환자의대다수를국가지정희 귀난치센터가있는본기관에서보는만큼, 본기관의결과를통해 대한민국국가차원의결과를엿볼수있다고생각한다. 아울러이 연구에서는 NIPPV 의종류나압력설정값이기흉발생에미치는 영향에대해유의한결과를얻지못하였는데, 여기에는 NIPPV 의 강도를높이는대신침습적기계환기로언제전환하느냐가기흉의 발생에영향을미치는보다중요한변수로작용하였기때문일수 있다. 긴시간높은강도의 NIPPV 를적용한뒤침습적기계환기로 전환하는경우더많은기흉이발생할수있으며, 반대의경우더적 은기흉이발생하였을것이다. 따라서이결과는호흡보조관련변 수를설정하는기준과침습적기계환기로전환하는기준에대해 다수의연구자들로부터합의된프로토콜이필요한이유를드러내 고있다. 마지막으로, 기흉이발생한이후절개관적용을결정하는 데있어서의학적필요성이외에도보호자와환자본인의선호도, 가정에서돌봄에투여할수있는자원의종류가지대한영향을미 쳤기에절개관적용을치료실패의지표로해석하는데는주의를 요한다.
