However, it is difficult to discriminate pulmonary nodule in patients with IPF, because underlying IPF can be expressed as lung nodules. We evaluated the diagnostic performance of FDG PET in discriminating lung nodule in patients with IPF. Methods: We retrospectively reviewed 8 lung nodules in 16 subjects (age; 67.53±9.83, M:F=1:). Two patients had previous history of cancer (small cell lung cancer and subglottic cancer). The diagnostic criteria on chest CT were size, morphology and serial changes of size. FDG PET was visually interpreted, and maximal SUV was calculated for quantitative analysis. Results: From 8 nodules, 18 nodules were interpreted as nodules, 10 nodules as nodules by histopahthology or follow-up chest CT. The sensitivity and specificity of FDG PET were 100% and 9.%, while those of CT were 70.0% and.%, respectively. Malignant nodule was higher maxsuv than that of lung nodules (7.68±3.96 vs. 1.±0.65, p<0.001). Inflammatory lesion in underlying IPF was significantly lower maxsuv than that of nodules (1.80±0.3, p<0.001). The size of and nodule were 3.95±10.15 mm and ±5.3 mm (p<0.01). Conclusion: FDG PET showed superior diagnostic performance to chest CT in differentiating lung nodules in patients with underlying IPF. FDG PET could be used to evaluate suspicious lung nodule detected by chest in patients with IPF. Discriminating Pulmonary Nodule in Idiopathic Pulmonary Fibrosis with FDG PET 있다. 특발성폐섬유증환자의질병경과는주로흉부전산화단층촬영을통하여이루어지고있으며, 추적과정중에폐결절이흔히발견된다. 특발성폐섬유증환자에서발생하는악성결절의방사선학적소견은결절성또는선형성밀도그리고벌집모양으로알려져있으며, 경계가불분명한종괴 (ill defined consolidation like mass) 로언급된적도있었다. 13,1) 그러나특발성폐섬유증환자에서관찰되는섬유증식부위는방사선학적으로그물음영또는벌집모양으로나타나므로결절의악성여부를감별하기어렵다. FDG PET는폐암의진단과재발판정에널리이용되고있으며, 폐단일결절의감별에도유용한것으로확립된방법이다. 현재까지특발성폐섬유증환자의폐결절의감별진단에 FDG PET가이용된연구는보고된바없다. 이연구에서는특발성폐섬유증환자의흉부전산화단층촬영에서관찰된결절의악성여부감별에서 FDG PET의유용성을평가하였다. 대상및방법 1. 대상 특발성폐섬유증으로진단받고추적관찰중인환자중, 001년 8월부터 005년 10월사이에촬영한흉부전산화단층검사에서결절이관찰된환자를대상으로하였다. 결절의감별을위하여 FDG PET 또는 FDG PET/CT 이시행된환자는 16명 ( 남 : 1, 여 :, 나이 : 67.5±9.8세 ) 이었다. 흉부전산화단층촬영과 FDG PET 또는 FDG PET/CT 의간격은평균 15±16일이었다. 명의환자에서소세포암과성문하암으로치료받은병력이있었다. 악성의최종진단은조직병리학적검사또는 6개월이상의흉부전산화단층촬영추적검사로판정하였다. 3. FDG PET/CT protocol 6명의환자에서 Gemini PET/CT system (Philips) 을사용하여영상을얻었다. 감쇠보정은 CT를이용하였고, F-18 FDG (kg당 0.1 mci) 를정맥주사하여 60분이경과한후에 bed당 분 30초씩총 9 bed를촬영하였다. CT 영상은 PET emission의영상에적용하기위하여 matrix를 matrix로변형하였다. 영상의재조합은 3D-RAMLA iterative reconstruction algorithm으로하였고, 보정상수로는영상의각 Hounsfield units으로부터구한감쇠계수를사용하였다. Standard uptake value (SUV) 는투여한 FDG 양, 체중그리고감쇠보정된국소영상의연조직섭취를고려하였다. 관심영역 (region of interest=roi) 은감쇠보정된전신혹은국소영상의단면상에서병변주변부를마우스로그려선정하였다.. 흉부전산화단층촬영결절의크기는흉부전산화단층촬영영상의장경을기준으로측정하였다. 결절의위치는흉막으로부터 3 cm 거리에가상의선을설정하여중심부와주변부로구분하였고, 해부학적폐의구조에따라상엽과하엽으로구분하였다. 결절의크기, 모양, 크기변화등을참고하여, 이전영상에비하여크기가증가하거나새로생긴결절이있는경우악성으로진단하였다. 5. 통계분석악성및양성결절간비교, 그리고특발성폐섬유증부위와결절간비교분석은 Mann-Whitney test를사용하였다. 적절한진단적 maxsuv값의선정을위해 ROC (receiver operating characteristics) 곡선을이용하였다. 통계분석에 SPSS 1 (SPSS INC., IL, USA) 프로그램을사용하였다.. FDG PET protocol 환자는 6시간이상금식후적정혈당 (180 mg/dl 이하 ) 이확인된후에 FDG PET 검사를시행하였다. 10명의환자에서 ECAT EXACT 7 PET scanner (Siemens-CTI, Knoxville, USA) 를사용하여영상을얻었으며, 감쇠보정은 triple germanium-68 ring source을이용하여 bed당 분으로투과스캔을하였다 MBq의 F-18 FDG를정맥으로투여받고 60분이경과한후, bed당 6분간격으로촬영하였다. 횡단면영상은 Shepp-Logan filter (cut off frequency, 0.35 cycles per pixel) 을사용하여재조합하였으며, 투과영상을이용하여감쇠보정을하였다. 결과 8명의환자에서발견된 10개의결절은폐암으로확인되었으며 ( 병리조직검사 : 6건 ; 흉부전산화단층촬영추적검사 : 건 ), 9명의환자에서관찰된 18개의결절은양성결절로판정되었다 ( 흉부전산화단층촬영추적검사 : 18건 ). 추적검사가시행된시간간격은 7.5±5.5 달이었다. 판정된결절의특징과 SUV는 Table 1에나타내었다. 흉부전산화단층촬영은 10개의악성결절중 7개를악성으로판정해예민도는 70% (7/10) 이었고, 18개의양성결절중 8개를양성으로판정해특이도는.% (8/18) 이었다. 위양 16

The size of nodule was mm. b: FDG PET showed mild diffuse hypermetabolic lesions (msuv:1.96; black arrow) in the lung fields and hypermetabolic nodular lesion (msuv:1.0; white arrow) in right lobe. c: In fusion image of PET and CT, there were hypermetabolic nodular mass (white arrow) within fibrotic area of right subpleural area and mild diffuse hypermetabolic lesions (black arrow) in peripheral area of both lung. 성을보이는결절은이전영상에비하여크기가증가하였으나, 추적검사에서변화가없는경우이었다. 악성결절의크기는양성결절보다컸으며통계적으로유의하였다.(p<0.0) (Table ) 육안분석에의한 FDG PET의예민도와특이도는 100% (10/10) 와 9.% (17/18) 이었다. 악성결절은양성결절보다높은 SUV를가졌으며통계적으로유의하였다.(p<0.001) (Table ) ROC 곡선분석에서가장높은진단성능이얻어지는 maxsuv 는.6이었으며, 이때예민도와특이도는각각 90% 와 9.% 이었다 (area under curve on ROC curve= 0.989, Fig. 3). 결절이없는특발성폐섬유증환자의섬유증부위의 maxsuv는 1.80±0.3 ( 좌엽 :1.8±0.6, 우엽 : 1.78±0., Table 1) 이었으며, 악성결절보다는통계적으로낮은값이었고 (p<0.001) 양성결절보다는오히려높았다.(p<0.001) 악성결절은폐의상엽에 60% (6/10) 가존재하였으며 (Table 3), 90% (9/10) 가폐실질주변부에위치하였다. 또한악성결절의 60% (6/10) 는 IPF에의한섬유증식성병변 165

5 김범산외. 특발성폐섬유증환자의폐결절에서 FDG PET 의감별유용성 준다는것을확인할수있었다. 폐의염증성질환에서 FDG 섭취가증가되어있으므로폐결핵이나폐렴에서도 FDG 섭취가관찰되며, 이는 FDG PET의제한점으로생각되고있다. 그러나 IPF 등의질환에서 FDG 섭취는국소적이지않으며, 비교적넓은범위에걸쳐경한 FDG 섭취를보인다. 따라서국소적인폐암이발생하는경우 FDG PET으로진단이가능할것으로기대할수있다. 특발성폐섬유증환자의흉부단층촬영소견인흉막하그물음영또는벌집모양 (subpleural reticular densities or honeycomb) 부위는정상폐실질혹은양성결절보다높은 SUV값을가졌으나, 악성결절에비해서는낮은값이었다 (p<0.001). Meissner 등 15) 은특발성폐섬유증에서특징적인 FDG 섭취증가가관찰되며, FDG PET에의하여평가되는포도당대사정도는특발성폐섬유증의활성도를반영하는것임을보고하였다 ) 이연구에서특발성폐섬유증의 FDG 섭취가그리높지않고, 국소적이지않아병변의 SUV 수치에따라악성과양성으로잘구분될수있음을보였다. 정상폐실질에발생한폐암의진단과비교하면 IPF 환자에서의예민도는떨어질것으로생각되지만, 흉부전산화단층촬영에서관찰된결절의악성도평가에서 FDG PET은감별에추가적인도움을줄수있을것으로보인다. 단일폐결절의진단에서흉부전산화단층촬영은주로모양과크기, 크기변화를기준으로악성도를판정하며크기변화가가장중요한판정기준이다. IPF 환자에서새로생긴결절은 IPF 자체에의한것일수도있고악성종양의발생에의한것일수도있으므로흉부전산화단층촬영으로진단하는데한계가있다. 이연구에서악성결절의크기가양성결절의크기보다큰것을확인할수있었으나전체적인진단성능은 PET보다떨어졌다. 이연구에서대상환자의악성결절은 60% 가상엽에존재하였고, 90% 는폐실질의주변부에서관찰되었다. Park 등 19) 에따르면특발성폐섬유증환자의악성결절은 5% 가폐상엽에서위치하고있었으나, 이전발표된대부분의논문에서는하엽의빈도가높았다. 0,1) 악성결절은주로폐실질의주변부 (90%), 특히섬유증식성부위에서발생빈도가높았다고보고되고있다. 이는폐섬유증과악성결절의연관성을추정케하는결과였다. Park 등 19) 의논문에따르면악성결절의 56% 는폐주변부에서, 6% 는섬유증식성부위에존재하였다. Mizushima 등 10) 의연구에서도특발성폐섬유증환자의폐암발생은주로폐의주변부, 특히섬유증식성부위에서높았으며다른논문에서도비슷한결과를보였다. 13,1) 본연구의대상군은대부분남자였으며성별에따른분석은어려웠다 (1/16). Lee 등 13) 의논문에따르면악성결절 비율은남자에서더높은것으로발표되었다 (31/3). 그리고악성결절은둥근형태가 70% 를차지하였으며, 0% 는선형성이었다. 이는이전에알려진악성결절의방사선소견과비슷한결과를보였다. 13,1) 대상환자의 PET 영상중 10명은 ECAT EXACT 7 PET scanner 에서영상을얻었으므로 germanium-68 을이용한감쇠보정을시행하였고, 6명은 Gemini PET/CT system 의 MX8000 D multi slice CT를이용하여감쇠보정을시행하였다. Nakamoto 등 1) 에따르면, CT를이용한감쇠보정에서얻어진 radioactivity concentration value는 germanium 을이용한감쇠보정에서보다일반적으로높은값을가짐이보고되고있다. 그러나이연구에서측정한정상폐의 SUV 는두방법간에차이가없었으며, Kim 등 ) 이 cesium과 CT 를비교한분석에서도비슷한결과를보였으므로, 검사법간의차이를보정하지않고분석하였다 3-6). 대상환자중조직학적진단을받은환자가 6명이고추적검사로판정된환자가 10명이었다. 조직학적검사로확인되지않은환자가많다는것이이연구의제한점이라고할수있다. 그러나추적 CT를통한방법으로임상적평가가이루어졌고, 항암제나방사선치료등의치료방침이결정되었으므로임상적으로는악성혹은양성으로확진된것으로볼수있다. 향후조직학적확인이이루어진환자만을대상으로한추가적인연구가필요하다. FDG PET은흉부전산화단층촬영으로발견된특발성폐섬유증환자의폐결절감별에서유용하였다. FDG PET에서얻어진 maxsuv 값은악성과양성의감별에도움이되었으며, maxsuv.6이기준점이되었다. 특발성폐섬유증환자의추적관찰중발견된폐결절의감별에서 FDG PET은흉부전산화단층촬영보다진단의정확도가높았다. 요약 목적 : 특발성폐섬유증 (Idiopathic pulmonary fibrosis: IPF) 에서폐암의발생빈도가정상인에비하여증가되어있음이알려져있다. IPF 환자의흉부전산화단층촬영 (chest CT) 에서폐결절이관찰되는경우폐암의발생과 IPF자체의결절을감별하기어렵다. 이연구에서는 IPF 환자의 chest CT에서관찰된결절의악성여부를 FDG PET을이용해분석하였다. 대상및방법 : IPF로진단된환자중, chest CT에서악성결절이의심되어 FDG PET을시행한 16 명을대상으로하였다. 총 16명 ( 남 : 1, 여 :, 나이 : ±9.83 세 ) 의환자에서관찰된 8개의결절에대하여 FDG PET과 CT소견을분석하였다. 대상환자중 명은소세포암 167

Discriminating Pulmonary Nodule in Idiopathic Pulmonary Fibrosis with FDG PET 과성문하암으로치료받은병력이있었으며, 나머지환자는악성종양의기왕력이없었다. 결절의악성도여부는조직검사와 CT 추적검사로판정하였다. 결과 : 10개의결절은폐암으로진단되었고, 18개의결절은양성결절로판정되었다 ( 조직병리검사 : 6예, chest CT 추적검사 : 예 ). FDG PET의예민도는 100% 이었으며특이도는 9.% 이었다. 크기와형태및크기변화등을참고한 CT의예민도는 70% 이었고, 특이도는.% 이었다. 악성결절의 maxsuv 는 7.68±3.96, 양성결절은 1.±0.65 이었다 (p<0.001). 폐섬유증부위에서측정한 maxsuv는 1.80±0.3 로써악성결절보다낮은값이었으며, 양성결절보다는높은값이었다 (p<0.001; p<0.001). CT에서측정한악성결절의크기는 3.95±10.15 mm, 양성결절은 10.83±5.3 mm이었다 (p<0.0). 결론 : FDG PET은 IPF 환자의 CT에서발견된폐결절을감별하는데도움이되었다. References 1.
