Aristolochic acid (AA) is believed to be associated with the development of CHN. The authors report a case of CHN in which AA in the herb regimen was identified by high-performance liquid chromatography (HPLC). A 32-year-old female presented with nausea, vomiting and generalized weakness. She had been taking Chinese herbs for symptomatic care. Clinical and laboratory examinations revealed Fanconi syndrome, renal failure, and severe anemia. Renal biopsy showed severe tubulointerstitial nephritis with moderate tubular atrophy and interstitial fibrosis. She developed end-stage renal failure 4 months after diagnosis. The herb she had been taking was Aristolochia fangchi. HPLC technique was used to identify AA and to measure its concentration in the herb. From the clinical and laboratory data, the patient was diagnosed with CHN caused by aristolochic acid.(korean J Med 71: , 2006) Key Words : Aristolochic acids, Aristolochia, Tubulointerstitial nephritis 서론 Chinese herb nephropathy (CHN) 는유럽에서체중감량을목적으로한약재가포함된약물을복용한환자에서급속하게진행하는간질성신염에의한말기신부전증을보고함으로써알려지기시작하였다 1). CHN을일으키는주요물질의하나로서 aristolochic acid를들수있는데, 이는체중감소또는이뇨목적으로사용되는한약 또는한약재로만든일부약물들에포함되어있는물질로서근위세관에손상을주어말기신부전증으로빠르게진행하는간질성신염을일으킨다. 저자들은신생검과환자가복용한광방기 (Aristolochia fangchi) 에포함된 aristolochic acid의농도를측정하여 aristolochic acid에의한간질성신염을진단하였기에보고하는바이다. 접수 : 2005년 7월 4일 통과 : 2005년 9월 20일 교신저자 : 한진석, 서울시종로구연건동 28, 서울대학교의과대학내과학교실 ( ) jshan@snu.ac.kr

2 -Jay Wook Lee, et al : A case of chinese herb nephropathy induced by aristolochia fangchi - 증례 1. 병력및검사결과 환자 : 박, 여자, 32세주소 : 전신쇠약감및구역, 구토현병력 : 특별한병력없이건강하였던환자는내원 1개월전부터전신쇠약감, 구역, 구토가발생하였다. 환자는내원 2개월전부터 1개월간호박을달여먹었고, 내원 1개월전부터약 2주간호박과함께 방기 를달여먹었다 ( 그림 1). 환자는내원 3일전부터전신쇠약감, 식욕저하, 구역, 구토를심하게호소하였고, 이와동시에양하지의쇠약감이심해져인근병원을방문하였다. 그곳에서대사성산증, 심한저칼륨혈증및고질소혈증이발견되어본원으로의뢰되었다. 과거력 : 특이사항없음. 가족력 : 특이사항없음. 이학적소견 : 내원시혈압 144/86 mmhg, 맥박 86회 /min, 호흡수 18회 /min, 체온 36.7 이었다. 급성병색이었으며, 구강점막의건조이외에다른진찰소견은없었다. 검사결과 : 인근병원의검사는혈청 Na 143 mmol/l, K 1.5 mmol/l, Cl 126 mmol/l, HCO mmol/l, osmolality 343 mosm/kg 이었고, 이로부터계산한혈청 anion gap은 2 mmol/l이었다. 혈청 creatinine은 2.4 mg/dl이었다. 간기능검사에서 aspartate aminotransferase (AST) 는 945 IU/L, alanine aminotransferase (ALT) 는 273 IU/L이었다. 동맥혈가스분석은 ph 7.22, pco mmhg, po mmhg 이었다. 요전해질은 Na 35 mmol/l, K 13.8 mmol/l, osmolality 190 mosm/kg이었고, transtubular potassium gradient (TTKG) 는 8이었다. 요검사에서는 specific gravity 1.010, ph 6.5, albumin 1+, glucose 2+, blood 3+, 현미경검사결과 RBC 30-49/HPF, WBC <1/HPF 이었다. 24시간요검사결과요량 2190 ml, 단백 3.52 g/d, glucose 25.4 g/d, cysteine mol/g Cr ( 정상범위 mol/g Cr), creatinine 청소율 23 ml/min 이었다. Bicarbonate loading 후측정한 fractional excretion of bicarbonoate (F EHCO - 3 ) 는 36.6% 이었다. 요단백전기영동검사에서는 albumin 19.1%, α1-globulin 10.5%, α2-globulin 26.9%, β-globulin 31.5%, γ-globulin 12.0% 로측정되 Figure 1. Phytological analysis identified these herbs as Aristolochia fangchi. 었고, M-spike 는발견되지않았다. 이러한소견은 Fanconi 증후군에합당하였다. 신생검에서는근위세관의괴사및세관상피세포의소실이관찰되었고, 근위세관의내강에단백질원주가있었다. 간질의부종및염증세포의침윤이뚜렷하게관찰되었다. 혈관및사구체에는특별한이상소견이없었다 ( 그림 2A). 2. 환자가복용하던한약재의확인 환자가구입하여복용한한약재는방기 (Stephania tetrandra) 가아닌광방기 (Aristolochia fangchi) 로확인되었다 ( 그림 1). 3. 한약재에서 HPLC를 이용한 aristolochic acid 농도측정 High performance liquid chromatography (HPLC) 를이용하여환자가복용한생약중의 aristolochic acid의농도를측정하였다. 1) 시료의전처리및 aristolochic acid의추출환자가복용한생약시료 ( 그림 1) 를갈아서분말로만든후하루밤동안건조시켰다. 분말의양을정확히측정한다음, 70% methanol 1 ml를넣고 30초간잘혼합시켰다. 혼합된검체를 18시간동안실온에서방치한다음원심분리하였다 (12,000 g, 5 min). 상층액을분리하여 liquid chromatography 기기에주입하였다

Aristolochic acid I and II were eluted respectively by using highperformance liquid chromatography. 2) Aristolochic acid 표준용액의제조 Aristolochic acid (Sigma Aldrich, USA) g을정확히측정하여 polypropylene tube에넣고 methanol 을채워 11.5 ml의용액을만들었다 (aristolochic acid I 580 μg /ml, aristolochic acid II 370 μg /ml). 이용액을 0.1% ammonium acetate로 serial dilution 하여검량선 (standard curve) 을만들기위한표준시료로사용하였다. 3) HPLC를이용한농도측정 Agilent 1100 Series LC/MS/MS system (Agilent Technologies, Palo Alto, California, USA) 을이용한 high-performance liquid chromatography를시행하였다. Aristolochic acid의 HPLC 측정결과 aristolochic acid I은 1.08 min, aristolochic acid Ⅱ는 1.05 min에각각용출되었으며 ( 그림 3), 검량선범위에서상관계수

Aristolochic acid의정량한계농도는각각 aristolochic acid Ⅰ은 10 ng/ml, aristolochic acid Ⅱ는 1ng/mL 이었다. 검출결과, 두종류의시료 A와 B의 aristolochic acid Ⅰ의농도는각각 mg/g, 3.68 mg/g이었으며, aristolochic acid Ⅱ의농도는각각 μg/g, μg/g이었다. 3. 치료및경과환자는내원 5일째부터 38.0 에이르는발열및오한, 기침, 객담을호소하였으나폐렴의증거는없었다. 이에대해상기도감염의증상을완화시키기위한약물을투여하였고, 이후환자의증상은호전되었다. 환자는 NaHCO3 및 KCl을경구로투여하여대사성산증과저칼륨혈증을교정하였으며, 혈청 creatinine 값도 2.0 mg/dl 이상으로증가하지않아경과관찰하기로하고퇴원하였다. 혈청 AST 및 ALT 값은입원경과도중감소하여퇴원시에는 AST 22 IU/L, ALT 42 IU/L이었다. 상기도감염의증상및독성간손상을고려하여간질성신염에대한스테로이드치료는시행하지않았다. 퇴원 4개월후, 환자는혈청 BUN 82 mg/dl, 혈청 creatinine 13.5 mg/dl로신부전증이악화되어다시입원하였다. 24시간요단백 1265 mg, 크레아티닌청소율 4 ml/min로신기능이저하되어있었다. 신기능이단기간에저하한원인을알기위하여신생검을다시시행하였다. 심한간질성신염및신세관의위축, 간질의섬유화가관찰되었다 ( 그림 2B). 이후환자는혈액투석을시행하다가, 처음내원한지 2년후, 사촌동생으로부터신이식을받고특별한문제없이지내고있다. 고찰본증례는 Chinese herb nephropathy (CHN) 에일치하는임상소견과신생검소견을보인환자가복용하던한약재를얻어이로부터 aristolochic acid (AA) 를검출한국내최초의보고이다. Chinese herb nephropathy (CHN) 는 1993년 Vanherweghem 등이체중감소를목적으로한약이들어있는약물을복용한벨기에여성들에서급속히진행하는간질섬유화및신부전증이나타남을보고하면서알려지기시작하였다 1). 이약물들은 Stephania tetrandra ( 방기, 한방기또는분방기 ) 및 Magnolia officinalis ( 당후박 ) 등의한약재성분을함유하고있다고알려졌으나, 약물제조과정에서 S. tetrandra 대신 Aristolochia fangchi ( 광방기 ) 가사용되었을가능성이제기되었다. 이후분석과정에서 S. tetrandra 의성분인 tetrandrine이아닌 A. fangchi 의성분인 AA가검출되어벨기에에서보고된 CHN의원인물질이 AA일가능성이높아지게되었다 2). CHN은다양한식물독소 (phytotoxin) 에의해일어날수있으며, 이중일부증례에서만 AA가발병기전에중요한물질일것으로생각되고있다. 따라서 AA를 CHN의원인물질로증명할수있는경우 AAN으로부르고, 의심되는원인물질을증명할수없는다른 CHN을 phytotherapy-associated interstitial nephropathy (PAIN) 으로정의하자는주장도제기되었다 3). AA는세포배양실험및동물실험에서신독성이있음이증명되었고, 여러증례로부터신독성과의관계가보고되어있다. Lebeau 등은 AA로인해근위세뇨관세포의 endocytosis가저해되고 AA-DNA 부가물 (adduct) 이발생함을보고하였고, Schmeiser와 Cosyns 등은 32 P-postlabeling method를이용하여 CHN 환자의신조직에서 AA-DNA adduct를검출하였다 4, 5). Bieler 등은이후다른증례보고에서환자가복용하던한약과환자의신조직에서각각 AA와 AA-DNA adduct를발견하였고, 이로부터 CHN을 aristolochic-acid nephropathy (AAN) 으로바꾸어명명할것을주장하였다 6). 또, DeBelle 등은 salt-depleted rat에 AA를투여하면 AA의투여용량에비례하여심한간질섬유화를동반한만성신부전이발생함을보고하였다 7). 따라서지금까지의보고들을종합하면 AA가직접적으로간질성신염과만성신부전을일으키는것은거의확실한것으로보인다. 요약본증례의환자는근위부신세뇨관산증, 진행성만성신부전증, 세뇨관위축및간질섬유화등 CHN에잘맞는임상증상및조직소견을보였고, 복용하던한약재에서 AA가검출되어 AA에의한 CHN 또는 AAN으로진단할수있었다. 이러한환자의신조직에서 AA-DNA adduct를추가로검출할수있다면진단의정확성을더욱높일수있을것으로보인다. 중심단어 : Aristolochic acids, Aristolochia, Tubulointerstitial nephritis
