Purpose: We report the causes and prognosis of anterior interosseous nerve syndrome (AIN) according to the treatment. Methods: From March 29 to December 215, the 2 patients with the clinical symptom of AIN syndrome were enrolled in the study and electromyography (EMG) of AIN was performed. We retrospectively reviewed hand function test, active range of motion, the disabilities of the arm, shoulder and hand (DASH) score and EMG during the recovery from disease. We further surveyed the time of recovery and residual symptoms. Results: The patients with unknown cause of the disease (12 cases), heavy work or trauma (6 cases) and infection (2 cases) were investigated in the study. Thirteen out of 15 cases with conservative treatment and 2 out of 5 cases with a surgical treatment at an average of 8 months from disease were recovered. In addition, 8 cases with fine motor disturbance and 3 cases with tingling residual symptom were observed. Conclusion: Due to the low possibility of entrapment neuropathy, conservative treatment for 7 months is the first choice rather than surgical treatment. If there is no improvement from the conservative treatment, surgical exploration of AIN is the indication of treatment. After recovery, patients may have the symptoms of fine motor disturbance and tingling. Keywords: Anterior interosseous nerve, Neuritis, Nerve compression syndrome 서론 전골간신경증후군 (anterior interosseous nerve syndrome) 은정중신경의순수운동분지의이상소견으로장무지굴근 (flexor pollicis longus), 인지의심수지굴근 (flexor digitorum profundus) 을주로침범하며때로는중지의심수지굴근및방형회내근 (pronator quadratus) 을침범하는마비질환이다. 년 Dissociated paralysis of the median nerve 라는주제로처음기술되었으며 Parsonage 와 Turner 에의해 년 명의신경학적근위축증 (neuralgic amyotrophy) 환자중 명의환자를보고하였다. 이후발생원인에대해서 년 예의신경염 (neuritis) 에의한마비를보고하였고 년과 상골절 (supracondylar fracture) 과관련된마비증상을기술하였으며 년에는처음으로전방전골간신경증후군에서포착신경병증 (entrapment neuropathy) 을보고하였다. 전골간신경증후군에대한보존적또는수술적치료의적응증에대해서는논란이있었다. 만약마비의원인이포착신경병증이라면초기수술적탐색술의적응증이되겠지만신경학적근위축증이나신경염에의한원인이라면수술적치료보다는보존적치료로좋은결과를보고하고있다,. 하지만두원인을감별할만한임상적 (clinical) 또는신경생리학적 (neurophysiological) 분별을위한적절한검사가정해져있지않아원인을밝혀내지못하는경우가많다. 저자들은전골간신경증후군의발병원인과원인에따른환 Copyright c 217. 2 J Korean Soc Surg Hand Vol. 22, No. 1, March 217 자들의치료과정과경과그리고예후를통해이질환의치료방향을제시하고자한다. 대상및방법 년 월부터 년 월까지본원에서전골간신경증후군으로진단된 명의환자를대상으로하였다. 남자가 예, 여자가 예였으며좌수와우수가각각 예로조사되었고평균연령은. 세 ( 범위, - 세 ) 였다. 개월이상추시된환자를대상으로하였으며평균추시기간은 개월 ( 범위, - 개월 ) 이었다. 전체환자에대해이학적검사및근전도검사를시행하였고장무지굴근과인지의심수지굴근만이환되고감각기능의변화가없고다른동반신경병증이없는전골간신경증후군으로진단된경우로한정하였다. 결과에대한추시및판정은수지기능검사 (key pinch, pinch meter [lb]), grip (dynamometer [lb]), 관절가동범위검사 ( ) 와상지기능검사 (the disabilities of the arm, shoulder and hand, DASH score) 그리고근전도검사로평가하였다. 통계적분석은 SPSS 통계분석프로그램 (IBM SPSS ver. ; IBM Co., Armonk, NY, USA) 을이용한 Wilcoxon signed-rank test 비모수검정을시행하였으며 p-value 가 % 의신뢰구간에서통계적유의성을검증하였다. 수술적치료는최소 개월이상보존적치료를시행하였으나근전도검사와마비의회복이없는경우에시행하였다. 또한설문을통한회복의시점, 현재남아있는증상을조사하였다. 결과 전체 명의환자중 예에서심한일을했거나외상과관련이있었으며 예에서감기와관련이있었고나머지 예에서는원인을알수없었다. 좌수와우수각각 예로조사되었고장무지굴근만이환된불완전형은 예였으며인지심수지굴근만이환된불완전형은 예였고나머지 예에서는무지와인지를침범하는완전형으로조사되었다. The Fate of Anterior Interosseous Nerve Syndrome 증상회복은수지기능검사에서집기 (pinch) 그리고관절가동범위검사에서무지와인지의관절의능동적굽힘이 % 이상회복된경우를조사하였으며보존적치료를시행한 예중 예에서평균 개월 ( 범위, - 개월 ) 에증상의회복이있었다. 수술적치료를시행한 예에서는수술까지의기간은평균 개월 ( 범위, - 개월 ) 이었으며수술적치료는 예에서포착부위를찾기위한외부신경박리술 (external neurolysis), 예에서포착부위이외신경다발의문제점을찾기위해내부신경박리술 (internal neurolysis) 을시행하였으며 예모두에서포착을의심할만한구조를발견할수없었다. 수술후회복된경우는 예에서수술후각각 개월, 개월에증상의회복이있었다 (Table ). 통계학적분석으로보존적치료를시행했을경우증상의회복에유의성을보였으며수술적치료에대해서는모집단 이작아통계적유의성을판단하기어려웠다. 특히, 설문조사상 예에서무지와인지의미세조절이힘든증상이남았으며근력의회복이되지않은경우근력약화이외에도저린증상도포함되어있었다 (Tables, ). 고찰 이전문헌에서보고된 명의임상적특징을분석한결과이환된환자중남자가 명여자가 명으로평균. 세 ( 범위, - 세 ) 로보고되었으며우수와좌수가각각 예와 예로우수에이환이많았지만, 본조사에서는 예중남자 ( 예 ) 가여자 ( 예 ) 보다많이이환된것으로조사됐고평균. 세 ( 범위, - 세 ) 로명확하게의미있는연령에따른이환은보이지않았으며우수와좌수가동일한예로우수가 Table 2. The clinical results of cases with surgical treatment Case Time to recovery (month) Pinch Grip DASH ROM ( ) Residual symptom Initial F/U Initial F/U Initial F/U Initial F/U (T) 7 (T) Fine motor disturbance 1 (I) 6 (I) (I) 5 (I) Weakness (T) (I) 5 (T) (I) Weakness Tingling (T) (T) Weakness Tingling (T) 1 (I) 5 (T) 6 (I) Fine motor disturbance p-value Pinch, pinch meter (lb); Grip, dynamometer (lb); DASH, the disabilities of the arm, shoulder and hand; ROM, active range of motion; T, flexor pollicis longus of thumb; I, flexor digitorum profundus of index finger. p<.5, Wilcoxon signed-rank test. 우세하게조사되지않았다. 전골간신경증후군은무지와인지의 O 형성이안되는것이전형적인증상으로장무지굴근과인지의심수지굴근의마비로인해무지의지관절의굴곡이안되고인지의원위지관절의굴곡이안되는것이원인이다. 방형회내근의마비로인한약화는환자가인지하기어렵고근육의신체검사가어렵기때문에정확한판단이어렵다. 전골간신경증후군은경우에따라불완전하게무지또는인지, 중지중하나만을침범하기도하고모두를침범할수있다. 보고에따르면대부분에서무지와인지모두를침범하거나무지를침범하는경우가많았으며인지만을침범하는경우는드물게보고되고있다,,. 본조사에서도 예중 예에서무지와인지를침범하였으며 예에서무지만을침범하였으며 예에서인지만을침범하였다. 때문에무지또는인지만을침범하는경우임상적증상으로도전골간신경증후군을의심할수있다. 급성으로나타나는마비증상은외상또는정맥채혈, 감기증상과같은선행증상이있는경우가자주보고되고, 마비증상이시작되기전전완부의통증을경험하는경우를보고하고있다,. 본조사에서는 예중 예에서전완부와관련된통증을호소하였다. 이러한전완부통증은포착신경증후군의초기증상으로보고되고있다. 하지만, 상완신경총신경염과구분되는중요한점은상완부의통증이고전완부와주관절에서는운동신경증상이선행된다고보고되고있다. 그래서통증은전골간신경증후군에서자주일어나지만신경염과구조적포착을구분할수있는예상인자는될수없다. 전골간신경증후군의원인은외상과연관이안된자연발 생또는감염에의한신경염의원인으로분류할수있다. 다시말하면신경적근위축증, 신경염, 포착신경병증으로분류할수있다. 연부조직, 혈관, 골격구조에의한신경의포착은 Spinner 에의해분류된후대부분의원인을포착신경병증으로보고하였다,. 하지만 년에서 년사이 예의외상과관련이안된마비증상중 예에서수술적탐색술을시행한결과, 예에서섬유띠에의한포착이발견되었고나머지 예에서는심하지않은종창과반흔조직만을보고하고있다. 본연구에서는 예에서수술적치료를시행하였고수술중시야상에서포착을의심할만한비정상적구조보다는신경의종창을확인할수있었다. 그래서확실한외상에의한신경증상이외에환자가표현하는단지외상의기왕력으로포착증후군과신경염을구분하기는어렵다 (Fig. ). 전골간신경증후군의해부학적원인의인식으로부터치료에대한논쟁이있어왔고이론적으로보존적인치료가신경학적근위축증과신경염에서받아들여지고있으며포착증후군에대해서는수술적치료가받아들여지고있다. 하지만두원인에대한증상과임상적검사만으로구분하기어렵다는한계점이있다. 그래서원인에대한인식이시대에따라달라지면서보존적치료의기간과수술적치료의시점에영향을주었다. 포착증후군의원인이지배적이었던 년대에자연발생한전골간신경증후군에대해서보존적치료를시작하지만 주에서 주까지임상적또는근전도에서증상의회복이없다면탐색술의적응증이된다고말하고있다. 년대에들어서면서신경염의원인이우세하게받아드려지면서평균 개월까지증상의회복을지켜볼수있다고보고하고 52

After 14 months, when he visited, he could flex the recovered power of thumb interphalangeal joint and index distal interphalangeal joint flexion. 능의회복을빠르게하지도않았다. 결국관통상이나중요한 손상또는압박되는부위가정확히발견되지않은한수술적 치료는도움이되지않는다고보고하고있다. 원위상지의압통이있는환자의보고에서정중신경탐 색술을시행한결과관다발간 (interfascicular) 신경유리술 (neurolysis) 시행부위의모래시계같은압축대 (hourglasslike constriction) 가발견되었으며외부압박이없는 명의 환자에서같은병변을발견할수있었고 명의환자중 명의환자에서관다발간신경유리술후증상호전을보고하 였다. 또한다른보고에서는압축대의부분을제거하고신 경이식을시행해서좋은결과를보고하기도하였다. 압축대 는염증 (inflammation) 이주요한역할을하며면역학적원인 (immunological in origin) 이라고최근보고되고있고 고해상 신경초음파 (high-resolution nerve ultrasound, HRUS) 가소 개되면서신경의압축대또는팽창을확인하는방법 이소 개되기도하였다. 진단후병변의확인을조기에할수있다면 더좋은치료의결과를얻을수있다. 정중신경의운동신경과감각신경침범없이전골간신경만 을침범함의의문에대해보고되었다. 인접한큰정중신경을 침범하지않고전골간신경만을침범하는것에대해설명하기 어렵다고보고되기도하였다,. 또한저자들은구조적포착 보다는신경염이말초신경을침범한것이라는가설을세우기 도하였다. 최근보고에서는전골간신경증후군으로진단된 환자의기존영상과근전도결과를다시판독한결과더욱광 범위한근육에침범과전골간신경이외의신경침범을보고하기도하였다. 실제로본연구에서 예에서저린증상이후유증으로남은것은신경염이정중신경에서전골간신경으로의이행부위에서신경염이일어나면처음에는운동신경증상만나타나다가신경염이진행하면서감각신경일부를침범한경우라고생각할수있다. 보존적치료를시행했고마비되었던모든근육이의학연구협의회 (Medical Research Council, MRC) 등급 이상으로회복되었지만증상의회복이 개월안에없을때증상의회복을위해오랜시간이필요하다고보고하고있다. 또한마비증상이시작하고 개월후인대이전술을시행하였고마비의원인과치료와관계없이운동기능의회복이없다면인대인전술로만족한결과를보고하고있다. 그래서만약 개월까지신경검사와마비증상의회복이없다면기능의장애가남을수있기때문에인대이전술도만족할만한결과를얻을수있을것이다. 본연구의한계점은단일기관에서조사된경우만을포함하고있으며 명의환자에서비교적짧은기간증상의추시가시행됐다는점이다. 또한후유증으로미세조절기능, 저린증상에대한정확한계측을할수없었다는점이다. 결론 전골간신경증후군은정확한외상의기왕력이없다면포착증후군보다는신경염을의심해야한다. 증상전전완부또는주관절의통증이있는경우가많지만포착증후군또는신경염을구분하는인자는아니며포착신경병증의경우가매우낮기때문에처음에는보존적치료가우선된다. 개월까지임상적또는신경검사에서호전이없다면수술적탐색술을시행할수있다. 대부분의환자에서증상의호전이있지만자신만이느끼는미세조절기능에불편함이있으며처음에나타나지않았던저린증상이신경염의경우에서는나타날수있다. 8 J Korean Soc Surg Hand Vol. 22, No. 1, March 217 전골간신경증후군의운명 유명재 안희찬 서재성 우상현 W 병원수부외과및미세재건수술센터 목적 : 전골간신경증후군으로진단되었던환자들의발병원인에서치료과정그리고예후에대해임상증례를중심으로그결과를보고하고자한다. 방법 : 년 월부터 년 월까지임상증상과근전도검사에서진단된 명을대상으로하였다. 추시된환자의수지기능검사, 관절가동범위검사와상지기능검사 (the disabilities of the arm, shoulder and hand, DASH score) 그리고근전도를이용한환자의회복정도를확인했으며설문을이용한회복시점, 현재남아있는증상을확인하였다. 결과 : 전체 명의환자중원인을모르는경우가 예로가장많았으며 예에서심한일을했거나외상과관계가있었고, 예에서감염과관계가있었다. 보존적치료를시행한 예중 예에서증상의회복이있었고수술적치료는 예에서증상발현후평균 개월에시행하였으며 예에서수술후약 개월에증상이회복됐다. 설문조사상 예에서무지와인지의미세조절이힘든양상으로남았으며근력의회복이되지않은경우근력약화이외에도저린증상도포함되어있었다. 결론 : 포착증후군의가능성이낮기때문에보존적치료를 개월시행한후증상과검사상호전이없다면수술의적응증이될수있다. 증상의회복후에도환자자신만이느끼는미세조절기능또는처음에없었던저린증상이남아있었다. 색인단어 : 전골간신경증후군, 신경염, 포착신경병증 접수일 216 년 12 월 11 일수정일 1 차 : 217 년 1 월 12 일, 2 차 : 217 년 1 월 16 일게재확정일 217 년 1 월 16 일교신저자우상현대구광역시달서구달구벌대로 1632 W 병원수부외과및미세재건수술센터 TEL FAX handwoo@hotmail.com 56
