The aim of this study was to compare rifabutin and levofloxacin rescue s in patients with first- and second-line Helicobacter pylori eradication failures. Methods: Patients, in whom a first treatment with proton pump inhibitor-clarithromycin-amoxicillin and a second trial with proton pump inhibitor-bismuth-tetracycline-metronidazole had failed, received treatment with either rifabutin or levofloxacin, plus amoxicillin (1 g twice daily) and standard dose proton pump inhibitor. Eradication rates were confirmed with 13 C-urea breath test or rapid urease test 4 weeks after the cessation of therapy. Results: Eradication rates were 71.4% in the rifabutin group, and 57.1% in the levofloxacin group, respectively. Although there was no significant difference in Helicobacter pylori eradication rates between two groups (p=0.656), rifabutin based showed relatively higher eradication rate. Conclusions: Helicobacter pylori eradication rates of rifabutin- or levofloxacin-based triple therapy could not achieve enough eradication rate. Further studies would be needed on combination of levofloxacin and rifabutin-based or culture based treatment. 2 402 정명호등. 헬리코박터파일로리의 3 차구제요법의비교 항균제에대한내성률의증가를초래하는데, 이삼제요법의제균율은 CLA의내성증가에따라전세계적으로 intention to treat (ITT) 제균율이 80% 가되지않고있으며, 년도이후국내에서도제균율이 80% 이하로감소중이다. 8 1차제균치료에실패한경우현재까지가장널리사용되고있는 2차제균치료로는 PPI, bismuth, tetracycline 및 metronidazole (MET) 을포함하는사제요법을추천하고있지만, 미국과유럽의경우 MET에대한내성 H. pylori의비율은약 20-30% 로증가하고있고, 9,10 사제요법으로 2차치료를하는경우제균율을 57-85% 정도로보고하였으며, 11 우리나라도역시제균율이 81-85% 정도로높지않게보고되었다 차제균치료로도실패한경우 3차제균요법을시행해야하나, 국내에서아직까지표준화된 3차제균요법은없으며, Maastricht III 가이드라인에서는 H. pylori 배양과항균제감수성검사를시행한후에감수성검사결과에맞추어제균요법약제를선택할것을권고하고있다. 6 그러나, 배양과항균제감수성검사는비용과시간적측면에서전의료기관에서 1, 2차제균치료에실패한모든환자에게시행되기힘들다. 13 따라서이를극복하기위해그동안 H. pylori 제균치료실패후구제요법으로여러항생제에대한많은연구가시행되었다. 그중에서 levofloxacin은광범위항균작용을하는 fluoroquinolone으로, 몇몇연구에서 H. pylori 제균에있어서의효능이보고되었는데, levofloxacin이포함된삼제요법은 66-83% 정도의다양한제균율을보여주고있다. 14,15 H. pylori의 3차제균에사용되는다른종류의항균제는 rifabutin이다. 몇몇연구에서 7일이상의 rifabutin-ppi-amx 삼제요법은표준삼제요법에실패한다제내성 H. pylori 균주의제균에매우효과적이었다. 16 따라서, 이번연구는 3차구제요법들중 levofloxacin 요법과 rifabutin 요법의제균율을비교하여어떤치료가유용한지알아보고자하는목적으로시행하였다. 대상선정의제외기준은 18세미만또는 75세이상인환자, 전에상부위장관수술을받았던경우, 최근 3개월이내에항생제복용력이있는경우, 심각한내외과적질환을동반한경우, 임신, 약물에의한부작용또는알레르기의기왕력이있는경우, 제균여부가확인되지않은경우였다. 2. 방법대상환자는 levofloxacin 투여군 (PPI 표준용량, AMX 1.0 g 하루 2회, levofloxacin 500 mg 하루 1-2회 ), rifabutin 투여군 (PPI 표준용량, AMX 1.0 g 하루 2회, rifabutin 150 mg 하루 2회혹은 300 mg 하루 1회 ) 의 2가지요법으로 levofloxacin 투여 14명, rifabutin 투여군 7명으로나뉘어졌다. 투여기간은 levofloxacin 투여군은 10일간투여하였고, rifabutin 투여군은평균 10.3일간 (7-14일) 을투여하였다. 투약종료 4주후에추적내시경검사를시행하여신속요소분해검사를시행하였거나, 요소호기검사를시행하여음성으로나온경우에박멸에성공한것으로판정하였다. 대상환자에서 levofloxacin 투여군과 rifabutin 투여군으로연령, 성별, 질병군, 치료기간등의관련인자를비교하였으며치료효과는 levofloxacin 투여군과 rifabutin 투여군의 H. pylori 제균율을비교하였다. 통계학적처리는 chi-square 분석을이용하였다. 유의성검정은 p값이 0.05 미만일때통계적인유의성이있는것으로판정하였다. 이번연구는환자의의무기록검토에의한후향적분석으로, 동의서면제를신청하여본원의임상시험심사위원회승인을받아연구를진행하였다. 결과 1. 환자특성비교대상환자는 21명이었고 levofloxacin 투여군 14명, rifa- 대상및방법 1. 대상 2002년 7월부터 2010년 8월까지서울아산병원과가천의대길병원소화기내과를방문한환자들중에서조직검사, 요소호기검사와신속요소분해검사에서 H. pylori 양성으로진단받은환자중 1차표준치료인삼제요법과 bismuth 기반 2 차사제요법에실패한경우를후향적으로분석하였다. Levofloxacin 투여군 14명중남자 12명, 여자 2명으로연령의중앙값은 51.5세 (26-85세) 였으며, 질환별로는위궤양 4예 (28.6%), 십이지장궤양 3예 (21.4%), 위선종이나조기위암으로내시경적점막하박리술을시행받은 4예 (28.6%), 비궤양성위증 2예 (14.3%), MALT 림프종 (7.1%) 였다. Rifabutin 투여군 7명중남자 7명, 여자 0명이었고, 연령의중앙값은 53.0세 (44-67세) 였으며, 질환별로는위궤양 3 예 (42.9%), 십이지장궤양 2예 (28.6%), 비궤양성위증 1예 (14.3%), MALT 림프종 1예 (14.3%) 였다. 각치료군에있어서연령, 남녀비및질환은유의한차이가없었다 (Table 1). 2. H. pylori 제균율 1차표준치료및 2차사제요법에서치료에실패한총 21명의환자에있어서제균여부를확인하였으며, 21명중 16명은요소호기검사를, 5명은신속요소분해검사를통해확인하였다. Levofloxacin 투여군총 14명중 8명 (57.1%), rifabutin 투여군총 7명중 5명 (71.4%) 이박멸되어양군간의제균율은통계학적으로유의한차이를보이지않았다 (p=0.656) (Table 2). 고 찰 이연구에서 levofloxacin 병합투여군은 57.1%, rifabutin 병합투여군은 71.4% 의제균율을보여 rifabutin군에서더높은경향을보였지만양군간의제균율은통계학적으로유의한차이를보이지않았으며, 양군모두에서만족할만한제균효과를보이지못하였다. 소화성궤양및 MALT 림프종과같은경우에 1, 2차제균요 Table 2. Results of Third-line Two Regimens for Eradication of Helicobacter pylori Levofloxacinbased (n=14) Rifabutinbased (n=7) p-value Dosing (mg qd-bid) Duration of treatment, (n=4) or 150 (n=3) 10.3 (7-14) day (range) Tests to confirm eradication Rapid urease test 5 0 Urea breath test 9 7 Eradication rate 8/14 (57.1%) 5/7 (71.4%) 법에도실패한경우환자는소화성궤양의재발또는그로인한합병증이생길가능성이크므로 3차혹은그이상의구제요법도실시되어야한다. 그러나, 기능성소화불량증과같은 H. pylori 제균의이득이명확하게증명되지않은경우는반복적인구제요법은신중하게선택하여야할것이다. 17 제균실패의원인은환자의순응도, CYP2C19의유전자형, 항균제내성들인데, 이중항균제내성이제일중요한인자이다. 18 표준삼제요법과 bismuth 기반사제요법에실패한경우 CLA와 MET를포함하여적어도한종류, 대개는두종류의항균제에대한다제내성균을가지고있을것이다. 따라서 3 차요법을구성하는약제로 macrolide나 nitromidazole과교차내성을보이지않는약제를선택하는것이중요하다. MET 내성률이높은경우에는 furazolidone으로대체할수있는데이는교차내성이없기때문이다. 임상연구에서는이약제에대해순응도가높은환자들에서높은제균율을보고하였다. 19 또한 MET과 CLA에동시에내성을보이는 H. pylori 감염환자들에게 14일간의고용량의 AMX (1,000 mg 하루 3회 ) 과 omeprazole (40 mg 하루 3회 ) 이제요법후 75% 의제균율을보였다는보고가있다. 이연구에서고용량의이제요법이 rifabutin을포함한삼제요법과효과와순응도면에서동등하다고보고하였다. 20 H. pylori에대한치료가실패한경우, 이에대한배양과항균제감수성검사를추천하고있으나, 비용및시간적측면에서모든환자에게적용하는것은아직논란의여지가있다. 6,13 따라서, 1차또는 2차제균요법에실패했더라도, 항균제다제내성 H. pylori에효력이있는항균제가존재한다면균배양을하지않더라도이항균제를사용할것이다. 이러한측면에서 rifabutin을포함한삼제요법이다제내성 H. pylori에대한만족할만한제균율을보이는항균제로대두되고있다. 한연구에서 7일이상의 rifabutin-ppi-amx 삼제요법은내 Vol. 59 No. 6, June 2012

4 404 정명호등. 헬리코박터파일로리의 3 차구제요법의비교 성 H. pylori 균주의제균에매우효과적이었다. 16 그러나, 이번연구의결과에서 3차구제요법으로서 rifabutin-ppi-amx 삼제요법의제균율은 71% 정도로이전다른저자들의보고인 72-79% 와유사하였다 그리고 rifabutin은이외에도여러가지문제점을가지고있다. 첫번째는가격이상당히높다는것이고두번째는발진, 구토, 소화불량과같은흔한부작용외에도시각장애, 24 백혈구및혈소판감소증의골수독성과같은심각한부작용이있다는점이며 25 마지막으로결핵균내성을증가시킬수있으므로결핵이많은국내에서는사용에항상주의해야하고, 대규모로사용해서는안된다는것이다. 26 한편 fluoroquinolone, 특히 levofloxacin을포함한삼제요법또한다제내성 H. pylori 균주의제균에효과적인요법으로인식되고있다. 14,27 그리고, in vitro 연구에서 fluoroquinolone 제재에 PPI를동시복용했을때 H. pylori 균주에대해서상승효과를나타낸다고보고하였다. 28 더불어 levofloxacin은다른 in vitro 연구에서 H. pylori가 CLA와 MET 에내성이있을때효과가있다는보고가있다. 28 이러한결과들은여러 in vivo 연구에서 CLA와 MET에내성이있는환자에서 levofloxacin을포함한삼제요법을통해치료가되어서근거가확립되었다. 23,29 그러나, 3차구제요법으로서 levofloxacin-ppi-amx 삼제요법도이번연구결과에서는제균율이 57% 정도로이전다른저자들에의해보고된 60-86% 에비해서높지않게나왔다. 13,15,27,30,31 반면에 levofloxacin은구역및구토, 복부팽만, 설사와같은경증의부작용만이관찰되었고, rifabutin과같은중증의부작용이관찰되지는않았다. 하지만, fluoroquinolone은항균제내성이쉽게전파되고, 이로인한항생제내성률의증가가관찰된다는것이문제점이다. 이와같이 fluoroquinolon의내성률이증가하는원인은주로 gyra의 Asn-87과 Asp-91의돌연변이에의한것으로보고되었다. 32 이번연구의결과에서 levofloxacin 포함삼제요법의제균율이높지않은이유도 levofloxacin에대한항균제내성률의증가에서원인을찾아볼수있다. 한국인을대상으로 H. pylori에대한항균제내성을조사한 Kim 33 의보고에의하면, levofloxacin의항균제의내성률은 1987년과 1994년에는전혀관찰되지않았으나, 2003년분리균주에서는 21.5% 를나타냈다. 또한 1차제균시행전 2004년과 2007년의 fluoroquinolone 내성률은 15.6% (29/185) 로높게관찰되었다. 34 이는우리나라에서 1998년의 fluoroquinolone계항균제의생산량이 1994년에비해 2.1배증가한것과연관성이있을것이다. 또한다른 floroquinolone인 moxifloxacin을사용한다른국내연구에서도최근제균율이감소함을보고하였는데, 이는이지역에서 moxifloxacin에대한내성률이 2004 년 5.6% 에서 2007, 2008년에는 28.2% 로증가하였기때문이 라고하였다. 35 몇몇연구에서여러제균요법을시행한후제균실패시다른구제요법들을통해누적했을때, 3차혹은그이상의누적제균율은거의 100% 에이른다고보고하였다. 13 한편 rifabutin과 levofloxacin을 rabeprazole에병합하여사용했을때, ITT 제균율이 85% 에서 91% 에이르는결과를보인보고가있다. 36 따라서항균제다제내성 H. pylori를치료하고자계획할때, 두개이상의순차적치료요법을고려한다면 100% 에근접하는치료성공률을보일수있을것이며, levofloxacin과 rifabutin 병합요법도향후치료방법으로고려해보아야할것이다. 이번연구의제한점으로후향적연구를기반으로한것으로인해군간에무작위배정이이루어지지않은점과주요항생제, 특히 rifabutin에대한용량및투약기간이통일되지않았고순응도를알수없다는문제점을지적할수있다. H. pylori에대한 3차제균은항생제내성의증가로인해점차대두될임상적문제점으로, 향후 rifabutin과 levofloxacin을동시에투여하는요법이나감수성검사를통한적절한제균요법의개발이필요할것이다. 요약 목적 : 항균제내성의증가로 H. pylori 3차구제요법의필요성이증가하고있다. 이번연구는 3차구제요법들중 levofloxacin요법과 rifabutin요법의제균율을비교하고자하였다. 대상및방법 : 1차표준치료와 bismuth 기반 2차사제요법에실패한환자들에게 levofloxacin을포함한삼제요법과 rifabutin을포함한삼제요법을투여하였다. 제균율은치료중지 4주후요소호기검사와신속요소분해검사를통하여비교하였다. 결과 : Levofloxacin 투여군총 14명중 8명 (57.1%), rifabutin 투여군총 7명중 5명 (71.4%) 이박멸되어양군간의제균율은통계학적으로유의한차이를보이지않았으나 (p=0.656), 상대적으로 rifabutin 투여군의제균율이더높게나왔다. 결론 : H. pylori 3차제균요법으로 levofloxacin 혹은 rifabutin 기반삼제요법두군모두충분한제균율을보이지못하였다. 향후 levofloxacin과 rifabutin 병합요법이나배양검사에기초한제균치료가필요하겠다. 색인단어 : 헬리코박터파일로리 ; 리파부틴 ; 레보플록사신 ; 3차 ; 구제요법 REFERENCES 1.
