Therefore, this study was conducted to determine whether perioperative intravenous lidocaine would reduce postoperative pain after a laparoscopic cholecystectomy. Methods: Fifty patients undergoing laparoscopic cholecystectomy were divided into two groups; a lidocaine group, in which patients were injected with a lidocaine bolus (1.5 mg/kg) and infusion (1.5 mg/kg/h); and a control group, in which patients were injected with the same volume of saline bolus and infusion. Intravenous lidocaine was initiated before anesthesia was administered and continued for 1 hour postoperatively. The intensity of abdominal and shoulder pain was then assessed 1, 6, 12 and 24 hours after surgery and recorded using a visual analog pain score (VAS) and verbal rating score (VRS). Results: The abdominal pain score (VAS and VRS) was significantly lower in the lidocaine group than in the control group at all times evaluated during the first 24 hours after surgery (P < 0.05). In addition, the shoulder pain score and incidence were significantly lower in the lidocaine group than the control group at 12 hours and 24 hours after surgery (P < 0.05). In the lidocaine group, the incidences of epigastric, right flank, and back pain were lower than that of the control group, but these differences were not statistically significant. Conclusions: Perioperative intravenous lidocaine reduces shoulder and abdominal pain for 24 hours after laparoscopic cholecystectomy. (Korean J Anesthesiol 2008; 54: 69 73) Key Words: laparoscopic cholecystectomy, lidocaine, postoperative pain. 서 론 최근에 수술 후 통증 치료의 중요성에 대한 관심이 증가 되고 있다. 이에 따라 통증을 조절하기 위한 여러 가지 약 제와 방법이 시도되고 있지만, 항상 효과적으로 수술 후 통 증이 치료되고 있지는 않다. 복강경 담낭 절제술은 기존의 개복 담낭 절제술에 비해 수술 절개 부위가 적어 수술 후 통증 제거의 필요성이 줄어들고 횡경막의 기능도 보존되어 호흡기계에 미치는 영향이 적게 되어 회복기간과 재원기간 의 단축 및 이병률 감소가 이루어지므로 선호되는 수술 방 법이다. 1,2) 그러나 최근에 보고된 바에 의하면 복강경 담낭 절제술 을 받은 환자에서도 술 후 통증 호소로 인하여 3,4) 복강경 수술의 장점인 장운동의 빠른 회복과 조기퇴원에 어려움을 주고 있다. 이러한 통증을 감소시키려는 방법으로 진통제의 근주, 국소마취제의 복강 내 투여 및 창상 내 침윤 등이 시 도되고 있고, 5,6) 이외에도 수술전후에 전신적으로 사용된 국 소마취제도 장기능의 회복과 통증 완화에 영향을 미친다는 보고가 있다. 7,8) 이에 저자들은 복강경 담낭 절제술 환자에 서 수술전후에 정주한 lidocaine이 수술 후 통증에 미치는 영향에 관해 알아보기 위해 본 연구를 시행하였다. 논문접수일:2007년 5월 4일 책임저자:강화자, 서울시 동대문구 회기동 1 경희의료원 마취통증의학과, 우편번호: Tel: , Fax: whakang@hotmail.com 대상 및 방법 대상 환자는 전신 마취 하에 복강경 담낭절제술을 받은 미국마취과학회 신체등급분류 1, 2에 해당하는 환자로서, 69

2 대한마취과학회지:제 54 권 제 1 호 2008 본원 임상윤리위원회 승인 하에 환자의 동의를 받고 연구 를 시행하였다. 국소마취제에 과민성 반응 병력이 있는 환 자, 심폐질환이나 통증 관련 질환을 동반하는 환자, 급성 담낭염 환자 그리고 24시간 이내에 진통제를 투여 받은 환 자를 제외한 30세에서 55세 미만의 환자 50명을 대상으로 무작위로 각각 25명씩 lidocaine군과 대조군으로 구분하였다. 군은 마취유도 직전에 lidocaine 1.5 mg/kg 정주하였 으며 기관내 삽관 후부터는 lidocaine 1.5 mg/kg/hr를 피부 봉합 1시간 후까지 회복실에서 계속 정주하였으며 대조군 은 생리 식염수를 동일한 방법으로 정주하였다. 대상 환자들의 연령, 성별 체중 및 평균 수술시간은 Table 1과 같으며 두 군간 차이는 없었다. 마취전투약으로 glycopyrrolate 0.2 mg과 midazolam 3 mg 을 마취 유도 30분전에 근주하였고 propofol 2 mg/kg과 rocuronium 1 mg/kg를 정주하여 마취유도후 기관내 삽관을 시행하였으며 마취유지는 산소 2 L/min, 아산화질소 2 L/min, isoflurane vol%로, 근이완은 rocuronium의 간헐적 투여로 유지하였다. 수술은 배꼽하부에 피부를 절개한 후 Trendelenburg 체위 로 Verres needle을 통하여 복압이 12 mmhg로 유지되도록 지속적으로 이산화탄소를 주입하면서 담낭절제술 조작 시 에는 역 Trendelenburg 체위에서 시행하였다. 수술 종료 후 glycopyrrolate와 pyridostigmine으로 근이완을 길항하고 환자 의 의식을 각성시켰으며 모든 수술은 같은 집도의가 시행 Table 1. Demographic Data Group Sex (M/F) 10/15 12/13 Age (yr) 45.6 ± ± 5.7 Weight (kg) 62.8 ± ± 10.2 Operation time (min) 56.8 ± ± 17.5 Values are mean ± SD. 하였다. 통증은 복통 및 어깨 통증으로 분류하여 수술전날 환자 를 방문하여 수술 후 휴식시 통증이 전혀 없을 때는 0, 참 을 수 없는 극심한 통증을 10으로 하는 10 cm visual analog scales (VAS) 9) 와 통증이 기침할 때 없으면 0, 심호흡할 때 없고 기침할 때 있으면 1, 정상 호흡때 없으나 심호흡 때 통증은 2, 정상 호흡시 통증은 3, 정상 호흡때 심한 통증은 4로 하는 verbal rating pain scales (VRS) 10) 에 대해 교육하였 고 수술 후 1시간, 6시간, 12시간, 24시간에 동일인에 의해 관찰하였으며, 그 외 다른 부위의 통증(어깨통증, 우측복부 통증과 상복부 통증, 두통, 허리통증)및 저혈압, 빈맥, 서맥 등의 심혈관계 부작용 그리고 이명, 시각 이상, 두통, 의식 혼돈 이나 간질 등의 신경계 부작용 여부에 대해서도 주의 깊게 관찰하였으며 lidocaine투여에 따른 부작용을 보인 환 자는 없었다. 수술 후 관찰된 복통과 어깨 통증의 점수는 평균값 ± 표 준편차로 표시하였으며 통계적 처리는 Student t-test와 Fisher's exact test로 비교분석하였으며 P값이 0.05 미만인 경우 통계적으로 유의한 것으로 간주하였다. 결 과 수술 1시간 후 복통의 VAS 점수는 대조군과 lidocaine군 에서 5.4 ± 0.8과 3.4 ± 0.6, VRS 점수는 3.8 ± 0.7과 2.0 ± 0.4로 전시간대에서 가장 높게 나타났으나 시간이 경과함에 따라 감소되었고 양군을 비교 할 때 수술 24시간 후까지 lidocaine군이 대조군에 비해 의미 있는 감소를 나타내었다 (P < 0.05)(Table 2). 어깨 통증의 VAS 점수가 가장 높은 시간과 점수는 수술 후 24시간으로 대조군은 2.3 ± 0.6이고 lidocaine군은 1.1 ± 0.4이었고 양군을 비교 할 때 수술 12시간 후 및 24시간 후 에 lidocaine군이 대조군에 비해 의미 있는 감소를 나타내었 다(P < 0.05)(Table 2). The comparison of postoperative side effect between control and lidocaine group. 수술 후 나타난 부작용중 어깨통증, 우측복부 통증과 상 복부 통증, 허리통증의 발생빈도가 대조군에 비해 lidocaine 군에서 적었으나 통계적인 의미는 없었다(Fig. 1). 고 복강경 담낭 절제술은 수술 절개 부위가 작아 수술 후 통증제거의 필요성이 줄어들고 재원기간의 단축으로 경제 적 부담이 적어지는 등 많은 장점이 있어서 사용이 증가 되고 있다 ) 그러나 최근 보고에 의하면 복강경 담낭절제 술을 받은 많은 환자에서 통증을 호소하여 약 32%의 환자 에서는 진통제의 사용이 필요하다고 하나 4,11) 약 10%의 환 자에서는 심한 통증이 발생하지 않는다고 한다. 2-4) 복강경 담낭절제술 후 나타나는 통증은 수술 조작과 용 해된 이산화탄소에 의해 횡경막이 자극되어 나타나는 내장 성 통증이 주원인이며 9,14) 그 외는 개복 후 주로 투관침에 의해 복벽에 생기는 구멍에 의한 체성통증이라고 한다. 11) Joris 등은 15) 복강경 담낭 절제술 후 나타나는 통증을 내장 성, 복벽 및 어깨통증으로 분류하여 관찰 할 때 수술 후 첫 8시간까지는 내장성과 복벽통증이 어깨 통증보다 심하게 나타나나 수술 24시간 후에는 복통이 감소되므로 어깨 통 증이 더 심하게 나타나고 발생 빈도는 12%에서 80%이라고 한다. 16) Elhakim 등도 5) 복강경 담낭절제술 환자에서 내장성 통증과 어깨 통증이 가장 많이 나타나며 어깨 통증은 40% 의 환자에서 나타나고 Kim 등은 9) 37.5%, Kang 등은 17) 45% 의 환자에서 어깨 통증이 나타났다고 한다. 본 연구에서도 48%의 환자에서 어깨 통증이 나타났으며 수술 후 24시간에 최고의 통증 점수를 나타냈다. 그러나 복통은 수술 후 1시 간에 최고의 통증점수를 나타내었으며 수술 후 24시간 내 에 감소되었다. 찰 내시경 후 발생되는 통증에 영향을 미치는 원인들에는 기복강에 사용된 잔여가스의 양, 18) 종류 19) 및 온도 20) 또는 수술시간과도 21) 연관되며 술 전의 담낭염 등의 염증은 중추 신경계를 민감하게 하여 술 후 통증을 악화시킨다고 한 다. 22) 따라서 본 연구에서는 급성 담낭염 환자를 제외하였 다. 내시경 수술 후 발생되는 통증의 정확한 기전은 규명되 지 않았지만 가능성 있는 기전으로는 횡경막의 긴장과 횡 경막 신경자극이며 23) 기복강에 의해 횡경막의 오목한 상태 가 증가되므로 횡경막의 긴장이 증가되고 또한 기복강은 내장 표면 긴장의 소실을 초래해서 간에 부착된 횡경막의 무게를 증가시켜서 어깨 통증을 유발한다고 한다. 24) 그 외의 다른 기전은 전신적 이산화탄소 혈증에 의해 교 감신경계가 흥분되므로 국소조직의 염증 반응이 확대되어 서 나타나는 것과 25) 기복강 후에 발생되는 내장점막의 허혈 에 의한 것 등이 있다고 한다. 26) 복강경 담낭 절제술 후 발생되는 통증을 감소시키는 방 법들에는 국소마취제의 복강내투여 5) 및 창상침윤, 6) 유출관 을 통한 주입된 가스의 지속적 제거, 저압력 가스주입 등이 있으나 27) 국소마취제를 이용한 통증제거 방법이 널리 연구 되고 있다. Amide계열의 국소마취제인 lidocaine은 steroid제 제와 구조상 유사해서 강력한 항염증 작용을 가지므로 수 술 후 조직에서 유해 염증물질의 작용과 방출을 억제하므 로 진통효과가 크다. 28) 전신적으로 투여된 lidocaine은 만성통증, 특히 neuropathic pain에서의 진통효과가 증명되었으며 29,30) 또한 많은 연구에 서 수술 중 전신적으로 사용된 lidocaine이 술 후 통증과 morphine의 소모량을 감소시켰다고 보고 됐으나 lidocaine의 급성 통증에 대한 제통 효과는 아직 논란이 많다. 7,8) Rimback 등은 31) 담낭절제술 환자에서 lidocaine을 수술전 후 24시간동안 정주 시 장기능의 회복이 빠르고 진통제의 사용량이 감소됐다고 하며 Cassuto 등도 7) lidocaine을 2 mg/min 속도로 수술 30분전에 정주를 시작하여 지속적으로 술 후 24시간까지 사용하여 술후 통증과 morphine 사용량이 뚜렷하게 감소되었다고 보고하였으며 lidocaine의 지속적 정주 는 수술 후에만 사용하면 진통효과가 없고 확실한 nociceptive input이 있는 수술 중에 lidocaine을 지속적으로 사용해야만 술 후 진통효과가 있다고 한다. 따라서 lidocaine 정주는 수 술 전부터 시작하여야 술 후 진통효과를 얻을 수 있을 것 으로 생각된다. Koppert 등도 32) 복부수술 환자에서 소량의 lidocaine을 수술전후에 지속적 정주를 하여 술 후 통증과 morphine 사용량을 감소시켰다고 한다. 본 연구에서도 lidocaine을 수술전후에 정주하여 통증점수가 의미 있게 감소하 였다. 전신적으로 사용된 lidocaine의 작용기간과 부위는 잘 알 71

4 대한마취과학회지:제 54 권 제 1 호 2008 려져 있지 않지만 말초에서 neuropeptide의 유리를 억제 한 다고 생각되나 32) 주 치료효과는 central antihyperalgesic 효과 라고 하며 33) mechanoinsensitive nociceptors는 특히 소량의 lidocaine에 예민하게 반응하여 central hyperalgesia의 유도를 예방하므로 술 후 통증 치료에 효과가 있다고 한다. Nagy와 Woolf 등도 34) lidocaine이 척수 후각세포에서 흥분성 glutamate와 tachykinin에 의한 반응을 억제해서 제통효과를 나타 낸다고 하였다. 그러나 수술 후 진통을 목적으로 한 lidocaine 정주는 오 래전부터 사용되어왔으나 7,31,32) 효능여부와 부작용에 대한 논란이 있어왔고 Koppert 등과 32) Rimback 등은 31) 술 후 진 통에 효과가 있다고 하였다. 그러나 De Kock 등은 35) 80명의 대장절제술 환자에서 fentanyl 단독 투여와 fentanyl과 clonidine 병합 투여, fentanyl과 clonidine과 lidocaine의 병합투여 를 비교하였을 때 진통효과의 차이가 없다고 하였다. 또한 Kaba 등의 36) 연구에서는 lidocaine의 지속 정주가 술후 장기 능 회복에서 flatus, defecation time등의 의미있는 감소를 가 져왔다고 하였지만 Wu 등의 37) 연구에서는 단독으로 lidocaine 정주는 뚜렷한 flatus time의 감소를 보여주지 못했다고 한 다. 이러한 차이는 각 연구의 lidocaine 투여기간과 사용량, 투여 방법 그리고 수술 부위 및 수술 종류의 차이에 따른 조직 침윤, 손상 정도의 차이에서 35,37,38) 기인한 것으로 생각 된다. 본 연구와 같은 용량의 lidocaine을 사용한 Koppert 등 의 32) 연구에서 측정된 혈중 lidocaine 농도가 독성농도인 5 μg/ml 이하인 1.9 ± 0.7μg/ml로 본 연구에서도 같은 농도 일 것으로 추측되며 lidocaine 투여에 따른 부작용은 없었다. 결론적으로 내시경 담낭절제술을 받는 환자에서 술 전후 의 lidocaine 정주가 술 후 통증에 대한 진통효과가 있었으 나 이에 대한 연구가 더 필요할 것으로 사료된다. 참 고 문 헌 1.
