Materials and Methods: A total of 20 female Sprague-Dawley rats were obtained at 8 weeks of age and were divided randomly into 3 groups: a control groups underwent sham operation and bladders were excised 4 weeks and 8 weeks after operation each. Other two groups underwent bilateral oophorectomy and bladders were excised 4 weeks (group 1) and 8 weeks (group 2) after operation each, too. Each group has 5 rats. Conventional proteomics was conducted via high resolution 2-D gel electrophoresis, followed by image analysis and protein identification through mass spectrometry. Results: Bladder weights were reduced significantly in group 2 as compared with the 8-week control group. A comparison of the bladders of the oophorectomy group subjects with those of the control group subjects demonstrated that the expressions of 11 proteins were altered Eukaryotic translation initiation factor 5A (elf-5a), chaperone grp 75 precursor, actin-depolymerizing factor, contrapsin-like inhibitor 1 precursor, guanine deaminase, actin, peroxiredoxin 2, phosphatidylethanolamine, putative protein kinase, Keratin complex 2 and Enol protein. Conclusion: The roles of 11 proteins are variable. A part of these proteins have a role of cellular apoptosis, acute inflammation, and muscle contraction. It seems that these changes of proteins have an influence on bladder functions and LUTS. 2 방광에서 양측 난소절제술 후 단백질 변화 서 론 재료 및 방법 에스트로젠이 하부요로에 영향을 주어 여러 가지 요로증상을 보일 수 있다는 여러 연구가 있지만 아직 논란이 많다. 에스트로젠과 프로게스테론 수용체는 여 성 생식기관뿐만 아니라 요도, 방광과 골반근육에서도 발견된다 [1-4]. 방광에서는 에스트로젠 수용체가 방광 정부의 이행상피세포층에서는 발견되지 않지만 방광 의 삼각부 부위에서는 존재한다고 한다 [5,6]. 또한 에 스트로젠은 무스카린 수용체의 변화와 세포외 칼슘이 온의 근세포 내로의 이동을 억제함으로써 직접 배뇨 근의 기능에 영향을 미칠 수 있다 [7,8]. 결과적으로 에스트로젠의 농도 변화는 하부요로의 기능과 증상에 영향을 주게 되는데 순환하는 에스트로젠 농도가 감 소하는 갱년기에는 빈뇨, 야간뇨, 배뇨통, 배뇨근 불안 정으로 인한 요절박, 복압성요실금 등의 하부요로증상 과 반복되는 요로감염 같은 질병의 발생빈도가 증가 할 수 있다 [9,10]. 노화가 진행되면 요실금의 발생이 증가하게 되는데 특히 절박성 요실금의 발생빈도가 높아지고 이는 갱년기 기간에 비례하여 증가하게 된 다고 한다 [11]. 점막 내 혈류의 감소와 허혈 및 점막 투과성의 증가 등으로 인한 감각신경의 손상이 발병 기전으로 제시되고 있으며 이외에도 점막 내에서의 세포자멸사의 증가와 여러 유전체 변이가 연구되고 있으나 아직 발생기전을 설명하기에는 부족함이 많다 [12]. 최근 한 연구에 따르면, 난소절제술 후 방광조직 에서의 단백질의 변화를 살펴본 결과, 난소절제로 인 한 호르몬 환경의 변화가 방광에서 세포고사 과정과 방광근육의 수축력 변화에 영향을 줄 것으로 보인다 고 하였다 [13]. 그러나 난소절제술 후 4주라는 짧은 기간 동안 관찰한 결과여서, 결론을 내리기에는 다소 무리가 있다고 보인다. 따라서 저자들은 본 연구에서 에스트로젠 부족 후 발생하는 하부요로증상의 원인 및 발병기전을 밝히기 위해 단백체학을 이용하여 난 소절제술 후 4주와 8주에 변화되는 방광 내의 단백질 변화를 알아보고자 하였다. 생후 8주 된 Sprague-Dawley 암컷 백서를 12시간 낮/ 밤 주기로 조명을 조정하고 기온 22±1, 상대습도 50±20%로 유지되는 창문이 없는 방에서 사육하였다. 가루사료 (삼양식품, 한국)를 실험기간 내내 기본식으 로 사용하였다. 모든 실험 백서들은 ARENA/OLAW IACUC (Institutional Animal Care and Use Committee) guidebook 2nd edition (2002)의 지침에 따라 다루었다. 일주일간의 안정기간을 지낸 후 백서들을 대조군 10마 리 (sham 수술군)와 양측 난소절제군 10마리의 두 군 으로 나누었다. 대조군은 다시 5마리씩 술후 4주군 5 마리와 술후 8주군 5마리로 나누었다. 난소절제군도 다시 술후 4주군 5마리 (group 1)와 술후 8주군 5마리 (group 2)로 나누었다. 케타민으로 마취한 후 (100mg/kg intramuscularly) 하복부를 절개하여 난소와 방광과 요 관, 요도를 확인한 후에 양측 난소를 완전히 제거하였 다. 복벽은 일반적인 방법으로 봉합하였다. Sham 수술 군은 난소절제군과 같은 방법으로 수술하였으나 난소 는 절제하지 않았다. 술후 항생제 (150mg/kg, ampicillin intramuscularly)를 투여하여 창상감염을 예방하고자 하 였다. 수술 후 4주나 8주에 방광을 적출하였다. 방광을 확인한 후 방광경부 직상방에서 방광을 요도로부터 분 리 적출하였다. 난소절제군에서 육안으로 보아 방광에 변화가 있거나 방광파열의 소견이 있는 백서는 실험에 서 배제하였다. 방광 적출은 실험동물을 경추를 탈구 시켜 사망시킨 후에 시행하였다. 적출된 방광은 체부 만을 따로 분리하여 무게 (평균무게±표준편차)를 측정 한 후 액화질소 탱크에 보관하였다. 시료 준비 백서의 방광에서 단백질을 German Heart Center의 방법을 이용하여 추출하였다 ( fu-berlin.de/~pleiss/tissue.html). 방광조직은 세척용액 (50mM Tris-HCl (7.1), 100mM KCl, 60mM EDTA, 5.8mM benzamidine, 0.2mM PMSF)으로 세척하고 1시 간 동안 냉동 건조시켰다. 30mg의 조직을 액화질소 안에서 파쇄한 후 100μl 버퍼용액 (7M Urea, 2M Vol. 13, No. 2,

3 이형철 김형지 Thiourea, 4% CHAPS, 100mM DTT, 25mM Tris-HCl (7.1), 50mM KCl, 0.2% Bio-Lyte 3/10 Ampholyt)에 용 해시켰다. 210U DNase를 첨가한 후 실온에서 30분간 방치하였다. 용해되지 않는 물질은 원심분리기로 제거 하였다 (13,000rpm, 20분, 10oC). 단백질 농도는 시판 중인 Bradford reagent (Bio-Rad protein assay kit, Bio-Rad, Richmond, USA)를 사용하여 측정하였다. 조 직은 사용할 때까지 영하 70 C에서 보관하였다. 형화된 IPG strip은 15% uniform polyacrylamide gel로 옮 기고 PROTEAN II Xi cell tank (30mA)에서 실험을 계 속하였다. 젤은 Coomassie Brilliant Blue R250으로 염색 하였다. 염색 후 2DE는 Powerlook 1100 (UMAX, Fremont, USA)을 사용하여 이미지화하였다. 이미지들은 Melanie III (Swiss Institute of Bioinformatics, Switzerland) 소프트웨어를 사용하여 분석하였고. 모든 실험은 재현 성을 위하여 5회 이상 시행하였다. 이차원 전기영동 단백질 동정 일차원적인 젤 분리는 17cm ph 4-7 IPG (immobilized ph gradient gel) strip을 가지고 제조회사의 프로토콜 (Bio-Rad)대로 시행하였다. 1mg의 조직을 IPG strip에 놓고 등전하 초점화 (isoelectric focusing; IEF)는 PROTEAN IEF (isoelectric focusing) cell을 가지고 시행 하였다 (145kVh, 20 C). IEF 후 IPG strip은 2% DTT를 포함한 6M Urea, 2% SDS, 0.05M Tris-HCl (8.8)와 20% Glycerol에서 15분 동안 평형화시킨 뒤 2.5% iodoacetamide를 포함한 같은 용액에서 15분간 평형화시켰다. 평 대조군과 다르게 표현되는 실험군의 단백질 스폿들 은 단백질 서치 프로그램인 ProFound와 MS-FIT와 함 께 질량분석기 (matrix assisted laser desorption/ionization-the time of flight mass spectrometry; MALDI-TOF MS)로 분석하였다. 재료 실험에 사용된 Sprague-Dawley 백서는 안성동물농장 4 pi 7 4 pi A 14.4 B 14.4 Fig. 1. (한국)에서 구입하였고 2DE 시스템 (PROTEAN IEF cell, PROTEAN II Xi cell), Trans-Blot Cell, CHAPS, DTT, SDS, iodoacetamide, ampholyte와 IPG strip 등은 Bio-Rad (Richmond, USA)에서 구입하였다. DNase와 protein pi marker는 Amersham-Pharmacia Korea (서울, 한국)에서 구입하였다. Acrylamide, N,N -methylene-bisacrylamide (Bis), TEMED, ammonium persulfate, trisma base, glycine, methanol, glacial acetic acid, glycerol, potassium chloride, urea, thiourea, phenylmethylsulfonylfluoride (PMSF), ethylenedinitrilotetra acetic acid disodium salt (EDTA), benzamidine와 brilliant blue R-250들 은 Sigma-Aldrich (St. Louis, USA)에서 구입하였다. 결 과 난소절제술 후 4주째의 방광무게는 실험군이 75.5±13.1 mg으로 대조군의 70.4±11.3 mg과 유의한 차이 는 없었으나 술후 8주째의 무게는 실험군이 75.9±14.4 mg, 대조군이 85.8±15.6 mg으로 group 2에서 유의하게 적었다. 외견상 대조군과 실험군의 방광에서는 특이 소 138 대한배뇨장애요실금학회지

6 방광에서 양측 난소절제술 후 단백질 변화 견이 보이지 않았다. 이차원 전기영동 상에서는 약 1,000개의 단백질 스폿이 보였고, 대조군과 비교하여 실 험군의 표현율 변화가 50% 이상 감소하고 200% 이상 증가한 단백질을 의미 있는 단백질로 간주한 결과, 총 11개의 단백질 변화가 관찰되었다 (Fig. 1,2). 1개의 단백질이 (eukaryotic translation initiation factor 5A; eif 5A) 4주째 대조군에 비해 실험군 (group 1)에서 과발현되었으나 8주째에는 (group 2) 감소하는 양상을 보였다. 또한 chaperone grp 75 precursor는 group 2에서만 과발현되었다. Actin-depolymerizing factor, contrapsin-like inhibitor 1 precursor, and guanine deaminase는 group 1에 서만 발현이 감소하였고 8주차에는 정상으로 회복되었 다. Actin, peroxiredoxin 2, phosphatidylethanolamine, and putative protein kinase는 group 2에서만 발현이 감소하였 다. Keratin complex 2과 d Enol protein은 group 1과 2에 서 모두 저발현되었다 (Fig. 3,4) (Table 1). 고 찰 저자들은 여성호르몬을 제거한 후 발생하는 방광내 단백질의 변화를 연구하고자 하였다. 저자들의 이전 연구에서는 난소 절제 후 4주에 변화되는 단백질을 관 찰한 결과로서 eif-5a가 대조군에 비해 실험군에서 과 발현되었고 7개의 단백질 (chaperone grp 75 precursor, guanine deaminase, keratin complex 2, Gelsolin precursor, peroxiredoxin 2, Enol protein, contrapsin-like inhibitor 1 precursor)은 대조군에 비해 실험군에서 저발 현되었다. 따라서 저자들은 변화된 단백질의 성질에 근거하여, 난소절제로 인한 호르몬 환경의 변화는 방 광에서 세포고사 과정과 방광근육의 수축력의 변화에 영향을 줄 것으로 생각하였다 [13]. 그러나 이는 술후 4주만을 가지고 한 연구이므로 좀 더 장기적인 관찰이 필요하였다. 따라서 본 연구에서는 난소절제 후 4주차 와 8주차에 방광 내 단백질의 변화를 보고자 하였다. 본 연구에서는 이전 연구와 다르게 대조군에 비해 8주 차 실험군의 방광무게가 유의하게 감소하였다. 또한 단백질의 발현도 이전 연구와 차이가 있었다. eif-5a는 tumor necrosing factor (TNF)-alpha가 유도 하는 proapoptotic 단백질로서 eif-5a의 증가는 난소절 제술 후에 세포자멸사가 진행됨을 보여주고 있다 [14]. 본 연구에서는 group 2에서 eif-5a가 다시 감소하는 것으로 보아 세포자멸사가 어느 정도 진행된 뒤에는 안정을 찾는 것으로 생각할 수 있겠다. Chaperone grp 75 precursor는 heat shock protein (HSP) 계열로서 HSP 의 과발현은 excitotoxic과 허혈성 세포사를 방지한다 고 한다 [15-17]. 본 연구에서는 group 1에서 발현에 변화가 없었으나 group 2에서 발현이 증가하였으므로 반응하는 데에 시간이 필요한 것으로 보인다. 술후 4주차에는 저발현되었다가 8주차에 정상으로 회복된 Actin-depolymerizing factor, contrapsin-like inhibitor 1 precursor와 guanine deaminase에서 actin-depolymerizing factor는 Gelsolin precursor라고도 하는 단백 질이며 근육병과 관련이 있으므로 이 단백질의 변화 는 방광근의 기능과 관련이 있을 것으로 생각된다 [18]. 급성 염증이 있을 때 감소하는 contrapsin-like inhibitor 1 precursor 단백질이 [19] 있으나 난소절제술 후 변화되는 기능을 연구하기에는 자료가 부족하였다. 또한 Guanine deaminase는 뇌에서 ammoniagenesis와 관 련이 있는 단백질로서 [20] 방광 내에서의 작용에 대 해서는 좀 더 많은 연구가 필요하다. 술후 8주, 즉 group 2에서만 발현이 감소한 Actin, peroxiredoxin 2, phosphatidylethanolamine과 putative protein kinase에서 actin gamma 2와 putative protein kinase 도 방광근과 관련이 있을 것으로 생각된다. 이 단백질 들은 모두 술후 4주에는 큰 변화를 보이지 않았으나 술후 8주에는 발현량이 감소하고 방광의 무게도 감소 하여, 단백질 변화를 관찰하려면 8주 정도의 시간이 필요한 것으로 보인다. Peroxiredoxin은 17에서 24kDa 의 분자량을 가진 핵을 가진 thiol 단백질로서 H2O2나 lipid hydroperoxide, peroxinitrite 같은 넓은 범위의 peroxides를 줄일 수 있는 산화력과 관련된 보편적인 페 록시다아제 (peroxidase)이고 산화성스트레스와 관련이 있다 [21]. 따라서 peroxiredoxin의 감소는 산화로 인한 세포손상에 대한 방어가 약화되었음을 시사한다. Phosphatidylethanolamine은 phospholipid의 한 형태로서 포유류 요로상피의 주된 phospholipid 요소이다 [22]. 따라서 phosphatidylethanolamine의 감소는 요로상피의 감소를 뜻하며 방광의 기능을 변화시킬 것으로 예상 된다. 술후 4주와 8주 모두에서 저발현을 보였던 Keratin Vol. 13, No. 2,
