The three main kinds of vaginitis are bacterial vaginosis, vaginal candidiasis, and trichomoniasis. The purpose of this study was to characterize Lactobacillus plantarum MG989 and L. fermentum MG901 isolated from the vaginas of healthy Korean women in terms of their inhibitory activity against the vaginitis associated pathogens such as Gardnerella vaginalis and Candida albicans. Co-culture experiments showed that the two Lactobacillus strains MG989 and MG901 significantly reduced the viability of G. vaginalis and C. albicans. Also, the two strains were resistant to bile acid up to 1% and their autoaggregation rates were as high as 83.33%. Further studies are underway to demonstrate that the two strains can be applied as pharmaceutical agents for recovering healthy vaginal ecosystem. 1 ( 주 ) 메디오젠 1 MEDIOGEN, Co., Ltd., Seoul 04157, Korea Tel: , Fax: nspaek@mediogen.co.kr 2 인하대학교생물공학과 2 Department of Biological Engineering, Inha University, Incheon 22212, Korea Keywords: Vaginitis, Gardnerella vaginalis, Candida albicans, Lactobacillus plantarum MG989, Lactobacillus fermentum MG INTRODUCTION 질염 (Vaginitis) 이란질의염증으로비정상적인질분비물이나오는상태를말한다 [1]. 질염의원인으로는여러종류가지목되고있는데, 감염, 알레르기, 화학적자극 ( 피임약등 ), 물리적자극, 임신, 항생제의과다투여로질내정상균까지파괴되는경우, 폐경 ( 호르몬변화, 위축성 ) 등질을자극하는많은요인이있다 [2]. 평소에는에스트로겐이질상피세포의글리코겐을유지하면서정상질내세균총 ( 유산균 ) 을유지하나, 폐경이나난소절제, 항생제투여등은질내산도를변화시키고이때문에유산균대신병원균이번식하기시작하여질염으로발전된다. 질염의종류는세균성질염 (bacterial vaginosis), 칸디다질염 (Candidasis), 트리코모나스질염 (Trichomonas vaginitis) 이있으며, 이중세균성질염과칸디다질염이전체발병률의 70~80% 를차지한다 [3, 4]. 세균성질염 (bacterial vaginosis) 이란질내의건강을유지하는유산균총이감소하여혐기성병원균인 Gardnerella va-

2 한국여성의질에서분리한 L. plantarum MG989 과 L. fermentum MG901 의 G. vaginalis 와 C. albicans 에대한억제효과및특성규명 41 ginalis, Mycoplasma hominis, Prevotella spp. 등이질내에서증식하면서발생하게되는질병이다 [4]. 질내에서혐기성병원균이증식하게되면질내휘발성아민화합물이증가하며, 이는세균성질염특유의악취를유발하게된다 [5]. 또한산성으로유지되는질내산도가높아져임신관련질병및성병감염의위험을높인다고알려져있다 [6,7]. 칸디다질염 (Candidasis) 이란질내부의효모균이증식하면서발생하는데, 가장흔한원인균은 Candida albicans 로알려져있다 [8]. 일반적으로약 75% 의여성이살아가는동안한번이상질과외음부의칸디다성질염을겪으며, 45% 의여성이 1 년에 2 회이상의재발을겪는다고알려져있다 [9]. 장기간항생제를사용할때잘생기며임신부나당뇨병환자에게자주발생하는질병으로, 재발률이높은것이문제가되고있다. 세균성질염과칸디다질염의치료는주로 metronidazole, clindamycin 등의항생제를복용또는주입하는방법이사용되고있지만, 치료후에도지속적으로재발하는문제점이있다 [10]. 따라서기존의항생제사용의보완방안으로병원균의제거뿐만아니라질내환경을개선하여질염이재발하는것을방지하는방안등이연구되고있으며, 그대안방안으로살아있는생균제인프로바이오틱스를사용하는방법이주목을받고있다. 여성질내에존재하는유산간균은그람양성으로포도당을발효시켜젖산을생성하여질내산도를 ph 4.5 이하로유지하여병원체로부터질감염을방지하고건강한질상태를유지하는역할을한다 [11,12]. 보호기전으로점액에부착하여병원체의유입을방해하는장벽을형성하거나항균물질은과산화수소, 젖산, bacteriocin 또는 bacteriocin 유사물질등을생성하여질내환경을건강하게유지한다 [13]. 따라서본연구에서는세균성질염과칸디다질염의치료및재발방지를위한대체방안의일환으로한국여성의질에서분리한 Lactobacillus plantarum MG989 와 Lactobacillus fermentum MG901 의프로바이오틱스로의가능성을확인하였고, 질염의주요원인균인 Gardnerella vaginalis 와 Candida albicans 에대한억제효과를확인하였다. 또한 Lactobacillus plantarum MG989 와 Lactobacillus fermentum MG901 의담즙염에대한내성및 autoaggregation 특성을확인하여프로바이오틱스로서의기능확인을통해관련연구의기초자료로사용하고자하였다. 2. MATERIALS AND METHODS 2.1. 균주및시료한국여성의질로부터분리한 Lactobacillus spp. 중항균활성이우수한 Lactobacillus plantarum MG989 와 Lactobacillus fermentum MG901 을선발하여본연구를수행하였다. Lactobacilli MRS (Difco, MI, USA) 배지를사용하여 37 o C 에서정치배양하였다. 유산균주는 MRS 액체배지에 24 시간정치배 양한배양액을 OD (10 8 ~10 9 CFU/mL) 으로조정하여새로운 MRS 배지에 1% 접종하였다. 18 시간동안배양한배양액을원심분리 (4,000 g, 4 o C, 5 분 ) 한후, 0.22 µm 필터를이용하여얻어진제균된상등액 (CFS, cell free supernatant) 을실험에사용하였다. Gardnerella vaginalis KCTC5096 의배양은 10% 말혈청 (Horse serum, Life technologies corp., NY, USA) 을함유한 BHI 배지 (Difco, MI, USA) 를사용하였으며, Anaerocult A (Merck, Germany) 로혐기상태로 37 o C 에서정치배양하였다. Candida albicans SC5314 의배양은 YM 배지 (1% glucose, 0.3% malt extract, 0.5% peptone, 0.3% yeast extract) 를사용하였으며, 37 o C 에서배양하였다 Gardnerella vaginalis 에대한항균활성본연구에서는액체배지희석법으로이용해세균성질염의주요원인균인 G. vaginalis 에대한항균활성을확인하였다. G. vaginalis 에대한항균효과를확인하기위하여, 유산균주의상등액 (CFS) 을 10% 말혈청이포함된 5 ml BHI 배지에 10% 첨가한후, G. vaginalis 를 OD 로조정하여 1% 접종하였다. G. vaginalis 에대한선별된유산균주와혼합배양을통한항균효과를확인하기위하여, MRS 액체배지에 24 시간정치배양한배양액 5 ml 과 G. vaginalis (10 5 CFU/mL) 배양액 5 ml 을혼합하였다. 각항균효과확인실험은 24 시간동안혐기배양한후 G. vaginalis 의생균수를아래와같이측정하여선별균주의항균효과를확인하였다. 항균효과 (%) = [ 대조군 (CFU/mL) 실험군 (CFU/mL)] / 대조군 (CFU/mL) Candida albicans 에대한항진균활성액체배지희석법으로칸디다질염의주요원인균인 Candida albicans SC5314 에대한항진균활성을확인하였다. C. albicans 에대한항진균활성을확인하기위하여, 유산균주의배양상등액 (CFS) 5 ml 를 5 ml YM 배지에첨가한후, 10 4 ~10 5 CFU/mL 로조정한 C. albicans 를접종하여 20 시간동안배양하였다. C. albicans 에대한유산균주의혼합배양을통한항진균활성은 MRS 액체배지에 24 시간정치배양한배양액 (10 5 CFU/ ml) 5 ml 과 C. albicans (10 5 CFU/mL) 배양액 5 ml 혼합한후, 20 시간동안배양하면서 C. albicans 의생균수를측정하여항진균활성을확인하였다 2.4. 유산균주의담즙염내성섭취하는유산균이질염증개선에효과가있었다는연구에기초하여 [14], 선별된유산균주의장내환경과같은담즙염에대한생균력을확인하기위해 Guo 등의방법으로실험을진행하였다 [15]. 선별된유산균주를 MRS 배지에 24 시간전배양한후, OD 으로조정하여새로운 MRS 배지에 1%

3 42 Korean Society for Biotechnology and Bioengineering Journal 31(1): (2016) 접종하여 18 시간배양하였다. 배양액을원심분리 (4,000 g, 4 o C, 5 분 ) 한후얻어진균체는 phosphate-buffer saline (PBS, ph 7) 으로 2 번세척하였고, OD (10 8 ~10 9 CFU/mL) 으로조정하여담즙염에대한내성실험에사용하였다. 담즙염내성은 0.3~1.0% oxgall (Difco, MI, USA) 을포함한 MRS 배지에유산균주의희석액을 1% 접종하고 37 o C 에서 24 시간배양한후생균수를확인하였다 Autoaggregation 유산균주의장내세포내부착능력을간접적으로확인하기위하여 Kassa 등의연구를변형하여 autoaggregation 실험을진행하였다 [16]. MRS 배지에각각 L. plantarum MG989와 L. fermentum MG901을전배양한배양액을 OD 으로조정하여새로운 5 ml MRS 배지에 1% 접종한뒤, 18시간배양하여실험에사용하였다. 배양한유산균은원심분리 (4,000 g, 4 o C, 5분 ) 한후, PBS로 2번세척하였고, OD (10 8 ~10 9 CFU/mL) 으로조정하여균주현탁액 5 ml을 10초간진탕한뒤 5시간동안방치하면서 autoaggregation을확인하였다. 처리직후 (A0) 와 5시간후 (A), 각각 0.1 ml의상등액을취해 0.9 ml PBS와혼합한뒤 600 nm에서흡광도를측정 (A0, A) 하였고, 다음계산식에따라 autoaggregation을계산하였다. Autoaggregation (%) = [(A0 A)/A0] RESULTS AND DISCUSSION 3.1. Gardnerella vaginalis 에대한항균활성건강한질내에는수많은유산균이존재하고있으며, 이들의분포가인종, 연령, 환경에따라변하는것으로알려져있다 [11,17]. 유산균주의분포가다르기에한국여성에서분리한 유산균주의특성을연구하는것이국내여성에맞는질염치료유산균제재를연구하는데있어중요하다. 이에본연구에서는국내건강한여성의질에서분리한 Lactobacillus plantarum MG989 와 Lactobacillus fermentum MG901 을사용하였다. 세균성질염을일으키는주요원인균중 G. vaginalis 는통성혐기성세균으로질내에서증식하면서질내산도를 ph 4.5 이상으로높이고, 이로인해다른병원성세균이증식할수있는환경을만들면서세균성질염증을일으킨다고알려져있다 [18]. 본연구에서 MG989 와 MG901 을세균성질염의주요원인균인 G. vaginalis 를대상으로항균력을확인하였다. 실험결과 (Fig. 1) MG989 와 MG901 의상등액 (CFS) 의항균효과는각각 99.4%, 98.9% 로확인할수있었다 (Fig. 1). 혼합배양을통한항균활성은 MG %, MG % 로각각조사되었다. 유산균은배양과정중젖산, 과산화수소, 박테리오신등을생산하며, 이를통해항균효과를보이는것으로알려져있다 [19] Candida albicans 에대한항진균활성가임기간에있는여성의약 5% 가재발성칸디다질염에걸리며, 치료후에도증세가완전히사라지지않는경우가많다 [20]. 본연구에서 MG989 와 MG901 의칸디다질염의주요원인균인 C. albicans 에대한항진균활성을상등액과혼합배양을통해확인하였다. MG989 와 MG901 배양상등액 (CFS) 의항진균활성은 Fig. 2 와같다. 20 시간배양후, C. albicans 만배양한실험결과 (CONTROL) 대비 MG901 은 84.5% 의항진균활성을나타냈으며, MG989 는 99.9% 의항진균활성을보였다. MG989 와 MG901 의혼합배양을통한항진균활성결과는 Fig. 3 과같다. 20 시간혼합배양후, C. albicans 만배양한실험결과대비, MG901 은 98.5% 의항진균활성을나타냈으며, MG989 는 99.9% 의항진균활성을보였다. 특히 C. albicans 와 Fig. 1. MG901 의혼합배양시에는 20 시간후에 C. albicans 의생균수가 10 CFU/mL 이하로나타나는것을확인할수있었다. 일반적으로 C. albicans 는호기성으로배양하는동안에산을생성하며, 배지의 ph 를낮추기도한다 [21]. Köhler 등의연구결과에따르면, 낮은 ph 상태에서유산균이생성한젖산은 C. albicans 을생장을억제하는중요한역할을한다고알려져있다 [22] 담즙염에대한내성프로바이오틱스로사용되는유산균이장내극한환경에서생존하기위해서는쓸개에서분비되는담즙에대한내성을지녀야한다 [23]. 일반적으로유산균의담즙염에대한내성을확인하기위해서는일반적으로 0.3% 의담즙염이사용되 고있는데 [24], 본연구에서는 1% 담즙염농도까지 MG989 와 MG901 의내성을확인하였다 (Fig. 4). 일반적인체내담즙염농도인 0.3% 에서 MG901 과 MG989 의생균수는각각 CFU/mL, CFU/mL 을보였으며, 1% 의담즙염농도에서도 MG901 과 MG989 의생균수는각각 CFU/mL, CFU/mL 로확인되어, 담즙염에대해내성을보이는것을확인할수있었다. Lin 등의연구결과에따르면, 0.3% 담즙염에서유산균들의생균수가 10 4 CFU/mL 이하로담즙염에민감하다고알려져있다 [25] Autoaggregation 프로바이오틱스로사용되는유산균의경우, 체내에서지속적으로생존하기위해서장및질세포에부착할수있는세

Autoaggregation 능력은세포부착능을간접적으로확인할수있는방법으로, Osset 등의연구결과에따르면, 유산균이질상피세포에잘부착할수록병원체의부착을방지하는능력이더크다고알려져있다 [26]. MG 901 과 MG989 의장내부착능을알아보기위하여 autoaggregation 실험을진행하였다 (Table 1). MG901 과 MG989 는 24 시간후에각각 83.33, 73.33% 의 autoaggregation 효능을나타내었다. Malik 등의연구결과에따르면, L. plantarum CMPG 5300 의 in vitro 상의 autoaggregation 이약 70% 로나타났으며, in vivo 상인질상피세포에서의부착능도 50% 로 in vitro 상의결과와 in vivo 상의결과가연관된다고보고되었다 [27]. 4. CONCLUSION 본연구는한국여성질에서분리한유산균인 Lactobacillus plantarum MG989 와 Lactobacillus fermentum MG901 을통해질염의주요원인균인 Gardnerella vaginalis 와 Candida albicans 에대한억제효과를확인하였다. MG989 와 MG901 의배양상등액과혼합배양처리시 G. vaginalis 와 C. albicans 에억제효과를나타내었다. 담즙염에대한내성과 autoaggregation 을확인한결과, MG989 와 MG901 은 1% 담즙염까지생존률을유지하며, 각각 83.33, 73.33% 의 autoaggregation 효능을나타내는것을확인할수있었다. 본연구를통해 Lactobacillus plantarum MG989 와 Lactobacillus fermentum MG901 균주는향후한국여성에게맞는유산균을이용한질염치료방법및치료제재의개발에있어서의주요한소재자원으로활 용될수있을것이라기대된다.
