The main features of antidepressants discontinuation syndrome are gastrointestinal complaints, dizziness, flu-like symptoms, sleep disorders, balance and sensory disorders, and affective symptoms. It has been postulated that multiple neuronal systems including the serotonin, norepinephrine, and cholinergic systems, and the possible interactions among them are likely to be engaged. They were reported with nearly all antidepressants but the symptoms are somewhat different among the antidepressant classes owing to the different mechanism of action and the pharmacologic properties such as half-life and active metabolite. The symptoms of discontinuation are usually mild and transient, but can range widely in severity. They commence within a few days of stopping medication or reducing the dosage rapidly, resolves spontaneously within 2 weeks. It is important to recognize the symptoms of discontinuation and distinguish them from relapse because misdiagnosis can lead to inappropriate treatment and adversely effect future antidepressant compliance. Received: May 19, 2010 Revised: June 7, 2010 Accepted: June 8, 2010 Correspondence author: Won-Myong Bahk, MD, PhD Department of Psychiatry, College of Medicine, The Catholic University of Korea, St. Mary s Hospital, Yeouido-dong, Yeongdeungpo-gu, Seoul , Korea Tel: , Fax: , wmbahk@catholic.ac.kr 오늘날항우울제는우울및불안증상에대한일차적인치료수단으로서널리처방되고있다. 처방빈도가늘어나면서항우울제의부작용에대한보고와연구들도증가하고있는데, 항우울제를복용하는도중에나타나는부작용뿐만아니라항우울제를중단하였을때나타날수있는부작용들에도연구자들의관심이모아지고있다. 일반적으로항우울제는의존 (dependence) 이나중독 (addiction) 과무관한것으로인식되고있지만, 최초의 tricyclic antidepressants( 이하 TCAs) 인 imipramine 이도입된이래 selective serotonin reuptake inhibitors( 이하 SSRIs), monoamine oxidase inhibitors( 이하 MAOIs), selective serotonin and norepinephrine reuptake inhibitors( 이하 SN- RIs), noradrenergic and specific serotonergic antidepressants( 이하 NaSSAs) 등모든종류의항우울제의복용을중단한직후환자가불편함을호소하는경우는꾸준히보 고되어왔다. 1-6) 임상에서가장많이사용되는 SSRIs의경우항우울제를중단한환자가운데약 1/3 가량에서항우울제중단으로인한증상이나타나기도했다. 4) 사실이들증상은기존증상의반동현상 (rebound phenomena) 이나재발 (relapse) 일수있고, 새롭게발현한신체적, 심리적증상일수도있으며, 또는정말로약물중단자체로인해유발되는금단증상일수도있다. 이가운데항우울제의중단이직접적인원인으로작용해서나타나는일련의증상들을항우울제중단증후군 (antidepressants discontinuation syndrome) 이라고한다. 항우울제중단증후군은비록대부분의경우증상이경미하게지나가지만, 충분히생활이나업무에지장이있을정도의불편함을초래할수있고, 다른정신사회적문제를유발할수있으며, 때로는입원이필요하기도하다는점에서중요하다. 또한항우울제중단증후군을인식하지못할경우다른신체질환혹은정신질환으로오진하게되어환자로하여금불필요한검사와치료를받게할수있으며, 항우울제중단증후군을경험한환자들이항우울제에대한선입 Copyright c 2010 대한정신약물학회 115

2 항우울제중단증후군 견을갖게되어복약순응도를떨어뜨려향후우울증이나불안장애의재발률을높일수있다는점에서그임상적인의의가있다. 7) 이러한증상들은연구자에따라항우울제금단증후군 (antidepressants withdrawal syndrome) 이라고불리기도한다. 두용어의의미는사실상큰차이가없으나금단 (withdrawal) 이란말이전반적으로중독이나의존성에대한인상을줄수있다고하여항우울제중단증후군이라는용어가보다선호되는편이다. 8) 현재 Diagnostic and Statistical Manual of Mental disorders( 이하 DSM-IV) 와 International Classification of Diseases( 이하 ICD-10) 에서제시하는의존성의진단기준에는금단증상이외에도내성 (tolerance) 과갈망 (craving), 약을구하려는행동 (drug-seeking behavior), 약물사용에과다한시간을소비함등과같은특징들을함께요구하고있다. 그러나항우울제를중단한환자는이같은요건을만족시키지못하고약물에대한통제력을유지한다. 금단증상또한벤조다이아제핀계열의항불안제, 알코올, 마약등에서나타나는금단증상들과는차이가있으며, 항전간제, β-차단제, 질산염제재, 이뇨제, 교감신경자극제, 헤파린, 타목시펜, 항파킨슨약물, 항정신병약물등에서도나타날수있는비특이적인것들이다. 9) 두가지예외가있다면 tranylcypromine 과 amineptine 인데, 이항우울제들은주로 dopamine 에작용하며현재는거의처방되지않고있지만과거의존과남용에대한보고가있었다. 10) 따라서이논문에서도항우울제중단증후군이라는용어를사용하도록하겠으며, 항우울제의복용을중단함으로인해나타날수있는여러증상들과이에대한논의들에대해살펴볼것이다. 임상의는항우울제중단증후군의증상들에대해분명히인식하고있어야하며, 그발생가능성을항상염두에두어야한다. 하지만증상이광범위하고혼재된상태로나타나고, 환자에따라증상의강도와기간도차이가나기때문에항 우울제중단증후군을인지하기란쉬운일이아니다. 1997년도의조사에따르면정신과의사의 72%, 일반의사의 30% 만이항우울제의중단으로인해유발되는증상들에대해인식하고있었다. 11) 따라서보다체계적인분류와기술을시도하려는노력이계속되고있으며, 현재항우울제중단증후군은어지럼, 소화기계증상, 감기유사증상, 이상감각증상, 수면증상, 우울, 불안, 초조등의정신증상등과같은 6개의증상군으로분류하고있다 (Table 1). 이가운데비교적흔하게나타나는증상들은어지럼과오심, 기면과두통이었고, 드물게나타나는증상으로는조증과경조증, 파킨슨증상등이있었다. Berber 13) 는전형적인항우울제중단증후군의증상들을모아 FINISH[Flu-like symptoms, Insomnia, Nausea, Imbalance, Sensory disturbances, Hyperarousal (anxiety/agitation)] 라는암기용용어 (mnemonics) 를제안하기도하였다. 보다세부적으로살펴보면항우울제의종류에따라서나타나는증상들에다소간의차이를보였다 (Table 2). 복통과설사등소화기계증상은콜린성작용이많은 TCAs 에서두드러지는경향이있었다. 3) 이에비해어지럼과이상감각증상은 TCAs보다 SSRIs에서많이나타나며, SSRIs와 SNRIs는항우울제중단증상들간에큰차이를보이지않았다. 5) SSRIs 가운데특별히콜린성작용이강한 paroxetine 은 TCAs에서보이는콜린성반동증상을흔히유발한다. 7,8) bupropion, mirtazapine, MAOIs는항우울제중단증후군의보고들이드문편인데, MAOIs 의경우불안, 초조, 섬망, 환각, 정신증, 인지기능의장애등이나타나는경향이있었다. 2) 임상에서흔히관찰할수있는항우울제중단증후군의또한가지경우는임신후반기동안계속 SSRIs를복용하던산모에게서출생한신생아로부터발생한증상들이다. 산모가복용한 SSRIs는태반을통해태아에게전달되는데출생후신생아가산모로부터분리되면서혈중항우울제농도가떨어지게되면약 1/3의신생아에서 1~10일가량호흡문제, 저혈당, 청색증, 심한울음, 경직, 아프가점수 (Apgar score) 의감소등을나타낼수있다. 사실이들증상은 SSRIs가태아에게직접적으로미친독성작용때문일수도있고, SSRIs 중단으로인한금단증상일수도있다. 14) 다만지속성신생아 Table 1. 4 항우울제중단증후군 이 SSRIs와그유사계열의약물들을복용하고있으므로 SSRIs를중단한후나타나는증상들에연구의초점이맞춰지고있다. 2000년 Black 등 15) 은 1986년부터 1997년까지출간된 46건의사례보고를바탕으로 SSRIs 중단증후군에대한진단기준을제안하였다 (Table 3). 1998년 Haddad에의해서도유사한진단기준이제안된바있으며, 세부적인사항은연구자에따라조금씩차이가나지만공통적으로강조되는 3가지사항이있다. 8,15,16) 1) 전형적인증상의발현. 2) 증상발현의시간적인과관계-대부분약물을중단하거나급격히감량한뒤 1주일이내나타난다. 3) 증상은경미하고, 단기간발현하며, 저절로호전되거나기존에복용하던항우울제를재복용했을경우빠르게호전된다. 하지만많은환자들이진단기준에서제시된전형적인증상외에도다수의부가적인증상들을호소한다. 따라서다양하고비특이적인항우울제의중단증상들을보다정밀하게측정하기위해연구자들은 Discontinuation-Emergent Signs and Symptoms( 이하 DESS) scales 를개발했다. 17) 이척도는 43개의항목으로이루어져있으며, 아직까지타당도에대한평가가완전히이루어진상태는아니지만약물임상시험을비롯한여러연구들에서유용하게사용되고있다. 항우울제중단증후군의경과에대한연구는흔하지않다. 97명의환자를대상으로한자연사연구에따르면항우울제를중단한지평균 2일후에항우울제중단증후군이나타났 Table 3. Proposed diagnostic criteria for the SSRIs discontinuation syndrome 15) Appearance within 1-7 days after discontinuation Occurrence after 1 month of exposure Duration of generally <3 weeks after abrupt discontinuation 2 or more of the following symptoms Dizziness Gait instability Light headedness Headache Vertigo/Faintness Insomnia Paresthesias Nausea or emesis Anxiety Tremor Diarrhea Visual disturbance Fatigue SSRIs: selective serotonin reuptake inhibitors 으며 18) 보통 1주이상지난다음에금단증상이출현하는일은드물다고하였다. 지속기간은길지않은데, 한전향적연구에서조사된항우울제중단증후군의평균지속시간은 7~14 일로항우울제의종류에따라차이가있었다. 17) paroxetine 의경우반동성의불안, 불면증상이수주 ~ 수개월가량지속되기도한다. 19) 운동조절의장애, 이상감각증상, 소화기계증상등항우울제중단증후군에비교적특이적인증상이나타날경우쉽게감별해낼수있을것이다. 그러나항우울제중단증후군은항우울제를끊은뒤에발현하고, 우울감, 식욕의변화, 수면이상, 인지기능장애, 피로감등을포괄하기때문에기존에있었던우울증의재발 (relapse or recurrence) 과구분이어려울수있다. 특히우울증이완전히호전되지않은상태에서약물복용을중단하였거나, 과거우울삽화의재발을여러차례경험한적이있는환자의경우에는우울증의재발가능성을간과해서는안될것이다. 이때는증상의발현시점이중요한감별요인이된다. 우울증의재발은일반적으로항우울제를중단한지 2~3주가지난다음에발생하는경우가많다. 20) 또한항우울제중단증후군은항우울제를다시복용하기시작하면수일내에가라앉는다는것도감별진단에유용한사항이다. 흔하지는않지만항우울제중단증후군의일환으로나타나는조증이나경조증또한양극성장애의첫발병이나재발로오인할수가있다. 양극성장애에서의자연적경과로나타나는조증과항우울제중단으로인해유발되는조증을구분하는것은매우중요한데이는양극성장애의진단여부에따라추후의치료계획이많이달라지기때문이다. 따라서흥분성, 수면의감소, 불안, 초조증상등을호소하는환자에게는즉각적으로자세한병력청취를하는것이권장된다. 일반적으로항우울제를중단한지일주일이내에조증이발현했다면항우울제중단증후군일가능성이높다. 21) 하지만증상만으로둘을구분하는것이어려울수도있는데, 이경우에는항우울제를다시투여해본다음 12~14시간뒤에조증증상이줄어든다면항우울제중단으로인해유발된조증이라고간주할수있다. 22) 그밖에항우울제에의해유발된조증과항우울제중단증후군도구분되어야한다. 이를위해서는증상발현시점에항우울제를복용하고있었는지를자세히조사해보아야한다. 하지만환자의항우울제복용여부를잘알수없을때가많으므로항우울제중단으로인한조증을항우울제에의해유발된조증으로오인하여항조증약물을처방하는경우도발생한다. 21) 118 대한정신약물학회지 2010;21:

5 송후림등 과도한 serotonin 자극에따른 serotonin 증후군도감별해야한다. serotonin 증후군의발생률은 1,000명중 0.4명정도로매우낮은편이지만, 고용량의항우울제를복용할경우 15% 까지도나타날수있다. 전형적인증상은의식상태의변화, 신경근계증상, 자율신경계증상, 생체징후의변화등이며, 보다진단에특이적인증상들로는근육경련, 반사의항진, 초조, 발한, 떨림등이있다. 23) 이같은 serotonin 증후군은증상면에서항우울제중단증후군과유사한점이많지만, 경과상항우울제를복용하는도중에나타나고, 보다심각하며, 저절로호전되기어렵고복용하던항우울제를즉각적으로중단해야하는것이차이점이다. 그러나항우울제를교체하는시점에서도 serotonin 증후군이나타날수있으므로주의깊은감별을요한다. 특히 fluoxetine 을다른항우울제로변경할때 serotonin 활성이증가하게되므로 serotonin 증후군이잘발생한다. 24) 이외항우울제중단증후군은뇌졸중, 신경학적장애, 감염성질환, 복용하던다른약물의부작용등심각한신체질환으로오인되기도한다. 또한항우울제를교체할때기존에복용하던항우울제와새로투여되는항우울제의약리학적기전에차이가있을경우에도항우울제중단증후군이발생할수있으며이증상들이새로투여되는항우울제로인한부작용으로오인될수있다. 25) 0.2) 이유의하게낮았다. 또한 sertraline 과 fluoxetine 에비해 paroxetine 환자군에서보다증상이일찍나타나는경향을보였다 ( 평균 4일 ). 27) 이후에이루어진유사한두실험에서도다른 SSRIs 에비해 paroxetine 에서나타나는항우울제중단증후군이많음을확인할수있었다. 28,29) 항우울제중단증후군의발생에영향을미치는가장중요한요인은복용하던항우울제의종류및투여기간, 그리고감량혹은중단에소요된기간이다 (Table 4). 일반적으로고용량을복용했을수록, 복용기간이길수록호발하는데, 그중단방식과무관하게 5주미만으로항우울제를복용하다가중단했을경우에는나타나는빈도가적었다. 31) 항우울제중단증후군은월경전불쾌장애를치료하기위해주기적으로월경전에만 2주가량단기간항우울제를복용하는여자에게서도드문데, 이러한사실들은항우울제로인해초래된시냅스내변화가안정화되는데시간이걸린다는사실을시사한다. 복용하던항우울제의반감기, 대사산물등약동학적특성도중요한요인이다. 같은종류의 SSRIs라도반감기가 84~ 144시간으로길고활성화대사산물인 norfluoxetine 이존재하는 fluoxetine 에비해, 반감기가 21시간으로짧은 paroxetine 에서항우울제중단증후군이잘나타난다. 32) 5시간내외의짧은반감기를가진 venlafaxine 또한다른항우울제에비해항우울제중단증후군에취약하다. 33) citalopram 다양한임상양상과경과로인해항우울제중단증후군에대한역학적자료는매우제한적이다. 항우울제를복용하였던 352명의환자의무기록을후향적으로조사한연구에서항우울제중단증후군의발생비율은 setraline 2.2%, fluvoxamine 14%, paroxetine 17.2%, clomipramine 30.8% 로항우울제마다편차가컸다. 4) 또한이연구에서는환자의나이, 성별, 진단에따라특별히항우울제중단증후군의위험성이차별화되어나타나지는않았다. 최초의전향적인무작위연구는 SSRIs가임상에도입된이후에야이루어졌는데, 공황장애환자에서 paroxetine 과위약을 12주간투여하다중단한뒤 2주간관찰하며나타나는증상들을비교하는것이었다. 이연구에서나타난부작용의빈도는 paroxetine 군에서 34.5%, 위약군에서 13.5% 였으며위약에비해유의하게높은빈도로나타난부작용은어지럼증이었다. 26) 3종의 SSRIs 를복용하던환자들을대상으로 5~8일간항우울제대신위약투여를시도한이중맹검무작위연구에서는 paroxetine (66%) 과 sertraline(60%) 에비해 fluoxetine(14%) 이유의하게낮은발생률을보였다. DESS 척도상에서도 sertraline ( 평균 : 5.7), paroxetine( 평균 : 7.8) 에비해 fluoxetine( 평균 : Table 4. 6 항우울제중단증후군 도경도의일과성항우울제중단증후군을일으킬수있으나 30~35시간의비교적긴반감기와덜강력한대사산물로인해보고되는빈도는적다. 34) 항우울제의중단에소요되는기간도영향을미친다. SSRIs 복용을중단한 74명의환자들을대상으로한설문조사에서급작스럽게약물을중단한환자와점진적으로약물을감량중단한환자들사이의 DESS 점수는 12.0과 5.9로두배가량의차이가났다. 또한사전에항우울제중단증후군에대한정보를제공받지못한환자와제공받은환자의 DESS 점수도 8.9와 5.5로차이가있었다. 35) 그밖에환자의연령, 식사와수분공급상태, 약물순응도, 같이복용중인항고혈압제, 항전간제, 항히스타민제등에의해서도영향을받을수있다. 36) 효과는전시냅스부위의 α-2 자가수용체를점진적으로둔감화 (desensitization) 되면서음성되먹임기전을감퇴시키는데이러한측면으로 NE의활성증상이설명되기도한다. 5,39) TC- As나 paroxetine 과같은항콜린성작용이강한약물들은 muscarine 수용체를상향조정 (up-regulation) 시키며, 약물이중단되면일시적으로콜린활성이증가 (cholinergic rebound) 하여절박뇨, 타액과다분비, 설사, 복통, 두통, 파킨슨증상등운동조절과관련한문제가나타나게된다. 3) 이외에도항우울제중단증후군을 N-methyl-D-aspartate( 이하 NMDA) 수용체증가와관련한 glutamate 활성, 산화질소 (nitric oxide), GABA 등이작용하는스트레스반응의일종으로보는시각도있다. 40) 여러가지가설들이제시되고있지만, 아직까지확증적인설명은없다. 항우울제중단증후군에는 serotonin( 이하 5- HT) 뿐만아니라 norepinephrine( 이하 NE), acetylcholine ( 이하 Ach) 작용이모두관여하고있을것으로생각되고있으며, 개인의유전적성향도영향을미칠수있다. 9,37,38) 이론적으로항우울제중단증후군은약물이신경계에새로운항상성을성립한이후에야발생가능하다. 그러다가약물의중단으로인해성립된균형이와해되면서나타나는증상이항우울제중단증후군이라고할수있다. SSRIs 를장기간사용했을경우시냅스내의 5-HT가증가하면서후시냅스부위의 5-HT 2 수용체가하향조정 (down-regulation) 되고, 항우울제의갑작스런중단으로시냅스내의 5-HT가결핍되어도하향조정된수용체는수일에서수주동안그상태를유지한다. 이로인해직, 간접적으로 NE, dopamine, γ- aminobutyric acid( 이하 GABA) 등다른신경전달물질체계에영향을미쳐각종신체적, 정신적증상들을심화시킬수있다. 38) 또한 SSRIs를장기간사용하여 5-HT가증가하면 NE 신경세포를억제하는효과가생기는데 SSRIs의중단은 NE의억제를차단하여반동성의고혈압, 두통, 안절부절못하는증상등을유발한다. 37) SNRIs와 TCAs, MAOIs 또한 5-HT 활성을증가시키므로동일한기전이적용된다. 한편 MAOIs는 α-2 자가수용체와 dopamine 수용체를하향조정시키는데 MAOIs의중단은일시적인 NE와 dopamine 의방출을유발하고이로인해인지기능의장애, 공격성, 긴장증, 불면, 흥분성, 편집망상, 환각등의증상이나타날수있다. 2) 또한 SNRIs와같은약물들은직접적으로 NE의재흡수를차단하여시냅스내의 NE를증가시키고전시냅스부의 α-2 자가수용체에대한자극은음성되먹임 (negative feedback) 기전에의해시냅스내의 NE를감소시킨다. 장기간의약물 항우울제중단증후군에대한인지와예방이우선적이다. 예방을위해서는항우울제의적용대상과용법에있어정해진규범을준수하는것이필요하다. 우울증이나불안장애외에도과민성대장증후군, 비만, 두통, 불면증등의상태에대해항우울제가대증적으로처방되는경우가많은데투약일정이나중단, 감량에있어보다주의를기울여야한다. 41) 또한임상의가항우울제중단증후군에대해사전에경각심을갖고있으면증상이발생했을때오진과불필요한검사및치료를최소화하고적절한대처방안을강구할수있다. 예방을위해서는환자에게약물복용에대해사전에충분히교육해서임의로항우울제를중단하거나복용을건너뛰지않도록하는것이필요하다. 기존질환이호전되기시작하면대부분의환자는약물중단의유혹을느끼게되며, 특히여성은임신과수유와관련하여항우울제의복용을중단하는경우가많으므로보다주의해야한다. 항우울제중단증후군의위험요인들에대해서도고려해야한다 (Table 4). 약물을중단해야할경우에는환자에게항우울제중단증후군의발생가능성에대한정보를제공하고 6~8주에걸쳐점진적으로감량하도록한다 (Table 5). 하지만임상실제에서항우울제를감량할때는개개인의반응을보며결정해야할때가많다. 특히반감기가짧은 paroxetine 이나 venlafaxine 을감량할때는보다점진적인감량이요구된다. 때로는점진적인감량후에도항우울제중단증후군이나타날수있고, 증상이심하거나장기간지속되기도한다. 이러한경우역시 paroxetine 이나 venlafaxine 을복용하던환자에서비교적많은데, controlled-release(cr) 형제재 (Paxil CR ) 나 extended-release(er) 형제재 (effexor XR ), 그리고 venlafaxine 의광학적이성질체인 desvenlafaxine(pristiq ) 을복용하던사람에게도증상이나타날 120 대한정신약물학회지 2010;21:

TCAs의복용중단으로인해나타나는콜린성반동증상에대해서는대증적으로 atropine이나 benztropine을투여하면도움이된다. 45) 이러한방법은 TCAs의재복용을원치않는환자에게대안이될수있다. 그밖에제시되고있는방법으로는반감기가짧은항우울제를반감기가긴항우울제로대체하는것이있다. venlafaxine 의중단으로유발된항우울제중단증후군을 fluoxetine(5~20 mg/day) 을 1~2주동안처방하여해결할수있다. 20) 같은경우에대하여 Benazzi 46) 는기존에복용하고있던용량의 venlafaxine 과 fluoxetine 20 mg을병용하다가 5일간격으로 venlafaxine 을절반씩줄여나가중단하고, 1주일뒤에 fluoxetine 을다시 10 mg으로감량하여 5일간유지하다가중단하는방법을제안하였다. 일반적으로장기간동안 SSRIs와 SNRIs를병용하는것은 serotonin 증후군을초래할가능성으로인해권장되지않으나추가하는 fluoxetine 의용량이 5~20 mg로소량이고, venlafaxine 의감량이조속히이루어진다면안전한것으로보인다. 지금까지살펴본바에따르면항우울제중단증후군이란항우울제를장기간복용하던환자가항우울제를갑작스럽게중단하거나감량하였을때나타나는일련의신체적, 정신적증상들을포괄하는용어이다. 대표적인증상으로는소화기계증상, 어지럼, 감기유사증상, 수면장애, 평형과감각의장애, 그리고정신적인증상들이있었다. 이러한증상들은장기간항우울제의복용으로인해형성되었던신경계의균형이깨어지면서 5-HT, NE, Ach 등의전달체계가서로상호작용하며나타나는것으로알려졌다. 항우울제중단증후군은거의모든종류의항우울제에서보고되고있지만, 작용하는수용체, 반감기, 활성화대사산물등항우울제가가진고유의작용기전과약동학적특성에따라나타나는증상의양상과빈도에차이가난다. 현재까지의보고들에따르면항우울제중단증후군은 paroxetine 과 venlafaxine 에서가장잘나타나고, fluoxetine 에서가장덜나타나는경향을보였다. 항우울제중단증후군은 6~8주에걸친점진적인약물감량과충분한환자교육을통해예방할수있으며, 만일증상이발생했을경우라면대증적으로접근한다. 대부분의증상들은경하고일과성으로지나간다. 약물을중단혹은감량한지수일내에발생하며 2주이내에저절로소실된다. 그러나증상의정도와지속기간은복용하던약물의종류와개인의특성에따라다양할수있다. 필요할경우에는기존에복용하던항우울제를다시투여하는것이증상소실을촉진시키며, 기존의항우울제를반감기가긴 fluoxetine 으로교체하는방법도유용할수있다. fluoxetine 은점진적인감량스케줄을적용하지않아도된다는권고도있다. 20) 가장중요한것은사 121

8 항우울제중단증후군 전에경각심을갖고주의깊은관찰을통해항우울제중단증후군을분명히인지하고기존질환의재발등을비롯한다른상황들과구분해내는것이다. 항우울제중단증후군을간과하거나오진할경우에는부적절한검사나치료를하게되어환자를위험에노출시키고향후항우울제에대한복약순응도를떨어뜨리게만들어병의경과를악화시킬수있다. 이러한항우울제중단증후군의보고는연구자들간에항우울제가의존성을유발하는가에대한논쟁을촉발시켰으며임상적으로도매우중요한이슈가되었다. 그이유는오늘날광범위하게사용되고있는항우울제에의존성이있다고밝혀질경우의사는환자들의알권리충족을위해정확한정보를제공해야하며, 동시에이는환자들로하여금약에대한두려움을갖게하여치료로부터멀어지게만들수있기때문이다. 일반적으로항우울제는비특이적인금단증상외에 DSM-IV와 ICD-10에서요구하는다른요건들을만족시키지못하고약물에대한통제력이유지된다. 하지만의존성에대한논쟁은아직도끊나지않은상태로항우울제에의존성이있다고주장하는측에서는항우울제의경우 DSM-IV와 ICD-10 에서제시한의존성의진단기준들을반드시모두충족시켜야할필요는없다는의견을제시하고있다. 47) 용어적인측면에서도대립은남아있다. 중독과갈망을연상시키는항우울제금단증후군이란용어대신항우울제중단증후군이라는용어가선호되고있기는하지만여전히항우울제금단증후군이라는용어를고수하고있는연구자들도많다. 이같은혼선에대하여 Haddad 9,10) 는중독과의존이란말이갖는모호함과상대성을지적하였으며, 의미론적인논쟁을지양하고현재까지밝혀진정보들을바탕으로환자들과정기적인대화를해나갈것을권유하였다. 향후지속적으로항우울제중단증후군에대해통제화된조사가다각도로시행되어야할것으로보이며, 그동안간과되어왔던항우울제의의존성문제에대해서도충분한논의를통해개념을정립하고환자들에게정확한정보를제공하는것이필요하겠다. 항우울제중단증후군이란장기간동안항우울제를복용하다가갑작스럽게감량혹은중단함으로써초래되는다양한신체적, 정신적반응들을말한다. 주된양상은소화기증상, 어지럼, 감기유사증상, 수면장애, 평형과감각의장애, 그리고정신적인증상들이지만, 보다광범위하고비특이적인증상들도포함한다. 항우울제중단증후군은 5-HT, NE, Ach 등의전달체계간상호작용의결과이며, 거의모든항우울제에서보고되고있으나그양상은약물고유의작용기전 과약동학적특성에따라차이를보인다. 대체적으로약물을중단한지수일만에발생하여 2주이내에가라앉으며, 증상은경미하다. 하지만때로는충분히생활이나업무에지장이있을정도의불편함을초래할수있고, 기존질환의재발이나다른상황으로오인되어불필요한검사나치료를시행하게만들기도한다. 결과적으로이는환자의복약순응도를떨어뜨려향후경과에부정적인영향을주게된다. 항우울제중단증후군은점진적인약물감량과충분한환자교육을통해예방할수있으며, 증상이발생했을경우에는대증적인치료를고려한다. 임상의는이와같은항우울제중단증후군의발생가능성에대해항상주의를기울이고있어야한다. 중심단어 : 항우울제중단증후군 항우울제금단증후군.
