It is an infrequent but serious complication of the revascularization of peripheral arterial disease, encountered in surgical bypass and thrombolytic procedures. We present two cases of intraarterial thrombolysis and percutaneous transluminal intervention of the lower extremity arteries, complicated by the development of compartment syndrome. 되는 우측 하지의 간헐성 파행을 주소로 내원하였다. 환 동맥의 완전 폐쇄 소견과 우측 후경골동맥과 비골동맥 자는 고혈압과 당뇨병 및 뇌혈관질환의 병력은 없었으 으로의 측부순환이 관찰되었다. 동맥 내로 유로키나제를 며 흡연력도 없었다. 내원 당시 시행한 흉부 단순촬영, 심 40만 단위 사용하며 우측 대퇴천동맥 원위부와 우측 슬 전도, 경흉부 심초음파도 소견은 정상이었다. 진찰 소견 와동맥에 풍선 혈관성형술을 시행하였으나, 우측 대퇴천 상 우측 슬와동맥과 우측 족배동맥의 맥박이 촉지되지 동맥 원위부에 50%의 잔류협착, 우측 슬와동맥에 60% 않아 시행한 자기공명 혈관촬영술상 우측 슬와동맥, 우 의 잔류협착, 전경골동맥과 후경골동맥의 분기부에 40% 측 전경골동맥, 우측 후경골동맥에 완전 폐쇄 소견이 관 의 잔류협착 소견이 관찰되었다. 유로키나제를 시간당 7 찰되었다(Fig. 1). 10만 단위의 속도로 18시간동안 사용한 후 우측 대 혈관조영술상 우측 대퇴천동맥의 원위부와 우측 슬와 퇴천동맥 원위부와 우측 슬와동맥에 보강 풍선 혈관성형 술을 시행하였다. 시술 후 시행한 혈관조영술상 우측 대 퇴천동맥 원위부와 우측 슬와동맥에 각각 10%와 40% 의 잔류협착 소견이 관찰되어 우측 슬와동맥 원위부에 서 후경골동맥으로의 이행 부위에 스텐트(7 60 mm, SMART, Cordis Co., Miami, FL, USA)를 삽입하였 다. 추적 혈관조영술상 우측 슬와동맥 원위부에 잔류협 착 소견은 없었다(Fig. 2). 시술 직후 우측 족배동맥에 맥박이 잘 촉지되었으나, 시술 후 20시간 경과하여 환자는 우측 하지의 동통을 호소하였다. 우측 종아리의 종창 소견(우측 종아리 둘레 40.6 cm, 좌측 종아리 둘레 35.6 cm)이 관찰되었고, 우측 발목 원위부에 감각 소실이 있었다. 감각 소실이 계 속되고 발목 관절의 구축이 발생하였으며, 시술 후 4일 째 creatinine kinase가 8,499 IU/L로 증가하여 재관류 손상으로 인해 발생한 근육괴사 및 부종 그리고, 이차적 인 우측 비골신경 손상을 의심하여 시행한 근전도 및 신 경전도검사상 우측 발목 부위의 비골신경과 경골신경 손 상을 보였다. 이후 환자는 물리치료를 받으면서 우측 종 Fig. 1.
