We investigated the changes in serial serum NSE levels in patients with acute cerebral infarction. Methods: We measured serial serum NSE levels at 24, 48, 72, and 96 hours, and 2 weeks after the onset of cerebral infarction in 30 patients (15 territorial and 15 lacunar infarctions). We also measured the NSE levels in age-matched controls (n=15) who had no evidence of acute stroke or other neurological disorders. The NSE level was measured using a radioimmunoassay. Results: The initial serum NSE level was significantly higher in the cerebral infarction group than in the control group (6.6±2 vs 4.7±1.6 ng/ml [mean±sd], p=0.006). This difference was also observed between the territorial and lacunar infarction groups until 72 hours after the cerebral infarction. The serum NSE level peaked at 72 hours after the infarction in both lacunar and territorial infarction groups. The correlation between the NSE level and the score on the NIH Stroke Scale was strongest at 48 hours after the cerebral infarction (r=0.469). Conclusions: Serum NSE level can be a good indicator for distinguishing lacunar from territorial infarction during the acute stage of cerebral infarction. 2 구본대정경천 급성기의열공성뇌경색과영역성뇌경색의혈청내 NSE의농도변화를시간간격을두고관찰하였다. 대상과방법 1. 대상및정상대조군 환자군은급성뇌경색으로입원한영역성및열공성뇌경색환자로선정하였다. 뇌경색환자군은증상발현후 24 시간이내에내원한환자로내원 3개월내에뇌경색, 뇌출혈, 뇌외상, 중추신경계감염증, 심장질환등의병력이없는환자를대상으로하였다. 과거력상혈액질환자가면역질환, 중추신경계종양을비롯한각종종양이의심되거나채혈당시폐렴이나요로감염등의염증이있는환자는배제하였다. 뇌경색환자군은내원시임상병력청취, 신경학적검사, NIH 뇌졸중척도, 혈액검사, 소변검사, 심전도검사, 흉부 X-선촬영등을하였다. 뇌영상으로 CT나 MRI 촬영을하였고뇌혈관영상으로뇌혈관조영술 (cerebral angiography) 이나자기공명혈관조영술 (magnetic resonance angiography) 을실시하였다. 대조군은정상인을대상으로환자군과연령을짝지어선정하였다. 채혈당시폐렴이나요로감염등의염증소견이있을경우제외하였다. 혈액질환, 자가면역질환혹은악성종양등의과거력이의심되거나뇌경색, 뇌출혈, 뇌외상, 중추신경계감염증등의중추신경계병력이있는경우도모두배제하였다. 뇌졸중위험인자로고혈압, 당뇨, 고지혈증, 심장질환에대하여평가하였다. 고혈압유무는과거력과함께항고혈압제를투여받았던병력이있는경우이고, 기왕력이없는경우안정상태의앉은자세에서측정한수축기혈압이 140 mmhg, 이완기혈압 90 mmhg 이상일때가 2회이상인경우로정의하였다. 4 당뇨병은당뇨병의과거력과함께인슐린이나혈당강하제를투여받았던병력이있는경우이고, 기왕력이없는경우공복시혈당이 126 mg/dl 이상이거나당 75 g 경구투여 2시간후에측정한혈당이 200 mg/dl 이상인경우로정의하였다. 5 고지혈증의유무는과거력상고지혈증치료를받은병력이있거나, 기왕력이없는경우공복상태에서정맥의총콜레스테롤, 중성지방, 고밀도지단백, 저밀도지단백을측정하여, 공복시혈중저밀도콜레스테롤이 130 mg/dl 이상인경우로정의하였다. 6 심장질환의유무는허혈성심질환, 심방세동, 심장판막질환, 심부전등의과거력이있거나, 심전도나심초음파검사에서진단된경우로정의하였다. 7 본연구는연구윤리위원회의승인을받았으며, 연구에참여하는모든대상자에게검사의목적을충분히설명한후에서면동의서를받았다. 2. 방법환자군은뇌경색발현후 24시간, 48시간, 72시간, 96시간및 2주에거쳐전주와정맥 (antecubital vein) 에서혈액을 2.5 ml 채취하여 4 에서 15분동안 3000 rpm으로 10분간원심분리하여얻어진혈청들을 -20 로냉동보관하였다. 대조군은입원시 1회환자군과동일한방식으로혈청을채취하여보관하였다. 채취된혈청의양은최소 5 ml 이상으로검체채취과정중용혈 (hemolysis) 되거나실온에서 1일이상방치된혈청은대상에포함시키지않았다. 모아진혈청은 NSE kit (ELSA-NSE, CIS bio international, USA.) 를사용하여방사면역측정법 (radioimmunoassay method) 으로측정하였다. 8 검사방법은항NSE 단클론성항체 (monoclonal Ab) 를함유한시험관에표준시험관 (standard tube) A-F, 대조시험관 (control tube), 검체시험관 (sample tube) 을각각표시하고각시험관에표준시료 (standard reagent) A-F, 대조시료 (control reagent), 검체 (sample) 를각각 50 ml씩넣은후 125 I 항NSE 단클론성항체를함유한 125 I 항NSE 를 300 ml 넣고실온에서 3시간동안 200 rpm 으로회전시켰다. 회전이끝난후시험관의내용물을완전흡입해내고세척용액 2 ml 로시험관세척을 3차례반복하였다. 시험관의내용물을완전흡입해내고적어도 1분동안감마계측기 (gamma counter, COBRA 5010II, PACKARD, USA) 를이용해활동성을측정하였다. 표준시료 (standard reagent) 의농도와분당계측률 (counter per minute, CPM) 로표준곡선 (standard curve) 을만들고각검체의 CPM 을측정하여표준시료의 CPM 에대한검체의 CPM 의백분율로농도를산출하였다 평가및통계분석뇌경색환자군과대조군의혈청 NSE 의농도를비교하였고뇌경색환자군을영역성뇌경색과열공성뇌경색으로나누어대조군과 NSE 의농도를비교하였다. 영역성뇌경색환자및열공성뇌경색환자들의시간에따른 NSE 농도의변동양상및차이를비교하였다. 또한시간별혈청 NSE 와내원당시의 NIH 뇌졸중척도의상관을비교하였다. 뇌경색환자군과대조군의혈청 NSE 농도차는 Mann-Whitney 검정을사용하였고, 열공성뇌경색및영역성뇌경색군의 24시간, 48시간, 72시간 96시간및 2주의농도차는 repeated measures ANOVA 를사용하였다. 또한열공성뇌경색환자군, 영역성뇌경색환자군및대조군에서혈청 NSE 의농도차는 Kruskal-Wallis 검정을사용하였다. 14 대한신경과학회지제 27 권제 1 호, 2009

3 급성뇌경색에서시간경과에따른혈청신경원특이에놀라제 (neuron specific enolase) 의변화 NIH 뇌졸중척도와의상관성은 Spearman correlation 을사용하였다. 뇌경색발생기전에따라열공성뇌경색, 혈전성뇌경색및색전성뇌경색으로구분하여각군간의 24시간, 48시간, 72시간 96시간및 2주의 NSE 농도차를 repeated measures ANOVA 를사용하여분석하였다. 통계분석은 SPSS 14.0 프로그램을사용하여유의수준 p<0.05 로검정하였다. 결과 시상이 3예, 뇌실주위백질 (periventricular white matter) 이 2예, 중뇌가 1예에서나타났다. 그중입원후신경학적결손의정도가심해지는진행성뇌경색의경과를보인예가 4예이고뇌경색이재발한경우가 2예였다. 영역성뇌경색군의내원당시 NIH 뇌졸중척도는 12에서 20 점이었고 ( 평균 =15.3±2.6), 열공성뇌경색군은 4에서 10점으로 ( 평균 =6.5±2.0) 으로두군간에유의한차이가있었다 (p<0.001, Mann-Whitney U test). 1. 뇌경색군과대조군의연령및위험인자분석뇌경색군은총 30명으로이중영역성뇌경색군이 15명, 열공성뇌경색군이 15명이었으며대조군이 15명이었다. 뇌경색군의평균병원내원시간은 14±4.6 시간으로 ( 영역성뇌경색군 14±3.1 시간, 열공성뇌경색군 14±5.6 시간 ) 으로두군간의차이는없었다 (p=0.758, Mann-Whitney 검정 ). 뇌경색군과대조군의인구학적변인과뇌졸중위험인자에는차이가없었으며영역공성뇌경색군과열공성뇌경색군간의인구학적변인과뇌졸중위험인자에도차이는없었다 (Table 1). 2. 영역성및열공성뇌경색군의뇌경색위치, 임상경과와 NIH 뇌졸중척도영역성뇌경색군은좌측뇌경색이 8예, 우측뇌경색이 7예였고경색부위로는전두엽이 9예, 두정엽이 6예, 측두엽이 3예, 후두엽이 2예, 소뇌가 2예, 뇌간이 3예였다. 그중입원후뇌경색이진행하여의식의악화및신경학적결손의정도가심해진경우가 6예이고그중 2예에있어서는각각우측중대뇌동맥폐색및기저동맥폐색후흡인성폐렴에의한패혈증으로발병 4주및 6주에사망하였다. 뇌경색이재발한경우가 3예였다. 열공성뇌경색군의임상양상은좌측뇌경색이 7예, 우측뇌경색이 8예이고경색부위로는내포 (internal capsule) 가 7예, 기저핵이 5예, 방사관 (corona radiata) 이 3예, 교뇌가 3예, 3. 뇌경색군과대조군의 NSE 농도차, 뇌경색군에서시간경과에따른 NSE 의농도변화및 NIH 뇌졸중척도와의상관관계뇌경색군에서증상발현 24시간에측정한혈청 NSE 의평균농도는 6.6±2.1 (5.2~13.6) ng/ml 이며, 대조군은 4.7±1.6 (2.2~7.2) ng/ml 으로두군사이에유의한차이를보였다 (p=0.006, Mann-Whitney U test). 이러한 NSE 의변화는뇌경색군에서 2주까지유지되었다 6.2±1.9 (1.7~10.1) ng/ml (p=0.003, Mann-Whitney U test). 이를영역성뇌경색군과열공성뇌경색군및대조군으로나누어보았을때영역성뇌경색군이가장높은 NSE 농도를보였고 (7.5±2.5 ng/ml) 세군간에유의한차이가있었다 (p=0.003, Kruskal-Wallis 검정 ). 뇌경색군에서증상발현 24시간, 48시간, 96시간, 72시간및 2주에실시한혈청 NSE 농도는 72시간에 9.0±9.6 ng/ml 로가장높은수치를기록했다. NIH 뇌졸중척도와의상관관계는 48 시간에 로가장높았다 (Spearman correlation, Table 2). 4. 영역성및열공성뇌경색군의시간경과에따른 NSE 의농도변화영역성뇌경색군과열공성뇌경색군모두에서증상발현 24 시간, 48 시간, 72시간, 96시간및 2주에실시한혈청 NSE 농도는 72시간에 9.0±9.6 ng/ml 로가장높은수치를기록했지만 Table 1. 4 구본대정경천 ( 영역성뇌경색군 =11.9±12.9 ng/ml, 열공성뇌경색군 =6.1± 2.0 ng/ml) 각군내의시간에따른농도차는경계선의유의성은있었지만통계적유의성을보이지않았다 (p=0.089, repeated measures ANOVA Tukey 사후검정, Table 3). 영역성뇌경색군은열공성뇌경색군에비하여시간경과에따른 NSE 농도가의미있게증가하였으며이러한차이는 72시간까지지속되었다 (Table 3). 5. 뇌졸중발병기전에따른 NSE 의농도변화뇌경색군을발병기전에따라열공성, 혈전성및색전성뇌경색군으로구분하였으며이들은각각 11명, 11명, 8명이었다. 이들의증상발현 24 시간, 48 시간, 72 시간, 96 시간및 2주에실시한혈청 NSE 농도는각군간에시간에따른농도차는유의하지않았지만 (p=0.149, repeated measures ANOVA), 혈전성뇌경색군이열공성뇌경색군에비하여시간경과에따른 NSE 농도가의미있게증가하였다 (p=0.025, repeated measures ANOVA Tukey 사후검정, Fig.). 고찰에놀라제 (enolase) 는해당경로중 2-phospho-D-glycerate 와 2-phospho-enolpyruvate 의상호전환반응을촉매하는효소로 α, β, γ의 3종류아단위로된이배체로서 αα, ββ, γγ, αβ, αγ의 5가지동종효소가존재한다. 10 그중 γγ 또는 αγ 에놀라제가중추신경계의신경세포및신경내분비세포에주로존재 Figure. NSE 24, NSE 48, NSE 72, NSE 96 and NSE 2wk; neuron specific enolase level (ng/ml) 24 hours, 48 hours, 72 hours, 96 hours and 2 weeks after cerebral infarction. 하여 NSE 라고명명되었다. 10 NSE 는뇌단백질의 1.5% 를차지하며뇌손상을검사하는민감하고유용한지표로뇌경색을비롯하여저산소증, 뇌외상, 간질중첩증,Creutzfeldt-Jacob 병, 지주막하출혈등여러종류의뇌손상에서그정도와예후를알수있는검사로사용되고있다. 3 신경세포손상을반영하기위한 NSE 측정은초기에는뇌척수액으로시행하였다. 뇌척수액의 NSE 농도증가가신경세포손상의정도와상관관계가있어뇌척수액의 NSE 농도변화를신경세포손상의지표로제시하였다. 11 뇌척수액의 NSE 측정은민감도는높지만채취가침습적이고반복측정이어려우며뇌압상승이동반된환자에서는채취가제한된다는단점이있다. 뇌손상에동반된성상세포변형및혈관-뇌장벽 (blood-brain barrier) 의투과성변형으로말초혈액에서 NSE 를측정하는것 Table 2. 5 급성뇌경색에서시간경과에따른혈청신경원특이에놀라제 (neuron specific enolase) 의변화 이가능하다는것이밝혀진후 NSE 의혈청농도를측정하여신경손상의예측인자로사용되는경우가많아졌다. 12 급성뇌경색에서 NSE 의증가는뇌허혈시세포내로유입된칼슘이온에의한다. 칼슘이온은신경세포내의단백분해효소작용을촉진시켜세포질이동의방해와세포막의손상을통하여뇌세포를파괴시킨다. 이러한일련의생화학반응을통하여뇌세포내에특이적으로존재하는 NSE 의유출이일어나게된다. 13 혈청 NSE 의반감기는약 48 시간이며급성뇌손상후약 3개월까지는정상군에비하여높은 NSE 를유지하는것으로알려져있다. 12 이는뇌손상후 3개월까지는신경손상에의한신경세포의통합성 (integrity) 이복구되지않음을의미한다. 14 본연구에서도뇌경색군에서 2주까지 NSE 의농도가대조군에비하여높은농도로유지되고있었으며, 이러한차이는열공성뇌경색군과영역성뇌경색군에서도확인할수있었다. 혈청내의 NSE 농도는뇌세포파괴정도와상관관계가있으며뇌허혈을포함한중추신경계의파괴성손상부의정도를가늠할수있는유용한지표이다. 12,15 본연구에서도대조군에비하여뇌경색환자군에서의미있는 NSE 의상승이 2주까지지속적으로관찰되었다. 영역성뇌경색환자군과열공성뇌경색환자군을비교해보면영역성뇌경색군이열공성뇌경색환자군보다 2주간지속적으로높은평균 NSE 농도를보였지만두군간의의미있는차이는뇌경색발병 72시간까지였다. 이러한결과가 72시간의초급성기를지나며 NSE 의혈중농도가떨어질때두군간속도차에의한것인지는좀더많은환자를대상으로확인하는것이필요하다고생각된다. 뇌경색발병기전에따른분류에서혈전성뇌경색군이열공성뇌경색군에비하여높은 NSE 농도를보인것은혈전성뇌경색군이열공성뇌경색군에비하여뇌경색영역이크기때문으로생각된다. 급성뇌경색에서일정간격을두고측정한 NSE 의최고치는최종적인뇌손상의정도를반영하며주기적측정을통하여뇌경색의진행정도를예측할수있는지표로도사용될수있다. 16,17 연속적인 NSE 농도측정을통한연구에서 NSE 농도의최고치는 1.9±0.8 일로보통 2일에서 3일사이에나타났고뇌경색발병 3시간부터혈청에서상승이관찰되었으며 12시간까지는완만한하강을보인다. 17,18 본연구에서는뇌경색군에서 NSE 의최고농도는발병 72시간에나타났으며이는영역성뇌경색군과열공성뇌경색군과모두에서같았다. 대부분의연구에서최고농도를보인시점을기준으로그전후의측정치와비교하여통계적유의성을획득하지는못하였는데 17 이는급성기뇌경색의혈청 NSE 농도의상승이비교적짧은시기에이루어지므로하루정도의간격을가지고측정할경우그차이가크지않음을보여준다. NSE 농도는나이, 성별. 고혈압, 당뇨, 고지혈증, 흡연, 혈청단백질등에영향을받지않는다 하지만급성뇌경색환자에서혈중포도당농도가높을경우혈청 NSE 수치가비례하여올라가며이는뇌경색당시의고혈당이신경세포의손상을촉진시키는지표임을시사한다. 19 혈청 NSE 농도는뇌손상용적에비례하여증가한다. 20 본연구에서도우측경동맥폐색에의한뇌경색으로뇌탈출이진행된환자에서뇌탈출이진행하는시기에측정된발병 72시간의혈청 NSE 가 57.7 ng/ml 로급격히증가된것을관찰할수있었다 ( 발병 24, 48, 72, 96시간및 2주후의 NSE 가각각 6.4, 15.2, 57.7, 10.5, 7.3 ng/ml). 하지만본연구에서는경색부위의용적을측정하지못하여정확한상관정도는알수없었다. 다만내원당시 NIH 뇌졸중척도와 NSE 의농도사이의상관관계는뇌경색발병 48시간에채취한 NSE 와 의가장좋은상관을보였다. 내원당시 NSE 농도와 NIH 뇌졸중척도와의상관을보인연구에서상관도는 0.42 정도로나타났고 NSE 농도를시간경과를두고측정하며내원당시 NIH 뇌졸중척도와의상관을구하지는않았다. 21 NSE 의측정은말초성어지럼과중추성어지럼을구별하는유용한지표로도사용될수있으며, 22 여러가지종류의뇌손상에대한각종신경보호제의효과를정량화하기위하여측정되기도한다. 23 이는 NSE 의상승이뇌손상에대한비특이적인반응으로뇌병변의감별보다는뇌손상의정도를측정할수있는검사이기때문이다. 또한급성기혈청 NSE 농도는뇌경색후유장애의장기적예후를반영해줄수있다는연구도있다. 24,25 본연구는급성기 NSE 의농도변화를측정하는것이주된목적으로대상환자에대한 3개월및 6개월후의신경학적상태에대한추적조사가이루어지지못하였다. 본연구에서대상환자수가적었고, 신경영상을통하여뇌경색환자의뇌손상용적을측정하지못하였으며, NIH 뇌졸중척도를연속적으로측정하지않았으며, 그리고초기 NSE 의농도와후유장애의장기적예후를측정하지못한제한점이지적된다. 하지만급성뇌경색에서시간경과에따른 NSE 농도변화를정량화하였으며이를다시영역성뇌경색군과열공성뇌경색군으로나누어두군간의시간경과에따른변화를관찰하였다는데에의의가있다. 결론적으로본연구는급성뇌경색환자의뇌경색의정도를알아보는데혈청 NSE 농도가유용한지표가될수있으며이러한유용성은초급성기에좀더민감할가능성을시사한다.
