Delirium can be appeared by various underlying disease and conditions that affect brain metabolism. Delirium has apparent negative impact on morbidity and mortality of the patent s underlying disease, and it is especially profound in elderly and critical care populations. But, since symptoms of delirium can be calm and fluctuating, nurses and physicians consistently under diagnose delirium in clinical practice. And, in intensive-care unit patients, especially in the neurocritical care population, severe neurological complications could be masked by delirium. Therefore, precise diagnosis and management for the delirium is essential. It is well known to the prevention, the early detection, and treatment of delirium make better prognosis. Therefore early optimal intervention for the delirium is the most important in the patient who has risks. There are a lot of conditions that can predispose or precipitate the delirium. According to many previous studies, it is concluded that patient who has more risk factors for delirium has more likely to have delirium and would have more severe and prolonged delirium. Medical staff s well understanding and continuous awareness about delirium would be most effective diagnostic strategy of the delirium and because delirium developed by interactions among the multiple factors, organized multifactorial approach to the prevention and management of the delirium is highly required. J Neurocrit Care 2011;4:25-34 KEY WORDS: Delirium Neuro-critical care. 서 섬망 은 급성으로 발현하는 가역적인 신경학적 증상으로 기복이 심한 주의집중력 장애를 포함한 전반적인 인지기능장애 및 행동장애를 보이는 것이 그 특징이다 섬망은 라틴어로 라는 어원을 가지고 있으며 이는 미친 또는 헛소리하다 라는 뜻을 가지고 있다 섬망은 급성 뇌부전 급성 기질적 뇌 증후군 - 급성 혼돈 상태 그리고 수술 후 정신증상 등의 많은 동의어를 가지고 있으나 현재 섬망 이라는 용어로 가장 흔하게 쓰인다 Address for correspondence: Sung Sang Yoon, MD, PhD Department of Neurology, College of Medicine, Kyung Hee University Hospital, 23 Kyungheedae-ro, Dongdaemun-gu, Seoul , Korea Tel: , Fax: hsyoon96@khu.ac.kr 론 입원한 환자의 경우 특히 노인 환자에게 섬망은 매우 중요한 의학적 문제이다 섬망은 의 노인 입원환자에서 관찰되며 특히 기저 인지기능장애가 있는 환자에서 더 빈번하다 또한 중환자들은 기저 질환에 따라서 패혈증 낮은 산소 포화도 진정제 및 진통제 등에 노출될 가능성이 매우 높아 섬망은 중환자실 환자에서 정도의 높은 빈도로 관찰되며 중환자실에 입원한 노인의 경우에는 정도로 더 높은 빈도로 관찰된다 연구방법의 차이로 신경계 중환자에서의 섬망의 빈도는 정확히 보고되어 있지는 않으나 허혈뇌졸중 뇌내출혈 - 또는 거미막하 출혈 환자의 가 섬망을 경험하는 것으로 알려져 있으며 급성관상동맥증후군 환자와 비교했을 때 섬망이 더 자주 동반된다는 보고도 있다 섬망은 한번 발생하면 사망률과 합병증의 빈도를 높이고 재원기간을 증가시키고 추가적인 경제적 비용을 발생시키는 Copyright 2011 The Korean Neurocritical Care Society 25

2 J Neurocrit Care 2011;4:25-34 등 원인질환의 예후에 매우 부정적인 영향을 끼친다 따라서 섬망에 대한 정확한 이해를 통한 그 일차적 예방 빠른 진단 처치 및 이차적 예방은 매우 중요하다 장소 그리고 현재 상황에 대한 지남력장애를 보일 수 있으 나 이들 증상은 보호자나 의료진이 직접적으로 물어보지 않 는다면 모른 채 지날 가능성이 높다 섬망의증상 섬망은 그 각각의 주요 증상이 매우 다양한 강도로 나타난다 아주 경한 섬망의 경우 복잡한 문제를 해결하지 못하는 정도의 가벼운 주의집중력장애로 나타날 수도 있지만 심각한 경우에는 집중력 외에 다른 전반적인 인지기능에도 심한 장애를 일으켜 극단적인 경우 자극에 반응은 하지만 다른 의미 있는 작업은 전혀 하지 못하는 상태까지도 이를 수 있다 섬망은 크게 공격성과 불안으로 대표되는 양성증상 을 보이는 과다활동 과다각성 섬망 - 과 심한 집중력장애를 비롯한 음성증상 을 주로 보이는 저활동 저각성 섬망 으로 나눌 수 있으며 두 가지 아형이 심한 기복을 두고 같이 발생하는 경우가 가장 많다 섬망의 주요 증상은 다음과 같다 급성으로발현하고기복이심한의식장애섬망은 급성으로 발현하는 수 시간 며칠 의식의 수준 및 내용의 변화가 특징이다 기복이 심한 환자들은 문진 도중 종종 잠에 들기도 한다 의식의 기복이 심한 환자의 경우 보호자가 자주 교체된다면 각각의 보호자가 환자의 의식수준을 서로 다르게 알고 있을 가능성도 있다 의식의 수준 외에 기억력 언어능력 추상적 사고 등의 다른 인지기능의 장애도 흔히 기복을 보인다 주의집중력장애섬망환자들은 주변자극을 인지하는 능력 및 그 주변자극을 선별해서 필요한 자극만을 받아들이는 능력이 현저히 떨어져 무시해도 되는 소리나 물체에 민감하게 반응하게 되어 산만해 보일 수 있고 정보를 분석하고 한 가지에 집중하는 능력이 떨어지게 된다 주변에서 오는 감각자극이 적어지고 또 이에 대한 반응이 무뎌지는 밤시간에 이 증상들이 더 심해져 이를 이라고 표현하기도 한다 주의집중력장애의 유무를 객관적으로 선별해낼 수 있는 기준으로 가 유용한 것으로 알려져 있다 기억력장애및지남력장애주로 최근 기억에 장애 왜 입원을 했는지 자신의 주치의나 간호사에 대한 기억을 못함 를 보이는 것이 특징이며 날짜 불안또는동요섬망을 보이는 환자들은 그들이 경험하게 되는 지남력장애 혼돈 등으로 인해 불안해 하게 되거나 공격적이 될 수 있다 예를 들어 환자는 자신이 가진 지남력장애 때문에 자신이 현재 집에 있다고 생각하고 의료진을 침입자로 생각해 의료진의 말을 따르지 않고 정맥라인이나 산소튜브를 뽑고 침대에서 뛰쳐나가 난동을 부릴 수 있다 이런 공격적인 행동과 반대로 우울해지고 감정이 둔마되고 식욕 및 의욕을 잃으며 수면 주기에 장애를 일으킬 수도 있다 그 외에도 다양한 감정장애를 보일 수 있는데 불안 슬픔 눈물을 흘리거나 상황에 맞지 않게 행복해 하는 등의 증상이 한 가지 이상 섬망기간 동안 발생할 수 있다 수면장애수면장애 또한 흔한 섬망의 증상으로 수면주기의 이상으로 낮에는 잠을 자고 밤에는 깨어있는 증상을 보이며 이에 혼돈 지남력장애가 동반되거나 밤시간이 되어 환자의 지남력을 유지할 수 있도록 도와주던 자극들이 감소하게 되면 침대에서 떨어지거나 정맥라인 산소튜브 도뇨관 또는 비위장관 등을 스스로 뽑는 상황이 벌어질 위험성이 커진다 지각장애시각 또는 음성 환각이 발생할 수 있으며 흔히 망상도 동반될 수 있는데 정도의 섬망 환자가 이를 경험한다 환각은 과다활동 과다각성 및 저활동 저각성 섬망에서 모두 관찰될 수 있으며 시각 환각이 더 흔하고 위험한 동물이나 이상한 이미지 등의 공포를 주는 환각이 가장 흔히 동반된다 또한 시각이나 청각 외에 미각이나 후각에 관한 환각도 종종 관찰될 수는 있지만 드물다 환자가 겪는 망상은 지남력장애 및 기억장애와 관련이 깊으며 주로 편집증적 망상이나 피해망상으로 나타나는 경우가 많다 신경학적징후섬망의 원인과 관계없이 몇몇 신경학적 징후가 관찰될 수 있다 보행장애 진전 근간대경련 사지나 목의 근긴장이상 등이 나타날 수 있으며 읽기 쓰기의 장애 따라 그리기 등의 시공간적 구성 장애 등이 동반될 수 있다 26

3 Delirium D Lee, et al. 섬망의진단 섬망은 환자에게 세심한 주의를 기울이지 않으면 진단을 내리거나 다른 일차성 정신질환 및 기타 인지기능장애를 동 반하는 일차성 뇌질환들과 감별을 하기가 어렵다 에서 의 섬망은 진단이 되지 않는 것으로 알려져 있으며 특히 노인의 저활동 저각성 섬망은 진단이 더욱 어렵다 섬망 이 잘 진단되지 않는 또 다른 이유는 그 증상의 기복이 크기 때문이며 기저에 인지기능 저하가 있는 노인인 경우 더욱 그 진단이 어렵다 약 의 노인 섬망환자는 기저에 치매를 앓 고 있었던 것으로 알려져 있다 따라서 기저의 인지기능 정 도 및 입원 후에 인지기능의 변화를 객관적으로 측정할 수 있 는 선별검사 방법을 이용하면 섬망환자의 진단과 치료에 큰 도움이 된다 이 가장 임상의사에게 익숙하며 - 가 섬망에 대한 연구목적으 TABLE 1. Confusion Assessment Method (CAM) The diagnosis of delirium by CAM requires the presence of features a and b and either c or d. 1. Acute onset and fluctuating course: evidence of an acute change in mental status from the patient s baseline that changes in severity during the day. 2. Inattention: patient has difficulty focusing attention- for example, is easily distractible or has difficulty keeping track of conversation. 3. Disorganised thinking: patient s thinking is disorganised or incoherent, as evidenced by rambling or irrelevant conversation and unclear or illogical flow of ideas. 4. Altered consciousness: a rating of a patient s level of consciousness as other than alert (normal)-that is, vigilant or hyperalert, lethargic or drowsy, stuporous or comatose. TABLE 2. Diagnostic and Statistical Manual of Mental Disorders: diagnostic criteria for delirium In order to make a diagnosis of delirium, a patient must show each of features 1-4 listed below: 1. Disturbance of consciousness (that is, reduced clarity of awareness of the environment) with reduced ability to focus, sustain or shift attention. 2. A change in cognition (such as memory deficit, disorientation, language disturbance) or the development of a perceptual disturbance that is not better accounted for by a pre-existing or evolving dementia. 3. The disturbance develops over a short period of time (usually hours to days) and tends to fluctuate during the course of the day. 4. There is evidence from the history, physical examination, or laboratory findings that the disturbance is caused by the direct physiological consequences of a general medical condition, substance intoxication or substance withdrawal. 로 가장 널리 쓰인다 정식 진단은 를 주로 이용한다 하지만 중환자실 섬망환자에게는 또는 을 적 용하기 어려운 경우가 매우 많아 - 을 더 선 호한다 은 에서 인 환자를 대상으로 시행했을 때 섬망 진단에 있어서 와 비교할 때 민감도 특이도 를 보인다 하지만 이 방법 또한 진정제 투여에 의한 환자의 의식변화는 반영하지 못하므로 반드시 정기적으로 진정제 투여를 중단 한 상태에서 순차적인 신경학적 검진과 을 측정하는 것이 권장되는데 이는 섬망의 유무 및 원인을 판단 하기 위함이고 섬망으로 오인될 수 있는 치명적인 신경계 합병증을 감별하기 위함이다 신경계 중환자실 환자에서 크 게는 까지 그리고 신경학적 질환이 없던 내과계 중 환자실환자에서도 는 간질 발작을 경험하게 되고 그 대부분은 비경련성 간질지속상태 - 였다는 보고가 있으며 또한 중환자실 입원기간 동안 뇌졸중의 발생 위험도 적지 않다고 알려져 있다 섬망과 감별해야 할 주요 진단으로 치매 우울증 조울증 TABLE 3. The Confusion Assessment Method for the Intensive Care Unit (CAM-ICU) Feature 1: Acute onset and fluctuating course AND Identify an acute change in mental status from the baseline exam OR Identify fluctuating changes in mental status or behavior over the past 24 h that may vary in severity Feature 2: Inattention AND Identify an inability to focus attention, easy distractibility or inability to process components of conversation (e.g. count backwards, say months backwards) Feature 3: Disorganized thinking OR If the patient is verbal (and not aphasic): identify illogical or incoherent thought processes, inability to understand proverbs or inability to perform simple calculations (e.g. How many things are in a dozen? Where does a cactus grow?) If the patient is intubated or nonverbal (and not aphasic): use yes/no questions or letter board to identify illogical or incoherent thought processes (e.g. Can a cat sing? Does wool come from an alligator?) Feature 4: Altered level of consciousness Identify if the patient s level of consciousness is anything other than alert (i.e. drowsy, lethargic, stuporous, comatose or agitated/ combative) 27

4 J Neurocrit Care 2011;4:25-34 정신분열증 등이 있으며 드물게 히스테리 상태 및 간질후 혼돈과 감별이 필요하며 특히 노인의 경우 뇌졸중에 의한 실어증이나 기타 인지기능장애 등도 감별이 필요하다 섬망에서 나타나는 환각이나 망상 같은 생각이나 지각이상은 정신분열증에서보다 기복이 심하고 덜 복잡하며 청각보다는 시각적 환각이 더 흔하다 저활동 저각성 섬망은 피질하혈관치매 나 우울증과 비슷해 보일 수 있다 하지만 치매나 우울증에서도 지남력이나 기억력이 저하될 수는 있지만 의식의 수준에는 이상이 없는 것이 섬망과의 가장 큰 차이점이다 레비소체치매 는 의식의 저하가 기복이 있고 시각 환각이 주로 있다는 점에서 섬망과 아주 비슷하지만 섬망에 비해 아주 긴 병력을 가지고 파킨슨증이 동반된다는 차이가 있다 섬망과 치매 및 우울증의 주요 감별점은 에 정리하였다 이들의 감별을 위해서 환자가 입원하는 경우 환자가 가진 섬망의 위험인자 및 기저의 인지기능장애 여부를 선별해야 하며 환자의 알코올 또는 약물복용력 등을 정확히 파악하는 것이 필요하다 또한 입원시 자세한 신체 검사 및 신경학적 검진이 필요하며 기본적인 혈액검사 및 감염에 대한 검사가 기저 질환을 확인하는 데 도움이 된다 특수한 경우 비타민 엽산 혈중 코르티졸 동맥혈 가스 분석 그리고 독극물 선별 검사 - 도 필요할 수 있다 지속적인 뇌파 - 감시와 뇌 컴퓨터단층촬영 또는 뇌 자기공명영상 - 등 뇌 영상기법 또한 섬망을 정신과적 질환 및 뇌의 구조적 문제와 감별하는 데 큰 도움이 된다 TABLE 4. 5 Delirium D Lee, et al. 섬망의원인및위험인자 의식의 수준은 말초 감각기관들에서 들어온 감각신호를 통한 뇌줄기의 망상체부터 시상 전반적인 대뇌피질에 이르는 상향망향부활성계 기능의 활성화 정도에 따라서 결정되며 또한 이는 아세틸콜린 모노아민 계 등의 여러 신경전달물질의 활성도에 따라서 조절을 받고 그 신경전달물질은 또한 피질 내에서의 뉴런 간의 상호작용에도 영향을 미쳐 의식의 수준뿐 아니라 의식의 내용에 해당하는 주의집중력 기억력 그리고 학습능력에 직접적인 영향을 미친다 그 중 아세틸콜린 활성도 의 결핍 또는 도파민계의 과다 활성이 섬망의 발생과 관련이 많은 것으로 알려져 있다 또한 그리고 도 직간접적으로 위에 기술한 신경전달물질들과 상호작용이 있어 섬망의 발생에 관련이 있을 것으로 예상된다 섬망의 병태생리는 정확히 밝혀지진 않았지만 뇌의 대사와 신경전달 기능에 전반적이고 비특이적인 장애에 의해 상기 어떤 뇌기능의 장애로도 일어날 수 있다고 알려져 있다 뇌의 정상적인 대사나 기능을 방해할 수 있는 어떤 신체질환에 의해서도 섬망은 일어날 수 있고 그 신체적 질환은 정도가 경할 수도 있다 예를 들어 전기적 충격 발열 통증 약물 반응을 포함한 독성물질 뇌손상 저산소증 수술 외상성 쇽 수분이나 영양섭취 부족 수면부족 그리고 특정 약물이나 알코올 중독이나 의존상태는 매우 잘 알려진 섬망의 원인이다 게다가 만성적인 뇌 기능장애를 가진 사람은 가벼운 급성 뇌장애만 그에 더해져도 섬망이 발생할 수 있다 현재까지 많은 연구결과 섬망의 발생에는 매우 다양한 위험 및 유발인자가 관여하는 것으로 밝혀졌다 하지만 대부분의 연구에서 몇 가지의 위험인자가 공통적으로 강조되었고 그것은 다음과 같다 세 이상 고령 그리고 기저의 인지기능장애가 가장 확립된 섬망발생의 위험인자라고 할 수 있으며 섬망은 기저에 인지기능장애가 진행되고 있던 노인인구에서 치매를 진단하게 하는 첫 증상이 될 수 있다 비교위험도를 측정한 다른 연구에서 시력장애 중증질환 인지기능장애 높은 비율 등도 섬망발생의 유의한 위험인자로 제시된 바 있다 또 다른 노인 섬망환자를 대상으로 한 연구에서 가장 흔한 원인으로 감염 뇌혈관질환 을 꼽기도 하였다 또 다른 연구에서는 보호대 적용 배뇨관 설치 영양 부족 가지 이상의 약물 복용 그리고 의학적 시술을 받은 경우의 다섯 가지 위험인자로 이루어진 통계적 모델이 섬망 발생을 예측하 는 데 가장 유의한 수치를 보였다고 밝혔다 이들 연구들을 토대로 볼 때 섬망은 위험인자의 종류가 많을 수록 위험인자의 중증도가 높을수록 발생 확률이 높을 것으로 생각된다 그러므로 기저에 섬망발생의 위험인자를 가진 환자가 다양한 섬망발생의 유발인자들에 노출이 되면서 이들의 복잡한 상호작용에 의해 섬망이 발생하는 것으로 생각된다 지금까지 알려진 섬망발생의 위험인자 및 유발인자는 에 나열하였다 섬망환자의치료및관리 섬망의 치료는 크게 두 가지 방법으로 구성된다 첫째로 원인이 되는 기저질환을 치료하고 둘째로 뇌의 상태를 최적으로 만드는 것이다 이는 섬망환자에게 적절한 산소 수분 및 영양을 공급하고 불필요한 약물의 영향을 최소한으로 줄이며 변비 통증 등을 치료하는 것 등이 포함된다 환자의 정신적 스트레스를 찾아내고 관리해주는 것 또한 중요하다 그러므로 단지 섬망의 원인만 제거해 주는 것은 효과적인 치료가 되지 않을 가능성이 있다 따라서 섬망환자에게는 환자의 뇌기능을 떨어뜨리는 모든 요소에 대한 실질적이고 전문적인 분석과 관리가 필요한 것이다 환자가 심하게 불안해 하며 폭력적이어서 자신이나 주변사람을 해 할 수 있는 상황이 아니라면 비약물요법이 섬망의 우선적인 치료이다 불필요한 움직임을 피하게 하고 되도록 가족들이 직접 치료의 초기부터 환자간호에 가담해서 익숙한 얼굴과 환경을 보게 해주는 동시에 지남력 향상을 위해 시계나 달력을 주변에 두고 시력이나 청력이 좋지 않은 환자들에겐 안경이나 보청기를 사용하게 하는 방법들이 권장된다 중환자실에 입원하는 환자들 중 이상은 수면주기의 장애 및 수면 결핍을 보이므로 이를 예방하기 위해 낮엔 커튼을 걷고 전등을 켜놓고 밤에는 되도록 불필요한 신체 검진을 피하고 전등을 끄도록 하는 것이 좋다 보호대 적용은 섬망이 지속될 위험도를 배 정도 증가시키고 욕창 흡인 그리고 섬망 악화의 직접적인 원인이 될 수 있으므로 되도록 피하는 것이 좋다 이들 일반적 환자관리 방법이 모두 실패했을 경우에 약물치료를 고려해야 한다 일반적인 섬망환자의 관리에 대해서 에 의 가이드 라인을 인용하였다 약물치료는 환자의 섬망의 원인에 따라 선택하며 항정신병약물 중 이 가장 흔히 쓰이고 연구가 많이 되어있다 그리고 같은 비 전형적 항정신병약물은 연구결과가 적어 아직 근거가 부족하지만 29

6 J Neurocrit Care 2011;4:25-34 에서는 또는 을 치료에 추천하고 있다 성인에서 중등도의 섬망증상을 보일 때 은 에서 씩 하루 두 번으로 시작해 볼 수 있다 필요시 네 시간 마다 추가 투여가 가능하며 경정맥 경구 근육주사 경로 모두 투여가 가능하다 을 정맥으로 투여하면 다른 경로로의 투여보다 추체외로장애가 덜 발생하는 것으로 알려져 있으나 등 치명적인 부정맥의 발생가능성이 높아 정맥 투여시엔 주의를 기울여야 한다 따라서 의 정맥 투여는 심전도 감시가 필요하며 연장이 이상이거나 기저치보다 이상 증가하면 즉시 투여를 중단해야 한다 노인 환자인 경우에는 하루에 에서 두 번 투여하는 정도로 반드시 더 낮은 농도에서 치료를 시작해야 한다 또한 모든 환자에 있어서 반응이 있는 치료 약물 농도를 시간 마다 평가해서 조절해야 한다 약물은 서서히 농도를 며칠간 줄이면서 끊어야 하며 TABLE 6. Clinical guidelines to prevent and treat delirium in hospital (British Geriatric Society) Step 1: Identify all older patients (. 65 years) with cognitive impairment using the Abbreviated Mental Test or Mini-Mental State Examination on admission. Step 2: Consider delirium in all patients with cognitive impairment and at high risk (severe illness, dementia, fracture neck of femur, visual and hearing impairment). Use the Confusion Assessment Method screening instrument. Step 3: Identify the cause of delirium if present from the history, examination and investigations, and treat underlying cause or causes - commonly drugs or drug withdrawal, infection, electrolyte disturbance, dehydration or constipation. Step 4: In patients with delirium and patients at high risk of delirium: Do - Provide environmental and personal orientation - Ensure continuity of care - Encourage mobility - Reduce medication but ensure adequate analgesia - Ensure hearing aids and spectacles are available and in good - Working order - Avoid constipation - Maintain a good sleep pattern - Maintain good fluid intake - Involve relatives and carers (carers leaflet) - Avoid complications (immobility, malnutrition, pressure sores, over sedation, falls, incontinence) - Liaise with old age psychiatry service Do not - Catheterise (if possible) - Use restraint - Sedate routinely - Argue with the patient Step 5: Ensure a safe discharge and consider follow-up with old age psychiatry team. Provide family/carer education and support 증상의 호전이 임상증상의 기복 때문인지를 반드시 확인해 야 한다 은 그 자체로 섬망을 일으키거나 악 화시킬 수도 있고 섬망의 치료 및 관리에 있어서 근거가 아직 미약하다 최근 중환자실 환자를 대상으로 전향적으로 이 루어진 섬망에 대한 연구에서 이 섬망의 발생률을 증가시킨다는 연구결과가 있어 중환자실 환자에 있어 계열 약물의 사용시엔 주의를 요한다 그러 나 섬망이 알코올 금단이나 금단에 의한 것 일 경우 일차적 치료로 쓰여야 하는 것에는 이견이 없으며 또 는 항정신병약물이 금기인 경우 파킨슨병 또는 신경이완 제악성증후군 - 을 사용해 볼 수 있다 또한 레비소체치매 - 의 경우 항정신병약물에 부작용이 있을 수 있으므로 소량으로만 을 사용하거나 약물사용을 피해야 한다 우울제인 도 때때로 섬망의 치료에 쓰일 수 있으나 과도한 진정을 일으킬 위험이 있고 아직 섬망에 대한 연구가 부족한 실정이다 통증에 대한 적극적인 약물치료 또한 중환자실 섬망환자의 관리에 중요한 부분이다 부적절한 진통제의 사용은 섬망의 위험인자로 잘 알려져 있으며 한 연구에서 심한 통증이 섬망의 발생 위험을 배 높였다는 결과도 있다 섬망의예후및예방 섬망은 기저 질환의 치료에 따라 가역적인 상태이다 그러나 최근 연구에 따르면 섬망은 때때로 입원기간 내내 혹은 퇴원 후에도 지속될 수 있다고 밝혀졌다 중환자실에서 인공호흡기 치료를 받았고 섬망을 보였던 환자를 대상으로 장기적인 인지기능의 예후를 관찰한 한 연구에서 입원 중 섬망기간이 긴 환자일수록 중환자실 퇴원 후 개월 개월에 측정한 인지기능이 나이 교육기간 기저인지기능 중증 패혈증 그리고 진정제 투여 여부를 통계적으로 보정하고도 유의하게 나빴다는 보고가 있었다 그러나 위의 연구를 비롯한 섬망 여부에 따른 예후의 차이를 관찰한 이전의 여러 연구들은 일차적인 신경계질환을 가진 환자들을 제외한 채 진행한 것으로 신경계 중환자실 환자 집단에 직접적으로 적용하기는 어려워 신경계 중환자실 환자들 대상으로 추후에 연구가 더 이루어졌다 명의 세 이상의 허혈뇌졸중 또는 뇌내출혈 환자를 대상으로 한 연구에서 섬망이 있었던 환자의 사망률은 였던 반면 없던 환자는 로 낮아 섬망의 유무에 따라 전반적인 예후의 차이가 있다는 것도 밝혀냈으며 또한 입원 후 개월 개월에 확인한 - 점수 역시 섬망을 겪은 환자에서 유의하게 낮은 것으로 나타났다 또한 섬망이 시간 이상 지속된 환자는 시간 이내로 지속된 환자에 비해 나쁜 점수를 보여 섬망의 지속시간 역시 예후에 관여하는 것으로 생각된다 그 외에 신경중환자실 환자의 섬망 유무에 따른 사망률과 합병증을 관찰한 연구가 추가로 수행되었으며 이들 연구 역시 섬망의 동반은 나쁜 예후와 관련이 있다는 결과를 냈다 하지만 이들 연구들 모두 나이 기저질환의 중증도 뇌졸중의 아형 등을 보정하지 않은 연구로 결과에 대한 해석에 주의를 요하며 따라서 추가적인 신경계 중환자를 대상으로 한 섬망의 예후에 대한 전향적인 연구가 더 필요한 실정이다 섬망의 예방은 기저질환의 합병증 사망 등의 나쁜 예후를 줄이는 데 필수적이다 섬망의 발생은 다양한 요소에 의해 발 31

8 J Neurocrit Care 2011;4:25-34 생하므로 섬망의 예방 방법은 다수의 환자관리에 대한 지침들로 구성되어 있다 따라서 섬망의 예방 방법에 대한 연구는 주로 그 위험인자들을 줄일 수 있는 다양한 지침들을 수행한 효과에 대해 중점을 두고 있다 년 등이 수행한 병원에 입원한 섬망 발생의 위험인자를 가진 노인환자를 대상으로 한 연구에서 다방면의 환자관리 방법이 섬망의 예방에 효과가 있었다고 밝혔다 상기 연구에서는 가족들을 입원 초기부터 환자 간호에 동참하도록 하고 적절한 시각적 청각적 자료를 이용해서 환자와 의료진들의 의사소통을 최적화하고 치료 과정에서 조기에 거동을 시작 수면장애와 불안에 대해서는 먼저 비약물요법을 사용하고 적절한 영양과 수분섭취 그리고 통증에 대해서는 적극적인 약물처방이 이루어지도록 하는 것을 목표로 하였다 이 방법으로 섬망의 예방에는 효과가 있었지만 하지만 한번 섬망이 발생하고 나면 위의 방법은 효과적이지 않았다 그 외에 많은 섬망의 예방에 대한 연구가 이루어졌는데 한 다른 무작위 연구에서 위나 결장 절제술을 시행하는 환자를 대상으로 - 을 하루 시간 이상 수술 후 일간 정맥으로 투여하는 을 사용했을 때 대조군보다 섬망의 발생률이 적었다는 보고도 있었다 은 섬망의 예방 방법들의 문헌고찰을 통한 보고에서 섬망의 위험인자에 대한 관리를 더 광범위하게 할수록 수술환자의 섬망발생의 예방에 효과적이었다고 밝혔으며 이 위험인자의 관리에는 환자 및 보호자에 대한 교육 및 재교육 불안 관리 그리고 수술 후 의학적 평가 등도 포함되었다 등 이 수행한 다른 연구에서도 또한 다방면의 환자관리가 섬망의 기간 그리고 재원기간 사망률을 감소시켰다고 밝혔다 이 연구에서는 병원 직원에 대한 섬망의 평가 예방 및 치료 그리고 보호자와 환자 개개인의 상호작용에 대한 교육에 중점을 두었다 등 이 수행한 환자 대조군 연구에서도 병원 직원들에 대한 교육프로그램이 섬망유병률을 낮췄다는 보고도 있다 또 최근 섬망의 예방 방법에 대한 에서 고관절 골절 환자에서 사전에 노인의학전문가의 자문을 받고 섬망 치료에 을 사용하는 것이 섬망의 발생률과 기간 재원기간을 줄여 줄 수 있다고 밝혔다 결론 섬망은 발생빈도가 높고 사망률을 높이며 재원기간을 연장시키고 인지기능장애를 비롯한 장기적인 합병증 발생에도 큰 영향을 끼치므로 섬망은 환자에게뿐 아니라 환자의 가족과 사회적으로도 매우 중요한 문제이다 하지만 의료진이 지속적인 관심을 가지고 섬망을 의심하지 않으면 이를 인지 하지 못해 관리를 적절히 하지 못하게 되는 경우가 많다 특 히 중환자실에서의 섬망은 뇌졸중 간질중첩증 등의 심각한 신경계 질환과 감별이 잘 되지 않는 경우가 많으므로 각별한 주의가 필요하다 섬망의 예방 및 조기 발견은 환자의 예후에 매우 중요하다 그리고 섬망의 발생에는 매우 다양한 요소들이 작용하므로 섬망의 발생에 대한 정확한 이해는 적절한 섬망의 처치에 필 수적이고 조직적인 접근을 통해서만 효과적으로 예방하고 진단할 수 있다 현재까지 섬망의 예방에 관한 많은 연구 결 과가 있었지만 신경계 중환자실 환자에 있어서 섬망의 진단 방법 및 그 예후에 관한 전향적인 연구는 아직 한정되어 있으 므로 앞으로 많은 연구가 필요할 것으로 생각된다 REFERENCES 1.
