Although dopamine agonists improve RLS sensory symptoms and periodic leg movements during sleep, there was no study regarding the effects of dopamine agonist on cognition in patients with RLS. Methods: Sixteen drug-naïve RLS patients were enrolled. They underwent sleep questionnaires, Beck Depression Inventory, and neuropsychological evaluation, consisting of Rey Complex Figure Test and recognition trial (RCFT), Korean-California Verbal Learning Test, Controlled Oral Word Association Test (COWAT) before and after treatment with pramipexole (PMP). PMP was administered over a period of 12 weeks every night 1 h before bedtime. Paired Student s t-test and Spearman s correlation analysis were used for analyses. Results: After treatment with PMP, the International RLS Severity Scale score was significantly decreased. Sleep quality and depression also significantly improved after the treatment. RCFT immediate recall, COWAT phoneme word frequency, and digit symbol coding scores significantly improved compared to those before the treatment (17.72±7.84 vs ±7.33, p=0.05; 36.50±12.62 vs ±12.58, p=0.03; 68.19±21.11 vs ±21.11, p=0.02, respectively). There were no significant correlations between changes in clinical variables and in cognitive functions before and after the treatment. Conclusions: This study suggests that treatment with PMP in RLS patients improves not only RLS sensory symptoms, but also cognitive functions. 2 Kim M et al. 수면부족이외에도 증상으로 인한주의력저하 동반된우울증 및대뇌피질 자체의기능 이상이 환자의인지기능에영향을 미칠수있다 사건유발전위연구에서는 환자가정상인에 비해 진폭이감소하고 잠복기가연장되어있었으며 이러한변화는 주로주의력저하 및대뇌피질자체의기능 이상과연관이 있는것으로 나타났다 도파민작용제 는중증도이상의 환자에서 일차적으로 추천되는 약제이다 도파민작용제는 의일차적인 감각증상을 호전시키며 이외에도수면의 질수면중주기적사지운동 우울증상삶의 질등을호전시키는데 이는 중증도의 호전정도와 연관이 있었다 이처럼 도파민작용제가 와관련된 다양한 증상들을 호전시키는것으로 알려져있지만 아직까지인지기능에 어떤 영향을미치는지 연구한논문은 없다 본연구는 환자에서도파민작용제를 투여한후인지기능의호전이 나타나는지 확인하고자 한다그리고 인지기능이호전된다면 호전되는임상적인 요인이무엇인지 알아보고자하였다 이전에한번도 치료받지않은 명의 환자들이연구에포함되었다 모든환자들이 정형화된수면 설문지를통한 질문에 응답했고 신경학적 검사를 시행받았다 는- 에서제안한 기준에따라 진단하였으며 제외기준은 다음과같다 이차성 를유발할수있는동반질환의 존재임신 만성신장질환 말초신경질환 유발가능약물의 복용력예 신경이완제 항우울증약 항히스타민제 등 수면무호흡증 렘수면행동장애 사건수면등의 수면질환 인지기능장애로 인하여증상을 정확히호소할 수없는경우 주의력부족과잉행동장애 본태성떨림 파킨슨병 신경이완제에 의한정좌불능증 울혈성심부전혈관 및신경성파행 척수병증 관절염등 유사증상을보일수 있는질환 수면습관과 약물복용력을 문진하였으며 - 수면설문지를 함께 시행했다 우울정도는 설문지를 통해 평가하였으며 의중증도는 을통해 평가하였다 모든환자에서 혈색소혈당 혈청크레아티닌 철페리틴 갑상선호르몬 수치는 정상이었다 모든환자에서 문서화된 사전동의를 받았고 연구계획서는 기관윤리심의위원회- 승인을받았다 신경심리검사는 집중력을 확인하기 위한숫자외우기 검사 언어기능을 평가하기 위한한국판 보스톤 이름대기 검사단축형 - 시공간구성능력과 기억력을측정하기위한 레이오스터리에스 복합도형 검사 언어기억및 학습을평가하기위한한국판 캘리포니아 언어학습검사 를시행하였다 또한이마엽의집행기능을 평가하기위하여통제 단어연상검사 숫자부호화검사 선추적 검사 한국판 색상단어 스트룹 검사- 를시행하였다 모든환자에게 도파민작용제인 을 주동안취침한 시간전에투여하였다 용량은하루에 으로시작하였고 주가지난 뒤배로증량하였다 효과와순응도를 고려하여투여시작 주이내에 적정용량으로 조절하였고 적정화후일정량을 다음신경심리검사 시행일까지유지하였다 수면설문지와 신경심리검사는 약물투여직전과 주사이에두 차례시행하였다 치료전후의 신경심리검사의 변화를확인하기 위해- 를이용하였고 상관관계는 로분석하였다p 가 이하일 경우유의한것으로 판정하였다 환자들의평균 나이는± 세이고이 중여자는 명이다평균 교육연수는 ± 년 이었다 체질량지수 는± 혈청 은± 로측정되었다 투여의평균 추적기간은± 일하루 투여량은 중앙값 이었다 17

4 Kim M et al. - - 검사에서는 호전되는 경향을보였지만 유의한차이는보이지않았다 신경심리검사 영역중의미 있는호전을 보인 검사의점수 변화와수면 및우울검사의 점수 변화정도 간에상관분석을 시행하였다 투여후- 점수 점수 점수의호전과 점수의호전정도와는 유의한상관관계를 보이지않았다 본연구에서는 도파민작용제인 를이전에한 번도치료받지않은 중등도이상의 환자에게 투여하였고 가증상의 중증도불면 우울및수면의질을 호전시킴을확인할 수있었다 이는이전의 여러연구들과 일치하는 소견이다 인지기능에서는 집행기능및시각기억이 호전됨을확인하였고 이런연구 결과는도파민작용제가 환자에서 인지기능을 가역적으로 호전시킬 수있음을 보여준다그러나 임상증상의 호전과인지기능의 호전사이에는 상관관계를보이지 않았다 환자에서는인지기능이 저하되며 주로이마엽영역에 해당하는기능의 저하를보인다 이마엽은수면부족에 민감하게영향을 받는뇌영역이고 환자에서수면부족이 흔하게동반되므로 주로이마엽 영역의인지기능이 환자에서 저하되는 것을설명할 수있다 뇌영상및신경생리검사를 이용한연구들에서 환자의 이마엽기능장애를 시사하는결과가 보고되었다 구조적뇌 영상연구에서는 뇌들보 앞쪽띠이랑- 중심앞이랑 의 백색질부피가 환자에서 유의하게 감소되었고 기능적 뇌영상 연구에서는 능동적으로 발의발등굽힘과 발바닥쪽굽힘을시행할 때등가쪽 앞이마피질 부위의활성이 환자에서유의하게 증가되었다 그리고안정 뇌파에서는 이마엽의 베타활성도가 증가되어 과각성상태와 유사한 소견을보였다 본연구에서 환자에서 도파민작용제 치료에 의하여 인지기능이호전된 기전으로는다음과 같은가지가능성을 생각할 수있다 첫째 도파민에 의해 수면부족 주의력결핍과같은 증상이호전되었고 이차적으로 인지기능이호 전되었을 가능성이 있다 두 번째 우울증상이 감소되면서 가 성치매효과가 사라지고 이로 인해 인지기능이 호전된 것과 같은 결과를 나타냈을 가능성이 있다 마지막으로 도파민 시 스템의 조절장애가 의 병태생리학적 원인으로 알려져 있는데 도파민의 투여가 불균형을 해소하여 도파민 시스템 과 관련된 이마엽 인지기능을 호전시켰을 가능성이 있다 인지기능의 호전 정도는 증상 개선 수면장애 및 우울 증의 호전과는 의미 있는 상관관계를 보이지 않았다 이 결과 는 위에서 기술한 첫 번째 및 두 번째 가설을 지지하지 않는 소견이다 그러므로 환자에서 도파민작용제 투여 후 인 지기능의 호전은 다른 임상적인 변수들의 호전으로 인한 이 차적인 결과가 아니고 도파민이 직접적으로 인지기능에 영 향을 미쳤을 가능성이 있다 도파민은 뇌의 중요한 신경전달물질로서 주로 운동계와 변연계의 기능을 조절하는 역할을 하고 중대뇌변연계경로 를 통해 인지기능에도 영향을 미친다 등가쪽 앞이마엽 및 뒤쪽 마루엽을 꼬리핵과 연결 시키는 회로가 관련되어 작업기억과 집행기능 등의 앞이마 엽에 해당되는 인지 기능을 조절하는 역할을 한다 건강 인과 파킨슨 환자를 대상으로 도파민작용제 또는 레보도파 를 투여한 연구에서는 약의 투여나 중단이 이마 엽 인지기능에 관련된 집행기능과 작업기억을 변화시킨다 고 보고하였다 도파민 투여는 시행하는 검사와 신경 계 도파민의 농도에 따라 인지기능을 호전시키거나 오히려 악화시킬 수 있으며 단순히 도파민의 부족이 인지기능을 저 하시키는 것이 아니라 적절한 농도가 유지되지 않는 것이 인 지기능 저하의 원인이다 본 연구에서도 투여가 도 파민 불균형을 조절하여 인지기능의 호전이 나타났을 것으 로 생각된다 또한 집행기능과 시각기억이 호전된 점으로 미 루어 환자에서 신경계의 도파민 농도가 감소되어 있음 을 유추할 수 있다 본 연구에서는 대조군 없이 연구를 진행하여 환자에 서 도파민의 인지기능 호전 효과가 정상인에서 보일 수 있 는 정도였는지 여부를 확인하지 못하였다 그리고 연구를 진 행한 환자의 숫자가 적어 인지기능의 호전과 다른 임상적 변 수와의 상관관계가 없다고 단정짓기에는 한계가 있다 그럼 에도 불구하고 본 연구는 환자에서 도파민의 인지기능 조절 효과를 보여주는 첫 논문으로 도파민 치료가 환 자에서 다른 증상뿐 아니라 인지기능의 호전도 초래할 수 있 음을 보여준다는 점에서 의미가 있다고 하겠다 REFERENCES 1.
