1 ORIGINAL ARTICLE 한국인과이집트인의치열궁형태비교 장갑수 a ㆍ석경은 b ㆍ Mohamed Bayome a ㆍ김윤지 c ㆍ김성훈 d ㆍ국윤아 e 본연구의목적은한국인과이집트인의 Angle 씨 I 급, II 급, III 급부정교합군에서하악치열궁의크기와형태적특성을상호비교분석하고자하였다. 한국인부정교합자 368 명 (114 Class I, 119 Class II, and 135 Class III) 과이집트인부정교합자 94 명 (35 Class I, 32 Class II and 27 Class III) 을대상으로하였다. 치료전하악치열모형을복사한사진에서 13 개의인접한접촉면에서가장협측에위치한점들을좌표화한후하악치아의두께자료에근거하여임상브라켓점들을설정하였고 4 개의선계측과 2 개의비율을측정하였다. 치열궁형태를 tapered, ovoid, squared 의 3 가지로분류하여두인종간분포특성에관하여비교하였다. 이집트인은한국인보다대구치간폭경과견치간폭경이유의성있게좁았으며 (p < 0.001), 대구치간장경과견치간장경이더짧았다 (p < 0.001). 이집트인에서는 3 가지의치열궁형태가동등한비율로나타났으나 (p = 0.46), 한국인에서는 square arch form (46.7%) 이가장빈번하게나타났고, tapered arch form 이가장적게나타났다 (18.8%). 본연구의결과는기성품의초탄성호선을선택할때인종간의차이에대한유익한정보를제공할수있을것으로생각된다. DOI: /kjod 치열궁형태의분류방법은다양하나통상적으로사용되는기하학적형태를생성하는지표들은다양하다. 대부분의과거연구에서절치절단이나교두정을지표로사용하였다. 5-7 하지만이들에의한방법은 straight wire 처방을위한기성의초탄성와이어사용에있어서부적합하다. 최근연구들에서는하악치열궁형태를평가하기위한임상적브라켓점을지표로사용하였다. 6,8 인종집단간치열궁형태에대한많은연구들은안면과두개크기에대한두부방사선학적계측과인류학적연구를통해다양한인종집단간의현저한차이가있음을보고하였다 특히, 한국인과다른인종집단간의치열궁특징에대하여 Kook 등 15 은한국인과북미유럽인과의치열궁형태를비교하였고, Lee 등 8 은한국인과일본인의치열궁형태를비교한바있다. 인종집단간의치열궁형태특성에대한연구는북미유럽인과일본인을대상으로이루어진바있으나중동인과의비교연구는비교적적다. 특히, 이집트는중동지역에서가장인구가많고, 아랍인구의 1/4 을차지하고있다. 16,17 본연구에서는한국인과이집트인의 Angle 씨 I 급, II 급, III 급부정교합군에서하악치열궁의크기와형태를 tapered, ovoid, squared 의 3 가지로분류하여 334

2 Vol. 40, No. 5, Korean J Orthod Comparison of arch form between Koreans and Egyptians 두인종간분포와특성에관하여비교하고자하였다. 연구방법 본연구의한국인대상은원광대학교치과대학병원교정과에내원한 237 명의여성과 131 명의남성으로총 368 명의하악모형으로구성되었으며, 이집트인대상은카이로치과대학병원교정과와카이로소재의개인교정치과의원에내원한 73 명의여성과 21 명의남성으로총 94 명의하악모형으로구성되었다. 한국인대상은구치관계에따라 I 급 114 명, II 급 119 명, III 급 135 명으로분류하였으며, 이집트 Fig 1. Digitized contact points on a mandibular cast. 인대상은 I 급 35 명, II 급 32 명, III 급 27 명으로분류하였다. 한국인평균연령은 16.3 세 (± 5.65), 이집트인평균연령은 18.2 세 (± 3.08) 였다. 대상의선택기준은다음과같다. 1. 제 3 대구치를제외한건전한영구치열 2. 정상적인치아크기와형태 3. 과잉치의부재 4. 치열궁길이부조화가 3 mm 이하 5. 접촉부위, 교두정, 절단연에연장되는수복물이없어야함 6. 과거교정치료경험이없는자하악모형의교합면을확대율수정을위한눈금자와함께스캔하였다. 소프트웨어프로그램 (Autocad 2007, Autodesk Inc., San Rafael, CA, USA) 을사용하여 13 개의인접접촉부위의가장협측에위치한인접점을설정하여각해당되는치아의임상적브라켓점을설정하였다 (Fig 1). 중절치간접촉면의중심점은 X, Y 좌표의원점으로정하고, 좌, 우측제 1 소구치와제 2 소구치의인접점을잇는직선과좌, 우측제 2 소구치와제 1 대구치의인접점을잇는직선의기울기를측정한후두직선간의평균기울기가 X 축과평행이되도록 XY 좌표를조정하였다. 각치아의근심과원심의인접점을연결한선을그린후, 제 1 소구치에서는이선의근심 1/3 지점에서, 나머지모든치아에서는이선을이등분하는지점에서각각수직으로순, 협측으로연장한선을그어각치아의임상적브라켓점을찾도록하였다 (Fig 2). 3 장갑수, 석경은, Mohamed Bayome, 김윤지, 김성훈, 국윤아 대치교정지 40 권 5 호, 2010 년 Andrews 18 가치관의순, 협측으로가장풍융한부위를고려하여설정한각치아의두께자료를이용하여각각의브라켓위치에해당하는 12 개의점들을결정한후 4 개의선계측과 2 개의비율을정의하였다 (Fig 3). 다음의 4 개의선계측과 2 개의비율을사용되었다 (Fig 3): 견치간폭경 (Intercanine width): 좌우측견치의임상적브라켓간의직선거리 대구치간폭경 (Intermolar width): 좌우측제 1 대구치의임상적브라켓간의직선거리 견치간장경 (Intercanine depth): 좌우측견치의임상적브라켓간을연결한선위에서중절치사이의중심점까지의수직거리 대구치간장경 (Intermolar depth): 좌우측제 1 대구치의임상적브라켓간을연결한선위에서중절치사이의중심점까지의수직거리 견치간폭경 / 견치간장경비율 (Canine width/depth (W/D) ratio) 대구치간폭경 / 대구치간장경비율 (Molar W/D ratio) 후 square, ovoid, 또는 tapered 형태로제작된 template 인 OrthoForm (3M Unitek, Monrovia, CA, USA) 을기준으로제 1 소구치부터반대쪽제 1 소구치사이에설정된 8 개의임상적브라켓점들과중첩하여가장많은점들이일치하는치열궁형태를선택하였다. 통계분석 치열궁형태의발현빈도와인종집단분류사이의연관관계의유의성유무는 chi-square test 로분석되었고, 한국인과이집트인의성별에따른 arch dimension 비교에서는 t-test 를이용하였다. 성과연령등관련요인을보정한상태에서 Angle 분류법과치열궁형태분류에의한두인종집단의평균 arch dimension 을비교하기위하여공분산분석법 (ANCOVA) 이사용되었다. 모든분석은 0.05 의유의성수준에서평가되었다. 측정오차는각집단으로부터무작위로선택한 24 개의모형을 2 주간격을두고중복측정하여차이를통계학적으로분석함으로써평가되었다. Intraclass correlation (ICC) 분석을통하여 2 주간격으로측정한값들간에높은신뢰도를보였다 (ICC > 0.8). 측정오차는대체로작고, 허용범위내에있었다. 12 개의임상브라켓점들을 1:1 비율로출력한 Table 1. Tapered arch form 의발생빈도는 square 와 ovoid arch form 보다현저하게낮았다 (p < 0.001). 두인종집단에서의치열궁형태의발현빈도는전체표본과 I 급부정교합자들에있어서유의한차이를보였으나, II 급과 III 급부정교합자에서의발현빈도차이는유의하지않았다. 한국인과이집트인 III 급부정교합자에서 square arch form 이가장빈발하였으나, 이집트인 I 급과 II 급부정교합자에서는 ovoid 와 tapered 형태가가장많이보였다. 하지만한국인에서는 square 형태가가장많은분포로나타났다 (Table 2). 모든치열궁형태에서이집트인이한국인보다좁은견치간폭경과장경을갖는것을알수있었다. 대구치간폭경또한이집트인이좁았으나대구치간장경은 square arch form 을갖는두인종간유의성있는차이를보이지않았다 (p = 0.073). 모든치열궁형태에서이집트인이한국인보다더큰견치간폭경 / 장경비율을보였으나, 대구치간폭경 / 장경비율은 square arch form 을갖는집단을제외하고는두인종간뚜렷한차이를보이지않았다 (p = 0.010). 하악궁의형태변화가 tapered 에서 ovoid, square 형태순으로갈수록견치간장경과대구치간장경, 그리고견치간, 대구치간폭경 / 장경비율의유의한증가를보였다. 한편견치간장경은치열궁의형태가 tapered 에서 ovoid, square 형태순으로유의한감소를보였으며, 대구치간장경역시 337

SE, Standard error; W/D ratio, width/depth ratio. p < 0.05; p < 0.01 감소하는경향을나타냈다 (Table 3). 한국인여성은남성보다대구치간폭경이현저히좁았으며, 이집트인여성은남성보다현저히작은대구치간장경을보였다 (Table 4). 고찰 교정치료는세계적으로확산된의료산업으로구강위생에대한관심과심미에대한욕구가증가함에따라교정치료에대한요구가증가하고있다. 그 러므로, 교정치료에서인종간에치열궁특징에관한연구의중요성이강조되고있다. 우리나라에는 100 만이상의타인종이거주하고있고, 다문화가정이증가하고있으며, 다른인종의치료에있어서골격및치열궁특징에대한관심이증대되고있다. 따라서, 교정치료에서인종간에치열궁특징에관한연구의중요성이강조되고있다. 인종집단간의치열궁형태특성에대한연구는북미유럽인과일본인을대상으로이루어졌으나중동, 특히이집트인과의비교연구는이루어지지않았 338

6 Vol. 40, No. 5, Korean J Orthod Comparison of arch form between Koreans and Egyptians 다. 이집트의인구는 8천만이넘고, 셈족집단의일부를구성하는아랍인구의 1/4을차지하고있다. 16,17 과거이집트인과다른인종집단과의치열궁형태비교를위한연구는대부분상악치열궁에초점을맞춰실행되었다. 본연구에서는한국인과이집트인의하악치열궁형태를처음으로비교분석하였다. Bishara 등 13 은이집트와북아메리카청소년간의안면과치열관계를비교하여두인종집단간에 cephalometric parameter에유의한차이가있음을보고하였다. 뿐만아니라이집트인과 Caucasian 또는사우디인을대상으로한연구는 Caucasian의치열궁이다른인종집단과다름을보여주었다. 8,15,19-21 치열궁의형태를표현하기위해몇몇의수학적방법이시행되었다. 2,22-27 본연구에서는치열궁의형태를특징화하는데정확한방법중하나인다항식함수를사용하였다. 26 한국인에서가장적은빈도로나타난치열궁형태는 tapered arch form이었다 (p < 0.001). 하지만이집트인에서는세가지치열궁형태가균등하게분포되어나타났다 (p = 0.46). 이것은아마도셈족, 베르베르족, 누비아족과같은다양한인종집단이이집트인구를구성하는데기인한것으로보인다. 연구대상을치열궁형태에따라분류하였을때이집트인이모든치열궁형태에서작은넓이와큰견치간폭경 / 장경비를보였다. 일본인과코카시안의유의성이없음을보고한 Nojima 등 28 의연구와는다르게, 본연구는한국인과이집트인의치열궁형태의발현빈도가전체표본과 I급부정교합자들에있어서유의한차이를보이며, 각각의치열궁형태에따라인종간의차이가있다는것을나타낸다. 몇몇의연구는치열궁크기에있어남성과여성의차이가없다고보고하였으나, 6,20,29 본연구결과한국인여성이남성보다좁은대구치간폭경을갖는것으로나타났으며, 이것은 Haralabakis 등의보고와도일치한다. 30 하지만전반적인치열궁크기에있어서남성과여성의차이에통계적유의성이없었기때문에하나의그룹으로비교하였으며, 성별을고려하여 ANCOVA로통계처리를하였다. 또한 I급부정교합자와 II급부정교합자의치열궁형태에유의한차이가없다는보고가있었으나, 3 본연구의결과한국인 II급부정교합자는 I급부정교합자에비해서 ovoid 치열궁형태의발생빈도가낮고, tapered 치열궁형태의발생빈도가증가함에따라 Kook 등 15 의결과와일치함을보여주었으며, 이 집트인에서는반대의결과를나타내었다. 하지만이러한같은인종내 I 급과 II 급간의차이는통계적으로유의성을나타내지는않았다. 흥미롭게도두인종집단간의치열궁형태비도분포의차이는전체샘플과 I 급부정교합자에서나타났으며, II 급과 III 급부정교합자에서는나타나지않았다. 본연구에서는골격적관계는고려하지않고, Angle 분류에따른단지구치부 key 관계만으로 sample 을분류하였다. 이것은한국인과이집트인간의골격적차이또는선호하는음식의이유로인한부정교합발현양상의차이가있을수있으며이에대한규명이앞으로진행되야할것으로생각되며, 또한골격적관계와치열궁형태간의관련성에대한연구가필요할것으로생각된다. Braun 등 31 은 II 급부정교합자가 I 급부정교합자의것보다작은하악치열궁폭경과장경을갖으며, I 급부정교합자는 III 급부정교합자에비해넓은치열궁장경과좁은치열궁폭경을보였다. 하지만본연구에서는견치간장경과대구치간장경에있어서세집단간에통계적유의성을보였다. III 급부정교합자는 I 급또는 II 급부정교합자에비해유의하게넓은견치간, 대구치간폭경을보였으며, 이결과는과거의연구들과일치한다. 8,15 이연구에서사용한방법은 2 차원적방법으로 facial axis point 를정확하게표기할수없었다. 그러므로과거의연구에서와같이임상적브라켓점을계산하기위해접촉점을계수화하여연구하였다. 8,15 임상적브라켓점은절치의절단면이나교두정보다실제브라켓위치와와이어와긴밀하게연관되므로이방법은교정치료에있어서의미가크다. 본연구에서는한국인과이집트인간에처음으로치열궁형태의차이를규명하였으나연구방법이 2 차원적으로시행되었다. 최근의연구에서와같이 3 차원영상기법을이용하여 facial axis point 를찾는다면임상적치열궁형태를보다쉽게평가할수있을것이다. 32 앞으로 3 차원영상기법을이용한임상적치열궁형태에관한연구와다양한인종집단과의비교에대한추가적인연구가필요하다. 결론 본연구에서이집트인은한국인보다일반적으로더작은치열궁크기를갖는것으로보였기때문에치료시와이어의변형이요구된다. 본연구의결과는기존에시판되고있는기성초탄성호선의선택 339

Methods: The Egyptian sample consisted of 94 mandibular casts (35 Class I, 32 Class II and 27 Class III). The Korean sample consisted of 462 mandibular casts (114 Class I, 119 Class II, and 135 Class III). The most facial portion of 13 proximal contact areas was digitized from photocopied images of the mandibular dental arches. Clinical bracket points were calculated for each tooth. The subjects were grouped according to arch form to compare the frequency distribution of the 3 arch forms between the ethnic groups in each Angle classification. Results: Egyptians had significantly narrower intermolar and intercanine widths (p < 0.001), and shallower intermolar and intercanine depths (p < 0.001) than Koreans. There was an even frequency distribution of the 3 arch forms within the Egyptian group (p = 0.46). However, in the Korean group, the most frequent arch form was the square arch form (46.7%), while the frequency of the tapered arch form was significantly lower (18.8%). Conclusions: These results might provide helpful information in evaluating morphologic differences between ethnic groups in selection of preformed superelastic archwires.
