No 足關節錄接의關節造影術 해군본부기지병원방사선과 - bstract - 韓文熙 rthrography of the nkle Sprains Moon Hee Han, M.D. Department of Radiologκ Naval Headquaters ase Hospital nkle arthrography, by direct puncture of joint cav i야, is considered to be a simple and accurate diagnostic method for a precise evaluation of ligamentous injury. Forty-seven cases of ankle arthrography were successively performed in the patients of acute ankle sprains. The purpose of this study is to demonstrate how ankle arthrography can delineate the pathologic anatomy in such cases. The res비 ts are as follows: 1. Thirty cases among forty seven revealed the findings of ligament tears 2. For better diagnostic accuracy, the arthrography should be performed within 72 hrs. after injury. 3.,The anterior talofibular ligament tears were the most common (twenty-nine cases) of all and seventeen of them revealed tears w ithout association 01 any other ligament tears 4. There were ten cases of calcaneofibular ligament tears and nine of them were associated with anterior talofibular ligament tears. 5. Three cases of anterior tibiofibular and one deltoid ligament tears were demonstrated I. 績論足關節옳 ; 뿔 (nkle Sprain) 는매우흔한관절손상으 로운동이냐사고에의해서뿐아니라일상생활중에도 흔히발생한다. 骨折을동반하지않는대부분의경우에서單純 X 線農影은聊帶 f몹흉흉을 응? 斷하는데아무런도움을주지못 하며足關節의負倚像 (Stress View) 도痛효에의해어 려움이많고또한충분한정보를얻을수없다. 關節흩의直接穿刺 (Direct p.mcture of joint cavity) 에 의한關節造影術은聊帶휩屬의여부및그部位를알 이논문은 1985 년 6 월 29 일에접수하여 1985 년 7 월 8 일에채태되었음. 수있는매우간단하고正確한檢훌方法이다1). 특히 f흡흉훌後비교적빠른시기에施行하면그정확성이더욱높으며檢훌後합병증도거의없어治鷹前에많은정보를제공해준다. 著者는急性足關節옳짧의 øu에서關節造影術을施行하여휩屬部位및그所見을觀察, 分析하였다. 11. 對象 1983 년 11 월부터 1985 년 4 월까지足關節의急性휩옳을받은海軍장병중에서 ~ 帶 f홉陽이의섬되는 47 例에서關節造影術을施行하였다. 對象, 뚫者들은軍人이었으므로모두成男子이었으며檢훌前單純足關節擺影上骨折이없었던 øu를對象

2 - 짧文熙 : 足뼈節옳뺀의 IìII mi 造影術 - 으로하였다. f 홉陽이심하여關節의不安定이의섬되는경우에는前後像에서負뼈像을같이最影하였다. 1. 關節의穿행 IJ 1II. 方 i 去 IV. 結果愚者를 lfp 명. í 立로하고透視下에足關節의前方穿刺를하였다. 穿刺前에足背動服 (Dorsalis Pedis artery ) 을觸談하여그部位를표시하여穿刺時에피하도록하였다. 穿刺部位는透視下에결정하며가능한한中央部로하고測面像에서針의方向이上方으로 5 ", 10도정도向하게하기위해穿刺部位는關節面보다약 lcm 遠位部로하였다. 足關節의測面像으로透視하며水 zp: 方向으로針을進行시켜針끝이關節面의前端에걸리게하였다針은 22G 휴推針 (Spi nal needle) 을주로사용하였으며 21G 일반칩도함께사용하였으나針의進行中방향수정하기에 22G 홉堆針이더욱편리하였다. 針끝은解헤學的으로關節脫의前홈 (nteri or rece. 55 ) 에위치하게되며正常的으로약간前方으로突出되어있 3므로針끝이關節內로완전히들어가지않아도穿刺가가능하였다. 1. 시간경과에따른차이 총 47 例중뚜렷한異常所見을보인例는 3 0 例 (63.8 %) 이었으며 f 홉陽後 24 시간이내에檢좁를施行한경 우가 15 例 24 ", 72 시간後에施行한경우가 18 例그 리고 72 시간이상이경과한후에 이었다 (Table 1). 施行한경우가 14 例 f륨陽部位및정도에따라다르겠지만 7 2 시간이후에 施行한例에서異常所見을보이는例가상대석으로적게 나타났다 (Table 1). Me glumine diatrizoate(uro. grafin@, Na: Meglumine = 10: 66 ) 또는 50% 의 Na diatrizoate(hypaque-50@) 를사용하였으며注入時或은淚出時의痛 ile 을없얘기위해造影顆 J 6",7 cc 에 2 % Li docaine 1 ", 2cc 를섞어사용하였다 透視下에서造影齊 j 가關節흩內로흘러들어가는것을 확인하며注入하였으며造影용에의大量編出이나非正常的인價彼훌훌造影時에는 7"'Scc, 그런所見이없을때 에는 5",6cc 를注入하였다. 注入直後足關節을운동시켜造影뺑 j 가고르게 퍼지게 한후樓影을하였고前後像 (-P view), 測面像 (Later 외 view) 그리고兩 { 則料位像 ( oth ob!ique view5 ) 을最影하였다. 足關節의前外部 (nterolaterai portion) 에 造影簡 j 偏出이있는경우外힘位像 (Oblique view in external rotation ) 을擺影할때에前 JW 빠骨聊帶 Cnt e rior tibiofib 비 ar ligament) 를보기위해뾰骨과빠骨의前面이 일치되게하여題影히였다. 2. 聊帶慣傷 가장많은것이 前距排骨聊帶 (nt erior talofi bular ligament) 破짧이었으며 29 例에서 所見을觀察할수 있었다 다른聊帶慣傷없이前距厭骨聊帶破짧의所見 만을보인 ØlJ 가 17 例, 鍾聊骨聊帶 (Calcaneofi bular ligament) 破짧의所見과동반된例가 9 例이었다 (Table 2) Table 2. natomical Location of Ligament Tears Site Case No. (%) nterior talofibular ligament only nterior talofibular and Calcaneofibular ligaments Calcaneofibular ligament only nterior talofibular and anterior tibiofibular ligaments nterior talofibular, Calcaneofibular and Deltoid ligaments 17 (56.7) 8 (26.7) 1 (3.3) 3 (10.0) 1 (3.3) Total 30 (100)

5 - 大짧 h',( M~ 짧學會끊第 21 卷第 4 號 s he ath) 과밀착되어있고後距聊骨聊帶는距骨과外縣 V. 考흉윷足 짧節은 R얻骨, 명F 骨및距骨로이루어진비교적복장한구조이며關節鍵 ( Joint caps비 e) 은이뼈들의骨 - 軟셉 경게부위에붙어있다. 關節覆은足關節의足底屆 (Plantar flexion) 및足背屆 (Dorsiflexion) 을위해前後로는상당히여유있게되어있어이부분에前後홈 (nterior and Posterior r ecess) 를형성하게되나兩測面은여러 찌帶들로보강되어있는팽팽한구조로휩傷時에關節製의測面聊帶가破앓된다 2) 外 f떼의聊帶는前距명 Fi풍聊帶, 훌훌없 F 帶및後距排骨 P 帶인데前距뺨骨聊帶는距骨의願部냐外썼 ( Lateral ma l1 eolus) 플연결하고있으며內面은關節覆과연결된샘彼顆으로되어있다 2) (Fig. 8). lid빠骨 YH뿜는훌E 骨의測面과外 t 果를연결하며關節켈훌과는떨어져있으내명F 骨節健 f 骨 R찾훌훌 (Peroneus tendon 의 後面을연결하는튼튼한구조로關節覆과접하고있 다 (Fig. 8). 內測에는三角聊帶가內짧와距骨을떤결하고있으여역시關節製과정하고있다 (Fig. 8). 이밖에股骨과빠骨의前後面에각각前後降밟骨聊帶가脫厭骨의前面과後面을연결하고있으며關節훌훌과접하고있다 (Fig. 8). 이러한足關節聊帶들과關節覆或은隔被훌과의解 gu 學的인관계가각聊帶의破앓時각각다론所見을보이게한다.! 調節造影術上正常關節용의모양은부분적 로불규칙한곳은있나대체로부드러운모양을보이며뚜렷한경계를갖는다. 前後部에각각前홈와後홈로약간씩突出된부분이있으며前용보다後용가약간불규칙한모양을하고있다 (Fig. 6). 正常의경우造影鄭뷔關節앓外偏出은없 나간혹長屆節鷹 * 옵被훌흩 (Long flex- Fig.4. 8 - 짧文! 쩍 : JËUnîi l 훨 : 뾰의빼 m 1 섰!t)ji;! 당 - 데足 l짧節홈과의연결에의해보이는것으로 Ka ye 등 ') 에의하연약 10% 에서보인다하냐著者의경우에서는 19% 에서觀察되었다 (Fi g. 3). 骨間鷹는비교적흔히관찰되는것으로그유무나길 흔히다른聊帶휩앓및骨折과도동반된다 2, 3) 이경우足關節內얘 1떼軟組織에造影齊 j의源出을보인다 (Fig. 5). 다른웰帶볍陽과흔히동반되므로상대적으로희미한隔出을보여觀察에주의를要한다. 이에 별의미가없으냐前股厭骨聊帶破짧時에는보이지 않게된다. 著者의 3 例에서도모두관잘되지않았다. 짧網員없중에서가장흔한것이前距밟骨聊帶破앓이며 所見은뺨骨外測의軟組織애造影齊 j 가源出되는것이다 5, 6,7)(Fig.1). 前後像과外씹位像에서잘보이며偏出되는造影햄 j 의量은휩없의정도보다는시간경과에더많은影響을 받는것으로생각된다이所見은약 48 시간이경과하 연偏出部가막혀보이지않을수도있으며 3) 帶慣蘭 l 다른聊 의한偏出또는消被鍵의造影에의한關節內壓의감소에의해희미하게보일수도있다 ') 계는다른聊帶협陽의경우에서도마찬가지이다. 이런관 鍾距骨聊帶의破앓은거의모든경우에서前距厭骨웰 帶의破짧과동반된다 5,8)(Fig. 2). 著者의例에서도 10 例中 9 例가그러했으며냐머지 1 例는협陽後 10 일이경과한例로같이破짧되었요냐 前距뺨骨聊帶破짧의所見은보이지않는경우로추측된 다 (Fig. 3). 睡뺨骨聊帶破짧의所見은밟骨節健뿜被훌훌의 造影이며 상당한量의造影齊 j 가들어가關節外뼈 ~ 에서없 F 骨 ll'lj 健 의주행을따라뚜렷이보인다 9) 경과해도지속적 로보일수있 며 이런所見은시간이 48 시간이내에施行한檢흉上밟骨節睡隔被짧造影의所見만보이연 은오래된협傷에의한것이라는報告도 ') 있다. 이것 後距排骨 g 帶는外뼈 O g 帶中가장튼튼한것 로聊쯤쁨 f 블陽의특이한所見은없으며항상다론顆帶擔傷과동 반되고 3) 이경우상당한關節의不安定을보이므로負倚像에서關節의內歸 ( 1 nversion) 을관찰함으로써추측 할수있다 1) (Fig. 2). 正常所見 로알려져 있는距骨下關節造影이聊帶휩傷時의한所見으로보일수있다는報告도있다 ') R 용骨과뺨骨사이의前面에있는前健厭骨聊帶도다른 웰帶 f 員傷時같이破짧될수있으며이때股 밟骨사이 에 t 方으로불규칙한造影齊 j 源出을보인다 6,7) (Fig. 4). 前距뺨骨聊帶破앓時外힘位像에서造影輝 j 偏出部位와 8 뀔 骨의前面사이의앓은호間이이聊帶의위치이며破쫓 ~ 時이부분에도造影簡 j 가들어가는것을볼수있다 1) 內맴 0 에있는三角웰帶는심한휩陽에의해破짧되며 VI. 結論海軍장병에서말생한急性足關節현 ; 짧 47 例를대상으 로聊帶휩陽을딸기위해關節뾰의直接穿刺에의한關節造影術을施行하여다음과같은結홉움을얻었다 例中 30 例에서聊帶破짧의소견을보였다. 2. 휩傷後 72 시간이상경과한例에서는異常所見 을보이지않는例가더많았다 例 (96.7%) 에서前 Jìé 厭骨聊帶破짧의所見을 보였으며그중다른聊帶破짧이동반되지않았던例는 17Ø~ (56. 7%) 이었다. 4. 鍾뺨骨聊帶破짧의所見은 10 例 (33.3%) 에서보 였으며그중 9 例 (30.0% ) 에서 見과동반되었다. 前距명 F 骨 1J~ 뿜破짧의所 5. 前睡뺨骨聊帶破짧의例가 3 例 (10.0%), 三角聊帶破짧이 1 例이었다. 6. 正常變型所見 로骨間홈의造影이 64%. 距骨下關節의造影이 19% 그리고長屆節健濟被훌훌의 i 훌影이 11% 에서관찰되었다.
