It most often occurs in the upper respiratory tract (85%), as well as the head and neck, and very rarely occurs in the retroperitoneum. Here we report the case of a 57-year-old woman with retroperitoneal EMP. Retroperitoneal plasmacytoma - 병색을보였다. 혈압 150/90 mmhg, 맥박수 90회 / 분, 호흡수 20회 / 분, 체온 36.8 였다. 흉부청진상심음은규칙적이었고, 심잡음이나수포음은청진되지않았다. 복부팽만이있었으나압통과반발통은없었고장음은정상이었다. 양측상하지의운동기능, 감각기능은정상이었으나좌측하지부종이관찰되었다. 검사소견 : 혈구계산검사에서백혈구 7300/mm 3 ( 중성구 71.1%), 혈색소 11.9 g/dl, 혈소판 /mm 3 였고말초혈액바른표본검사에서특이소견은없었다. 일반화학검사에서총단백 6.3 g/dl, 알부민 3.8 g/dl였고혈청요소질소는 12.3 mg/dl, 크레아티닌 1.5 mg/dl였다. 혈청칼슘 8.6 mg/dl, 인 3.9 mg/dl, 요산 12 mg/dl, 젖산탈수소효소 (lactate dehydrogenase, LDH) 2512 IU/L ( 정상범위 : IU/L) 로 LDH가 뚜렷하게상승되어있었다. 프로칼시토닌 0.13 ng/ml, C-반응성단백 (C-reactive protein) 5.2 mg/dl로특이소견은없었다. 혈청 IgD는 229 mg/dl ( 정상범위 mg/dl) 로증가되어있었고 IgM는 16 mg/dl( 정상범위 mg/dl) 로감소되었으며 IgG는 712 mg/dl ( 정상범위 mg/dl), Ig는 mg/dl ( 정상범위 mg/dl) 로정상소견이었다. 혈청단백전기영동검사에서 M band는명확하지않았고, 요단백전기영동검사에서는 α2 globulin에 band가있으나이단백뇨양은 489 mg/ 일이었다. 혈청과요면역고정전기영동검사에서단클론감마글로불린병증 (Ig D Lambda) 소견이보였다 (Fig. 1). 혈청 kappa 유리경쇄는 1.25 mg/l ( 정상범위 mg/l) 였고 lambda 유리경쇄는 1,024.7 mg/l ( 정상범위 mg/l) 로 kappa 유리경쇄와 lambda C D Figure 1. 병리 검사상 lambda light chain (+), CD 이 둔각이었으며, 복부 엑스선 검사상 특이 소견은 없었다. 138 (+)의 형질세포종으로 진단되었다(Fig. 4).

로 수신증과 요관의 확장이 있었다(Fig. 2). 흉부 전산화단 치료 및 경과: 입원 후 시행한 전산화단층촬영 검사에서 층촬영 검사상 양측에 다량의 흉수가 있었다.

입원 8일째 의 미만성으로 fludeoxyglucose 섭취가 있었다(Fig. 3). 단순 소변량이 감소하여 혈청요소질소(blood urea nitrogen) 28.

(2014) 26 M Rt adrenal gland Surgery live Int J Clin Exp Pathol 2014;7: M, male; RTx, radiotherapy; Rt, right; CTx, chemotherapy; OP, operation. 루설치술을시행하였고이후신기능은회복되었다 ( 크레아티닌 0.9 mg/dl). 후복막강조직검사결과와면역고정검사및영상검사를종합하여골수외형질세포종 (Ig D lambda) 으로진단하였다. 치료로골수외형질세포종의표준인방사선치료를고려하였으나병변이광범위하여합병증의위험성이클것으로판단하였고전신항암요법 (bortezomib 1.3 mg/m 2 1, 4, 8, 11일 ; dexamethasone 40 mg 1, 2, 3, 4, 8, 9, 10, 11일 ; 매일 thalidomide 50 mg) 을시행하였다. 1주기치료가끝나고시행한전산단층찰영에서후복막강과좌하복강침윤이감소하며양측수신증이호전되는등경과가양호하였다 (Fig. 6). 고찰형질세포종은연조직또는골조직에형질세포단일클론이과증식하는악성질환이다. 형질세포종은골형질세포종, 골수외형질세포종, 다발성단일형질세포종으로나뉘는데골형질세포종은형질세포종이단일의뼈에만있는경우이며, 골수외형질세포종은연부조직에형질세포종이발생한경우이며, 다발성단일형질세포종은형질세포종이다발성 골병변또는다발성연부조직병변으로나타난것이다 [3]. 골수외형질세포종은형질세포종가운데 3% 로두경부 (80%) 특히인두부주변과비강, 잇몸, 구강점막에발생한다 [1]. 드물게위장관, 림프절에발생하는경우가있으나본증례처럼후복막강에서발생하는예는매우드물어전세계적으로 11예, 한국에는 1예의보고가있었다 (Table 1). 골수외형질세포종은남성에서많고 (75%) 평균발병연령은 55세이다 [4]. 골수외형질세포종은연부조직에단일클론형질세포의증식소견이있고골수, 혈액, 영상검사에서다발골수종이배제될시진단된다 [3]. 조직검사에서 CD 138(+) 또는 CD 38(+) 이며, light chain 편향성을보이는데본증례는 CD 138(+), lambda 유리경쇄 (+) 인형질세포종이확인되었다. 본환자는혈액검사상빈혈과고칼슘혈증이없었고영상검사에서다발성골병변이없었으며골수흡인검사에도다발골수종소견은없었다. 급성신부전이발생하였으나이는다발골수종에서동반되는신성신부전이아닌신후성신부전이었다. 종양에의한수신증으로경피적신루설치술후신기능이회복되었기때문이다. 그러므로의료진은본환자를다발골수종이아닌골수외형질세포종으로진단하였다. 골수외형질세포종은 14-25% 에서만 M band 가존재하는데, 본환자의경우

7 - 대한내과학회지 : 제 91 권제 2 호통권제 672 호 혈청및요단백전기영동검사에선 M band가명확하지않았다 [4]. 골수외형질세포종은 90% 이상에서방사선치료에반응이좋아단독방사선치료가표준치료이다. 보통 40 Gy로 4주간치료하는데, 5 cm 이상의큰골수외형질세포종은치료실패및재발하는경우가있어더많은방사선량 (50 Gy) 으로치료한다 [1]. 병변의완전절제가가능하다면, 수술적제거도가능하며치료결과는좋다. Sharma 등 [5] 은병변이넓어방사선치료가힘든골수외형질세포종환자에서전절제술을시행하였고, 이후 M단백의소멸과완전관해를유지하였다. 항암화학요법은방사선치료에효과가없거나방사선치료후재발한경우에시행하며 [1] 치료반응은비교적좋은편이다. 주로다발골수종에사용되는항암제로시행한다 [6]. 골수외형질세포종의초기치료로전신항암치료를시행한예는많지않아몇증례만보고된바있다. Zeng 등 [7] 은경막외에발생한골수외형질세포종의초기치료로 bortezomib/dexamethasone/thalidomide 복합화학요법을시행하여완전관해를유지한증례를보고하였다. 본증례는표준치료인방사선치료를고려하였으나급성신부전및나쁜전신상태로방사선치료를초기에시행하는것은무리가있었고광범위한후복막강과골반내의병변으로인해, 방사선치료시합병증이심할것으로판단하였다. 본원에서는스테로이드를포함한전신항암요법인 bortezomib/dexamethasone/thalidomide 를시행하였고경과는양호하였다. 골수외형질세포종치료후, 10년생존율은 70% 이상으로예후가양호하다. 방사선치료후국소재발률은 7% 미만이며, 원거리재발률은 30% 미만이다. 골수외형질세포종의 15% 가다발골수종으로진행된다 [8]. 크기 (5 cm 이상 ) 가골수외형질세포종의재발과다발골수종진행의나쁜예후인자로알려져있으며 [8], 그외에도고령 (60세이상 ), 조직학적특성 ( 이형성정도, 신생혈관형성정도 ) 이예후인자로보고되고있다 [9]. 종양의위치가예후에미치는영향은명확하지않다. 두경부에발생할경우더예후가좋다는보고가있지만, 치료방법의차이가있어이는명확하지않다. 1,185명의골수외형질세포종환자를대상으로한 Gerry와 Lentsch [10] 에따르면두경부병변환자의중간생존기간이 13년으로두경부외병변환자의중간생존기간 4년에비해더우수하였다. 이는골수외형질세포종이두경부에발생하면주로수술 -방사선복합치료를받았기때문인것으로판단된다 [10]. 전세계적으로보고된증례 (Table 1) 에서후복막강골수 외형질세포종이남성에많고, 크기가크다 (5 cm 이상 ) 는점을알수있다. 단독방사선치료보다는수술, 항암화학요법등다양한치료를시행하였는데병변의위치와크기가중요한요인이었을것으로추정된다. 본증례는골수외형질세포종이후복막강에발생했으며, 크기가거대했다는점에서예후가불량한경우이다. 치료후에도골수외형질세포종의재발및다발골수종진행가능성이크므로지속적인주의가필요한것으로사료된다. 요 본증례는후복막섬유증이의심되어내원한환자에서, 골수외형질세포종이진단된사례이다. 골수외형질세포종이두경부가아닌후복막강에발생했다는점에의의가있고표준치료인방사선치료가아닌전신항암화학요법으로초기치료를했다는점에서의의가있다. 후복막강종양또는후복막섬유증이의심되는환자에서골수외형질세포종의가능성을고려하여검사가진행되어야한다. 중심단어 : 형질세포종 ; 다발골수종 ; 후복막강 ; 후복막섬유증 약 REFERENCES 1.
