A 29-year-old man who had a 7-year history of ulcerative colitis was admitted to our hospital complaining of nausea, vomiting, bloody diarrhea, and colicky abdominal pain. Fourteen days after admission, he developed sudden-onset left pure motor hemiplegia. Magnetic resonance imaging and angiography revealed an infarction involving the right temporo-occipital hemisphere and diffuse middle cerebral artery territory. Neck computed tomography angiography showed no evidence of cervical carotid or vertebral artery abnormalities, and transthoracic and transesophageal echocardiograms revealed no evidence of thrombi. Factor V was elevated to 176% (normal 50~150%) and protein S antigen had decreased to 57% (normal 60~150%). He was managed with antiplatelet agents and a total proctocolectomy. (Korean J Med 77: , 2009) Key Words: Ulcerative colitis; Cerebral infarction; Thromboembolism 서론궤양성대장염환자에서피부병변, 관절염, 담도염, 안구증상등의다양한장외합병증이알려져있는데흔치않은합병증의하나로혈전증이있다 1-3). 이합병증의발생빈도는 1.2~7.5% 로알려져있는데발생부위는대부분하지정맥, 골반정맥총 (pelvic venous plexus) 그리고폐정맥등에서발생한다 4,5). 이러한정맥내혈전색전증의보고는많으나동맥내혈전증으로뇌경색이발생된경우는전세계적으로 30예미만이보고되고있다 6,7). 혈전색전성질환 (thromboembolic disease) 은복막염, 대장암에이어서 3번째로중요한염증성장질환의사망원인으로써, 혈전증이생기면사망률이 25% 에이르고특히뇌경색이발생한경우심한신경계의손상을가져오는것으로알려져있으며 8,9), 현재이러한합병증에대한명확한치료지침도없는상태이다 10,11). 본증례는궤양성대장염으로치료받고있던환자에서급작스런좌측전신마비의형태로증상이나타나뇌경색을발견하였으며심장및경동맥혈전등의병소여부확인과함께혈액검사를시행하여다른 2차적요소를배제하였고, 궤양성대장염과관련된혈전증임을확인한 1예를경험하여이를보고하고자한다. B 증례 29세남자가지속되는복통과혈변을주소로본원내과외래를방문하였다. 과거력상특이병력은없었으며내원 7년전부터혈변및하복부통증이있어오다가 4년전에상기증상으로중소병원에서전대장을침습한광범위궤양성대장염으로진단받았으며내원 6개월전지속되는혈변및하복부통증으로본원에입원하여당시심한활동성상태의궤양성대장염으로진단받고스테로이드 (hydrocortisone) 와싸이클로스포린 (cycloserine) 정주요법을사용한후증상이호전되어퇴원하였으나더이상본원외래를방문하지않고중소병원에서경구약제 (PENTASA : mesalazine) 을투여하며지내온환자였다. 내원 2주전부터하루 1~2회정도의구토와간헐적복통그리고하루 15~20회이상발생되는혈변증상을다시보여본원에입원하였으며, 대장내시경검사상전대장에걸쳐서궤양과자발성출혈소견을보이고있었다 ( 그림 1). 입원당시부터스테로이드정주요법과좌약 (ASACOL : mesalazine) 을투여하며관찰하였으나증상에호전이없어입원 8일째부터싸이클로스포린 (cycloserine) 정주요법을추가로사용하며관찰하던중입원 14일째오후 2시경갑작스런좌측편마비를보였다. 당시환자의의식은명료했으나묻는말에정확히대답하지못하는상태였으며, 환자의활력징후에는큰변화가없었다. 신경학적검사상감각의이상은 없었으나 Grade IV 정도의좌반신마비에해당하는근력저하를보였다. 혈액검사상혈색소 10.4 g/dl, 적혈구용적률 31.4% 로약간저하되었으나백혈구는 6,740/mm 3, 혈소판은 232,000/mm 3, 적혈구침강속도 5 mm/hr으로정상이었으며, 혈액응고검사 (prothrombin time, partial thromboplastintime), 간기능 (aspartate amino-transferase and alanine aminotransferase), 신장기능 (blood urea nitrogen/creatinine) 및혈중전해질농도등은모두정상범위였다. 뇌전산화단층영상에서우측후두엽실질에서비정상적인저음영의소견이관찰되었다 ( 그림 2A). 뇌자기공명영상에서는우측후두엽과측두엽의피질하부위에서경색을시사하는고신호강도의병변 ( 그림 2B) 과중간대뇌동맥영역 (middle cerebral artery territory) 에서피질및피질하경색을시사하는광범위한비정상적인신호 ( 그림 2C) 가관찰되었다. 뇌자기공명혈관조영술 (magnetic resonance angiography: MRA) 에서도중간대뇌동맥의상행분지인뇌섬엽구 (M2, Insular segment) 부위에폐쇄소견이관찰되었다 ( 그림 2D). 뇌영상검사를고려할때심장성색전에의한뇌경색의가능성이있어서시행한심전도에서동성리듬을보였으며경흉부심장초음파 (trans-thoracic echocardiography) 와경식도심장초음파 (trans-esophageal echocardiography) 검사에서혈전의병소를확인할수없었다. 경동맥의혈전여부를확인하기위해시행한두경부전산화단층혈관조영술 (neck CT angiograpy) 에서도특이사항은관찰되지않았다

D 혈전증의원인을찾기위해시행한혈액응고기능검사상항트롬빈 III (antithrombin III), C 단백항원 (protein C antigen), 호모시스테인 (total homocysteine), 항인지질항체 (antiphospholipid antibody; anti-cardiolipin IgG and IgM antibody), 루푸스항응고물질 (lupus anticoagulant screening), 항핵항체 (anti-nuclear antibody), 항 Ro와항 La 항체 [anti-ro (SS-A) and anti-la (SS-B) antibody] 는모두정상으로확인되었으나제 V 인자 (Factor V) 가증가되어있었고, S 단백항원 (protein S antigen) 이감소된소견을보였다 ( 표 1). 신경과협진하에환자에게뇌경색에대해서클로피도그렐 (clopidogrel) 과실로스타졸 (cilostazol) 을투여하였으며중증궤양성대장염에대해서는싸이클로스포린 (cycloserine), 스테로이드 (steroid), 메살라진 (mesalazine) 을경구로투여하였다. 환자는항혈전제제를투여한 4일후부터증상이빠르게호전되어좌측근력의경미한저하외에는다른신경학적이상소견을보이지않았다. 입원 23일째환자는복통이없어지고혈성설사도하루 1~3회로호전되어경구약제복용을 지속하기로하고퇴원하였으며뇌경색발병 1개월후외래에서경과추적하였다. 뇌경색에의한신경과적증상은소실된상태였으나하루 1~2회정도의간헐적혈변을호소하여경구약제로싸이클로스포린 (cycloserine), 스테로이드 (steroid), 아자치오프린 (azathioprine) 을투여하였으며좌약 (ASACOL : mesalazine) 을사용하였다. 이후에도외래추적시환자는경구약제에임상적관해를보이지않고복통및혈변이악화되어뇌경색발병 3개월후총직장결장절제술을시행하였다. 고찰염증성대장질환에서혈전색전증의유병률은 0.3% 에서 41% 까지다양하게보고되고있고, 궤양성대장염과크론병에서실제적인유병률을아직정확히알수는없지만연간유병률은모두 0.5% 정도로알려져있다 4,5). 궤양성대장염에서발생하는혈전색전증의대부분은심부정맥이나폐에서발생하고그외에도뇌혈관이나망막혈관, 문맥및장간

Results of coagulation studies Coagulation study Result (normal range) Antithrombin III 85.8% (normal 70~130%) Protein C antigen 102% (normal 72~160%) Protein S antigen 57% (normal 60~150%) Factor V 176% (normal 50~150%) Total homocysteine 8.4 µmol/l (normal 5.0~13.9 µmol/l) Antiphospholipid antibody Lupus anticoagulant screening Antinuclear antibody Anti-Ro (SS-A) antibody Anti-La (SS-B) antibody 막정맥 (mesenteric vein) 에서발생하며드물게는동맥에서도발생한다 1). 궤양성대장염이뇌혈관에서발생한경우는드물며실제로 Mayo clinic에서 7,199명의염증성장질환환자를대상으로한연구에서궤양성대장염을가진환자중 43 명에서혈전색전증의합병증이발생하였으며그중 4명만이뇌혈관에발생하였다 8). 궤양성대장염환자에서혈전색전증이발생한다는것은 1936년 Bargen 과 Baker 의보고 12) 가있은후잘알려져왔으 나혈전증의기전에대해서는분명히밝혀진바가없다. 여 러증례에서제시된발생기전에대한가설을살펴보면, 섬유 소원과혈소판의상승, 트롬빈과제 V 인자, 제 VIII 인자의생 성증가그리고항트롬빈 III 의감소가혈전증의발생에기 여할것이라는가설 13,14) 이있고, 항카르디오리핀항체가혈전 증의발생과관련되어있으며약 70% 의환자에서한가지 이상의응고장애를보였다는보고 15) 가있다. 그리고내독소 와친염증성사이토카인등에의한응고연속단계의활성화 로인해활동성궤양성대장염에서혈전색전증의위험이더 높다는보고 16) 와염증성장질환환자의혈전성향증 (thrombophilia) 에대한평가에서 33% 의환자가적어도하나이상의 혈전성향증을보였다는보고 17) 등이있다. Mieshler 등 18) 은정상인과비교하였을때염증성장질환 환자에서혈전색전증이발생할위험이 3.6 배더높다고보고 하였으며, 이러한발생위험은셀리악병 (celiac disease) 이나 류마티스관절염 (rheumatoid arthritis) 이동반된환자에서는 나타나지않았고, 염증성장질환환자에서발생한혈전색전 증의 60% 가누공이나농양과같은위장관합병증이존재하 는경우나염증성장질환의질병활성도가높은경우에발생 하였다고보고하였다. 국내에서는궤양성대장염과연관하여대부분하지정맥, 골반정맥총그리고폐정맥에서발생한혈전색전증의보고 들이있으며, 그외상장간막정맥 (superier mesenteric vein) 및 간문맥 (portal vein) 에서발생한경우 19) 와뇌정맥혈전증 20) 에 대해서보고하였다. 혈전증이일단생기면사망률이 25% 에 이르고특히뇌혈관에발생한경우 47% 의사망률과 14% 의 영구적인심한신경계손상을가져오는것으로알려져있 다 20). 위의보고들에비추어볼때본증례에서는혈소판증다증 이나항트롬빈 III, 트롬빈증가는나타나지않았고, 호모시 스테인도정상범위내에있었으며, 루푸스항응고물질과항 카르디오리핀항체의증가도나타나지않았다. 그러나본증 례에서뇌혈관발생의위험요소와기저질환이없는궤양성 대장염환자에서뇌경색이발생하는데기여할수있었던요 인으로는대장내시경검사에서궤양성대장염이전대장을침습한심한활동성상태였고제V인자의증가와 S단백이경미한감소를보였다는점을들수있겠다. 뇌경색에대해서클로피도그렐 (clopidogrel) 과실로스타졸 (cilostazol) 을투여하였으며뇌경색발병 1개월후경과추적시뇌경색에의한신경과적증상은소실되었다. 요 궤양성대장염환자에서혈전증의보고는많으나동맥내혈전증이관찰된경우는매우드물며, 이러한합병증에대한명확한치료지침도없는상태이다. 이에저자들은궤양성대장염환자에게서발생한뇌경색을항혈소판제제인클로피도그렐 (clopidogrel) 과실로스타졸 (cilostazol) 을투여하여합병증없이치료한경우를경험하였기에문헌고찰과함께보고하는바이다. 약 중심단어 : 궤양성대장염 ; 뇌경색 ; 혈전색전증 REFERENCES 1) Rankin GB.
