1 대한안과학회지 2010 년제 51 권제 9 호 J Korean Ophthalmol Soc 2010;51(9): pissn: eissn: DOI : /jkos = 증례보고 = 눈물길협착이없는눈물흘림환자에서가쪽눈꺼풀판띠고정술과실리콘관삽입술의효과 류원열 서지영 안희배 동아대학교의과대학안과학교실 목적 : 눈물길의폐쇄가없는환자에서가쪽눈꺼풀판띠고정술만을시행한경우와실리콘관삽입술을병행한경우의효과를비교하고자하였다. 대상과방법 : 아래눈꺼풀늘어짐을동반한눈물흘림환자 20 명 27 안에가쪽눈꺼풀판띠고정술을시행한경우와 18 명 29 안에가쪽눈꺼풀판띠고정술과실리콘관삽입술을병용한경우의증상, 눈물띠높이의변화, 수술효과를비교하였다. 결과 : 가쪽눈꺼풀판띠고정술군과가쪽눈꺼풀판띠고정술및실리콘관삽입술병용수술군의평균연령은각각 60.3 세, 64.8 세, 술전눈물띠높이는각각 0.62 ± 0.23 mm, 0.62 ± 0.19 mm, 술후는 0.43 ± 0.20 mm, 0.26 ± 0.10 mm 였다. 술후눈물흘림증상은가쪽눈꺼풀판띠고정술군에서 74.1%, 병합수술군에서 86.2% 의호전을보였다. 가쪽눈꺼풀판띠고정술의방법으로외안각보존법과외안각절개술과증상의변화는연관성이없었다. 결론 : 눈물길협착이없는눈물흘림환자에서가쪽눈꺼풀판띠고정술및실리콘관삽입술병용수술방법은효과적으로눈물흘림을치료할수있을것으로사료된다. 눈물길세척술및눈꺼풀늘어짐검사를통해수술방법의선택결정에도움이될것으로생각된다. < 대한안과학회지 2010;51(9): > 기능성눈물흘림이란, 비루관의막힘이없이과도한눈물흘림증상을보이는상태로정의할수있다. 1,2 눈물흘림환자에서눈물점이나눈물길의협착이없는경우눈물펌프의기능장애가원인중하나로알려져있다. 3,4 눈물펌프의기능장애는외상이나수술등기계적손상으로눈둘레근이약화될때, 안면신경마비로눈둘레근마비가있을때, 그리고퇴행성변화로아래눈꺼풀의늘어짐 (laxity) 이증가할때발생할수있다. 5,6 아래눈꺼풀늘어짐과관련된눈물펌프기능의저하가의심되는경우에는가쪽눈꺼풀판띠고정술을시행해볼수있다. 가쪽눈꺼풀판띠고정술은눈꺼풀의수평방향의힘을강화시켜눈물펌프기능을호전시킴으로써증상을완화시키는방법으로알려져있다. 4 또한아래눈꺼풀늘어짐과관련된눈물흘림환자에서가쪽눈꺼풀판띠고정단독술또는여러수술의병합요법이효과적이라는보고가있었다. 7 접수일 : 2010 년 6 월 4 일 심사통과일 : 2010 년 7 월 1 일 책임저자 : 안희배부산광역시서구동대신동 3 가 1 동아대학교병원안과 Tel: , Fax: hbahn@dau.ac.kr * 본논문의요지는 2010 년대한안과학회제 103 회학술대회에서구연으로발표되었음. * 이논문은동아대학교학술연구비지원에의하여연구되었음. 본연구는아래눈꺼풀늘어짐과관련된눈물흘림으로가쪽눈꺼풀판띠고정술만을시행한환자들과실리콘관삽입술을병행한환자들을비교분석하여그효과를알아보고자하였다. 대상과방법 2008년 2월부터 2009년 9월까지눈물흘림으로본원안과에내원하여아래눈꺼풀늘어짐이있으면서눈물길협착이없다는진단하에수술을받은환자 38명 56안을대상으로하였다. 의무기록을기초로환자의나이, 성별, 술후경과관찰기간, 증상의호전정도, 눈물띠높이의변화등을후향적으로조사하였다. 8 수술전후눈물띠높이는 1인의관찰자가세극등현미경을이용하여 0.2 mm나 1.0 mm 세극광선으로아래눈꺼풀에서안구표면을따라눈물띠상연까지의거리를측정하였고눈물띠높이가 0.5 mm 이하일경우에는 0.2 mm 세극광선을사용하여측정하였다. 술전아래눈꺼풀늘어짐유무를확인하기위해눈꺼풀떼어당김검사 (eyelid distraction test) 에서 7 mm 이상당겨지고아래눈꺼풀탄력검사 (snap back test) 에서바로정상적으로되돌아가지않거나눈깜박임으로제위치로돌아가는환자만을대상으로하였다. 또한눈물길세척술에서정상소견을보이고눈물소관의협착소견이없으며눈물점폐쇄, 결막염, 1174

2 - 류원열외 : 가쪽눈꺼풀판띠고정술과실리콘관삽입술 - 눈물주머니염, 결막결석, 결막늘어짐증, 각막이상, 안구건조등의안질환이없는환자를선택하였다. 안검내반, 반흔성안검외반이동반된환자는대상에서제외하였다. 20명 27안에서는가쪽눈꺼풀판띠고정술단독수술을시행하였고 18명 29안에서는가쪽눈꺼풀판띠고정술과실리콘관삽입병합수술을시행하였다. 가쪽눈꺼풀판띠고정술의수술방법으로는외안각절개법과외안각보존법을안검이완의정도에따라술자의판단하에선택하였다. 외안각절개법은 Jordan and Anderson 7 의술식과같은방법으로시행하였다. 가쪽눈구석절개술을시행하고가쪽눈구석인대의아래갈래를절단하여아래눈꺼풀을가쪽안와가장자리에서분리시켰다. 눈꺼풀의앞층판과뒤층판을분리한뒤, 가쪽눈꺼풀판의결막부착부를절단하여눈꺼풀판띠를만들었으며, 가쪽아래눈꺼풀을가쪽으로당겨서절제량을측정한다음여분의눈꺼풀판띠를잘라내었다. 그후눈꺼풀판띠를 5-0 Dacron으로골막에깊이재부착시켰다. 외안각보존법의경우는 Lemke 9 가발표한방법과같이시행하였다. 외안각에서가쪽으로 6-8 mm 떨어진곳에서피부절개를하고눈둘레근을박리해들어가가쪽안와연에이른다. 그리고안와골가쪽모서리에서골막피판 (periosteal flap) 을만들었다. 그후골막거상기 (periosteal elevator) 로휘트날안와결절 (lateral orbital tubercle of Whitnall) 근처, 가쪽안와골 2 3 mm 안의지점정도까지골막피판을들어올린후눈구석인대와부착시켰다. 실리콘관삽입술은 1:1000 에피네프린을거즈에적셔하비강내에밀어넣어비점막을수축시키고눈물점확장기로눈물점을넓힌후, 눈물길더듬자로탐침법을시행하였다. 그후눈물소관삽입세트 (bicanalicular intubation set) 및내시경을이용하여실리콘관이상하눈물소관을통하여삽관되어하비도에서관찰되면실리콘관용겸자 (Alligator forceps) 를사용하여비강외로견인하고, 매듭을지어코전정부위에고정하였다. 삽입한실리콘관은증상에따라술후 3개월에서 6개월사이에제거하였다. 수술결과는술후환자의눈물흘림의호전여부에대한주관적인평가와눈물점및눈꺼풀의해부학적위치, 최종내원시눈물띠높이에대한객관적인평가로판정하였다. 술후눈물흘림의호전여부는문진을통해평가하였는데, 증상의완전한소실을보이는경우를 Grade 1, 호전을보이지만일부증상이남은경우는 Grade 2, 수술이전과비교하여증상의변화가없는경우는 Grade 3, 수술전에비하여증상이악화된경우는 Grade 4로하여 4단계로분류하였다. 10 그중 Grade 1과 Grade 2에해당하는경우를증상의호전으로판단하였다. 수술전후의눈물띠높이는 Wilcoxon singned rank test 를이용하였고두군간의비교는 Mann-Whitney U test 를통하여통계분석하였다. 모든통계는 SPSS software program (SPSS for Window, version, 12.0K) 을사용하였으며 P 값이 0.05 미만일때통계적으로유의한것으로하였다. 결과 전체 38명 56안중가쪽눈꺼풀판띠고정술단독수술군은남자가 11명 15안, 여자가 9명 12안이었고, 나이는최소 21세에서최대 83세까지분포하였으며, 평균연령은 60.3 ± 15.74세였다. 병합수술군은남자가 9명 14안, 여자가 9 명 15안이었고, 나이는최소 54세에서최대 83세까지분포하였으며, 평균연령은 64.8 ± 9.06세였다. 술후평균경과관찰기간은단독수술군에서 14.6 ± 6.87개월, 병합수술군에서 13.3 ± 6.44개월이었으며, 두군간에남녀비와술후경과관찰기간은큰차이가없었다. 술전에측정된눈물띠의높이는단독수술군이 0.62 ± 0.23 mm, 병합수술군은 0.62 ± 0.19 mm로통계학적으로차이가없었으며 (P=0.809, Mann-Whitney U test), 95% 신뢰구간에서단독수술군은 mm, 병합수술군은 mm로유사한분포를보였다. 수술후최종내원시측정한눈물띠의높이는단독수술군이평균 0.43 ± 0.20 mm, 병합수술군이평균 0.26 ± 0.10 mm로두군모두수술전과비교하여통계적으로유의하게감소하였고 (P<0.001, Wilcoxon signed rank test) 병합수술군이단독수술군보다더욱우수한결과를보였다 (P<0.001, Mann- Whitney U test )(Table 1). 술후눈물흘림증상의호전을보인환자는단독수술군 Table 1. Grade 1 = no epiphora; Grade 2 = improved but intermittent epiphora; Grade 3 = no change; Grade 4 = aggravated. 에서 Grade 1, 2에해당하는환자가 74.1% 였고병합수술군에서는 86.2% 로병합수술군에서더많았다 (Table 2). 가쪽눈꺼풀판띠고정술의수술방법에따른술후눈물흘림의호전정도는 Grade 1, 2에해당하는환자가외안각절개법을시행한경우는 72.9%, 외안각보존법의경우에는 94.7% 로나타났다 (Table 3). 고 눈물흘림은눈물의과다생성또는눈물배출의감소로인해발생한다. 기능적눈물흘림은개방된코눈물관의명백한원인이없는경우에서눈물이과다한것으로정의할수있다. 1 눈물이배출되는과정은능동적이고복잡한기전의펌프작용에의해이루어진다. 눈물펌프작용기전에대한가설중에서는크게두가지이론이주로받아들여지고있는데, 하나는눈을깜박이는동안눈물소관팽대부가눈물주머니로눈물을밀어내고이때눈물주머니의음압이눈물을 찰 끌어당긴다는 Rosengren-Doane의능동적눈꺼풀눈물소관펌프 (active palpebral canalicular pump) 이론이다. 다른하나는눈을깜박일때안와사이막앞눈둘레근과눈꺼풀판앞눈둘레근같은근육들의작용에의해눈물의위와아래눈물배출계로의이동이유발된다는 Jones의눈물펌프이론이다 그리고이러한눈물펌프의기능장애가기능적눈물흘림의원인으로생각되고있다. 4 오래전부터아래눈꺼풀의늘어짐은눈물흘림의원인중하나로인식되어왔다. 14 Lew et al 15 의보고에따르면눈물흘림환자군전체에서눈물펌프의장애로인한눈물흘림이차지하는비율은 11.1% 였으며, 이중아래눈꺼풀의노인성이완에의한경우가가장많았다 (5.6%). 또한 Liu and Stasior 16 는아래눈꺼풀늘어짐을측정해서만성눈물흘림을포함한이물감, 만성재발성결막염등의안증상과의관련성을보고하였다. 각막염이나기타염증성질환을포함한여러가지원인에의하여눈물이과도하게분비되거나눈물점의경미한외반 (punctual ectropion), 협착등이아래눈꺼풀늘어짐과동반되어있을수있기때문에눈물흘림의정확한원인을찾아교정해주는것이필요하다. 아래눈꺼풀늘어짐의교정에관하여여러수술법들이개발되었으며, 많은문헌에서기술되었다. 4,9,17,18 Vick et al 4 은기능적눈물흘림환자 21명 34안을대상으로가쪽눈꺼풀판띠고정술을시행하여단지 3명에서만증상호전을보이지않았음을보고하였다. 가쪽눈꺼풀판띠고정술은아래눈꺼풀늘어짐을수평방향으로강화시켜눈물펌프기능을향상시킴으로써눈물흘림을개선시키는효과가있다. 19 아래눈꺼풀이강화되면눈둘레근이눈을감을때더효과적으로작용하여, 눈물점으로눈물을보낼수있고, 눈물주머니벽을당기는힘이증가하여, 눈물주머니로의음압을증가시키는효과가있다. 20 또한 Chung and Park 21 은기능적눈물길폐쇄환자 9명 12안을대상으로결막눈물주머니코안연결술을시행하여 12안모두에서만족할만한결과를얻었다고보고하였고, Wormald and Tsirbas 22 는기능적눈물길폐쇄환자 32명에서눈물주머니코안연결술을시행하여 27명 (84%) 에서호전된결과를보였다고보고한바있다. 이처럼최근에는아래눈꺼풀이완에의한기능적눈물흘림환자에서가쪽눈꺼풀판띠고정술을단독또는다른수술과같이시행하는것이권장되고있다. 7 본연구에서는가쪽눈꺼풀판띠고정술단독수술과가쪽눈꺼풀판띠고정술과실리콘관삽입병합수술을시행하여두군간의효과를비교하고자하였다. 두군모두에서의미있게눈물띠높이가감소하였고, 특히병합수술군이눈물띠높이의감소와증상호전에서더욱나은결과를보였다. 이 1176

4 - 류원열외 : 가쪽눈꺼풀판띠고정술과실리콘관삽입술 - 는실리콘관삽입술이눈물점을확대하는효과가있어서나타난결과로사료된다. 가쪽눈꺼풀판띠고정술의방법에서는외안각보존법이더욱나은결과를보였지만, 비교대상환자수가적고, 술전아래눈꺼풀늘어짐의정도에따라수술방법을다르게선택하였기때문에결과의차이가발생할수있을것으로생각된다. 향후보다많은대상군에서아래눈꺼풀늘어짐의교정방법과눈물흘림호전과의연관성에대한연구가필요하리라생각된다. 참고문헌 1) Sadiq SA, Downes RN. Methods: The present study investigated 27 eyes of 20 patients underwent lateral tarsal strip procedure (LTS) alone and 29 eyes of 18 patients underwent combined lateral tarsal strip procedure and silicone tube intubation and who were diagnosed with functional lacrimal duct obstruction. The authors retrospectively reviewed patients medical records for information including age, gender, history, follow up period, tear meniscus height (TMH), and symptoms. Results: The average age of the LTS alone group was 60.3 years, and that of the combined groups was 65.9 years. The TMHs in the LTS only and combined operation groups were 0.62 ± 0.23 mm and 0.62 ± 0.19 mm before the operation and 0.43 ± 0.20 mm and 0.26 ± 0.09 mm after the operation, respectively. The postoperative improvement in epiphora was 74.1% after the LTS alone and 86.2% after combined LTS with silicone tube intubation. The postoperative improvement in epiphora and the operative technique of the lateral tarsal strip canthus sparing or cantholysis were not relevant. Conclusions: Lateral tarsal strip combined with silicone tube intubation appeared useful in the treatment of epiphora patients without anatomic lacrimal duct obstruction. Determining which operative technique of lateral tarsal strip procedure should be chosen is difficult, and the preoperative syringing and snap back test may be helpful.
