S. lugudunensis infections resemble Staphylococcus aureus infections in terms of virulence, tissue destruction and clinical course. Although correct identification and determination of the susceptibility profile are important, some commercial systems may misidentify S. lugdunensis. We report a case of native valve infective endocarditis caused by S. lugdunensis, which was misidentified by the Vitek 2 system but identified correctly by 16S ribosomal RNA (rrna) gene sequencing in a 72-year-old male patient. The patient had multiple vegetations on his mitral valve, and the largest one was found on the posterior mitral valve leaflet. It was 2.5 cm in size and hypermobile. Diffuse valvular abscess was also observed. He had persistent bacteremia for appoximately 8 days, which was resolved after immediate surgery and antibiotic therapy. When a patient with severe sepsis syndrome grows S. aureus or CNS other than S. lugdunensis on a commercial automatic culture system, the possibility of S. lugdunensis should be considered and further confirmatory testing such as 16S rrna sequencing may be very useful. Key Words: Staphylococcus lugdunensis, Infective endocarditis, Coagulasenegative staphylococci, 16S ribosomal RNA sequencing 서론 Staphylococcus lugdunensis 는응고효소음성포도알균 (coagulase-negative staphylococci, CNS) 의일종으로서, 1988 년처음동정된이후로감염성심내막염을비롯하여골수염, 관절염등다양한침습성감염이보고되어온균이다. S. lugdunensis 에의한감염성심내막염은비교적드문질환이지만외국에서는약 60예이상보고되었고 [1, 2], 국내에서도 2예가보고된바있다 [3, 4]. 2 Infect Chemother 2011;43(4): 과는달리침습적이고위중한감염을일으킨다는특징이있지만 [5], 현재상용화되어있는자동화장비배양시스템에서는 CNS 의균종수준까지정확하게동정되지않는경우가많고, S. lugdunensis 가다른균종으로잘못동정되는경우가많아서 [6] 더정확한균종동정법이필수적이라하겠다. 저자들은자연판막에서발생한 S. lugdunensis 에의한감염성심내막염 1예를배양검사및 16S rrna 염기서열분석으로진단하고치료하였기에이에대해문헌고찰과함께보고하는바이다. 증례 72세남자환자가내원 15 일전부터발생한열과오한을주소로연고지지역병원에내원하였다. 환자는내원 20 일전이슬람지역으로성지순례여행을시작하였고, 내원 15 일전여행지체류중에열과오한이발생하여지속되었다. 내원 8일전귀국하여연고지지역병원에입원하여열에대한진단적검사를시행하였고, 혈액배양검사에서 S. lugdunensis 가동정되어내원 4일전 ampicillin-sulbactam 정주치료 (3 g IV q 6hr) 를시작하였다. 내원 2일전경흉부심초음파상승모판막의우종 (vegetation) 및중증승모판역류가동반되어본원으로전원되었다. 환자는내원 8일전, 내원 6일전, 내원 4일전에각각시행한 3차례의혈액배양검사에서모두배양양성소견을보이는상태였다. 과거력상당뇨병으로혈당강하제복용중이었다. 입원당시활력징후는혈압 127/82 mmhg, 맥박수분당 72회, 호흡수분당 18 회, 체온 36.4 이었다. 의식은명료하였고, 흉부청진상심첨부에서수축기잡음이청진되었다. 그외특이사항은없었다. 말초혈액검사에서백혈구 7,900/ mm 3 ( 호중구 75.4%, 림프구 15.6%), 혈색소 11.4g/dL, 혈소판 136,000/ mm3, 적혈구침강속도 55 mm/hr 이었고, 혈청생화학검사에서 C 반응성단백은 7.72 mg/dl, 젖산탈수소효소 569 IU/L, 신기능및간기능검사는정상소견이었으며혈청 procalcitonin 은 0.13 ng/ml 이었다. Diffuse thickening of the mitral valve was also observed, which was found to be valvular abscess following surgery. 심전도검사상정상동율동을보였으며단순흉부방사선검사상심비대및폐침윤소견은관찰되지않았다. 경흉부심초음파 (transthoracic echocardiogram) 상, 승모판후첨판 (posterior mitral valve leaflet) 에 cm 크기의우종 (vegetation) 이관찰되었는데매우유동적으로움직이는양상이었다 (Fig. 1). 승모판전첨판 (anterior mitral valve leaflet) 에도각각 0.77 cm, 0.45 cm 크기의우종 (vegetation) 이관찰되었고, 승모판비후, 승모판후첨판탈출 (prolapsed posterior mitral valve leaflet) 및건삭파열 (chordae rupture) 로인해중증도의편심승모판역류가관찰되었다. 경식도심초음파 (transesophageal echocardiogram) 상, 상기병변외에도승모판전체가미만성비후 (diffuse thickening) 를보여농양이의심되는소견이었다. 본원입원당일혈액배양검사를재차시행한후즉시 nafcillin 과 gentamicin 을정주하였다. 심초음파상관찰되는우종이원격병소로색전증을일으킬위험성이높고중증승모판역류가동반되어있어입원당일응급수술을시행하였다. 수술소견상, 승모판전첨판에추가로농양을확인하였고, 우종및염증성조직을모두제거한후우형심장막 (bovine pericardium) 을이용하여승모판치환술을시행하였다. 입원당일시행했던혈액배양검사결과 Vitek 2 (bio Mérieux, Hazelwood, MO, USA) 장비에서 Staphylocuccus warneri 가동정되었고, 수술장에서의뢰된판막조직의그람염색상소수의그람양성균이관찰되었고배양검사결과 Vitek 2 장비에서 S. lugdunensis 가확인되었다. 승모판막의생검조직검사상우종과석회화된알균을동반한급성화농성염증 (acute suppurative inflammation with vegetation and calcified bacterial cocci) 소견이관찰되었다. 내원 8일전타병원에서시행한혈액배양검사에서동정되었던균주및본원입원당시혈액배양검사와판막조직배양검사에서동정되었던균주를합쳐총 3균주에대해 16S rrna 염기서열분석 (16S ribosomal RNA sequencing) 을시행하였다. DNA 를추출하기위하여균집락을 lysis buffer (100 mm NaCl, 10 mm Tris-HCL, 1 mm EDTa, and 1% Triton X-100) 에잘섞은후 90 에서약 10 분간배양하여세포를용해한후원심분리하여 aqueous phase 를분리하였다. 시발체 16S-F0 (5 -GAT CCT GGC TCA GGA CGA AC-3 ), 16S-R0 (5 -CTT GTT ACG ACT TCA CCC CA-3 ) 를이용하여 16S rrna 유전자절편을증폭한후염기서열을결정하여 1,419bp 의염기서열을얻었다 [7, 8]. 이를 EzTaxon ( 의 database 와비교하였다. 분석결과 3 균주모두동일한염기서열을가졌으며, S. lugdunensis 표준균주인 ATCC 43809T (GenBank accession number, AB009941) 와비교하여 99.93% 상동성을가진것으로판명되었다 (Table 1). 다른 CNS 균종과의염기서열비교에서는모두 99.5% 미만의상동성을보여배제할수있었다. 항생제감수성검사상 penicillin 내성, oxacillin 감수성 (oxacillin MIC, 0.5 μg/ml) 균으로써 naficillin 및 gentamicin ( 초기 2주 ) 정주치료를지속하였다. 환자는수술후 2일째부터혈액배양검사결과음성으로전환되었고, 수술후 2주째 nafcillin 에의한약열 (drug fever) 진단의심하에 ampicillin-sulbactam 제제로변경하여유지하였으며수

(in press) LMG 25332(T) FJ / Staphylococcus hominis subsp. hominis Kloos and Schleifer 1975 DSM 20328(T) X / Staphylococcus pasteuri Chesneau et al ATCC 51129(T) AF / Staphylococcus warneri Kloos and Schleifer 1975 ATCC 27836(T) L /1418 술후 6주째에항생제를종결한후특별한증상없이퇴원하였다. 현재외래추적관찰중이다. 고찰 1988 년처음으로동정된이후 S. lugdunensis 는인체감염의중요한병원균으로부각되고있으며 [9], 임상적으로유의한검체에서분리되는 Coagulase-negative staphylococci 의발생빈도는 Staphylococcus epidermidis (50.5%), Staphylococcus haemolyticus (18.5%), Staphylococcus saprophyticus (16%), S. lugdunensis (6.0%), S. warneri (2.5%) 로 S. lugdunensis 가 4위를차지한다 [10]. S. lugdunensis 는서혜부와유방주위의피부에정상적으로존재하는상재균으로써, 심내막염, 뇌농양, 뇌수막염, 피부및연조직감염, 척추염, 인공삽입물관련감염, 복막염등다양한감염질환이보고되었는데, 특히감염성심내막염은매우치명적인질환으로잘알려져있다 [11]. Choi 등이최근에발표한자료에따르면, 국내소재 7개병원에서 10 년동안총 15 예의임상적으로의미있는 S. lugdunensis 균혈증이발생하였고 ( 발생율은 100,000 입원당 1.3 예였음 ) 그중 4 예에서감염성심내막염이진단되었는데 4예중 2예가사망하였다 [12]. S. lugdunensis 감염은임상양상이 S. aureus 감염증과비슷하고유사한독성인자 (virulence factor) 를공유하는것으로알려져있으며, 실제로치명적인패혈증을유발하는균이므로정확한동정및감수성을확인하는것은매우중요하다 [6]. 또한다른 CNS 균종들은 oxacillin 감수성기준이최소억제농도 (MIC) 0.25 μg/ml 이지만, S. lugdunensis 는 S. aureus 와동일하게 oxacillin MIC 2 μg/ml 까지 oxacillin 감수성으로간주되기때문에 [13] S. lugdunensis 을다른 CNS 로잘못동정할경우, oxacillin 에대한위내성으로보고될수있어 S. lugdunensis 의정확한동정이더욱중요하다고하겠다. S. lugdunensis 는종종슬라이드응고효소검사에서응고인자 (clumping factor) 양성소견을보여임상적으로 S. aureus 로오인되기도하며 [14], 이경우시험관법응고효소검사 (tube coagulase test) 음성을통해감별진단에도움을얻기도하나, 처음부터응고인자음성인경우엔 S. lugdunensis 의진단을놓칠수있다. 이외에도 novobiocin, pyrrolidonyl arylamidase, ornithine decarboxylase, mannose 를이용한간단한표현형검사를통해균종수준까지정확히동정한다는보고도있다 [10]. 상용화된자동화장비또한이문제를해결하지는못 하며, 연구에따르면 S. lugdunensis 진단을놓치는경우가 23.5% 에달하기도한다 [6, 10]. 이중 Mateo 등이발표한연구에따르면 17 개의 S. lugdunensis 균주중 Vitek 2가오진한경우는 4예 (23.5%) 로써, 모두 S. haemolyticus 로동정되었다. De Paulis 등의연구에서는 12개의 S. lugdunensis 균주중 Staph-Zym (Rosco) 장비가오진한경우는 1예로써 S. warneri 로동정되었으며 API-Staph (biomeriuex) 는 12 균주중 1 균주를동정하는데실패했다고한다. 본증례에서도혈액배양에서분리된균주의 Vitek 2 동정결과는 S. warneri 로나왔으나실제 16S rrna 염기서열분석검사에서는 S. lugdunensis 로확인되어 Vitek 2 의오류를알수있었다. S. lugdunensis 의항생제감수성은과거에는 oxacillin 최소억제농도 (MIC) 가 0.5 μg /ml 인경우를내성으로제시하였으나, MIC 가 0.5 μg/ml 이어도 meca 유전자를갖고있지않은경우가많아 [6, 15] 이를반영하여개정된 CLSI 지침에서는 S. lugdunensis 의경우 S. aureus 와마찬가지로 oxacillin MIC 2 μg/ml 인경우감수성, 4 μg/ml 인경우내성으로기준이바뀌었다 [13]. 본증례에서는 penicillin MIC 2 μg/ml 로내성이어서베타락탐분해효소를생성하는균이었으며, oxacillin MIC 는 0.5 μg/ml 로감수성을보였고, meca 유전자는음성이었다. CNS 에의한감염증이증가하면서임상검체에서 CNS 가분리되는경우오염균과감별을위하여균과질병의임상적상관관계를평가하는것이필수적이며, 이를위해서는정확한균동정이필요하다. 자동화장비가 CNS 를정확히동정하지못함은이전여러연구에서잘알려져있는데, 표현형검사를이용한균동정법의단점은균주가가지고있는표현형발현정도가다양하다는점, 표현형발현의해석이다양할수있다는점및이로인해표현형검사의재현성이떨어진다는점에있다 [16]. 본증례에서도혈액배양에서분리된균은 Vitek 2가 S. warneri 로동정하였고판막조직에서배양된균은 Vitek 2가 S. lugdunensis 로동정하였지만유전자형검사결과동일한균으로확인되었는데, 자동화장비에서사용하는표현형검사의재현성이부족함을단적으로보여준다고할수있겠다. 다른세균들과마찬가지로 CNS 도 16S rrna 염기서열분석법을이용한진단기법이수립되어있다 [17]. 16S rrna 염기서열은각개체별로고유하게존재하며또한매우잘보존되어있기때문에세균의속 (genus) 및종 (species) 수준까지정확하게동정을하는데도움이된다. 이에, 세균의정확한동정, 배양이어려운세균의및계통발생학적분류등다양한분야에서유용하게사용되고있다 [18]. 감염성심내막염의경우, 판막조직배양검사와 16S rrna 염기서열분석법의수행능

4 Infect Chemother 2011;43(4): 력을비교한한연구에따르면 16S rrna 염기서열분석법의민감도, 특이도, 양성예측도, 음성예측도는각각 94%, 100%, 100% 및 90% 였으며, 이에반해판막조직배양검사는각각 18%, 89%, 75% 및 36% 였다 [19]. S. lugdunensis 의경우, 배양음성감염성심내막염환자의색전조직에서 16S rrna 염기서열분석법을시행하여 S. lugdunensis 를진단한증례가보고된바있다 [20]. 16S rrna 염기서열분석법의결과판독시, 표준균주와비교하여염기서열의유사성기준치 (cut-off value) 는 97% 로제시되기도하였으나, 그의미는명확하지않다. 16S rrna 염기서열분석법이균종수준의감별에서는분별력이다소낮아서, 염기서열차이의기준치를 >0.5% 로제시하는저자들도있으며, 균종에따라 97-99% 에이르는다양한기준치가제시되고있다. Janda 등은동일균종이라고판단할수있는염기서열유사성은최저 >99% 이어야하며, 이상적으로는 >99.5% 의유사성을지녀야한다고제시한바있다 [17]. 아직까지는모든세균에일괄적으로적용할수있는염기서열차이의기준치 (cut-off value) 는없는상태이다 [17, 18]. 본증례에서는타병원혈액에서분리된균, 본원혈액배양검사에서분리된균, 판막조직에서분리된균이배양장비의표현형검사만으로는균종이상이한결과를보였으나, 유전형검사결과, 모두염기서열이일치하여동일한균주임을알수있었고, S. lugdunensis 표준균주와비교하였을때염기서열유사성이 99.93% 이었으며, 함께비교한다른 CNS 균종들의염기서열은모두본증례의균주와는 >0.5% 이상의차이를보였다. 이에저자들은 Janda 등의기준에의거하여본증례의균주를 S. lugdunensis 로확진하였다 (Table 1). Mateo 등의연구에서는 Vitek 2가 S. lugdunensis 의 23.5% 를 S. haemolyticus 로오진하였는데 [6], 본증례의균주염기서열을다른 CNS 균종과비교했을때, S. haemolyticus 와는 99.01% 의유사성을보였고, 본증례에서 Vitek 2가오진했던 S. warneri 와는 98.66% 의유사성을보였다. S. lugdunensis 는임상적으로중요한감염증을일으키는 CNS 이지만, Vitek 2에서정확히동정되지못하는경우자칫임상에서진단을놓칠수있어, 환자가임상적으로유의한감염증을가지고있고검체에서 CNS 가분리될경우엔 S. lugdunensis 의가능성을염두에두고염기서열분석까지시행해보는것이필요할것으로생각된다. 저자들은기저판막질환이없던자연판막에서발생한 S. lugdunensis 에의한감염성심내막염으로서 Vitek 2에서상이한균종동정결과를보였지만 16S rrna 염기서열분석법을통해 S. lugdunensis 로확진된증례를경험하고수술및항생제치료로성공적으로치료하였기에문헌고찰과함께보고하는바이다.
